황색포도알균의 항생제 내성 (Antibiotic Resistance of Staphylococcus Aureus)

  • Staphyloccus aureus is one of the most important pathogens in clinical settings. It is also one of the leading causes of nosocomial infections and the dissemination of multiple drug-resistant strains, mainly methicillin resistant Staphyloccus aureus, and the recent emergence of a vancomycin resistant MRSA is the concern to hospital worldwide. MRSA strains have acquired multiple resistance to a wide range of antibiotics, including aminoglycosides and macrolides. $\beta$-Lactam resistance of methicillin-resistnat Staphyococcus aureus is determined by the function of penicillin binding protein 2'(PBP2') encoded by the methicillin resistance gene mec A. MRSA strains carry methicillin resistance gene mecA, encoded by a mobile genetic element designated staphylococoal cassette chromosome mec(SCCmec). MRSA clones are defined by the type of SCCmec element and the genotype of the methicilline-susceptible Staphyococcus aureus chromosome in which the SCCmec element is integrated.

국내 분리 Brucella canis의 항균제 감수성 (Antimicrobial Susceptiblity of Brucella canis Isolated from Korea)

  • Little is known to data about the in vitro activity of antimicrobial agents aganist Brucella cams (B cams) isolated from Korea. Our study aimed at determining the in vitro activities of 15 antimicrobial agents against 3 isolates and 52 isolates of B cams from dogs in 1994 and 2002, respectively. In minimal inhibitory concentration (MIC) study, minocycline and doxycycline showed the lowest MICs ( < 0.06-0.5 ug/ml). Gentamicin, streptomycin, ciprofloxacin, norfloxacin and rifampin showed MICs in the range of less than 1 ug/ml. Lincomycin and sulfisox azole showed the highest MICs ( > 32 ug/ml). Interestingly, MICs of macrolides (erythromycin, spiramycin, tylosin) against 52 isolates in 2002 were 16-64 times higher than that of 3 isolates in 1994. In minimal bactericidal concentration (MBC) study, gentamicin, streptomycin, ciprofloxacin and norfloxacin showed the lowest MBCs [0.12-1 ug/ml (1-2 times higher than MIC)], but minocycline and doxycycline showed the highest MBCs [8-32 ug/ml (128 times higher than MIC)]. Rifampin showed the MBCs in the range from 2 to 4 ug/ml (2-4 times higher than MIC).

중요 항생제의 분류와 식품안전분야에서 활용 (Classification of Critically Important Antimicrobials and their Use in Food Safety)

  • WHO에서는 항생제 사용으로 인한 위해를 줄이고 사람의 건강을 지키기 위한 목적으로, 더욱 신중하게 사용해야 하는 항생제를 '중요 항생제'로 분류하였다. 중요 항생제 선정은 2가지 기준으로 분류하였는데, 기준 1은 사람에서 심각한 세균 감염을 치료하기 위하여 유일한 또는 치료제가 제한적인 항생제 계열, 기준 2는 세균이 비인체 유래로부터 사람으로 감염된 경우 또는 세균이 비인체로부터 내성유전자를 획득할 가능성이 있는 경우이다. 그 결과 '매우 중요한 항생제(Critically important antimicrobials, CIA)', '중요도 높은 항생제(Highly important antimicrobials, HIA)', '중요항생제(Important antimicrobials, IA)' 등 3개 그룹으로 분류되었다. 특히, 매우 중요한 항생제 중에서도 관리의 우선순위가 높은 항생제를 '최우선 중요 항생제(Highest priority critically important antimicrobials, HPCIA)'로 재분류하였는데, 여기에는 3세대 이상 Cephalosporins, Macrolides, Quinolones, Glycopeptides 및 Polymyxins가 속하였다. 중요 항생제 목록은 많은 국가에서 항생제의 올바른 사용과 내성 감소를 위한 위해평가 및 위해관리 정책 등 다양한 분야에 과학적 근거를 제공하며, McDonalds, KFC 등 식품산업 분야에서도 식품안전을 위하여 중요 항생제 목록이 활용되고 있다. 중요 항생제 목록(CIA list)는 앞으로도 새로운 연구 결과를 반영하여 지속적으로 보완될 것이다. 국내에서도 우리의 항생제 내성에 대한 조사결과 등을 신중하고 면밀하게 검토하여 우리 실정에 적합한 목록을 개발함으로써 식품안전성을 확보해 나가는 것이 필요할 것으로 생각된다.

Monitoring of Veterinary Antibiotics in Animal Compost and Organic Fertilizer with CHARM II System

  • Veterinary antibiotics (VAs) in animal compost and organic fertilizer can have adverse effect on ecosystem and eventually human health. The main purpose of this research was to evaluate feasibility of Charm II system for monitoring residuals of VAs in animal compost and organic fertilizer. Four different VAs (Tetracyclines: TCs, Sulfonamides: SAs, Macrolides: MLs, and ${\beta}$-lactams: ${\beta}$-LTs) were analyzed and total of 100 samples were monitored. Results reveled that SAs in animal compost showed the highest detection frequency (64%) with exceeded concentration of criteria. However, very low detection frequency (0-12%) for ${\beta}$-LTs was observed in animal compost and organic fertilizer. Depending on physicochemical properties of each VAs, detection frequency of VAs was determined. In conclusion, charm II system can be utilized to screen if residual of VAs is in animal compost and organic fertilizer. Also, more research is necessary to establish standard method for analysis of VAs in complex matrix and to minimize adverse effect of VAs from source to environment.

안개 D형 저지원을 갖는 MLS 내성 황색 포도상 구균의 분포와 내성 기전 (Characterization of S. aureus Showing MLS Resistance with Foggy D Shape (fMLS))

  • A new type of macrolides, lincosamides and streptogramin B antibiotics (MLS) resistance, showing the characteristic phenotype growing within the inhibition zone area around the clindamycin disk, was identified among clinically isolated Staphylococcus aureus. We named the phenotype as MLS resistance with foggy-D shape (fMLS). The average frequency of MLS isolates was 9%. All of fMLS isolates have only erm(A) for the resistance determinant. The growth pattern of the challenged MLS isolate looks intermediate phase between the patterns of inducible MLS resistance and constitutive MLS resistance.

전국에서 도축된 가축의 잔류검사에서 검출된 잔류물질 비교조사 (Comparison of antibiotic residues detected by the residue tests of slaughtered livestock in Korea)

  • It was carried out to investigate the antibiotic residues detected by the residue tests for edible tissues of slaughtered livestock in Korea from January to November in 2019. Positive cases of qualitative test and analytical test for 11 months were 689 and 341, respectively. Positive cases of analytical test for cattle, pigs, horses and poultry were 164, 168, 2 and 7, respectively. Antibiotics were detected from 472 materials from 341 cases and classified as β-lactams 162 (37.9%), aminoglycosides 141 (33%), quinolones 69 (16.2%), sulfonamides 23 (5.4%), amphenicols 10 (2.3%), tetracyclines 9 (2.1%), macrolides 6 (1.4%) and the rest 7 (1.4%). Other materials were benzyl penicillin 119 (27.9%), (dihydro)streptomycin 105 (24.6%) and enrofloxacin (including ciprofloxacin) 39 (9.1%). In conclusion, strict management of benzyl penicillin and streptomycin in cattle and pigs should be implemented to reduce the positive cases of slaughtered livestock in Korea.

$Ureaplasma$ infections in pre-term infants: Recent information regarding the role of $Ureaplasma$ species as neonatal pathogens

  • Although numerous clinical observational studies have been conducted over a period of over 30 years, the clinical significance of $Ureaplasma$ infection is still under debate. The $Ureaplasma$ speices. is a commensal in the female genital tract and considered to have of low virulence; however, $Ureaplasma$ colonization has been associated with infertility, stillbirth, preterm delivery, histologic chorioamnionitis, and neonatal morbidities, including congenital pneumonia, meningitis, bronchopulmonary dysplasia, and perinatal death. Recently, $Ureaplasma$ was subdivided into 2 separate species and 14 serovars. $Ureaplasma$ $parvum$ is known as biovar 1 and contains serovars 1, 3, 6, and 14, and $Ureaplasma$ $urealyticum$ (biovar 2) contains the remaining serovars (2, 4, 5, and 7-13). The existence of differences in pathogenicities of these 14 serovars and 2 biovars is controversial. Although macrolides are the only antimicrobial agents currently available for use in neonatal ureaplasmal infections, in the current clinical field, it is difficult to make decisions regarding which antibiotics should be used. Future investigations involving large, multicenter, randomized, controlled studies are needed before proper recommendations can be made for clinical practice.

Cloning and Expression of Glucose-1-Phosphate Thymidylyltransferase Gene (schS6) from Streptomyces sp. SCC-2136

  • The deoxysugar biosynthetic gene cluster of Sch 47554/Sch 47555 was cloned from Streptomyces sp. SCC-2136. One of the ORFs, schS6, appeared to encode glucose-1-phosphate thymidylyltransferase, which converts dTTP and glucose-1-phosphate to TDP-D-glucose and pyrophosphate. The dTDP-D-glucose is a key metabolite in prokaryotics as a precursor for a large number of modified deoxysugars, and these deoxysugars are a maj or part of various antibiotics, ranging from glycosides to macrolides. SchS6 was expressed in E. coli vector pSCHS6 and the expressed protein was purified to apparent homogeneity by ammonium sulfate precipitation and Ni-NTA affinity column chromatography. The specific activity of the purified enzyme increased 4.7-fold with 17.5% recovery. It migrated as a single band on SDS-PAGE with an apparent molecular mass of 56kDa. The purified protein showed glucose-1-phosphate thymidylyltransferase activity, catalyzing a reversible bimolecular group transfer reaction. In the forward reaction, the highest activity was obtained with combination of dTTP and ${\alpha}-D-glucose-1-phosphate$, and only 12% of that activity was obtained with the substrates $UTP/{\alpha}-D-glucose-1-phosphate$. In the opposite direction, the purified protein was highly specific for dTDP-D-glucose and pyrophosphate.

Incidence of Erythromycin Resistance Genes, erm(B) and mef(A), in Streptococci Isolated from Dental Plaques of Koreans

  • Erythromycin is a macrolide antibiotic and inhibits bacterial protein synthesis by stimulating the dissociation of the peptidyl-tRNA molecule from the ribosomes during elongation. The use of macrolides has increased dramatically over the last few years and has led to an increase in bacterial resistance to these antibiotics. Bacterial resistance to erythromycin is generally conferred by the ribosome methylation and/or transport (efflux) protein genes. Among the identified erythromycin-resistant genes, erm(B) (erythromycin methylation) and mef(A) (macrolide efflux) are generally detectable in erythromycin-resistant streptococcal species. The distribution of these genes in oral streptococcal isolates has been reported in studies from other countries but has not been previously examined in a Korean study. We here examined by PCR the presence of erm(B) and mef(A) in oral streptococci isolated from Korean dental plaques. Among the 57 erythromycin-resistant strains tested, 64.9% harbored erm(B) whereas 40.4% were positive for mef(A). Eleven isolates had both the erm(B) and mef(A) genes. Twenty six isolates had only erm(B) and 12 isolates had only mef(A). Eight of the 57 strains examined were negative for both genes.

Staphylococcus aureus의 항생제 내성 plasmid에 관한 연구 (R-plasmids in staphylococcus aureus)

  • Small size antibiotic resistance plasmids having molecular weights less than 10 Mdal were isolated and characterized from ten clinically isolated multiple resistant Staphylococcus aureus. Agarose gel electrophoresis profiles and antibiotic resistance patterns divided these strains into four groups. Strain 2-23-6, the representative strain of a group of five strains conferred two plasmids of molecular weights $1.6{\times}10^6\;dal\;and\;2.0{\times}10^6$ dal. The small plasmid (pSBK 112) specified macrolides, lincosamides and streptogramin type B (MLS) resistance gene which are expressed constitutively. Lage plasmid (pSBK 125) specified chloramphenicol resistance gene which is inducible. Strain 10-5 conferred a $3.0{\times}10^6$ dal plasmid (pSBK 141) which carry an inducible ampicillin resistance gene and strain P-H-2 conferred and $1.6{\times}10^6$ dal plasmid (pSBK 190) which carry a constitutive MLS resistance gene. Strain D-H-1 conferred four plasmids of molecular weights $0.8{\times}10^6$ dal (pSBK 201), $1.6{\times}10^6$ dal (pSBK 202), $2.5{\times}10^6$ dal (pSBK 203), and $1.2{\times}10^7$ dal (pDBK 204), respectively. Among those four plasmids, only pSBK 203 specified chloramphenicol resistance gene. Curing of constitutive MLS resistance using acriding orange or ethidium bromide in 2-23-6 and P-H-2 strains produced 'inducible' MLS resistance strains which are less resistant to MLS than the wild type strains, suggesting that there are two resistance genes in both strains; one is constitutive and the other is inducible.
