Accuracy in target localization in stereotactic radiosurgery using diagnostic machines (정위적 방사선수술시 진단장비를 이용한 종양위치결정의 정확도 평가)

  • The accuracy in target localization of CT, MR, and digital angiography were investigated for stereotactic radiosurgery. The images using CT and MR were obtained out of geometrical phantom which was designed to produce exact coordinates of several points within a 0.lmm error range. The slice interval was 3mm and FOV was 35cm for CT and 28cm for MR. These images were transferred to treatment planning computer using TCP/IP in forms of GE format. Measured 3-D coordinates of these images from planning computer were compared to known values by geometrical phantom. Anterior-posterior and lateral films were taken by digital angiography for measurement of spatial accuracy. Target localization errors were 1.2${\pm}$0.5mm with CT images, 1.7${\pm}$0.4mm with MR-coronal images, and 2.1${\pm}$0.7mm with MR-sagittal images. But, in case of MR-axial images, the target localization error was 4.7${\pm}$0.9mm. Finally, the target localization error of digital angiography was 0.9${\pm}$0.4mm. The accuracy of diagnostic machines such as CT, MR, and angiography depended on their resolutions and distortions. The target localization error mainly depended on the resolution due to slice interval with CT and the image distortion as well as the resolution with MR However, in case of digital angiography, the target localization error was closely related to the distortion of fiducial markers. The results of our study should be considered when PTV (Planning Target Volume) was determined.

High Resolution Time Resolved Contrast Enhanced MR Angiography Using k-t FOCUSS (k-t FOCUSS 알고리듬을 이용한 고분해능 4-D MR 혈관 조영 영상 기법)

  • Purpose : Recently, the Recon Challenge at the 2009 ISMRM workshop on Data Sampling and Image Reconstruction at Sedona, Arizona was held to evaluate feasibility of highly accelerated acquisition of time resolved contrast enhanced MR angiography. This paper provides the step-by-step description of the winning results of k-t FOCUSS in this competition. Materials and Methods : In previous works, we proved that k-t FOCUSS algorithm successfully solves the compressed sensing problem even for less sparse cardiac cine applications. Therefore, using k-t FOCUSS, very accurate time resolved contrast enhanced MR angiography can be reconstructed. Accelerated radial trajectory data were synthetized from X-ray cerebral angiography images and provided by the organizing committee, and radiologists double blindly evaluated each reconstruction result with respect to the ground-truth data. Results : The reconstructed results at various acceleration factors demonstrate that each components of compressed sensing, such as sparsifying transform and incoherent sampling patterns, etc can have profound effects on the final reconstruction results. Conclusion : From reconstructed results, we see that the compressed sensing dynamic MR imaging algorithm, k-t FOCUSS enables high resolution time resolved contrast enhanced MR angiography.

Elliptical Centric Techniques and Tricks About the Usefulness of the Clinical Application (Elliptical Centric과 TRICKS 기법의 임상 적용에 관한 유용성 연구)

  • To prospectively determine the diagnostic performance a combination of standard bolus-chase magnetic resonance (MR) angiography and MR angiography with time-resolved imaging of contrast kinetics (TRICKS) for depicting severity of the head and neck vascular diseases. Over a period of two months, A total of 100 patients(average ages : $50{\pm}8$, male : 60, female : 40) with head and neck vascular diseases were performed on the GE excite 3.0 T units with 8-channel head coil and 4-channel NV coil. Imaging parameters for a typical study were as follow: SBC(TR/ TE/ FA/ SliceThicken./ Slab/ Freq./ FOV/ BW/Scan Time) = 5.4/ min/ 30/ 2/ zip2/ 70/ $224{\times}448$/ 30/ 62.50/ 28, TRICKS(TR/ TE/ FA/ Slice Thicken/Slab/ Freq./ FOV/ BW/ Temp Res./ Scan Time = 3.6/ min/ 25/ 4/ 30/ $160{\pm}384$, zip512/ 30/ 100/ 1 to 1.5/ 23). The analysis of all MR images, which have respect-ively classified two techniques into some diseases. The results of the former were divided into two groups(SBC, TRICKS)with 4 grading of two reader, respectively. Wilcoxon signed rank test was used to determine if a significant difference between imaging techniques existed(p < 0.05). In 33 of 100 patients, arterio-venous malformation was 11% at TRICKS, subclavian vein stenosis : 8%, fistular sinus : 4%, jugular vein stenosis:6%, Middle Cerebral Artery bypass surgery : 4%, p < 0.05). The rest of 67 patients were considered as the results of SBC(14% in the basilar artery stenosis, carotid stenosis : 16%, vertebral stenosis : 17%, central neuro-cytoma : 5%, meningioma : 5%, Not appliable : 10%, p < 0.05). Sensitivity and specificity of TRICKS MR angiography in SVS, FS, JVS, MCABS were improved compared with those at standard MR angiography. In SBS MR angiography which were improved in BAS, CS, VS, CN, Meningioma. In conclusion, TRICKS MR angiography of the SVS, FS, JVS, MCABS is superior to standard MR angiography regarding the number of diagnostic grading. The SBS MR angiography were improved in BAS, CS, VS, CN, Meningioma. and assessment of the degree of luminal narrowing on both TRICKS and SBS.

Diagnosis of Subclavian Steal: Contrast Enhanced 3D MR Angiography vs 2D TOF

  • Purpose: Subclavian steal refers to the retrograde flow of blood in a vertebral artery that supplies t ipsilateral shoulder and arm caused by proximal subclavian artery stenosis or occlusion. T purpose of this exhibit is to demonstrate MR findings of subclavian steal on contrast-enhanc 3D (CE 3DMRA) and 2D TOF MR angiography. Method: Four patients(men 3 and women 1, age: 28-78years) with subclavian steal obtained both CE 3DMRA and digital subtraction angiography(DSA) including subclavian arterie Sequential imaging was undertaken during first pass after double dose of Gd-DTPA (0 mM/kg) Injected by a power injector. Coronal source images were obtained with coronal D-fast low angle shot sequence(TR/TE/flip angle=3.8/l.3/35, acquisition time= 10sec/one measurement). Precontrast imaging was subtracted from enhanced images and maximu intensity projection was done. 2D time-of-flight MR angiography(2D TOF) of the carot bifurcation was added in all cases with post-saturation. All studies were review retrospectively.

Magnetization Transfer Contrast Angiography for Organized Thrombosed Intracranial Aneurysm in TOF MR Angiography: a Case Report

  • A 66-year-old woman was referred for treatment of incidental detection of two intracranial aneurysms. Time-of-flight MR angiography (TOF MRA) revealed two aneurysms at the M1 segment of the right middle cerebral artery, and clinoid segment of left internal carotid artery, respectively. On digital subtraction angiography, there was a saccular aneurysm on the left internal carotid artery, but the other aneurysm was not detected on the right middle cerebral artery. Based on comprehensive review of imaging findings, organized thrombosed aneurysm was judged as the most likely diagnosis. In the presented report, magnetization transfer (MT) pulse to TOF MRA was used, to differentiate aneurysm-mimicking lesion on TOF MRA. We report that MT technique could be effective in differentiating true aneurysm, from possible T1 high signal artifact on TOF MRA.

Contrast-Enhanced Magnetic Resonance Angiography: Dose the Test Dose Bolus Represent the Main Dose Bolus Accurately?

  • Objective: To determine whether the time-intensity curves acquired by test and main dose contrast injections for MR angiography are similar. Materials and Methods: In 11 patients, repeated contrast-enhanced 2D-turbo-FLASH scans with 1-sec interval were obtained. Both test and main dose timeintensity curves were acquired from the abdominal aorta, and the parameters of time-intensity curves for the test and main boluses were compared. The parameters used were arterial and venous enhancement times, arterial peak enhancement time, arteriovenous circulation time, enhancement duration and enhancement expansion ratio. Results: Between the main and test boluses, arterial and venous enhancement times and arteriovenous circulation time showed statistically significant correlation (p < 0.01), with correlation coefficients of 0.95, 0.92 and 0.98 respectively. Although the enhancement duration was definitely greater than infusion time, reasonable measurement of the end enhancement point in the main bolus was impossible. Conclusion: Only arterial and venous enhancement times and arteriovenous circulation time of the main bolus could be predicted from the test-bolus results. The use of these reliable parameters would lead to improvements in the scan timing method for MR angiography.

  • X-Ray Angiography has been gold standard or imaging of blood flow. However, patients have to take many risks such as catheterization procedure, iodine contrast agent and ionizing X-rays. MR Angiography has been suggested as a substitute for its non-hazard to patient. But the resolution and contrast was inferior to that of X-ray Angiography. Recently the resolution and the contrast have been much improved due to the development of fast imaging technique.

  • Contrast-enhanced MR angiography has become a widely used method useful for clinical diagnosis. Early studies identified a number of technical issues, and many of these have been addressed with various MRI physics innovations over the last several years. The quality of the results is high enough that CE MRA is replacing conventional x-ray angiography methods at many institutions. Ongoing research is expected to provide further improvements in performance, most notably in additional reductions in examination time, in time-resolved 3D imaging, and in improved imaging of the peripheral vasculature with extended fields of view.

Optimization of Flip Angle at Head & Neck MR Angiography using Gadoteridol (Gadoteridol을 이용한 Head & Neck MR Angiography에서의 적정 Flip Angle)

  • In this research, we tried to suggest moderate FA(Flip Angle) for CE(Contrast Enhnaced)-Head&Neck MR Angiography with Gadoteridol. For this study, we did test MR phantom and clinical study according to FA change. After that, quantitative analysis was progressed. The results of MR phantom study were as follow: RSP(Reaction Starting Point)was recorded within 300~400 mmol. MPSI(Max Peak Signal Intensity) was 2,086, 3,705, 5,109, 6,194, 7.096, 7,192 [a.u]. MPP(Max Peak Point) was shown at 30, 50, 50, 40, 50, 40 mmol. IRMPSI(Increase Rate of MPSI) was 77.6%, 37.9%, 21.2%, 14.6%, 1.4% as increasing of FA. The results of clinical study were as follow SICB(Signal Intensity of Carotid artery Bifurcation) was recorded respectively 392.5, 4165.2, 4270, 3502.2, 3263.7, 3119.6 [a.u]. ORA(Occurence Rate of Artifact) was increased as 0, 0, 20, 40, 50, 70%. According to this research, we are not only able to assure that increase of FA can be effect on H1 spin's SI(Signal Intensity) which was combined with gadolinium agent, but also be effect on artifact rate in blood vessel. In clinical field, we expect that CE-Head&Neck MR Angiography can be performed in a practical way with this research.
