Individual approach in the recanalization treatment of the acute ischemic brain stroke according to the various MR findings in hyperacute stage

  • We will present various MR findings of hyperacute ischemic stroke with our own experiences in the management of the patients according to the findings. 대상 및 방법: A total of 441 patients were underwent 'acute stroke MR' imaging protocol between Mar. 2001 and Jun. 2003. The protocol included initial T2-weighted image (WI), diffusion WI (DWI, b=2000), time-of-flight (TOF) MR angiography (MRA), and pefusion WI(PWI), and follow-up T2WI, DWI, TOF MRA, and neck vessel contrast-enhanced MRA obtained three to five days after the insult. Among them, we retrospectively reviewed the MR findings and clinical courses of 193 patients with anterior circulation territorial infarction. Those ICA and MCA lesions were divided into six and five groups respectively according to the level and mechanism of the occlusion. PWI findings can be another factor in the management planning.

MR Findings of Seizure-Related Cerebral Cortical Lesions during Periictal Period

  • Purpose: This study investigated the MRI, MR angiography (MRA) and MR perfusion findings of seizure-related cerebral cortical lesions during the periictal period. Materials and Methods: From a retrospective review of the institutional database between 2011 and 2014, a total of 21 patients were included in this study. Two radiologists assessed periictal MRI, including MRA and MR perfusion, in patients with seizure-related cortical lesions. The parameters examined include: location of cortical abnormality, multiplicity of the affected cortical region, cerebral vascular dilatation, perfusion abnormality and other parenchymal lesions. Results: All patients showed T2 hyperintense cerebral cortical lesions with accompanying diffusion restriction, either unilateral (18/21, 85.7%) or bilateral (3/21, 14.3%). Of the 21 patients enrolled, 10 (47.6%) had concurrent T2 hyperintense thalamic lesions, and 10 (47.6%) showed hippocampal involvement. Of the 17 patients (81%) who underwent MRA, 13 (76.5%) showed vascular dilatation with increased flow signal in the cerebral arteries of the affected cortical regions. On MR perfusion, all 5 patients showed cortical hyperperfusion, corresponding to the region of cortical abnormalities. Conclusion: Seizure-related cerebral cortical lesions are characterized by T2 and diffusion hyperintensities, with corresponding cerebral hyperperfusion and vascular dilatation. These findings can be helpful for making an accurate diagnosis in patients with seizure.

Optimal Region of Interest Location of Test Bolus Technique in Extra Cranial Carotid Contrast Enhanced Magnetic Resonance Angiography

  • This study is aimed to optimize a location of region of interest (ROI) in test bolus carotid contrast enhanced magnetic resonance angiography (CE-MRA) at 3.0T. A total of consecutive 270 patients with no cardiovascular and vessel diseases were selected. Patients underwent elliptical centric 3D CE-MRA with the test bolus technique to identify the individual arterial arrival time. Quantitative measurements were performed by drawing ROIs of $25mm^2$ and signal intensities (SI) were measured in the center of common carotid artery (CCA), internal carotid artery (ICA) and aortic arch (AA). As a result, ROIs located within AA showed a significantly clarified arterial peak and over three times increased SI, while no significant arterial peak time differences were observed compared to ROIs located within CCA. In conclusion, it was demonstrated that the aortic arch is the optimal position to locate ROI in test bolus images of the carotid CE-MRA.

경동맥 MRA 영상을 이용한 새로운 내경 측정 방법 (New Carotid Artery Stenosis Measurement Method Using MRA Images)

  • 현재 경동맥 내막절제술 시행을 위한 경동맥 협착증의 정도 측정에는 디지털감산조영술(DSA), 회전조영술(rotational angiography), 컴퓨터단층조영술(CTA) 및 자기공명조영술(MRA)로부터 얻어진 경동맥의 투영 영상을 이용하여 북미, 유럽 표준 및 총경동맥 방법이 사용되고 있다. 본 논문에서는 기존의 기계적인 측경기를 이용하는 전형적인 경동맥 협착 측정 방법의 단점을 극복하고, 측정자간의 변화율을 최소화하기 위해 자기공명조영술의 단면 영상을 사용하고 컴퓨터화한 새로운 협착증 정도 측정 방법을 개발하였다. 영상 분할에 사용되는 방법중 가장 널리 사용되고 효율적인 명암값 임계치 방법을 사용하여 경동맥 및 동맥의 내강을 분할하였다. 또한, 각 증례의 측정된 총경동맥의 혈관두께를 사용하여 분할된 경동맥으로부터 혈관을 제거 하였고, 혈관이 제거된 경동맥을 혈류 영역과 플라그 영역으로 분할하였다. 각 단면 영상에서의 경동맥 협착증 정도 측정은 (분할된 플라그 영역/혈류영역 및 플라그를 합한 면적) * 100% 식으로 계산된다.

Primary Antiphospholipid Antibody Syndrome: Neuroradiologic Findings in 11 Patients

  • Objective: To describe the neuroradiologic findings of primary antiphospholipid antibody syndrome (PAPS). Materials and Methods: During a recent two-year period, abnormally elevated antiphospholipid antibodies were detected in a total of 751 patients. In any cases in which risk factors for stroke were detected - hypertension, diabetes mellitus, hyperlipidemia, smoking, and the presence of SLE or other connective tissue diseases - PAPS was not diagnosed. Neuroradiologic studies were performed in 11 of 32 patients with PAPS. We retrospectively reviewed brain CT (n = 7), MR (n = 8), and cerebral angiography (n = 8) in 11 patients with special attention to the presence of brain parenchymal lesions and cerebral arterial or venous abnormalities. Results: CT or MR findings of PAPS included nonspecific multiple hyper-intensity foci in deep white matter on T2-weighted images (5/11), a large infarct in the territory of the middle cerebral artery (4/11), diffuse cortical atrophy (2/11), focal hemorrhage (2/11), and dural sinus thrombosis (1/11). Angiographic findings were normal (5/8) or reflected either occlusion of a large cerebral artery (2/8) or dural sinus thrombosis (1/8). Conclusion: Neuroradiologic findings of PAPS are nonspecific but in young or middle- aged adults who show the above mentioned CT or MR findings, and in whom risk factors for stroke are not present, the condition should be suspected.

뇌졸중 환자에서 3 Tesla 자기공명혈관조영술의 유용성 (The Usefulness of 3T-TOF MR angiography in Patients with Cerebral Infarction)

  • 목적 : 뇌혈관 평가에 있어서 경두개 도플러 초음파(transcranial doppler; TCD)와 고식적 뇌혈관조영술(CA)과 비교하여 3T 장비를 이용한 Time-of-flight(TOF) 자기공명 뇌혈관 조영술(3T-TOF MRA)의 유용성에 대해 알아보고자 하였다. 대상 및 방법 : 뇌혈관 질환이 의심된 54명의 환자를 대상으로 3T-TOF MRA를 시행하였다. 54명 모두에서 TCD를 시행하였으며, 이중 11명은 CA를 함께 시행하였다. 뇌혈관의 평가를 위해 추골동맥과 기저동맥을 포함하여 I군, 총경동맥의 분지부위 2 cm 전부터 내 외경동맥 분지 부위에서 내경동맥의 무릎(genu)부위 까지를 II군, 내경동맥의 추체부위에서 전 중 대뇌동맥의 1차분지 부위까지를 III군, 그리고 각각 전 중 대뇌동맥 1차분지 그 이하 부위를 IV군으로 임의로 구분하였다. 평가는 2명의 방사선과 의사가 3T-TOF MRA와 TCD, 그리고 CA의 결과를 모르는 상태에서 각 군에서 혈관의 양상이 잘 나타나는 경우를 3점, 양상이 잘 나타나지 않으나 판독이 가능한 경우를 2점, 잡상 등으로 혈관의 평가가 어려운 경우를 1점으로 하였다. 혈관의 협착 정도에 대한 평가는 TCD 및 CA와 각각 비교하여 그 일치 정도를 평가하였다. 결과 : 각 군당 좌우를 합하여 108 혈관분절, 총 432 혈관분절을 평가하였다. 정성평가에서 I, II, III, IV군은 각각 2.98, 2.96, 2.91, 2.88로 혈관의 양상에 대한 파악은 잘 되는 것으로 나타났다. TCD만을 시행한 43명 중 41명에서 3T-TOF MRA와 TCD가 일치하였고, TCD와 CA를 모두 함께 시행한 11명에서는 3T-TOF MRA가 TCD 및 CA와 모두 일치하였다. 결론 : 3T-TOF MRA는 뇌혈관 평가에 있어서 CA 및 TCD와 비교해서 빠르고 비침습적이며 혈관상태를 적절하게 평가할 수 있는 유용한 검사라고 생각한다.

Fast MRI in Acute Ischemic Stroke: Applications of MRI Acceleration Techniques for MR-Based Comprehensive Stroke Imaging

  • The role of neuroimaging in patients with acute ischemic stroke has been gradually increasing. The ultimate goal of stroke imaging is to make a streamlined imaging workflow for safe and efficient treatment based on optimized patient selection. In the era of multimodal comprehensive imaging in strokes, imaging based on computed tomography (CT) has been preferred for use in acute ischemic stroke, because, despite the unique strengths of magnetic resonance imaging (MRI), MRI has a longer scan duration than does CT-based imaging. However, recent improvements, such as multicoil technology and novel MRI acceleration techniques, including parallel imaging, simultaneous multi-section imaging, and compressed sensing, highlight the potential of comprehensive MR-based imaging for strokes. In this review, we discuss the role of stroke imaging in acute ischemic stroke management, as well as the strengths and limitations of MR-based imaging. Given these concepts, we review the current MR acceleration techniques that could be applied to stroke imaging and provide an overview of the previous research on each essential sequence: diffusion-weighted imaging, gradient-echo, fluid-attenuated inversion recovery, contrast-enhanced MR angiography, and MR perfusion imaging.
