MR Spectroscopy and Diffusion Weighted Imaging Findings of Primary Non-Hodgkin Lymphoma of the Breast: Two Case Reports (유방에 발생한 일차성 비호지킨림프종의 자기공명 분광법 및 확산강조 영상: 2예)

  • Primary non-Hodgkin lymphoma (NHL) of the breast is a very rare disease, and the mammographic and ultrasonographic findings of breast lymphoma are variable. There are several reports of magnetic resonance (MR) imaging findings in patients with breast lymphomas; however, few reports have described the findings observed on MR spectroscopy or the features of diffusion weighted (DW) imaging. The authors report the findings of classical MR imaging, MR spectroscopy and DW imaging of a 48-year-old woman and a 40-year-old woman with primary non-Hodgkin's lymphoma of breasts. Mammography and breast ultrasonography revealed a mass with circumscribed margin. The mass showed strong enhancement after contrast injection on MR imaging. DW imaging showed reduced diffusion and high-amplitude choline (Cho) peak at 3.22 ppm was detected by single voxel MR spectroscopy which was consistent with malignancy.

Hepatic Parenchrmal Changes After Percutaneous Injection of Holmium-166 in Rabbit (가토 간내에 Holmium-166 주입 후의 간 실질 변화)

  • Purpose : To evaluate changes in rabbit liver parenchyma on MR images following percutaneous Holmium-166 injection, and to correlate those changes with histologic findings. Materials and methods. Holmium-166 (10-25 mCi) was percutaneously injected into the liver of rabbit (n=12) under sonographic guidance. MR images were obtained between one to two weeks (acute phasea) after the injection in four rabbits, and between two to four weeks (subacute phase) after the injection in four rabbits. Tissue specimens of these eight rabbits were obtained immediately after MR imaging. Tissue specimens were obtained without MR imaging in four rabbits (between one to two weeks in one rabbit and between three to four weeks in three rabbits). Results : Tissue specimens showed central liquefactive necrosis and peripheral coagulative necrosis containing deposition of small particles and hemorrhage. The peripheral margin of the lesions showed formation of the granulation tissue with fibrosis, which tended to be more prominent in subacute phase. The area of the necrosis tended to correlate with the dose of the radioactive Holmium-166. On MR images, the central portion of the necrosis showed hyperintensity on 72-weighted image, hypointensity on the precontrast T1-weighted images, and no enhancement on the dynamic MR images. The peripheral portion of the necrosis showed hypointensity on T2-weighted images, iso or mild hypointensity on the T1-weighted images, and mild peripheral enhancement on the delayed dynamic MR images. The peripheral margin of the lesion showed hypointensity on both T1- and T1-weighted images with increased enhancement on the delayed phase images of the dynamic MR images. Conclusion : After percutaneous Holmium-166 injection into rabbit liver parenchyma, the central portion showed liquefactive necrosis, the peripheral portion showed coagulative necrosis with granulation, fibrosis, hemorrhage and depostition of small granules. MR imaging may be helpful in evaluation of the histological change of the liver after percutaneous Holmium-166 treatment.

Usefulness Comparative Experimental Study of the CT and MR Imaging in the Dog Clonorchiasis (잡견 간흡충증의 전산화단층촬영과 자기공명영상의 유용성에 관한 실험적 연구)

  • Purpose : Be aware of clinical possibilities on image quality by comparison of contrast-enhanced dynamic CT and MR imaging applied of MIP technique after the experimentally induced clonorchasis infection in dogs. Materials and Method : Twenty mongrel dogs prepared in zoo-laboratory were followed up with serial CT scans and MR imaging for 13 weeks after the experimental infection in liver. Two-phase helical CT was acquired in the supine position with the following scanning parameters. After the injection of contrast material, the arterial phase was initiated using a bolus-racking method. The portal phase scan was started 15 seconds after the arterial phase scan. CT protocol was determined after single level dynamic scans. MR imaging used the CP body coil and images get a 2D image using HASTE, FLASH, TSE pulse sequence. Bile duct MR imaging were obtained in three plans. Then each image was post processed by using target MIP algorithm. Two experimentation above, as a method of evaluation, one pathologist, three radiologist and five radiological technologist were analyzed visually for evaluation of following findings, enhancement of the bile duct wall, dilatation of bile duct tip, liver parenchyma, background suppression. Results : Five dogs was died of a disease after the infection, the rest one else shows the chronic dilatation of the intrahepatic bile duct with CT and MR imaging. Contrast administration of CT shows the contrast-enhanced of the bile duct walls with live parenchyma. MR imaging calculated of CNR and CR from pulse sequence for comparative evaluation and shows the pattern of the intrahepatic bile duct, dilatation of bile duct tip using MIP technique. CNR of the clonorchiasis, HASTE was $16{\pm}0.83$, TSE $7.06{\pm}3.0$, FLASH $1.19{\pm}0.2$ and CR, HASTE was 73.3%, TSE 62.3%, FLASH 6.4%. Conclusion : CT and MR imaging is very usefulness in diagnosis of dog clonorchiasis.

High Resolution Time Resolved Contrast Enhanced MR Angiography Using k-t FOCUSS (k-t FOCUSS 알고리듬을 이용한 고분해능 4-D MR 혈관 조영 영상 기법)

  • Purpose : Recently, the Recon Challenge at the 2009 ISMRM workshop on Data Sampling and Image Reconstruction at Sedona, Arizona was held to evaluate feasibility of highly accelerated acquisition of time resolved contrast enhanced MR angiography. This paper provides the step-by-step description of the winning results of k-t FOCUSS in this competition. Materials and Methods : In previous works, we proved that k-t FOCUSS algorithm successfully solves the compressed sensing problem even for less sparse cardiac cine applications. Therefore, using k-t FOCUSS, very accurate time resolved contrast enhanced MR angiography can be reconstructed. Accelerated radial trajectory data were synthetized from X-ray cerebral angiography images and provided by the organizing committee, and radiologists double blindly evaluated each reconstruction result with respect to the ground-truth data. Results : The reconstructed results at various acceleration factors demonstrate that each components of compressed sensing, such as sparsifying transform and incoherent sampling patterns, etc can have profound effects on the final reconstruction results. Conclusion : From reconstructed results, we see that the compressed sensing dynamic MR imaging algorithm, k-t FOCUSS enables high resolution time resolved contrast enhanced MR angiography.

  • 의료영상에서 인공물(Artifacts) 이라 함은 영상이 얻어지는 신체부위와 아무런 관련이 없으나 얻어진 영상에는 마치 영상의 일부분으로 나타나는 모든 것을 가리킨다. 따라서 영상에서 이들 인공물들은 실제 조직의 해부학적인 구조를 나타내지 않으므로 영상 판독에 영향을 주어 잘못된 진단을 초래할 수도 있다. 그러나 MR 영상이 가능한 이래로 새로운 여러 종류의 MR 인공물들이 많이 발견 되었으나 다행스럽게도 거의 모든 MR 인공물들은 쉽게 설명이 가능하며, 따라서 이들 인공물들에 의한 진단 오류의 가능성은 매우 희박한 실정이다. 그러나 새로운 영상방법이나 혹은 새로운 펄스대열이 계속 고안됨에 따라 새로운 종류의 인 공물들이 생겨날 가능성은 항상 존재하고 있다. 지금까지 알려진 여러 MR 인공물들은 그 생겨난 원인에 따라 다음과 같이 크게 세 가지로 분류가 가능하다. 견관절의 영상의학적 검사

  • 단순촬영에서 견관절을 이루는 골격의 형태를 파악하고 MRI에서는 주로 회전근 개 건의 fraying및 파열, 오훼견봉인대의 견봉 부착부위의 fraying, 점액낭염, 활막염, 관절순의 병변, 연골연화와 같은 연부조직의 병변을 관찰한다. 회전근 개 파열의 고식적 MR 진단은 정확도가 93%, 민감도가 84~100%, 특이도가 88~97%, 지방억제 MR 관절강 조영술은 민감도 및 특이도가 90%및 75%에서 100%까지 최근 보고 되고 있다. 단순촬영검사로 충돌중후군의 일반적인 소견을 파악할 수 있고 impingement series에는 견관절 전후면, suprascapular outlet view (SSO), craniocaudal 30 degree tilt view (CC30T), axillary view를 포함한다. 특히 SSO에서 견봉 및 ACJ의 형태 및 골극의 유무를 파악하고, CC30T에서는 견봉 돌출의 정도를 파악할 수 있다. 최근 고해상도의 MDCT로서 얻은 3차원 영상이 충돌증후군의 전반적인 영상 소견을 파악할 수 있고 수술전 평가에 이용되고 있다. MRI 및 MR Arthrography의 영상은 일반적으로 극상건의 주행에 따른 고식적 MR 사위 관상면이 중요하고 사위 시상면에서 견봉돌기의 형태와 대결절에 부착되는 극상건의 파열 유무와 정도를 파악할 수 있다. MR 관절조영술 후 외전 및 외회전후 (ABER view)의 영상도 관절면 쪽으로 부분 파열된 극상건의 진단에 유용하다.

Pulse Sequence based MR Images for Compressed Sensing Algorithm Applications (펄스열을 이용한 MR 영상의 Compressed Sensing 알고리즘 적용)

  • In recent years, compressed sensing (CS) algorithm has been studied in various research areas including medical imaging. To use the CS algorithm, the signal that is to be reconstructed needs to have the property of sparsity But, most medical images generally don't have this property. One method to overcome this problem is by using sparsifying transform. However, MR imaging, compared to other medical imaging modality, has the unique property that by using appropriate image acquisition pulse sequences, the image contrast can be modified. In this paper, we propose the possibility of applying the CS algorithm with non-sparsifying transform to the pulse sequence modified MR images and improve the reconstruction performance of the CS algorithm by using an appropriate sparsifying transform. We verified the proposed contents by computer simulation using Shepp-Logan phantom and in vivo data.

Efficient Compression of MR Images Using Fractal Coding in Wavelet Transform Domain (웨이브릿 변환 영역에서의 프랙탈 부호화를 이용한 효율적 MR 영상 압축)

  • We propose an efficient MR image compression technique using fractal coding in wavelet transform domain. In the Proposed method , we construct significant coefficient trees with the absolute values of discrete wavelet transform coefficients and then perform the fractal coding with the information of significant coefficients having high energy. For MR images, most Pixels including background have very low gray level values, the number of significant coefficients is small. so we can expect high compression rate. In addition. since this method uses the fractal coding in wavelet transform domain, blocking artifact is reduced prominently and edges sensitive to human visual system are well preserved. As a result of computer simulation, we obtained the reconstructed images with better quality than those by JPEG at the low bit rates below 0.33[bpp].

Automatic Brain Segmentation for 3D Visualization and Analysis of MR Image Sets (MR영상의 3차원 가시화 및 분석을 위한 뇌영역의 자동 분할)

  • In this paper, a novel technique is presented for automatic brain region segmentation in single channel MR image data sets for 3D visualization and analysis. The method detects brain contours in 2D and 3D processing of four steps. The first and the second make a head mask and an initial brain mask by automatic thresholding using a curve fitting technique. The stage 3 reconstructs 3D volume of the initial brain mask by cubic interpolation and generates an intermediate brain mask using morphological operation and labeling of connected components. In the final step, the brain mask is refined by automatic thresholding using curve fitting. This algorithm is useful for fully automatic brain region segmentation of T1-weighted, T2-weighted, PD-weighted, SPGR MRI data sets without considering slice direction and covering a whole volume of a brain. In the experiments, the algorithm was applied to 20 sets of MR images and showed over 0.97 in comparison with manual drawing in similarity index.

Under standing of MR pulse suquences (MR 펄스파형의 이해)

  • MR 영상을 획득하기 위해서 사용하고 있는 가장 기본적인 펄스파형(pulse sequense)은 스핀반향(spin echo)과 경사자계반향(gradient echo)이며, 최근 다방면으로 응용되고 있는 고속영상기법들은 이 두가지 펄스파형을 기보능로 하고 있다. 펄스파형의 종류와 펄스파형의 변수(TR, TE TI등의 펄스 상이의 시간 간격 및 flip angle 등)에 따라서 MR영상의 특징(대조도, SNR, artifact 등)이 달라지므로 임상응용시에는 목적에 따라 적절한 펄스파혀의 선택과 함께 변수값을 결정해야 한다. 이를 위하여 펄스파형에 관한 기본적인 지식과 함게 간단한 임상응용에 관하여 기술하고자 한다.
