Energy cost of walking in older adults: accuracy of the ActiGraph accelerometer predictive equations

  • BACKGROUND/OBJECTIVES: Various accelerometer equations are used to predict energy expenditure (EE). On the other hand, the development of these equations and their validation studies have been conducted primarily without including older adults. This study assessed the accuracy of 8 ActiGraph accelerometer equations to predict the energy cost of walking in older adults. SUBJECTS/METHODS: Thirty-one participants with a mean age of 74.3 ± 3.3 yrs were enrolled in this study (20 men and 11 women). The participants completed 8 walking activities, including 5 treadmill and 3 self-paced walking activities. The EE was measured using a portable indirect calorimeter, with each participant simultaneously wearing the ActiGraph accelerometer. Eight ActiGraph equations were assessed for accuracy by comparing the predicted EE with indirect calorimetry results. RESULTS: All equations resulted in an overall underestimation of the EE across the activities (bias -1 to -1.8 kcal·min-1 and -0.7 to -1.8 metabolic equivalents [METs]), as well as during treadmill-based (bias -1.5 to -2.9 kcal·min-1 and -0.9 to -2.1 METs) and self-paced (bias -1.2 to -1.7 kcal·min-1 and -0.2 to -1.3 METs) walking. In addition, there were higher rates of activity intensity misclassifications, particularly among vigorous physical activities. CONCLUSIONS: The ActiGraph equations underestimated the EE for walking activities in older adults. In addition, these equations inaccurately classified the activities based on their intensities. The present study suggests a need to develop ActiGraph equations specific to older adults.

A Study on Energy Efficiency in Walking and Stair Climbing for Elderly Wearing Complex Muscle Support System

  • Objective: This study was conducted to analyze the effect of wearable complex muscle support system on energy efficiency during walking in elderly. Design: Cross sectional study Methods: Twenty healthy elderly participated in this study. All subjects performed a 6 minuteswalk test(6MWT) and stair climbing test in dual, slack and no suit conditions. In each condition, oxygen consumption(VO2), metabolic equivalents(METs), energy expenditure measures(EEm), physiological cost index(PCI), walking velocity and heartrate were measured. Through repeated measured ANOVA, it was investigated whether there was a statistically significant difference in the measurement results between the three conditions. Results: In over-ground walking, VO2, METs and EEm showed significant differences between no suit and slack conditions(p<0.05). In stair climbing, VO2 showed significant difference between slack and dual conditions(p<0.05). Also, METs and EEm showed significant differences between no suit and slack, and between slack and dual conditions(p<0.05). Conclusions: Wearing the wearable complex muscle support system for elderly does not have much benefit in energy metabolism efficiency in over-ground, but there is a benefit in stair walking.

COVID-19 이환자의 브루스 프로토콜을 이용한 유산소 운동이 심박수, 산소포화도, 혈압에 미치는 영향 (Effect of Aerobic Exercise using Bruce Protocol on Heart Rate, Oxygen Saturation, and Blood Pressure after Recovery from COVID-19 Infection)

  • Purpose : This study aimed to find out the effect of aerobic exercise using Bruce protocol on heart rate, oxygen saturation, and blood pressure after recovery from COVID-19 infection. Methods : In this study, 34 students from D University located in J city were targeted, COVID-19 infected group (17 people) and non-COVID-19 non-infected group (17 people). The Bruce protocol using a treadmill was applied to the aerobic exercise of this study. The Bruce protocol has in the first stage of METs 4 (slope 10 %, speed 2.7 km/h). The second stage was METs 5 (slope 12 %, speed 4 km/h), and the third stage was METs 6 (slope 14 %, speed 5.4 km/h). All measurements were measured 3 times and the average value was used. Results : As a result of this study, as a result of comparing heart rate changes according to aerobic exercise using the Bruce protocol. EG group and the CG group increased significantly according to the progressive exercise load (METs 4~5), and in the third stage of the Bruce protocol between groups, The EG group showed a significantly lower heart rate. As a result of comparing changes in oxygen saturation and blood pressure, there was no significant difference between the EG and C groups according to the gradual exercise load. Conclusion : In conclusion, there was no difference between normal and pressure when MET of moderate intensity exercise (4 to 6) was applied to the effect on heart rate, oxygen saturation, and blood pressure in healthy adults who were fully recovered from COVID-19. Secondary side effects may occur when high intensity exercise with a MET of 6 or higher, so it is strongly recommended that hospitals or specialized institutions measure exercise and physical ability according to individual exercise intensity.

Association between Exercise Capacity and Cardiovascular Risk Factors among Obesity Types in Adult Man

  • Increased waist circumference has shown to be more strongly associated with cardiovascular disease risk factors. The purpose of this study is to investigate the association between exercise capacity and cardiovascular risk factors among obese types in adult men. The subjects of this study were a total fifty-four obese persons and obesity criteria is body mass index $(BMI){\geq}25kg/m^2$. Diagnostic criteria for obesity was defined as a waist circumference of ${\geq}90cm$. The BMI in the obese subjects, as judged by the presence or absence of abdominal obesity, were classified into two groups (non-AO: without abdominal obesity group, AO: with abdominal obesity group). AO presented lower total exercise time, metabolic equivalents (METs) than Non-AO. AO showed slow HRR (heart rate recovery) response. HRR was negative correlated with BMI, body fat mass, waist circumference. AO had a high heart rate and a low cardiac output in submaximal exercise stage 1~2. In conclusion, AO's (with abdominal obesity groups) total exercise time, METs and HRR are lower than Non-AO. HRR is related with BMI, body fat mass and waist circumference.

Obesity and Metabolic Syndrome in Adults with Prader-Willi Syndrome

  • Body fat distribution in patients with Prader-Willi syndrome (PWS) is characterized by reduce lean body mass (LBM), increased total body fat mass (FM), and lower percentage of visceral adipose tissue (VAT). Individuals with PWS seem to have a lower risk for insulin resistance with high levels of adiponectin, an anti-atherogenic adipocytokine that is decreased in visceral fat hypertrophy subjects compared to simple obese subjects, both in children and in adults. The mechanism of the reduction in visceral adiposity in PWS is still unclear. It might be related to qualitative intrinsic characteristics of adipocyte or novel genetic influences on the control of fat distribution. However, obesity remains a critical problem, and obesity status plays a crucial role in individual metabolic risk clustering and development of metabolic syndrome (Mets) in PWS children and adults. Long-term growth hormone (GH) treatment after cessation of skeletal growth improved body composition, with an increase in lean body mass and a reduction in total body fat and subcutaneous and visceral fat in PWS adults. Thus, the role of GH is important after childhood because it might attenuate obesity and Mets in PWS adult by adipocyte modification.

Improved block-wise MET for estimating vibration fields from the sensor

  • Modal expansion technique (MET) is a method to estimate the vibration fields of flexible structures by using eigenmodes of the structure and the signals of sensors. It is the useful method to estimate the vibration fields but has the truncation error since it only uses the limit number of the eigenmodes in the frequency of interest. Even though block-wise MET performed frequency block by block with different valid eigenmodes was developed, it still has the truncation error due to the absence of other eigenmodes. Thus, this paper suggested an improved block-wise modal expansion technique. The technique recovers the truncation errors in one frequency block by utilizing other eigenmodes existed in the other frequency blocks. It was applied for estimating the vibration fields of a cylindrical shell. The estimated results were compared to the vibration fields of the forced vibration analysis by using two indices: the root mean square error and parallelism between two vectors. These indices showed that the estimated vibration fields of the improved block-wise MET more accurately than those of the established METs. Especially, this method was outstanding for frequencies near the natural frequency of the highest eigenmode of each block. In other words, the suggested technique can estimate vibration fields more accurately by recovering the truncation errors of the established METs.

심장재활 프로그램이 심근경색증 환자의 건강행위와 생리적 지수에 미치는 효과 (Effects of a Cardiac Rehabilitation Program on Health Behavior and Physiologic Parameters for Myocardial Infarction Patients)

  • Purpose: The purpose of this study was to investigate the effects of a cardiac rehabilitation program for patients with myocardial infarction. Method: The subjects were 45 myocardial infarction patients(22 for experimental group and 23 for the control group). Data were collected through questionnaire survey, blood test, and the treadmill test, between October 1, 1999 and December 31, 2000. As for data analyses, paired and unpaired t-test and $\chi^2$ test were adopted using the SAS program. Result: 1. The post-test revealed that increase in compliance score of health behavior was significantly higher in the experimental group than in the control group. 2. The increase in HDL cholesterol was significantly higher in the experimental group than in the control group, in the post-test. 3. The experimental group showed significantly higher duration of exercise time and maximal METs than the control group after the program. Conclusion: The above findings indicate that a cardiac rehabilitation program was effective in increasing compliance of health behaviors, serum HDL cholesterol level, duration of exercise time, and maximal METs in patients with myocardial infarction. Accordingly, we can adopt the individualized cardiac rehabilitation programs as a nursing intervention.

운동 강도와 에너지소비량에 따른 단기간의 유산소운동이 면역세포에 미치는 영향 (The Effects on Immune Cell of Short-Term Aerobic Exercise by Exercise Intensity and Expenditures Calorie)

  • 본 연구는 남자학생 8명을 대상으로 GXT로 사전검사를 실시하여 최대운동검사를 통하여 측정된 피험자 개개인의 VO2max를 기준으로 각각 50%, 70%에 해당되는 산소섭취량과 경사도 및 속도를 산출하고, 산소섭취량(ml/min/kg)의 METs 및 칼로리 소비량을 대입, 전체 300kcal와 600kcal가 소모되는 운동시간을 산출하였다. 유산소운동에 따른 T, B, NK cell의 변화는 림프구 전체에서 T, B, NK 림프구가 차지하는 상대적 비율이 운동 강도에서는 유의한 차이가 없었고, T cell에서 에너지 소비량(p<.01), 상호작용(p<.05)효과에서, B cell은 에너지소비량(p<.01)에서, NK cell은 에너지소비량(p<.001), 상호작용(p<.05)효과에서 통계적으로 유의한 것으로 나타났다.

가속도계와 신체활동일기를 이용한 초등학생 축구선수 남녀의 신체활동수준, 신체활동 패턴 및 에너지소비량 비교 (Comparison of Physical Activity Level, Physical Activity Pattern and Energy Expenditure in Male and Female Elementary School Soccer Players using Accelerometer and Physical Activity Diary)

  • Objectives: The purpose of this study was to compare the physical activity level (PAL), the physical activity pattern and the energy expenditure in male and female elementary school soccer players using the accelerometer and the physical activity diary. Methods: Twenty-five (male 11, female 14) elementary school soccer players (9-12 years) participated in this study. During their daily activities, they wore an accelerometer for seven days (five weekdays and two weekend days) and completed the physical activity diary for three days (two weekdays and one weekend day). PAL was calculated by using the physical activity diary and three equations (Pate Preschool, Freedson Children, and Freedson) were used to calculate the intensity of physical activity and energy expenditure from the counts of accelerometer. Results: The average of physical activity by day of the week, CPM (Counts Per Minute) and METs (Metabolic Equivalents) were significantly higher in males ($723.3{\pm}149.2CPM$, $2.07{\pm}0.18METs$) compared to females ($505.6{\pm}119.9CPM$, $1.79{\pm}0.20METs$), but there was no significant difference in PAEE (Physical Activity Energy Expenditure) between the two groups (males: $15.5{\pm}9.1kcal/day$, females: $11.5{\pm}6.0kcal/day$). During weekdays, physical activity intensity was significantly higher in males compared to females at lunch time and training time than at other times. In both genders, the PAL was higher during weekdays (males 1.98, females 1.89) compared to weekend (males 1.62, females 1.61) (p<0.05). Conclusions: Our study observations suggest the necessity to develop an accelerometer equation for accurately evaluating the physical activity of elementary school athletes.

Relationship between Circadian Variation in Ictus of Aneurysmal Subarachnoid Hemorrhage and Physical Activity

  • Objective : The circadian pattern of the onset time of aneurysmal subarachnoid hemorrhage (aSAH) has been reported by various authors. However, the effect of the degree of physical exertion on the circadian pattern has not been studied in detail. Therefore, we conducted this study to investigate the effect of physical exertion on the circadian pattern of aSAH. Methods : Of the 335 patients presenting with aSAH from January 2012 to December 2017, 234 patients with identifiable onset time and metabolic equivalent (MET) values were enrolled. The onset time of aSAH was divided into 4-hour intervals. The patient's physical exertion was then assessed on a scale between 1 and 8 METs using generally accepted MET values, and categorized into two groups-light exertion (1 to 4 METs) and moderate to heavy exertion (5 to 8 METs)-to determine the effect of the degree of physical exertion on the onset time distribution of aSAH. Multivariate analysis was used to calculate the odds ratio (OR) between the two groups to determine the effect of the degree of physical exertion on each set of time periods. Results : There was a definite bimodal onset pattern that peaked at 08:00-12:00 hours followed by 16:00-20:00 hours (p<0.001). MET values at all time intervals were found to be significantly higher than the night time (00:00-04:00 hours) values (p<0.031). The MET value distribution showed a unimodal pattern that slightly differed from the bimodal distribution of the onset time of aSAH. There were no significant differences in the ORs of each time interval according to the degree of the MET value. Conclusion : This study reaffirmed that aSAH occurs in a bimodal pattern, especially showing the highest prevalence in the morning. Although aSAH could be related to daily activity, there were no significant changes in diurnal variations affected by the degree of physical exertion.
