주요 우울장애와 준임상적 우울증에서의 감정표현불능증 (Alexithymia in Major Depressive Disorder and Subclinical Depression)

  • 연구목적 : 본 연구는 주요 우울장애 환자들과 준임상적 우울증 환자들의 감정표현불능증 정도를 조사하기 위해 시행되었다. 방법 : 지역사회에 거주하는 386명(남/여=148/238, 평균 연령 : $43.0{\pm}13.9$, 연령 범위 : 19~79)이 연구에 참여했다. 구조화된 임상면담(Structured Clinical Inteview for DSM-IV, SCID)를 이용하여 주요 우울장애 및 기타 1축 정신질환을 진단했다. 우울증상를 평가하기 위해 역학연구센터 우울척도(the center for epidemiologic studies-depression scale, CES-D)를 자가 작성하게 했다. 감정표현불능증은 토론토 감정표현불능증 척도(Toronto alexithymia scale, TAS)로 평가했다. 주요 우울장애의 병력이 없는 이 중 SCID에서 경도 우울장애가 있거나, CES-D 점수가 16점 이상인 경우를 준임상적 우울증으로 정의했다. 주요 우울장애군, 준임상적 우울증군, 정상 대조군 세군으로 나누어 TAS 총점과 각 요인별 점수를 비교했다. 결과 : 전체 386명 중, 38명(9.8%)이 DSM-IV 주요 우울장애의 진단 기준에 부합하는 병력이 있었고, 57명(14.8%)이 준임상적 우울증군으로 분류되었다. 각 군간 TAS 총점($F_{2,383}=14.0$, p<0.01), 요인 1(감정인식의 장애) ($F_{2,383}=23.4$, p<0.01), 요인 2(감정표현의 장애)($F_{2,383}=7.8$, p<0.01)에 차이가 있었다. TAS 요인 3(외부지향적 사고)는 유의한 차이가 없었다($F_{2,383}=1.8$, p=0.16). 주요 우울장애군과 준임상적 우울증군은 모두 대조군에 비해 TAS 총점, 요인 1, 요인 2의 점수가 높았다(모두 p<0.01). 반면, 주요 우울장애군과 준임상적 우울증군은 TAS 총점 및 각 요인 점수에서 유의한 차이가 없었다. 결론 : 준임상적 우울증 환자는 우울증상이 없는 대조군에 비해 감정표현불능증이 더 심했다. 한편, 주요 우울장애와 준임상적 우울증은 감정표현불능증에서 유의한 차이가 없었다. 이러한 결과는 감정표현 불능증이 주요 우울장애 뿐아니라 준임상적 우울증과도 관계있음을 시사한다.

소아에서 aminoglycoside의 1일 1회 요법 (Once daily dosing of aminoglycoside in children)

  • Aminoglycosides are frequently used antibiotics in children. The multiple daily dosing (MDD) in infants and children is twice or three times daily depending on age. Recent studies in adults have shown that once daily dosing (ODD) maximizes the bactericidal activity and might minimize the toxicity of antibiotics. So, I reviewed many studies about efficacy, toxicity and cost effectiveness of ODD of aminoglycosides in children. Most studies suggest that ODD compared with MDD of aminoglycosides is theoretically more efficacious and has no higher toxicity in infants and children. But, the total number of patients included in the studies is not large. Multi-center, controlled prospective studies are required in larger numbers of infants and children to determine the efficacy and safety of the ODD regimen in children before ODD of aminoglycosides can be recommended for routine use.

Modeling of GN type III with MDD for a thermoelectric solid subjected to a moving heat source

  • We design the Green-Naghdi model type III (GN-III) with widespread thermoelasticity for a thermoelectric half space using a memory-dependent derivative rule (MDD). Laplace transformations and state-space techniques are used in order to find the general solution for any set of limit conditions. A basic question of heat shock charging half space and a traction-free surface was added to the formulation in the present situation of a traveling heat source with consistent heating speed and ramp-type heating. The Laplace reverse transformations are numerically recorded. There are called the impacts of several calculations of the figure of the value, heat source spead, MDD parameters, magnetic number and the parameters of the ramping period.

주요 우울 장애, 정신분열병, 양극성 장애 및 정상 대조군에서의 N100 진폭경사 (N100 Amplitude Slopes in Major Depressive Disorder, Bipolar Disorder, Schizophrenia and Normal Controls)

  • Objectives : N100 amplitude slope(the intensity dependence of the cortical auditory evoked potentials) is widely considered as an indirect indicator of central serotonergic neurotransmission. However, there are only a few studies about N100 amplitude slopes of major psychiatric disorders. In this study, we examined N100 amplitude slope differences among major depressive disorder(MDD), bipolar disorder(BD), schizophrenia (SCZ) and normal controls(NC). Methods : We measured the N100 amplitude slopes of 35 patients with MDD, 33 patients with BD, 27 patients with SCZ and 35 NC subjects. Amplitude differences from N1 to P2 at the five different sound intensities(55, 65, 75, 85 and 95dB) were examined at Cz electrode. The N100 amplitude slope was calculated as the linear regression of five N1/P2 peak-to-peak amplitudes across stimulus intensities. Results : BD patients showed significantly reduced N100 amplitude slope compared with NC(0.54${\pm}$0.70 vs. 0.96${\pm}$0.72, p=0.035). N100 amplitude slope of SCZ patients was significantly reduced compared with NC(0.50${\pm}$0.47 vs. 0.96${\pm}$0.72, p=0.027). N100 amplitude slope of BD patients was significantly lower than that of MDD patients(0.54${\pm}$0.70 vs. 0.94${\pm}$0.60, p=0.046). SCZ patients also showed significant reduction of N100 amplitude slope compared with MDD patients(0.50${\pm}$0.47 vs. 0.94${\pm}$0.60, p=0.036). There was no significant difference of N100 amplitude slope between MDD patients and NC(0.94${\pm}$0.60 vs. 0.96${\pm}$0.72, p=1.000). Conclusion : Interestingly, the N100 amplitude slopes of BD and SCZ were reduced compared to NC and MDD patients. Our results suggest the predictive use of N100 amplitude slope in making differential diagnoses of major psychiatric disorders. Clinical implications of N100 amplitude slope in major psychiatric disorders were discussed.

  • 복잡한 하드웨어 및 소프트웨어를 설계함에 있어 그 안정성에 대한 보장이 매우 중요하다. 이를 위해 정형 검증이나 테스팅과 같은 많은 기법을 활용하고 있다. 그러나 안정성 검증을 위해 시스템을 모델링하고 데스트 케이스를 만드는 과정에서 상태 폭발에 따른 메모리의 한계에 부딪히게 된다. 본 논문에서는 이러한 문제를 해결하고, 메모리를 효율적으로 이용할 수 있는 탐색방법을 이용한 데스트 케이스 생성 알고리즘을 제안한다.

Lack of Association between Serotonin Transporter Promoter Gene Polymorphism and Citalopram Response in Major Depressive Disorder

  • The 5-HTT gene is a candidate gene for influencing the clinical response to antidepressant treatment. The purpose of this gene study was to determine the relationship between serotonin transporter gene polymorphism at the SLC6A4 and the response to citalopram in a Korean population with major depressive disorder (MDD). Citalopram was administered for 8 weeks to the 80 patients who completed this study. The severity of depression was assessed with the 21-item Hamilton Depression Rating scale, and the 5-HTTLPR genotypes in the patients were determined using the polymerase chain reaction. Our result did not showed significant differences in, allele, and carrier distribution between the normal group and MDD patients. This study suggest that polymorphism of the 5HTT gene was not associated with citalopram response to MDD in the Korean population.

흙의 공학적 성질에 관한 연구 (Study on Engineering Properties of Earth Materials)

  • This study was made to investigate various engineering properties of earth materials resulting from their changes in density and moisture content. The results obtained in this study are summarized as follows: 1. The finner the grain size is, the bigger the Optimum Moisture Content(OMC) is, showing a linear relationship between percent passing of NO. 200 Sieve (n) and OMC(Wo) which can be represented by the equation Wo=0.186n+8.3 2. There is a linear relationship of inverse proportion between OMC and Maximum Dry Density (MDD) which can be represented by the equation ${\gamma}$d=2.167-0.026Wo 3. There is an exponential curve relationship between void ratio (es) and MDD whose equation can be expressed ${\gamma}$d=2.67e-0.4550.9), indicating that as MDD increases, void ratio decreases. 4. The coefficent of permeability increases in proportion to decrease of the MDD and this increase trend is more obvious in coarse material than in fine material, and more obvious in cohesionless soil than in cohesive soil. 5. Even in the same density, the coefficient of permeability is smaller in wet than in dry from the Optimum Moisture Content. 6. Showing that unconfined compressive strength increases in proportion to dry density increase, in unsaturated state the compacted in dry has bigger strength value than the compacted in wet. On the other hand, in saturated state, the compacted in dry has a trend to be smaller than the compacted in wet. 7. Even in the same density, unconfined compressive strength increases in proportion to cohesion, however, when in small density and in saturated state, this relationship are rejected. 8. In unsaturated state, cohesion force is bigger in dry than in wet from OMC. In saturated state, on the other hand, it is directly praportional to density. 9. Cohesion force decreases in proportion to compaction rate decrease. And this trend is more evident in coarse matorial than in fine material. 10. Internal friction angle of soil is not influenced evidently on the changes of moisture content and compaction rate in unsaturated state, On the other hand in saturated state it is influenced density. 11. Cohesion force is directly proportional to unconfined compressive strength(qu), indicating that it has approximately 35 percent of qu in unsaturated state and approximately 70 percent of qu in saturated state.

아증후군적 우울증 (Subsyndromal Depression)

  • Subsyndromal depression (SSD) is found to be more prevalent than major depressive disorder (MDD) and minor depressive disorder (MnDD). SSD is also associated with adverse clinical outcomes, increased risk of suicide, increased social dysfunction and disability, increased risk for future mood disorders, and increased uses of medical and mental health services. DSM-IV diagnostic criteria are not suitable for capturing SSD. Although there is no agreement on gold standard to define SSD so far, three definitions of SSD are available. First, SSD is defined as having two or more current depressive symptoms without core depressive symptoms (depressive mood or loss of interest) and with time threshold (most of the day and nearly every day over at least two weeks). Second, SSD is defined as having two or more current depressive symptoms with core depressive symptoms and without time threshold. Third, SSD is defined by using cutoff points of depression rating scales. SSD may represent a prodromal, residual, or interepisode symptomatic state in the course of MDD and MnDD. More than a half of SSD patients became any type of depressive disorders (SSD, MnDD and MDD) at 1 year. SSD may represent a discrete category of its own, without prior or consequent episodes of MDD. Considering clinical significance of SSD such as its high prevalence, significant psychosocial impairment and chronicity and serious outcomes, researchers and clinicians should be more vigilant in capturing and caring for patients with SSD.

Resolution of Molecular Species of the Triacylglycerol Containing Petroselinic Acid $(cis-C_{18:1{\omega}12)$ by Silver Ion-HPLC

  • On the analysis of triacylglycerol (TG) from the kernels of Acanthopanax sessiliflorus by reversed phase-HPLC, it was separated into three main fractions of PN 44, 46 and 48, according to partition number (PN). On the contrary, it could be clearly classified into seven fractions of SMM, MMM, SMD, MMD, SDD, MDD and MDT by silver ion-HPLC by the number of double bond in the acyl chains of TG species. But resolution of so-called critical pairs of TG molecular species such as molecular pairs of $P_eLL$ $[C_{18:1{\omega}12}/(C_{18:2{\omega}6)2}]$ and OLL $[C_{18:1{\omega}9}/(C_{18:2{\omega}6)2}]$ and OOL $[(C_{18:1{\omega}9)2}/C_{18:2{\omega}6]$, and $P_eP_eL$ $[(C_{18:2{\omega}12)2}/C_{18:1{\omega}6]$ was not achieved $(P_e;$ petroselinic acid, L; linoleic acid, O; oleic acid). On the other hand, TG extracted from Aralia continentalis kernels were also fractionated into seven groups of SSM, SMM, MMM, SMD, MMD, SDD and MDD (S; saturated acid, M; monoenoic acid, D; dienoic acid) by silver ion-HPLC, although it's were classified into three groups of PN 44, 46 and 48 by reversed phase-HPLC. The fractions of SMM, MMM, MMD and MDD were divided into two subfractions, respectively; the fractions of SMM, MMM, MMD and MDD were resolved into the subfraction of $PP_e/P_e$ and POO (critical pairs from each other), that of $P_e/P_e/P_e$ and OOO, that of $P_e/P_e/L$ and OOL, and that of $P_e/L/L$ and OLL.

주요우울장애 환자에서 생애초기스트레스와 연관된 심박변이도의 특성 (Characteristics in Heart Rate Variability associated with Early Life Stress in Patients with Major Depressive Disorder)

  • Objectives : Early life stress (ELS) may have impact on functions of the autonomic nervous system. Heart rate variability (HRV) is a reliable psychophysiological marker for functions of the autonomic nervous system. The purpose of this study was to investigate characteristics of HRV associated with ELS in patients with major depressive disorder (MDD). Methods : We compared HRV measures of MDD patients with ELS and without ELS in a 5-minute resting-state electrocardiogram recoding. Forty subjects participated in the study (25 with ELS, 15 without ELS). The Mann-Whitney test was conducted to identify group differences. Results : We found significant group differences in standard deviation of the NN interval (SDNN) and total power (TP). SDNN was lower in the ELS group (M=38.80 ms, SD=13.05 ms) than in the Non-ELS group (M=53.53 ms, SD=19.47 ms). TP was lower in the ELS group ($M=7.07ms^2$, $SD=0.69ms^2$) than in the Non-ELS group ($M=7.72ms^2$, $SD=0.77ms^2$). Conclusion : ELS may have a negative impact on the autonomic nervous system function in patients with MDD. ELS and dysfunction of autonomic nervous system should be considered in treatment for patients with MDD.
