A Monitor Unit Verification Calculation in IMRT as a Dosimetry QA

  • In standard teletherapy, a treatment plan is generated with the aid of a treatment planning system, but it is common to perform an independent monitor unit verification calculation (MUVC). In exact analogy, we propose and demonstrate that a simple and accurate MUVC in Intensity Modulated Radiotherapy (IMRT) is possible. We introduce a concept of Modified Clarkson Integration (MCI). In MCI, we exploit the rotational symmetry of scattering to simplify the dose calculation. For dose calculation along a central axis (CAX), we first replace the incident IMRT fluence by an azimuthally averaged fluence. Second, the Clarkson Integration is carried over annular sectors instead of over pie sectors. We wrote a computer code, implementing the MCI technique, in order to perform a MUVC for IMRT purposes. We applied the code to IMRT plans generated by CORVUS. The input to the code consists of CORVUS plan data (e.g., DMLC files, jaw settings, MU for each IMRT field, depth to isocenter for each IMRT field), and the output is dose contribution by individual IMRT field to the isocenter. The code uses measured beam data for Sc, Sp, TPR, (D/Mu)$\_$ref/ and includes effects from MLC transmission, and radiation field offset. On a 266 MHZ desktop computer, the code takes less than 15 sec to calculate a dose. The doses calculated with MCI algorithm agreed within +/- 3% with the doses calculated by CORVUS, which uses a 1cm x 1cm pencil beam in dose calculation. In the present version of MCI, skin contour variations and inhomogeneities were neglected.

경도인지장애의 침 치료에 대한 무작위배정 대조 임상시험(RCT) 연구 논문에 대한 질 평가 : STRICTA와 CONSORT 지침을 바탕으로 (Quality Assessment of Studies on Randomized Controlled Trials (RCTs) of Acupuncture Treatment for Mild Cognitive Impairment: Based on the STRICTA and CONSORT Guidelines)

  • Objective: The purpose of this study was to investigate quality of reporting acupuncture intervention for mild cognitive impairment (MCI) based on the STRICTA and CONSORT guidelines Methods: We searched for randomized controlled trial studies of acupuncture treatment for MCI in the MEDLINE (PubMed) database. Once the online search was completed, studies were selected manually by the inclusion criteria, and the selected studies were evaluated by STRICTA and CONSORT guidelines. Results: Five studies were included. The most frequently selected points for acupuncture treatment of MCI were on the head, such as GV20, EX-HN1, GV24, and GB20. By STRICTA guidelines, all items from STRICTA were reported except three items on the checklist. By CONSORT guidelines, apart from six items which were not available, quality assessment was performed with 31 items. Eighteen items were reported by 80% of the studies. Four items were reported by 40% of the studies and another four items were reported by 20% of the studies. One item was not reported in all studies. Conclusion: Most items were reported by STRICTA guidelines, whereas many items were insufficiently reported based on CONSORT guidelines. This needs to be considered in future RCTs of acupuncture treatment for MCI.

Effect of Education on Discriminability of Montreal Cognitive Assessment Compared to Mini-Mental State Examination

  • Background and Purpose: The Montreal Cognitive Assessment (MoCA) has been known as a screening test for detecting mild cognitive impairment (MCI) better than Mini-Mental State Examination (MMSE). However, in previous domestic studies, no significant difference was found in the discriminability between MoCA and MMSE. Researchers have suggested that this might be because older Koreans are less educated than older Westerners. This study was conducted to examine the effect of education on the discriminability of MoCA compared to the MMSE. Methods: Participants were 123 cognitively normal elderly, 118 with vascular MCI, 108 with amnestic MCI, 121 with vascular dementia, and 113 with dementia of the Alzheimer's type. The Korean-MoCA (K-MoCA) and Korean-MMSE (K-MMSE) were administered. Multiple regression analyses and receiver operating characteristic (ROC) curve analyses were performed. Results: In all participants, education significantly affected both K-MoCA and K-MMSE scores along with age. The effect of education was re-examined by subgroup analysis after dividing subjects according to the level of education. Effect of education on K-MoCA and K-MMSE was only shown in the group with <9 years of education. ROC curve analyses revealed that the discriminability of K-MoCA to differentiate between vascular MCI and normal elderly was significantly higher than that of K-MMSE. When re-examining subgroups divided by education level, however, this higher discriminability of K-MoCA disappeared in the group with <9 years of education. Conclusions: These results indicate no difference in discriminating cognitive deficits between K-MoCA and K-MMSE in Korean elderly with <9 years of education.

Gender Differences in Items of the Instrumental Activities of Daily Living in Mild Cognitive Impairment and Alzheimer's Disease Dementia

  • Background and Purpose: Each item in the instrumental activities of daily living (IADL) questionnaire has differential importance to an individual's life functioning based on gender. However, IADL has mostly been utilized for its total score alone, without gender specificity. We identify the impact of each item on the transition from amnestic mild cognitive impairment (aMCI) to Alzheimer's disease dementia (ADD), and determine if the impact of each item differs by gender. Methods: Subjects were aMCI or ADD with a global clinical dementia rating of 0.5 or 1. The sample size was 146 men and 154 women. We used logistic regression analysis to determine the effect of each item of IADL on the transition from aMCI to ADD. Results: The odds ratio (OR) for "remembering recent events" had similar values: 27.2 for men, and 27.7 for women. Gender difference was identified in the item with the highest OR value. For women, the "using transportation" item was 63.3, and for men, "conducting financial affairs" was overwhelmingly high at 89.1. Conclusions: Functional decline on items with relatively higher ORs may indicate higher probability of a transition from aMCI to ADD. The OR of "conducting financial affairs" was relatively higher for both genders. In terms of gender differences, "conducting home repair" for men, and "using transportation" for women, have relatively higher impact. This study demonstrates that during the transition from aMCI to ADD, each item of IADL shows a staggered decline in functioning, and that this decline is gender-specific.

  • This paper has developed a system for early diagnosis of senile dementia and mild cognitive impairment (MCI) by developing software to measure the volume of hippocampus. This software consists of two parts; segmentation and analysis. The segmentation part uses ROI and region growing to segment hippocampus region. On the other hand, the analysis part creates a volume rendering of hippocampus. This software is expected contribute in these research fields for dementia diagnosis and its medication planning.

Citations of Journal of Korean Neurosurgical Society Assessed by KoMCI-Trends in Recent Five Years

  • The authors analyzed the citation of Journal of Korean Neurosurgical Society, based on Korean Medical Citation Index[KoMCI] from 2000 to 2004. In this analysis the impact factor showed no significant further improvement in 2004 compared to 2003. However, the number of received citations was increased remarkably. If this increase can be kept up, it may have positive effect on impact factors. In the era of open competition of medical journals, the awareness and cooperations of every members of Korean Neurosurgical Society is essential for improvement of the status of our journal in a steady trend.

아스팔트포장 평가지수의 개발 (Development of Pavement Condition Index for Asphalt Pavement)

  • 효율적인 포장의 관리를 위해서 포장상태를 객관적으로 평가하는 포장평가지수의 사용은 필수적이다. 현재 사용되고 있는 PSI, MCI, UPCI는 사용자나 관리자의 입장만을 각각 고려하여 종합적인 포장평가지수의 역할을 못하고 있는 실정이다. 따라서 본 논문에서는 기존 평가지수들의 문제점을 파악하고 이를 개선한 새로운 포장평가지수를 개발하였다. 또한 개발된 평가지수로 실제 포장자료를 이용하여 포장재료의 성질에 따른 지수의 민감도 분석을 실시하였다. 표층두께, 아스팔트점도, 아스팔트함량의 순서로 포장평가지수에 영향을 주었다.

경도인지장애, 알쯔하이머형 치매 환자에서 우울증상 유무에 따른 혈중 호모시스테인의 차이 (The Differences of Serum Homocysteine Levels in Mild Cognitive Impairment and Dementia of Alzheimer's Type with or without Depressive Symptoms)

  • 연구목적 경도인지장애와 치매는 인지기능의 퇴행과 우울 등과 같은 신경정신증상을 나타낸다. 이들의 진단에 도움을 줄 수 있는 다양한 진단 도구 중 혈중 호모시스테인 농도를 측정하는 방법이 있으며, 연구에 따르면 혈중 호모시스테인 농도의 증가는 알쯔하이머병, 우울증 그리고 다른 신경정신질환의 발생위험을 증가시킨다. 본 연구는 혈중 호모시스테인이 인지기능장애와 연관된다는 사실을 기초로 경도인지장애와 알쯔하이머형 치매에서 우울증상 유무에 따른 혈중 호모시스테인 농도를 분석해보았다. 방법 경도인지장애, 알쯔하이머형 치매로 진단된 환자 86명이 참여하였고 혈액검사를 통해 혈중 호모시스테인 농도를 측정하였다. 환자들의 전반적인 인지기능 및 심각도의 단계 평가는 한국판 간이정신상태검사(MMSE-KC)와 전반적 퇴화척도(Global Deterioration Scale, GDS), 치매임상평가척도(Clinical dementia rating, CDR)를 이용하여 측정하였고, 우울증상 유무에 대한 평가는 한국판 노인우울척도(Korean version of Geriatric Depression Scale, K-GDS)를 통해 측정하였다. 결과 혈중 호모시스테인 농도는 경도인지장애군 중 우울증상이 있는 군에서 우울증상이 없는 군보다 통계적으로 유의한 높은 수치를 보였다. 치매환자군에서는 우울증상이 있는 군과 우울증상이 없는 군사이에 통계적으로 유의한 차이는 없었다. 상관분석에서 혈중 호모시스테인 농도와 MMSE-KC는 유의한 음의 상관 관계를 보였고, CDR, GDS는 유의한 양의 상관 관계를 보였다. 결론 이러한 결과는 혈중 호모시스테인 농도의 증가가 인지기능저하와 우울증의 위험인자라는 사실을 나타낸다. 특히 본 연구에서는 경도인지장애 환자군에서 혈중 호모시스테인 농도와 우울증상 간의 의미있는 연관성을 제시하였다. 본 연구 중의 몇 가지 제한점을 보완한 좀 더 대규모의 연구가 필요하다.

Target engagement of ginsenosides in mild cognitive impairment using mass spectrometry-based drug affinity responsive target stability

  • Background: Mild cognitive impairment (MCI) is a transitional condition between normality and dementia. Ginseng is known to have effects on attenuating cognitive deficits in neurogenerative diseases. Ginsenosides are the main bioactive component of ginseng, and their protein targets have not been fully understood. Furthermore, no thorough analysis is reported in ginsenoside-related protein targets in MCI. Methods: The candidate protein targets of ginsenosides in brain tissues were identified by drug affinity responsive target stability (DARTS) coupled with label-free liquid chromatography-mass spectrometry (LC-MS) analysis. Network pharmacology approach was used to collect the therapeutic targets for MCI. Based on the above-mentioned overlapping targets, we built up a proteineprotein interaction (PPI) network in STRING database and conducted gene ontology (GO) enrichment analysis. Finally, we assessed the effects of ginseng total saponins (GTS) and different ginsenosides on mitochondrial function by measuring the activity of the mitochondrial respiratory chain complex and performing molecular docking. Results: We screened 2526 MCI-related protein targets by databases and 349 ginsenoside-related protein targets by DARTS. On the basis of these 81 overlapping genes, enrichment analysis showed the mitochondria played an important role in GTS-mediated MCI pharmacological process. Mitochondrial function analysis showed GTS, protopanaxatriol (PPT), and Rd increased the activities of complex I in a dose-dependent manner. Molecular docking also predicted the docking pockets between PPT or Rd and mitochondrial respiratory chain complex I. Conclusion: This study indicated that ginsenosides might alleviate MCI by targeting respiratory chain complex I and regulating mitochondrial function, supporting ginseng's therapeutic application in cognitive deficits.

Computer programme to assess mandibular cortex morphology in cases of medication-related osteonecrosis of the jaw with osteoporosis or bone metastases

  • Purpose: The purpose of this study was to evaluate the morphology of the mandibular cortex in cases of medication-related osteonecrosis of the jaw (MRONJ) in patients with osteoporosis or bone metastases using a computer programme. Materials and Methods: Fifty-four patients with MRONJ (35 with osteoporosis and 19 with bone metastases) were examined using panoramic radiography. The morphology of the mandibular cortex was evaluated using a computer programme that scanned the mandibular inferior cortex and automatically assessed the mandibular cortical index (MCI) according to the thickness and roughness of the mandibular cortex, as follows: normal (class 1), mildly to moderately eroded (class 2), or severely eroded (class 3). The MCI classifications of MRONJ patients with osteoporosis or bone metastases were evaluated with the Pearson chi-square test. In these analyses, a 5% significance level was used. Results: The MCI of MRONJ patients with osteoporosis(class 1: 6, class 2: 15, class 3: 14) tended to be higher than that of patients with bone metastases(class 1: 14, class 2: 5, class 3: 0)(P=0.000). Conclusion: The use of a computer programme to assess mandibular cortex morphology may be an effective technique for the objective and quantitative evaluation of the MCI in MRONJ patients with osteoporosis or bone metastases.
