Efficacy of Bee-venom Acupuncture on Central Post Stroke Pain -Single-blind Randomized Controlled Trial- (중풍 후 중추성 통증환자에 대한 봉독약침의 임상효과 -단일맹검 무작위배정 환자 대조군 비교연구-)

  • Objectives : This study was designed to evaluate objectively the efficacy of bee-venom acupuncture on Central post-stroke patients. Methods : After screening, 25 patients recruited with eligible criteria. Among them 1 dropped out in treatment group, 4 in control group. We applied bee-venom acupuncture 6 points of body($LI_{15}$, $GB_{21}$, $LI_{11}$, $GB_{31}$, $ST_{36}$, $GB_{39}$) 2 times a week for 3 weeks for treatment group, and normal saline for control group in the same way. After 3 weeks treatment and 2 weeks follow up we measured VAS, categorial rating scale as a pain assessment also MBI, Modified Rankin Scale as a stroke recovery. Results : 1. Treatment group showed significant difference in VAS, categorial rating scale after treatment and 2 weeks follow up, compared with baseline. The control group showed significant difference between baseline and after treatment But the 2 weeks follow up, there is no significant difference compared with baseline. Also there's no significant difference in categorial rating scale. 2. MBI showed significant difference in treatment group between baseline and after treatment, but the control group did not have any difference. There's no significant difference in Modified Rankin Scale for treatment group and control group. 3. There's no significant difference between control group and treatment group. This is because of the small sample size in the study, Thus it seems to reflect individual difference strongly. Conclusions : There are positive effect on CPSP patients in pain and stroke recovery treating with bee-venom acupuncture than control group.

The Effects of Trunk Stabilization Training on a Stroke Patient's Postural Control and Activities of Daily Living: A Sing Subject Design (뇌졸중 환자의 체간안정화 훈련이 자세조절과 일상생활동작에 미치는 영향: 단일사례연구)

  • Objective : This study intends to suggest an effective clinical intervention method for stroke patients by applying a trunk stabilization training program to a single stroke patient and investigating and analyzing the effects of the program on postural control and activities of daily living. Methods : The subject of this study was a 75 year-old female stroke patient hospitalized in C Hospital located in Gyeongju, Gyeongsangbuk-do. As for the research design, A-B-A reversal design was used with a single subject design; and the research period was a total of five weeks from April 21, 2014 to May 23, 2014. As for the research process, a total of 15 sessions were carried out including three sessions of baseline(A), nine sessions of an intervention period(B), and three sessions of maintenance(A'); and the trunk stabilization program was applied during the intervention period. To evaluate the participant's postural control and activities of living life, the Postural Assessment Scale for Stroke(PASS) and the modified Barthel Index(MBI) were used. Visual analysis was used in which collected data were plotted on a graph, using Microsoft Office Excel 2013. Results : In the postural control test, the mean scores improved from 11.7 in the baseline phase to 14.8 in the intervention phase and 15.3 in the maintenance phase. In the evaluation of activities of daily living, the score improved from 66(medium dependence) during the baseline period to 80(low dependence) after intervention and at the maintenance phase. Conclusion : It was found that the trunk stabilization training program showed effects on the stroke patient's postural control or alignment, and furthermore a beneficial improvement in the patient's performing the activities of daily living, as well.

A short-term longitudinal study of mental health and academic burnout among middle school students (중학생의 정신건강과 학업소진의 단기종단연구)

  • This is a short-term longitudinal study investigating the relationships between changes in mental health and academic burnout among Korean middle school students. Study sample consisted of 409 middle school students in Seoul provision, with 161 male, 216 female, and 32 unidentified. Both Symptom Check List(SCL-47) and Maslach Burnout Inventory-Student Survey(MBI-SS) were used. In order to examine the pattern of changes in mental health and academic burnout among students, standardized residuals were calculated using regression equations which were then put into canonical correlation analysis. The results of this study are as follows. First, according to Function 1, among sub-factors of mental health, decreases in depression, compulsion, anxiety, and hostility were particularly associated with decreases in exhaustion and cynicism among academic burnout. Put in another way, students who showed increases in depression, compulsion, anxiety, and hostility experienced more academic exhaustion and cynicism. Second, according to Function 2, increases in phobic anxiety, compulsion with decreases in anxiety, depression were associated with decrease in academic exhaustion and increase in cynicism. Considering Russell's dimensional theory of emotion, Function 1 showed that mental health symptoms including both aroused and non-aroused affects were related with increases in exhaustion and cynicism while Function 2 showed that mental health symptoms including only aroused aspects were related with decrease in exhaustion and increase in cynicism. Thus, a conclusive intervention program seems to be required to deal with both aroused and non-aroused affects of students who are experiencing increased exhaustion and cynicism. On the other hand, an intervention program focused on aroused affects seems appropriate to students who are experiencing decreased exhaustion and increased cynicism. This study has its value in that it has enhanced understanding of students in school and counseling settings by revealing the relationships between mental health and academic burnout among adolescents and suggested differentiated intervention strategies based on patterns of students' academic burnout.

The Effects of Self-Exercise Based on Health Care Application on Upper Extremity Function and Daily Living, Satisfaction in Patients with Stroke (헬스케어 애플리케이션 기반의 자가운동이 뇌졸중 환자의 상지기능, 일상생활, 만족도에 미치는 효과)

  • The aim of this study was investigated the effect of self-exercise based on health care application on upper extremity function and daily living, satisfaction in patients with stroke. This experiments were participated in thirty patients with stroke. All subjects allocated that randomized each fifteen patients in experimental and control groups. Subjects of both group received the conventional rehabilitation therapy during 30 min/day, 5 for week, for 4 weeks. Additionally, experimental group performed that self-exercise based on health care application, supervised under caregivers, during 30 min/day. Additionally, control group conducted only self-exercise supervised under caregivers, during 30 min/day. The outcome measures were the JHFT(: Jebsen-Taylor Hand Function Test), FMA(: Fugl-Meyer Assessment), K-MBI(: Korean Modified Barthel Index), VASS(: Visual Analog Satisfaction Scale). In results of study. Two groups showed significant improvements after intervention in all tests(p<.05). In comparison of change score between both group, experimental group showed greater significant improvements than control group in JHFT(p<.05). The experimental group was significant higher than control group in VASS(p<.05). We suggested that self-exercise based on health care application can have a positive effects of the improvements of hand function and satisfaction than usual self-exercise in patients with stroke.

The Effect of Task-Oriented Training After Botulinum Toxin Injection on Upper Extremity Function and Activities of Daily Living For Patients with Stroke: Case Study (보툴리눔 독소 주사 후 과제 지향적 훈련이 뇌졸중 환자의 상지기능 및 일상생활활동에 미치는 효과: 사례연구)

  • Objective : This study was examined the effects of task-oriented training therapy after botulinum toxin injection, upper extremity function and activities of daily living for stroke patient. Methods : The subject was a 44 year old male who suffered from hemiplegic patient received a task-oriented training therapy after botulinum toxin injection on involved upper extremity for 30-min sessions 5 times every week during 8 weeks. The scores were obtained before the injection, at 4 weeks, and 8 weeks after the injection. The upper extremity function was assessed using a Manual Function Test and Box and Block Test, and also Modified Barthel Index was applied for evaluation of Activities of Daily Living performance ability. Results : The results of this study show that the subject's In-hand manipulation and dexterity function increase significantly. Also, In activities of daily living, personal hygiene area, feeding area, toilet area, dressing area scores are improved significantly. Conclusion : Through this study, task-oriented training after botulinum toxin injection can improve upper extremity function and Activities of Daily Living performance ability of patient with stroke.

  • Objective : This study is conducted to find the influence on upper extremity function, cognitive function and activities of daily living when stroke patients receive task-oriented training in group or individually. Methods : Twenty-six inpatients are assigned to two groups(task training group and individual training group) randomly, who receive rehabilitation therapy after stroke diagnosis for 5 months(june to november, 2012) in a hospital. Both groups receive a task-oriented training for 30 minutes a day for 3 weeks. FMA were used to measure upper extremity function, K-MMSE were used to measure cognition, and MBI for ADL. Results : Before training. two groups were not different significantly in upper extremity function and cognitive function. But in activities of daily living, bathing self(p<.001), feeding, personal hygiene and total score(p<.05) are higher in group training group. After training, upper extremity function is higher in individual training group(p<.001). In both training group, upper extremity function, feeding, personal hygiene, bathing self, dressing, toilet, chair/bed transfers, ambulation and stair climbing, total score are improved significantly(p<.05). In comparing of variation before and after training, upper extremity function(p<.001), feeding and total score of activities of daily living are more improved significantly in individual training group(p<.05). Conclusion : The outcome shows that task-oriented training can improve upper extremity function and activities of daily living in both training group. Especially, the more upper extremity function is improved, the more activities of daily living is improved. In the future, it will be necessary longitudinal study for a long time for more patients.

A Study on the Activities of Daily Living, Self-efficacy and the Health Promoting Behavior in Stroke Patients (뇌졸중 환자의 일상생활동작 수행능력, 자기효능감 및 건강증진행위에 관한 연구)

  • Purpose: The purpose of this study to analyze the relationship the activities of daily living(ADL), self-efficacy, and health promoting behaviors. Method: The research was a descriptive survey study. The subjects of this study were 115 hospitalized stroke patients to two General hospitals and one Oriental treatment hospital in G. Metropolitan. Data was collected through questionnaires from December 1st, 2003 to January 30th, 2004. The survey instruments used in the study Shah, Vanclay and Cooper's MBI(11 items), and Sherer and Maddux's self-efficacy(10 items), Walker, Sechrist, and Pender's HPLP(23 items) was developed by the researcher. The obtain data were analyzed with an SPSS 10.0 program using frequency, percentage, Cronbach's alpha, t-test, ANOVA, Duncan test, and Pearson's correlation coefficient. Result: The results were as follows; The level of ADL the score was 50.5, self-efficacy 40.4, and health promoting behavior 54.6. There were significant differences of ADL by general and disease related characteristics as follows : for monthly income(F=2.72, p= .048), duration of stroke(F=7.98, p= .001), number of attack(F=5.21, p= .007), operation (F=17.88, p= .000). There were significant differences of self-efficacy by general and disease related characteristics as follows : for monthly income(F=4.83, p= .003), number of attack (F= 6.20, p=. 003), operation(F=4.03, p= .047). There were significant differences of health promoti ng behaviors by general and disease-related characteristics as follows : for aphasia(F=4.24, p=.042). There were significant correlated between ADL and self-efficacy(r= .698, p=.000), self- efficacy and health promoting behavior(r=.398, p= .000), ADL and health promoting behavior(r= .235, p= .011). Conclusion: As a result, There was a significant correlation between ADL, self-efficacy and health promoting behaviors. There will be considered useful nursing intervention effect to progress, support health promoting behavior of stroke patients.

Application of Sociodrama for Reducing Burnout of Nurses in General Hospital (종합병원 간호사의 소진개선을 위한 소시오드라마의 활용)

  • Objectives : The aim of this study was to evaluate the effect of sociodrama for reducing burnout among health care workers and to investigate relationship between the job satisfaction, conflict with other professions, and burnout. Methods:15 nurses in general hospital participated in sociodrama directed by psychiatrist. One week after acting sociodrama, they were requested to complete ward organizational feature scale(WOFS) and maslach burnout inventory(MBI). The other twenty nurses in same ward requested to complete questionnaire without participating in sociodrama at the same time. Results : Sociodrama group(N=15) showed significantly lower score in burnout when compared to control group(N=17). Nurse/physician relationship was significantly correlated with job satisfaction and burnout. After controlling for interaction between variables by multivariate regression analysis, only job satisfaction was significant predictive factor to burnout. Conclusion : Our result suggests that the sociodrama can be effective in reducing burnout of health care workers in general hospital. These finding also underline the importance of relationship with physicians in job satisfaction and burnout of healthcare workers in general hospital.

Need Survey of Home Modification on Outpatient with Stroke and Spinal Cord Injury in Rehabilitation Center (재활치료를 받는 뇌졸중과 척추손상 외래환자들의 주거환경개선에 대한 요구도 조사)

  • The purpose of this study was to investigate the need of home modification for outpatient with stroke and spinal cord injury. The subjects of this study were 54 outpatients who have experience in rehabilitation therapy at hospitals in Seoul and Gyeonggi-do. A questionnaire was distributed and collected form January 2th to February 4th, 2012. Descriptive statistics and $x^2$-test were used to analyze data. There were statistically significant difference in stairs of access, in width of front door, in bathroom, space of room according diagnosis. There were statistically significant difference in light of sensor, tissue holder and socket cover in bathroom for the comparison of demands for home modification by living type. There were statistically significant difference in width and threshold of front door, tissue holder and towel bar, nonslip floor in bathroom, space and table, chair of kitchen, space and threshold of porch, light for the comparison of demands for home modification according score of MBI. Based on the results of the survey, needs of home modification is different according disability level, diagnosis, living type, ADL performance. The results of this study show a need for the further occupational therapy of home modification and supporting policies in home modification.

Effectiveness of Motion-Based Virtual Reality Training(Joystim) on Cognitive Function and Activities of Daily Living in Patients with Stroke (체감형 가상현실 훈련(Joystim)이 뇌졸중 환자의 인지기능과 일상생활활동에 미치는 효과)

  • The present study was to investigated the effects of motion-based virtual reality training on cognitive function and activities of daily living in patients with stroke. This study was participated in forty one patients with acute stroke. All subjects were randomly assigned into either the experimental group(n=20) or the control group(n=21). The both groups received the conventional occupational therapy during 30 min a day, 5 a week, 4 a weeks. Additionally, experimental group performed motion-based virtual reality training on each session during 30 min/day, and control group conducted conventional occupational therapy on each session during 30 min/day. The outcome measures were the LOTCA(Loewenstein Occupational Therapy Cognitive Assessment), TMT(Trail Making Test), VM(Visual Memory), K-MBI(Korean Modified Barthel Index). In comparison of change score of cognitive function, the experimental group showed a significantly greater improvements in attention and visual working memory than the control group(p<.05). As a result for change score of activities of daily living, the experimental group showed a significant greater improvement in self care than the control group(p<.05). Our findings suggest that motion-based virtual reality training may have a effects of the improvements of cognitive function and activities of daily living than conventional therapy in patients with acute stroke.
