Deficiency or activation of peroxisome proliferator-activated receptor α reduces the tissue concentrations of endogenously synthesized docosahexaenoic acid in C57BL/6J mice

  • BACKGROUND/OBJECTIVES: Docosahexaenoic acid (DHA), an n-3 long chain polyunsaturated fatty acid (LCPUFA), is acquired by dietary intake or the in vivo conversion of ${\alpha}$-linolenic acid. Many enzymes participating in LCPUFA synthesis are regulated by peroxisome proliferator-activated receptor alpha ($PPAR{\alpha}$). Therefore, it was hypothesized that the tissue accretion of endogenously synthesized DHA could be modified by $PPAR{\alpha}$. MATERIALS/METHODS: The tissue DHA concentrations and mRNA levels of genes participating in DHA biosynthesis were compared among $PPAR{\alpha}$ homozygous (KO), heterozygous (HZ), and wild type (WT) mice (Exp I), and between WT mice treated with clofibrate ($PPAR{\alpha}$ agonist) or those not treated (Exp II). In ExpII, the expression levels of the proteins associated with DHA function in the brain cortex and retina were also measured. An n3-PUFA depleted/replenished regimen was applied to mitigate the confounding effects of maternal DHA. RESULTS: $PPAR{\alpha}$ ablation reduced the hepatic Acox, Fads1, and Fads2 mRNA levels, as well as the DHA concentration in the liver, but not in the brain cortex. In contrast, $PPAR{\alpha}$ activation increased hepatic Acox, Fads1, Fads2, and Elovl5 mRNA levels, but reduced the DHA concentrations in the liver, retina, and phospholipid of brain cortex, and decreased mRNA and protein levels of the brain-derived neurotrophic factor in brain cortex. CONCLUSIONS: LCPUFA enzyme expression was altered by $PPAR{\alpha}$. Either $PPAR{\alpha}$ deficiency or activation-decreased tissue DHA concentration is a stimulus for further studies to determine the functional significance.

  • Objective: The purpose of this study was to compare the effects of exercise program combined with spaced retrieval and exercise program to show the effects on elderly people with dementia by presenting them to clinics and welfare facilities such as long-term care facilities. Method: This study was conducted in 20 elderly patients with dementia and randomly assigned to exercise program combined with spaced retrieval and exercise program. After screening the subjects for compliance with the criteria, Before starting the experiment, activites of daily living, depression, and nitive evaluation were performed. After 8 weeks, 3 times per week, 40 minutes per intervention, and 4 and 8 weeks, respectively K-MBI, GDSSF-K and MMSE-K were used to evaluate the differences between the experimental and control groups. Results: There was no statistically significant difference in the daily activities, depression, and cognitive scores between the groups of exercise program combined with spaced retrieval and exercise program group. However, there was a significant difference between the two groups after training (p<.05). Particularly, there was statistically significant difference in post-training cognitive evaluation (MMSE-K) only in the exercise program combined with spaced retrieval group (p<.05) Conclusion: This study suggests that exercise program combined with spaced retrieval is more effective in improving cognitive ability. This suggests that the exercise program combined with spaced retrieval is more effective.

Jeonbuk Tri-Pull Taping과 고유수용성신경근촉진법 운동이 뇌졸중 환자의 어깨관절 가동범위, 통증, 아탈구, 팔 기능 및 일상생활수행능력에 미치는 영향 -사례연구- (Effect of Jeonbuk Tri-Pull Taping and Proprioceptive Neuromuscular Facilitation Exercise on Shoulder Active Range of Motion, Pain, Subluxation, Upper Extremity Function and Activities of Daily Living in Patients with Stroke -A Case Study-)

  • Purpose: This study aims to determine the effect of Jeonbuk tri-pull taping and proprioceptive neuromuscular facilitation (PNF) exercise on the shoulder's active range of motion, pain, subluxation, upper extremity function, and activities of daily living in patients with stroke. Methods: In this study, Jeonbuk tri-pull taping and PNF exercise were applied to three patients with stroke and subluxation. The tape was removed and new tape applied for two days every Monday, Wednesday, and Friday over six consecutive weeks. PNF exercise was applied five times a week for six weeks. To measure the range of motion, a smart phone clinometer application was used, and the degree of pain was measured using a visual analogue scale (VAS). A jig measuring method was employed to measure the distance of subluxation. The Fugl-Meyer Assessment (FMA) was used to evaluate arm function, and the modified Barthel Index (MBI) was employed to evaluate the activities of daily living. Results: The shoulder's active range of motion was improved in the patients compared to the range of pre-tests, and the pain and subluxation distance were reduced compared to those of pre-tests. Arm function and activities of daily living were increased compared to those of pre-tests. Conclusion: The study results verified that Jeonbuk tri-pull taping and PNF exercise are useful when applied to patients with subluxation and stroke.

30-40대 여성의 미병상태에 따른 월경양상 비교연구 (A Study on comparison of menstrual pattern according to Mibyeong Index of Korean women in their thirties and forties)

  • Introduction : The aim of this study was to investigate menstrual pattern according to Mibyeong Index(MBI) and compare Mibyeong symptom score according to severity of menstrual pain. Mibyeong Index was designed to measure inconvenience and resilience of subject's complains including four physical symptoms(fatigue, pain, low sleep quality, indigestion) and mental distress including anxiety, anger, depression. Method : We used the clinical data of Korean medicine Date Center(KDC) for subjects who participated in the study of 'Clinical research for collecting of clinical cases based on the personal type of Mibyeng' conducted in Seoul, from June 26, 2015 to June 26, 2017. A total of 566 fertile women aged 30 to 50 who completed the questionnaires were included in this study. In this study, we used items of Mibyeong Index and the menstrual pattern by self-report questionnaires. The date were analyzed through Kruskal Wallis test, Pearson's chi-square test, and one-way ANOVA using SPSS statistics 19.0. Results : Mibyeong status was significantly associated with severity of menstrual pain(p<0.001), worst day of menstrual pain(p<0.05), and type of menstrual pain(low back pain, nausea, depression, and none (respectively, p<0.05)). In addition, The 7-subtype score (fatigue (p<0.001), pain(p<0.001), low sleep quality(p<0.001), indigestion(p<0.001), anxiety(p<0.05), anger(p<0.05) and depression(p<0.05)) of Mibyeong index and total score of Mibyeong Index (p<0.001) showed significant difference for severity of menstrual pain. Conclusion : This result indicates that a close relationship between Mibyeong status and severity of menstrual pain. Further studies are needed but, we hope that this results will be used as the basic data to improve mibyeong status through health care to alleviate dysmenorrhea.

사지위약감과 안면마비를 주소로 하는 길랑바레 증후군 환자의 한방치료 증례보고 1례 (A Case Report of a Patient with Guillain-Barre Syndrome Complaining of Limb Weakness and Facial Paralysis That Improved After Korean Medicine Treatment)

  • Objectives: This study reports a case of Guillain-Barre Syndrome (GBS) in which the patient experienced improved limb weakness, facial paralysis, paresthesia, and systemic pain after Korean medicine treatment. Methods: A 25-year-old female patient diagnosed with GBS received the herbal medicine Banhasasim-tang Soft Ext., acupuncture, electroacupuncture, moxibustion, cupping, and rehabilitation treatment. To confirm the change in symptoms, the manual muscle test (MMT), Korean Version of Modified Barthel Index (K-MBI), Yanagihara grading system (Y-score) and Numeric Rating Scale (NRS) were performed. Results: After Korean medicine treatment, there was a significant improvement in GBS-related clinical symptoms. Conclusions: Korean medicine treatment could be effective in improving symptoms of limb weakness, facial paralysis, paresthesia, and systemic pain related to GBS. However, this study has limitations as a case report, and more studies are needed.

Effects of Group Rehabilitation Gymnastics for Stroke Patients

  • Objective: This study is an experimental study to investigate the effect of group rehabilitation gymnastics on stroke patients. Design: A randomized controlled trial. Methods: From August 1, 2017, to February 30, 2018, 49 subjects hospitalized after being diagnosed with a stroke at K Rehabilitation Hospital had at least 8 weeks (more than 3 times a week) in the group rehabilitation exercise experimental group (25 patients), and the control group (24 patients) did not attend. Data analysis was performed using paired t-test for dependent variables before and after the experiment using SPSS 25. Results: In the experimental group, daily activities were statistically significantly increased from 59.04±25.19 points before treatment to 66.96±24.35 points after treatment (t=2.24, p=0.035). Lung capacity also significantly increased from 280.00±86.99 points to 334.40±93.23 points (t=4.21, p<0.001), and hospital reuse intention also significantly increased from 8.04±1.57 points to 8.88±1.16 points (t=2.67, p=0.013). In the case of the control group, although the MBI, lung capacity, stress, hospital reuse intention, and hospital use satisfaction increased after the experiment compared to before the experiment, there was no significant difference. Conclusions: In conclusion, group rehabilitation gymnastics for stroke patients is effective for daily living movements, lung capacity, and hospital re-use intention. Therefore, if it is continuously applied to stroke patients, it can be used as an intervention to improve the quality of nursing by enhancing physical function.

Participation and Activities of Daily Living, Cognition, And Motor Function According to Residential Area in Stroke Survivors

  • PURPOSE: Social participation is essential for stroke survivors. Although participation restrictions are affected by several factors, a few studies have focused on the effects of living in a particular residential area (urban versus rural) on the participation restrictions after a stroke. This study examined the factors affecting participation restrictions in stroke survivors according to the residential area. METHODS: One hundred sixty-six stroke patients (including 130 living in urban areas and 36 living in rural areas) were recruited for this study. The Korean Modified Barthel Index (K-MBI) was used to assess the activities of daily living (ADL). The Korean Mini-Mental Examination Status (MMSE-K) was used to assess cognition. The motor function was evaluated using the Fugl-Meyer function assessment (FMA). Path analysis was used to test the hypothesized model of participation restriction. RESULTS: The proposed path model showed good fit indices. In rural and urban areas, the direct effects were significant between the ADL and participation restrictions (β = -.673 and -.457, respectively). For urban areas, the direct effects were significant between cognition and participation restrictions (β = -.252). In both area types, motor function and cognition had a significant direct effect on the ADL. CONCLUSION: For urban community-dwelling stroke survivors, the ADL and cognition had direct effects on participation restrictions. For rural area stroke survivors, the ADL had direct effects on participation restrictions. Activity level exercise programs help reduce the participation restrictions. Moreover, it is essential to address cognition training to improve participation in urban community-dwelling stroke survivors.

인지기반 작업수행 중재가 아급성기 뇌졸중 환자의 팔 기능 및 작업수행에 미치는 영향 (The Effect of Cognitive Orientation to Daily Occupational Performance(CO-OP) on Upper Extremity Function and Occupational Performance in patients with Subacute Stroke)

  • PURPOSE: The purpose of this study was to investigate the effects of cognitive orientation to daily occupational performance (CO-OP) interventions on the upper extremity function and occupational performance in subacute stroke patients. METHODS: A total of 30 eligible stroke patients were randomly distributed into the CO-OP experimental group (n = 15) and the traditional occupational therapy (OT) control group (n = 15). The CO-OP approach was applied for twelve 60-minute sessions. The Jebson Taylor Hand Function Test (JTHFT), the Korean version of the Modified Barthel Index (K-MBI), and the Performance Quality Rating Scale (PQRS) were used for outcome evaluations. RESULTS: There was a statistically significant difference in the paralytic upper extremity function before and after treatment (p < .01) between the group treated with the CO-OP approach and the group treated with traditional OT. The occupational performance of the CO-OP approach group showed a significant improvement (p < .01), and this group also showed a statistically significant improvement in quality of performance compared to the traditional OT group (p < .01). CONCLUSION: The CO-OP approach may be useful in clinical practice as a therapeutic intervention for improving the functional recovery and performance of subacute stroke patients. Future research should be carried out to investigate the persistence of this effect involving more participants and follow-up studies should also be undertaken.

하지 강직으로 인한 보행장애를 동반한 특발성 파킨슨병 환자에 대한 두침 및 추나 치료 임상증례 보고 (A Case Report of Scalp Acupuncture and Chuna Manual Therapy for a Patient with Idiopathic Parkinson's Disease with Walking Disorders due to Lower Extremity Rigidity)

  • The purpose of this study is to report the case of a patient with idiopathic Parkinson's disease treated with scalp acupuncture and Chuna manual therapy. A total of 10 sessions of scalp acupuncture and Chuna manual therapy were performed. At the beginning of the treatment, the Korean version of modified Barthel index (K-MBI) was 25 points, and the range of motion (ROM) of both knee joints was 150° in passive flexion, while active movement was impossible. In addition, both knee joints were flexed and contracted at -40° on the right and -30° on the left. Over the course of treatment, the ROM of both knee joints improved to -20° on the right and -10° on the left. The patient was even able to maintain a standing position and was allowed to walk approximately 10 steps with support. Temporary improvement was observed as a result of applying scalp acupuncture treatment and Chuna manual therapy to a patient whose symptoms were gradually worsening.

일상생활활동 수행능력에 따른 뇌졸중 환자의 신체 및 인지기능의 변화 (Changes in physical and cognitive functions according to the activities of daily living performance in stroke patients)

  • Background: The purpose of this study is to investigate changes in physical and cognitive function according to the level of independence in performing activities of daily living in stroke patients. Design: Retrospective study. Method: This study is a retrospective study analyzing medical records. This study utilized data collected from 123 stroke patients at admission in a local rehabilitation hospital between 2019 and 2022. Stroke patients were classified into 5 groups based on the scores of the Korean Modified Barthel index (K-MBI) evaluated at the time of hospitalization at a rehabilitation hospital, and investigated the change in physical (spasticity (modified Ashworth scale), muscle strength (manual muscle test), gait ability (functional ambulation category), upper extremity function (manual function test), and balance function (berg balance scale)) and cognitive function (Korean mini mental status examination) according to the level of independence in performing activities of daily living. Result:: As a results, significant differences were observed in the physical (muscle strength, gait ability, upper extremity and balance functions) and cognitive functions of stroke patients according to the level of independence in performing activities of daily living (p<0.05). However, there was no significant difference in upper and lower extremities spasticity. Conclusions: Through the results of this study, we found that the physical (muscle strength, gait ability, upper extremity and balance functions) and cognitive function were influenced by the level of independence in performing activities of daily living in stroke patients.
