검체채취실의 고객만족도 향상 (Improvement for the Degree of Client Satisfaction in the Sample Collection Room)

  • The sample collection room(SCR) will have much more influence than all the other departments for the improvement of hospital image, if anyone coming to the SCR in the hospital goes back with the perfect complacency and because most clients who have much stresses and fatigues pay a final visit to the SCR via receipt-diagnosis- acceptance process. SCR has improved its image for the purpose of gratifying clients, in order for clients to visit the hospital again, the quality improvement(QI) team in the Diagnosis Inspection Medical Department has come to a conclusion as follows. The degree of client gratification before improvement marks 65.9 point, but the degree after improvement was 74.2 point. Therefore, satisfaction has increased by 8.3 points. The degree of client gratification in groups before improvement marks (1) service parts-89.2 points (2) facilities and environments-49.1 point (3) toilet facilities-46.3 point. But its gratification after improvement marks (1) 92.5 point (2) 60.1 point (3) 61.0 point. Therefore the degree of satisfaction has increased by (1) 3.3 point, (2) 11.0 point, (3) 14.7 point. The progress of facility improvement plans and the exclusion of improvement on the facility contents in the hospital have made facilities and environments of SCR and toilet facilities to be poorly improved. Although service parts have a good mark, and the facilities and environments are not scoring well, the whole degree clients' gratification of SCR couldn't be helped by the low grade. Therefore the bottom line for the clients' gratification of SCR in the future is to ameliorate the facilities and environments. SCR will take the clients' gratification survey every year and if any items get low marks, that is, below 90 point throughout the survey, SCR will immediately starts the improvement work for the clients' gratification with operating the programs of controlling quality continually, and SCR should induce the operation of services, participating in the kind campaign drive for clients. So SCR will adopt the incentive system for the best staff members who perform these kinds of services.

호스피스 완화의료 전문인력의 죽음에 대한 태도가 임종돌봄 스트레스에 미치는 영향 (The Effects of Attitude to Death in the Hospice and Palliative Professionals on Their Terminal Care Stress)

  • 목적: 본 연구는 호스피스 완화의료 전문인력의 죽음에 대한 태도의 임종 돌봄 스트레스에 대한 효과를 탐구하고, 이 두 변수와 관련된 우울, 대처전략들의 변수와의 상관관계를 분석하기 위해 시도되었다. 방법: 연구 대상자는 호스피스 완화의료 전문인력 131명이며, J도를 중심으로 2개의 상급종합병원과 2개의 종합병원 암병동, 2개의 호스피스 시설, 2개의 전문요양병원 및 2개의 노인병원에서 실시되었다. 자료는 2015년 3월부터 6월에 수집되었으며, 자료 분석에는 SPSS/WIN 21.0과 AMOS 18.0 programs을 사용하였고, t-test, factor analysis, ANOVA ($Scheff{\acute{e}}$), Pearson's correlation 및 path analysis를 실시하였다. 결과: 죽음에 대한 태도는 낮았으며(2.63점), 우울 점수는 0.45점이였으나 15.0%의 대상자는 우울관리가 요구되었다. 임종 돌봄 스트레스가 높고(3.82점), 그 중 의료 한계에 대한 갈등이 가장 높았다(4.04점). 스트레스 대처는 낮았으며(3.13점), 대인관계 기피(4.03점), 간식이나 잠을 취하는 기본욕구 충족(3.65점)과 같은 소극적인 방법을 사용하였다. 죽음에 대한 태도는 임종 돌봄 스트레스에 직접적으로 부적 영향을 주었으며, 우울과 기본 욕구 충족(CS2)을 통한 간접적인 영향을 주었다. 결론: 호스피스 완화의료 전문인력들에게 죽음에 대한 태도를 향상시키고 효과적인 대처전략을 활용하게 하는 인지적 지지와 정서적 지지를 제공하는 프로그램 제공이 요구된다.

Studies on the Preparation of Organic Compounds Labelled by $^{38}Cl$.(I) - Inorganic Yields of $^{38}$ Cl in Szilard Chalmer Reaction of Aromatic Chloro Derivatives

  • 방사성 $^{38}$ CL로 표지된 유기 하로겐 화합물의 제조방법을 가려내기 위하여서 Phenyl halides(7종), thloro nitrobenzene(6종), Chloro nitroanisole(2종), Chloro aniline (3종), Chloro anisidine(3종), Chloro toluene(3종), Benzyl chloride 유도체(4종) 및 기타 대조 화합물 3종 등을 원자로 내에서 조사하고 생성물을 알카리 수용액으로 주출하고 각각의 무기 $^{38}$ CL 수율을 조사시간 별로 검토하였다. 그 결과 화합물의 구조 보다도 표적물의 상태에 따라서 무기물 수율에 변화가 있고 고체상의 표적물의 경우에는 수율이 일반적으로 얕고 변화가 고르다는 것을 화인 하였다. 화학구조에 따르는 무기 $^{38}$ CL0의 생성수율의 감소는 현재까지의 결과로 보아서 Phenyl chlorides$^{38}$ CL 수율간에 Linear relationship가 있었으며 염소원자수가 많을수록 그 수율이 얕았다. 실험 결과를 논의하였으며 교지 반응에 대한 본 실험 결과의 응용성가능 여부를 고찰하였다.

울진 봉평리 신라비의 재판독과 보존과학적 진단 (Reconsideration and Conservational Scientific Diagnosis of Silla Stone Monument in Bongpyeong-ri, Uljin)

  • 이 연구에서는 울진 봉평리 신라비를 대상으로 비문에 대한 인문학적 조사와 손상도에 대한 보존과학적 진단을 융합하여 역사적 가치의 재인식과 원형유지를 위한 보존방안을 수립하였다. 먼저 봉평비의 비문을 재판독한 결과, I행 마지막 글자는 오(五)자로 재확인하였으며, 선행연구와 다르게 판독된 글자는 모두 13자로 조사되어 이를 통해 기존 판독문의 의미와 해석이 일부 달라졌다. 봉평비의 구성암석은 편마상 우백질화강암으로 이 암석의 산지로 가장 유력한 지점은 색, 광물의 입도와 조성, 편마상 구조, 자화강도 등이 거의 유사한 죽변 해변 일대(2.1km)로 판명되었다. 봉평비는 편마상 구조를 따라 구조상(균열지수: 0.4) 및 미세 균열(균열지수: 2.0)이 발달하여 수평방향의 물성이 수직방향에 비해 취약하므로 균열계의 보강과 처리가 필요하다. 그러나 초음파 측정에 따른 전체 풍화등급은 3등급(3,404m/s, 0.32)의 중간 풍화단계를 나타내는 것으로 보아 강화처리가 시급한 단계는 아니다. 또한 화학적 손상은 토양, 철산화물 및 탁본흔적에 의한 흑화현상(85.2%)과 바다로부터 전이된 NaCl 염결정(17.3%)이 주요 문제점으로 나타났다. 따라서 오염물과 염결정은 일차적으로 증류수 압력 분무법과 펄프종이를 이용하여 제거하고, 임상실험을 통해 습포제 적용을 검토해야 한다.

PRESENT DAY EOPS AND SAMG - WHERE DO WE GO FROM HERE?

  • The Fukushima-Daiichi accident shook the world, as a well-known plant design, the General Electric BWR Mark I, was heavily damaged in the tsunami, which followed the Great Japanese Earthquake of 11 March 2011. Plant safety functions were lost and, as both AC and DC failed, manoeuvrability of the plants at the site virtually came to a full stop. The traditional system of Emergency Operating Procedures (EOPs) and Severe Accident Management Guidelines (SAMG) failed to protect core and containment, and severe core damage resulted, followed by devastating hydrogen explosions and, finally, considerable radioactive releases. The root cause may not only have been that the design against tsunamis was incorrect, but that the defence against accidents in most power plants is based on traditional assumptions, such as Large Break LOCA as the limiting event, whereas there is no engineered design against severe accidents in most plants. Accidents beyond the licensed design basis have hardly been considered in the various designs, and if they were included, they often were not classified for their safety role, as most system safety classifications considered only design basis accidents. It is, hence, time to again consider the Design Basis Accident, and ask ourselves whether the time has not come to consider engineered safety functions to mitigate core damage accidents. Associated is a proper classification of those systems that do the job. Also associated are safety criteria, which so far are only related to 'public health and safety'; in reality, nuclear accidents cause few casualties, but create immense economical and societal effects-for which there are no criteria to be met. Severe accidents create an environment far surpassing the imagination of those who developed EOPs and SAMG, most of which was developed after Three Mile Island - an accident where all was still in place, except the insight in the event was lost. It requires fundamental changes in our present safety approach and safety thinking and, hence, also in our EOPs and SAMG, in order to prevent future 'Fukushimas'.

  • 섬강에 서식하는 꾸구리의 서식개체수를 추정하기 위하여 2010년 9월부터 10월까지 강원도 원주시 섬강 일대에서 조사를 실시하였다. 꾸구리가 출현한 곳은 14개 지점이었으며, 자갈과 돌이 깔린 하류의 여울부에서 주로 서식하고 있었다. 출현개체수가 가장 많고 서식지가 안정된 두 개 지점에서 표지-재포획법과 방형구(족대조사와 수중관찰)를 이용하여 서식개체수를 추정한 결과 표지-재포획법으로 $8,295{\pm}4,922$개체와 $39,846{\pm}14,232$개체로 각각 추정되었고, 방형구의 족대조사는 9,216개체와 37,873개체로, 수중관찰은 5,593개체와 24,347개체로 추정되었다.

  • The GMT-Consortium Large Earth Finder (G-CLEF) is a fiber-fed, optical band high dispersion echelle spectrograph that selected as the first light instrument for the Giant Magellan Telescope (GMT). This G-CLEF has been designed to be a general- purpose echelle spectrograph with the precisional radial velocity (PRV) capability of 10 cm/sec as a goal. The preliminary design review (PDR) was held on April 8 to 10, 2015 and the scientific observations will be started in 2022 with four mirrors installed on GMT. We have been participating in this preliminary design study in flexure control camera (slit monitoring system), calibration lamp sources, dichroic assembly and the fabrication of the proto-Mangin Mirror. We present the design concept on the parts KASI undertaken, introducing the specifications and capabilities of G-CLEF.

일본의 금융분야 ADR 에 관한 검토 (On the Japanese New Alternative Dispute Resolution System in the Financial Sector)

  • In the past, ADR has not been used as frequently in Japan as it has in other parts of the industrialized world. However, though litigation is still the most utilized vehicle of dispute resolution by Japanese financial institutions, this will be changing. The New Financial ADR system, which was created by a June 2009 amendment to the Financial Instruments and Exchange Act, is meant to deal with every stage of financial-related disputes and, as such, strives to resolve disputes before they become significant and acts to ameliorate any post-ADR issues that may remain, thereby completing the FIEA's purpose to protect investors. Since the foundation of the New Financial ADR system applies to all related industries, new provisions were set out in 16 business related acts, such as the Banking Act, the Insurance Business Act, and FIEA itself. October 2010 will mark the formal introduction of a new system of financial ADR in Japan. New Financial ADR in Japan will be modeled on the Financial Ombudsman Service in the United Kingdom, but will not feature one comprehensive dispute resolution system in which one dispute resolution institution covers all disputes in the financial field. The New Financial ADR system is merely one step towards a foundation of comprehensive financial ADR such as FOS. It must be noted, however, that this all important first step was over seven years in the making, involving a great deal of discussion, debate, and compromise amongst many parts of Japanese government, business, and society. The New Financial ADR system grants participating parties the ability to stop the clock on any statute of limitations which may correspond to any future possible court cases related to the dispute,13 and further grants the ability to suspend related court proceedings while the parties are utilizing the New Financial ADR system. In addition, where financial institutions have not accepted dispute resolution proceedings or have not accepted a special conciliation proposal, the Ministry of Finance may issue an order compelling compliance if it is found that certain actions are necessary to ensure the appropriate operations of a financial institution's business. In Japan, as best practices have not yet been created.

Initial Experience with Epicardial Ultrasound Scanning in Coronary Artery Bypass Grafting

  • Background: The benefits of epicardial ultrasound scanning (EUS) in coronary artery bypass grafting (CABG) have not yet been established. The aim of this study was to evaluate the usefulness of EUS in CABG, including in the assessment of the quality of distal anastomoses, the identification of epicardial target vessels, and the evaluation of any graft issues other than the distal anastomoses. Methods: Fifty-three patients undergoing CABG were enrolled between March 2018 and February 2019. Intraoperative EUS was performed along with transit-time flow measurement (TTFM). Graft evaluations were performed early (shortly after surgery) and 1 year after surgery for 53 (100%) and 47 (88.7%) patients, respectively. Results: EUS was applied to assess the quality of all distal anastomoses, 32 target vessels, and 2 conduit trunks. Insufficient TTFM findings were obtained for 18 grafts. However, graft revision was performed for only 3 distal anastomoses; based on the EUS findings, the remaining 15 sites were not revised. The early and 1-year overall graft patency rates were 100% (141 anastomoses) and 96.1% (122 of 127 anastomoses), respectively. All 15 of the distal anastomoses that were not revised despite insufficient TTFM results were patent at the 1-year mark. Conclusion: The routine application of EUS in CABG could be beneficial by confirming the quality of surgery and reducing unnecessary procedures.

Impact on Retrievability by Cement Variety for Implant Restorations Equipped with a Lingual Slot

  • Purpose: The purpose of this study is to measure and compare the removal torques of different cements applied in attachments of zirconia restorations on titanium (Ti) abutments fitted with retrievable cement-type slot (RCS) on the lingual side for the better retrievablity by use of a slot driver. Materials and Methods: Three types of cements were used in the experiment: two permanent cements in $RelyX^{TM}$ U200 (RU) (3M ESPE) which is a resin cement and $FujiCem^{TM}$ (FC) (GC) which is a resin-modified glass ionomer cement, and a temporary cement in $Freegenol^{TM}$ temporary cement (TC) (GC). Measurements of removal torques were conducted as follows; an attached sample was fixed on the equipment customized for the experiment; a slot driver was connected to a MGT12 (Mark-10 Corp.), a torque measurement instrument; the sample had the driver fitted to its RCS and then was rotated until the it was removed; and finally, the maximum torque value was recorded. Result: As for the removal torque measurement results, the average values were $47.9{\pm}2.6Ncm$ for RU, $43.4{\pm}1.5Ncm$ for FC, and $20.9{\pm}1.0Ncm$ for TC. The statistical analysis using Kruskal-Wallis test yielded the significance probability of P<0.05 (P=0.002), which confirmed the presence of significant differences between the three groups. Conclusion: All three cements exhibit clinically acceptable levels of removal torque when applied to an upper zirconia implant restoration fitted with a lingual slot, with RU and FC, the two permanent cements, having the significantly higher values than that of TC, the temporary cement.
