비소세포폐암에서 Maspin의 발현과 임상적 의의 (Maspin Expression and Its Clinical Significance in Non-Small Cell Lung Cancer)

  • Background: Maspin (mammary serine protease inhibitor) is a member of the serpin superfamily. A few studies have examined the role of maspin in tumor suppression of non-small cell lung cancer (NSCLC); however, its role in the development and progression of NSCLC still remains controversial. We evaluated the immunohistochemical expression of maspin in order to elucidate its clinical significance in NSCLC. Methods: We analyzed 145 patients with pathologically confirmed NSCLC, including 66 cases of squamous cell carcinomas (SCCs) and 79 cases of adenocarcinomas (ADCs). We performed a immuno-histochemical stain with maspin and PCNA (proliferating cell nuclear antigen) using tissue microarray blocks. Results: There were 108 men and 37 women in the study population. The mean age of patients in the study was 63.7 years (range, 40.0~82.0; median, 65.0). The proportion of maspin expression was significantly higher in SCCs (52/66, 78.8%; p<0.01) than in ADCs (17/79, 21.5%; p<0.01). Maspin expression was not associated with PCNA (p=0.828), lymph node involvement (p=0.483), or tumor stage (p=0.216), but showed correlation with well-to-moderate tumor differentiation (p=0.012). There was no observed correlation between maspin expression and survival with NSCLC (p=0.218). Conclusion: The present study suggests that maspin expression was significantly higher in SCCs than in ADCs and was associated with low histological grade. However, maspin expression was not an independent factor to predict a prognosis in NSCLC.

Analysis of CEA Expression and EGFR Mutation Status in Non-small Cell Lung Cancers

  • Background: The serum carcinoembryonic antigen (CEA) level can reflect tumor growth, recurrence and metastasis. It has been reported that epidermal growth factor receptor (EGFR) mutations in exons 19 and 21may have an important relationship with tumor cell sensitivity to EGFR-TKI therapy. In this study, we investigated the clinical value of EGFR mutations and serum CEA in patients with non-small cell lung cancer (NSCLC). Materials and Methods: The presence of mutations in EGFR exons 19 and 21 in the tissue samples of 315 patients with NSCLC was detected with real-time fluorescent PCR technology, while the serum CEA level in cases who had not yet undergone surgery, radiotherapy, chemotherapy and targeted therapy were assessed by electrochemical luminescence. Results: The mutation rates in EGFR exons 19 and 21 were 23.2% and 14.9%, respectively, with the two combined in 3.81%. Measured prior to the start of surgery, radiotherapy, chemotherapy and targeted treatment, serum CEA levels were abnormally high in 54.3% of the patients. In those with a serum CEA level <5 ng/mL, the EGFR mutation rate was 18.8%, while with 5~19 ng/mL and ${\geq}20ng/mL$, the rates were 36.4% and 62.5%. In addition, in the cohort of patients with the CEA level being 20~49 ng/mL, the EGFR mutation rate was 85.7%, while in those with the CEA level ${\geq}50ng/mL$, the EGFR mutation rate was only 20.0%, approximately the same as in cases with the CEA level<5 ng/mL. Conclusions: There is a positive correlation between serum CEA expression level and EGFR mutation status in NSCLC patients, namely the EGFR mutation-positive rate increases as the serum CEA expression level rises within a certain range (${\geq}20ng/mL$, especially 20~49 ng/mL). If patient samples are not suitable for EGFR mutation testing, or cannot be obtained at all, testing serum CEA levels might be a simple and easy screening method. Hence, for the NSCLC patients with high serum CEA level (${\geq}20ng/mL$, especially 20~49 ng/mL), it is worthy of attempting EGFR-TKI treatment, which may achieve better clinical efficacy and quality of life.

흰쥐규폐증에서 기관지 폐포 세척액내 세포변화 (Changes of the Cellularities in the Bronchoalveolar Lavage Fluid of the Experimental Silicosis)

  • Pneumconiosis is a sort of pulmonary fibrosis consequent to the inhalation of the respirable dusts. Thus, the pathogenesis of silicosis have concentrated largely on the early response of alveolar macrophage and the later fibroblastic stimulation. But the role of the other cells and continuing cell injury in the pathogenesis has not been fully studied. And the chemical factors such as prostaglandin, fibroblast stimulating factor and inhibiting factor and chemotaxin are also participated in the mechanism of pulmonary fibrosis in silicosis. In order to clarify the role of alveolar cells and prostaglandin, we investigated the changes of the cellularities in bronchoalveolar lavage fluid and tissue pathology in the experimental silicosis with the time sequence. The experimental animals were divided into 3 groups; control group received only intratracheal injection of 0.5 ml saline, silica group received the intratracheal instillation of 40 mg silica with the same amount saline, and aspirin group received 450 mg/kg of aspirin after silica instillation. The results were as follows: 1) The total cells of bronchoalveolar lavage fluid in the silica group markedly increased in comparison with the control group, but there was no significant difference between the silica and aspirin groups. 2) The percentages of alveolar macrophages to the total number of cells in the silica group tended to be lower than those in the control group and also lower than those in the aspirin group at the 1st week after silica instillation. 3) The percentages of neutrophils to the total number of cells in the silica group were significantly higher than those in the control group during the entire period and also higher than those in the aspirin group at the 3rd day after silica instillation. 4) In the silica group, the percentages of lymphocytes to the total number of cells were increased 143 progressively with the time course and those were significantly higher than those in the control group from the 3rd week after silica administration. There were marked differences of lymphocyte percentages between the silica and aspirin groups at the 1st week after silica instillation. 5) The inflammatory change was observed in the rat lung at the 1st day after silica instillation. Also the silicotic nodule appeared in the silica group at the 1st week but we could not find out that nodule in the aspirin group at that time. The fibrotic changes in the rat lung tended to be increased progressively with the time course, therefore, the diffuse fibrotic pattern appeared in the whole field at the 20th week after silica instillation. 6) By the electron microscopy, there were gradual increases of phagosomes and vacuoles in the alveolar macrophage in the silica group as compared with the control group. These results suggest that the neutrophils and the lymphocytes have also participated in the pulmonary fibrosis even though the alveolar macrophage has a major role, and prostaglandin mediate the inflammation and pulmanary fibrosis in the experimental silicosis.

자기장(磁氣場) 및 BEP 조사(照射)가 식이성(食餌性) 고지혈증(高脂血症)이 유발(誘發)된 흰쥐에 미치는 영향(影響)에 대(對)한 연구(硏究) (A Study on the Effects of Magnetic Field and BEP on Hyperlipidemia and Enzymatic Activities in Rats)

  • It has been known that Ki(氣) energy is very effective on many adult diseases. Oriental Medicine has acknowledged Ki as an existing reality and investigated its effects on the body. However, the existence of Ki has not been fully explained. In order to find a conclusive evidence on the existence of Ki, this experiment was done to study the mutual relationship of Ki with a magnetic field and BEP (biological energy projector). The BEP apparatus was irradiated under the magnetic field on rats in the hyperlipidemic induced state. Following criterias were measured in this experiment: weight change, weight of the visceral organs, serum, hepatic lipid peroxide, bleeding time, tissue factor, and etc. The following results were obtained in this study: 1. The weight of rat significantly decreased in the magnetic field treated group and radically reduced in the group treated with both magnetic field and BEP. 2. The weight of liver, heart, and kidney increased in both the magnetic field treated group and magnetic field+BEP group compared to the normal group, but decreased in comparison to the control group. No changes were witnessed in the weight of spleen. 3. Serum and hepatic total cholesterol, total lipid, and lipid peroxide level significantly decreased in both magnetic field treated group and magnetic field+BEP treated group, while lipase activity has increased noticeably. 4. Serum HDL showed a significant increase in both magnetic field treated group and magnetic field+BEP treated group compared to the control group, while LDL and VLDL level decreased significantly. 5. A bleeding time significantly increased in both magnetic field treated group and magnetic field+BEP treated group compared to the control group. A tissue factor value of the lung decreased in the magnetic field treated group and magnetic field+BEP treated groups while increased in the control group. 6. Serum and hepatic lipid peroxide and glutathione level were significantly decreased in the magnetic field treated group and magnetic field+BEP treated group, while hepatic glutathione level was significantly increased compared to the control group. 7. A significant increase was found in the serum hydroxyl radical and SOD activity in the dietary hyperlipidemic rats, and significant decrease was found in the serum lipid peroxide content and superoxidase activity. 8. Hepatic cytosolic enzyme xanthine oxidase and aldehyde oxidase showed a significant decrease in the magnetic field treated group and magnetic field+BEP treated group. Through the above experimental results, one can suggest that the magnetic field with BEP can suppress hyperlipidemia and boost lipid metabolism and restructuring a lipid in liver, which increases the function of liver. To conclude, BEP is considered to show more potent effects under the exposure of magnetic field because magnetic field seems to increase the flow of Ki in the body.

Reference Gene Screening for Analyzing Gene Expression Across Goat Tissue

  • Real-time quantitative PCR (qRT-PCR) is one of the important methods for investigating the changes in mRNA expression levels in cells and tissues. Selection of the proper reference genes is very important when calibrating the results of real-time quantitative PCR. Studies on the selection of reference genes in goat tissues are limited, despite the economic importance of their meat and dairy products. We used real-time quantitative PCR to detect the expression levels of eight reference gene candidates (18S, TBP, HMBS, YWHAZ, ACTB, HPRT1, GAPDH and EEF1A2) in ten tissues types sourced from Boer goats. The optimal reference gene combination was selected according to the results determined by geNorm, NormFinder and Bestkeeper software packages. The analyses showed that tissue is an important variability factor in genes expression stability. When all tissues were considered, 18S, TBP and HMBS is the optimal reference combination for calibrating quantitative PCR analysis of gene expression from goat tissues. Dividing data set by tissues, ACTB was the most stable in stomach, small intestine and ovary, 18S in heart and spleen, HMBS in uterus and lung, TBP in liver, HPRT1 in kidney and GAPDH in muscle. Overall, this study provided valuable information about the goat reference genes that can be used in order to perform a proper normalisation when relative quantification by qRT-PCR studies is undertaken.

편평상피성 폐암에 있어서 S-Phase Fraction과 생존기간과의 관계 (Relationship between S-Phase Fraction and Survival Time in Patients with Primary Squamous Lung Cancer)

  • 연구배경 : 종양세포는 세포의 분열성장이 매우 왕성하므로 정상 세포에 비하여 세포분열 주기 중 S-Phase fraction(SPF)의 증가를 예상할 수 있다. 같은 종양증에서도 SPF가 높으면 종양의 성장이 훨씬 빠를 것으로 생각되며, 이러한 종양세포의 생물학적 특성을 이용하여 원발성 편평상피성 폐암 환자에서 SPF와 생존기간과의 관계를 연구하였다. 방법 : 1985년 9월 부터 1990년 3월 까지 원광의대 부속병원에서 원발성 편평상피성 폐암으로 진단받고나서, 수술, 방사선, 화학요법 등을 실시받지 않고 사망한 21례를 대상으로 하였다. SPF 분석방법은 paraffin에 보관된 병리조직을 처리하여 유식세포 분석법에 의한 DNA histogram으로서 분석하였다. 결과 : 1) 전 대상군의 평균 생존기간은 225(${\pm}162$)일 이었고, TNM 병기, PS scale이 진행할 수록 단축되는 양상이다. 2) 전 대상군의 평균 SPF는 23.4(${\pm}11.3$)% 이었으며, TNM 병기, PS scale 별로 구분하여 비교한 결과, TNM 병기, PS scale의 진행정도와 SPF는 관계가 없었다. 3) SPF 20%를 기준으로 한, high SPF 군과 low SPF 군의 평균 생존기간은 각각 153(${\pm}99$)일, 342(${\pm}180$)일 로서 유의한 차이가 있었다(p<0.01). high SPF군과 low SPF군을 TNM 병기, PS scale 별로 구분하여 비교하여도 역시 유의하게 high SPF군이 low SPF 군 보다 평균 생존기간이 짧았다. 결론 : 편평상피성 폐암 환자에서 종양세포의 SPF가 증가되어 있으면, 예후가 유의하게 좋지 않으며, SPF는 TNM 병기, PS scale과는 무관한 독립된 예후추정 인자로서의 임상적 이용이 가능하다고 생각된다.

폐암 환자의 정위적방사선 치료 시 이중 에너지를 이용한 치료 방법의 유용성 평가 (Evaluation of useful treatment which uses dual-energy when curing lung-cancer patient with stereotactic body radiation therapy)

  • 목 적 : 종양의 위치에 따라 단일에너지를 이용한 치료계획과 이중에너지를 사용하는 치료계획을 정위적방사선 치료에 적용하여 실제 종양에 부여되는 선량의 변화와 종양과 인접한 부분에 위치하는 정상조직의 선량을 비교하여 임상적 유용성을 평가하고자 한다. 대상 및 방법 : 총 10명의 환자 CT 영상을 획득한 후 각각 단일에너지, 이중 에너지에 대한 체적변조회전치료 기법에 대한 치료 계획을 수립하였다. 종양 측 변화 요인을 분석하기 위하여 조형계수(CI)와 균질성 지수(HI), 최대 선량을 각각 계산했으며, 정상 조직에 대한 선량 분포를 비교하기 위하여 $V_{10}$$V_5$, 종양에 가장 근접한 첫 번째 ~ 네 번째 갈비뼈($1^{st}{\sim}4^{th}$ Rib), 척수(Spinal Cord), 식도(Esophagus)와 기관(Trachea)를 선정하였다. 또한 계획한 선량 분포가 실제 전달되는 정확성을 확인하기 위하여 2차원 이온전리함 배열을 이용하여 선량 측정을 시행하였다. 결 과 : 종양 측 인자의 경우 조형계수와 균질성 지수는 두 에너지를 사용했을 경우가 1에 가까운 값을 나타냈다. 최대 선량의 경우 앞쪽 흉벽은 약 2%, 등쪽 종양의 경우는 동등한 값을 나타냈다. 정상 조직의 경우 앞쪽 흉벽 종양은 인접한 갈비뼈에서 두 에너지를 동시에 사용한 경우 각각 4%, 5% 감소하였고, 기관지의 경우 11%, 17% 감소하였다. 폐의 선량 분포의 경우 $V_{10}$의 경우 1.5%, $V_5$의 경우 1%로 감소함을 나타냈다. 뒤쪽 흉벽의 경우 종양에 인접한 갈비뼈는 두 에너지를 이용한 경우에서 각각 6%, 1%, 4%, 12% 감소하였고, 폐의 선량 분포에서는 $V_{10}$ 3%, $V_5$ 3.1% 감소됨을 나타내었다. 선량 측정의 경우 모든 에너지에서 감마지표 3mm/3%의 결과에 부합하였다. 결 론 : 단일 에너지를 이용한 치료 계획 보다 두 에너지를 동시에 사용하는 경우가 표재성 종양에 대해 보다 유용하게 적용될 수 있을 것으로 사료된다.

자연 기흉 환자의 혈액 내 TGF-beta 1 Ligand 양과 폐 기포 형성과의 연관관계에 대한 연구 (The Correlation between TGF-beta 1 Blood Levels and the Formation of Bullae in Patients with Spontaneous Pneumothorax)

  • 배경: 저자들은 자연 기흉 환자의 폐 기포에서 transforming growth factor-beta 1 receptor II (TGF-${\beta}1$RII)와 transforming growth factor-beta 1 (TGF-${\beta}1$) ligand를 면역조직화학염색법으로 조사하여 상기 유전 물질이 과 발현되어 폐 기포 형성에 관여될 수 있음을 보고한 바 있다. 그러나 TGF-${\beta}1$ ligand는 혈액 내에도 존재하고 있으므로 혈액내의 TGF-${\beta}1$ ligand 양이 폐 기포 조직의 형성에도 관여가 될 수 있는 가능성 여부를 알아보기 위하여 연구를 하였다. 대상 및 방법: 자연 기흉으로 폐 기포 절제술을 실시한 환자 19명에서 폐 기포 조직과 혈액을 채취하였다. 대조군으로 25∼27세의 정상인 5명에서 혈액을 채취하였다. 획득된 폐 기포 조직은 formalin 용액에 고정하였으며 파라핀에 포매하여 $5{\sim}6{\mu}m$ 두께로 블록을 만들었으며 면역조직화학염색방법으로 염색하여 관찰하였다. 채취된 혈액에서 ELISA assay로 혈액내의 TGF-${\beta}1$의 양을 측정하였다. 결과: 19명 중 16명에서 TGF-${\beta}1$에 양성이었으며 10명에서 TGF-${\beta}1$RII에 양성의 소견을 보였다. TGF-${\beta}1$에 양성인 16명 중 9명에서 TGF-${\beta}1$RII에 양성으로 확인되었다. 강하게 염색된 부위는 폐 기포 조직과 정상 폐 조직의 경계선 부위였다. 폐 기포 조직에서 TGF-${\beta}1$과 TGF-${\beta}1$RII가 동시에 양성인 환자 9명의 혈액내의 TGF-${\beta}1$의 양은 $38.36{\pm}16.2ng/mL$이었으며 대조군은 $54.06{\pm}15ng/mL$이었다. 결론: 폐 기포를 갖는 수술 환자 군의 혈액내의 TGF-${\beta}1$의 양이 대조군보다 높지 않은 수치를 보이는 것으로 보아 혈액 내 TGF-${\beta}1$ 양은 폐 기포 형성에 직접적으로 관여될 가능성은 적고, 폐 조직에서 국소적으로 과 발현되는 TGF-${\beta}1$RII and TGF-${\beta}1$ ligand가 폐 기포 형성에 더 많이 관계하는 것으로 예상한다.

지방 육종의 생존율과 예후 인자 (Survival Rate and Prognostic Factors of Liposarcoma)

  • Liposarcoma is second in frequency only to malignant fibous histocytoma among the soft tissue sarcoma. Many different factors which might affect the survival rate of liposarcoma have been reported by many authors. The purpose of this study was to evaluate survival rate of liposarcoma and define the prognostic factors that affected survival rate. The authors analysed retrospectively 17 patients of liposarcoma in extremities from May 1984 to Dec. 1995 who had been treated in department of orthopaedic surgery of Kosin University Medical Center. All cases were resected with marginal or wide margin. There were 9 men and 8 women. The mean age was 48 years. The follow-up period ranged from 15 to 96 months. We compared the prognosis of the patients with several factors; age, sex, surgical staging, size, site, histologic type and treatment modality. At last follow-up, the presence of local recurrence was in 3 cases and the presence of lung metastasis was in 8 cases. The survival rates by Kaplan-Meier product limit method at 2 years and 5 years were 87% and 57% respectively. The statististically significant difference was estimated in histologic type, but was not estimated in age, sex surgical staging, size, site and treatment modality. In conclusion, the histologic type is considered as the most important factor of the prognosis in liposarcoma. Although it was too few patients for the differences to be statistically significant, we consider that surgical staging, site, size, the radio-therapy and chemothrapy in liposarcoma will affect the prognosis.
