PET/CT촬영 시 호흡에 의해 병소의 움직임으로 인한 영상의 왜곡이 발생한다. 본 연구에서는본원에서 보유하고 있는 호흡보정 Plumonary Toolkit을 이용한 폐결절부위를 영상화 함으로써 보정을 하지 않은 영상과 비교하여 SUV값의 변화와 영상의 왜곡을 어느 정도 교정할 수 있는지 실험하였다. 2008년 5월에서 8월까지 폐암을 진단받은 환자 17명을 대상으로 하였다. 실험결과 Max SUV값은 최소 4.08%에서 최대 43.10%까지의 증가율을 보였고 폐결절의 평균 Max SUV값은 6.07에서 7.00로 12.16%로 증가가 되었다. 호흡보정 PET/CT의 경우 영상의 왜곡이 개선되었다. SCC-Adenocarcinoma에서는 호흡보정 전.후에 통계적의로 유의한 수준(P<0.05)을 보였으나 SCC와 Adenocarcinom에 대한 각각의 비교에서는 유의성을 보이지 않아 Cell type과 관계없이 호흡보정에 효과가 있었다. 실험 결과Pulmonary Toolkit을 사용할 경우 표준섭취 계수값과 영상에서의 왜곡이 보정되었다. 따라서 폐암의 진단 및 추적관찰에 도움을 줄 수 있을 것이다.
목적 전산화단층촬영에서 호흡에 의한 폐 용적의 변화가 폐 반고형결절의 크기와 용적에 미치는 영향을 알아보고자 한다. 대상과 방법 총 31명의 환자에서 42개의 반고형결절이 연구에 포함되었다. 먼저 총 폐활량 상태에서 전산단층화촬영을 시행 받은 후, 결절이 포함된 부분만 일호흡용적 상태에서 추가로 촬영하였다. 각각의 반고형결절의 직경과 용적을 측정하였고, 전체 폐 용적은 결절의 중심이 있는 동측 폐의 단면적으로 추정하였다. 동일한 개인 내에서 총 폐활량과 일회 호흡량 간 측정값 변화의 유의성을 통계적으로 평가하였다. 결과 총 폐활량 상태와 비교하였을 때, 일회 호흡용적 상태에서 폐 단면적은 평균 12.7 cm2, 반고형결절의 직경 및 용적은 평균적으로 각각 0.5 mm와 46.4 mm3 감소하였다(p < 0.001). 총 폐활량 상태와 일호흡량 상태 간 폐 면적 변화는 반고형결절의 직경 변화(rho = 0.341; p = 0.027) 및 부피 변화(rho = 0.401; p = 0.014)와 유의한 양의 상관관계를 보였다. 그러나 폐 용적의 변화를 고려하여 보정한 후에는 반고형결절의 평균 직경과 부피가 총 폐활량 상태와 일호흡량 상태 간 유의한 차이가 없었다(각각 p = 0.062, p = 0.124). 결론 전산화단층촬영에서 측정한 반고형결절의 크기와 용적은 동일한 환자 내에서도 촬영 당시의 폐 용적에 유의한 영향을 받는다.

Objective: Bronchogenic carcinoma can mimic or be masked by pulmonary tuberculosis (TB), and the aim of this study was to describe the radiologic findings and clinical significance of bronchogenic carcinoma and pulmonary TB which coexist in the same lobe. Materials and Methods: The findings of 51 patients (48 males and three females, aged 48-79 years) in whom pulmonary TB and bronchogenic carcinoma coexisted in the same lobe were analyzed. The morphologic characteristics of a tumor, such as its diameter and margin, the presence of calcification or cavitation, and mediastinal lymphadenopathy, as seen at CT, were retrospectively assessed, and the clinical stage of the lung cancer was also determined. Using the serial chest radiographs available for 21 patients, the possible causes of delay in the diagnosis of lung cancer were analyzed. Results: Lung cancers with coexisting pulmonary TB were located predominantly in the upper lobes (82.4%). The mean diameter of the mass was 5.3 cm, and most tumors (n=42, 82.4%) had a lobulated border. Calcification within the tumor was seen in 20 patients (39.2%), and cavitation in five (9.8%). Forty-two (82.4%) had mediastinal lymphadenopathy, and more than half the tumors (60.8%) were at an advanced stage [IIIB (n=11) or IV (n=20)]. The average delay in diagnosing lung cancer was 11.7 (range, 1-24) months, and the causes of this were failure to observe new nodules masked by coexisting stable TB lesions (n=8), misinterpretation of new lesions as aggravation of TB (n=5), misinterpretation of lung cancer as tuberculoma at initial radiography (n=4), masking of the nodule by an active TB lesion (n=3), and subtleness of the lesion (n=1). Conclusion: Most cancers concurrent with TB are large, lobulated masses with mediastinal lymphadenopathy, indicating that the morphologic characteristics of lung cancer with coexisting pulmonary TB are similar to those of lung cancer without TB. The diagnosis of lung cancer is delayed mainly because of masking by a tuberculous lesion, and this suggests that in patients in whom a predominant or growing nodule is present and who show little improvement of symptoms despite antituberculous or other medical therapy, coexisting cancer should be suspected.

Background: To determine the effect of essential oil obtained from a traditionally used medicinal plant Tridax procumbens L, on lung metastasis developed by B16F-10 melanoma cells in C57BL/6 mice. Materials and Methods: Parameters studied were toxicity, lung tumor nodule count, histopathological features, tumor directed capillary vessel formation, apoptosis and expression levels of $P^{53}$ and caspase-3 proteins. Results: In vitro the MTT assay showed cytotoxicity was found to be high as 70.2% of cancer cell death within 24hrs for $50{\mu}g$. In vivo oil treatment significantly inhibited tumor nodule formation by 71.7% when compared with untreated mice. Formation of tumor directed new blood vessels was also found to be inhibited to about 39.5%. TUNEL assays also demonstrated a significant increase in the number of apoptotic positive cells after the treatment. $P^{53}$ and caspase-3 expression was also found to be greater in the essential oil treated group than the normal and cancer group. Conclusions: The present investigation showed significant effects of the essential oil of Tridax procumbens L in preventing lung metastasis by B16F-10 cell line in C57BL/6 mice. Its specific preventive effect on tumor directed angiogenesis and inducing effect on apoptosis warrant further studies at the molecular level to validate the significance of Tridax procumbens L for anticancer therapy.
본 논문에는 관심 영역의 폐실질 영역을 양성과 악성 결절의 분류를 위한 특징인자에 포함으로써 분류성능을 개선하였다. CT를 통해 확인되는 매우 작은 폐결절(4~10mm)은 고형 종양 내에 CT 데이터 복셀 수가 제한되어 기존 컴퓨터보조 진단도구를 통해 처리하기가 어렵다. 이러한 아주 작은 폐 결절의 경우 분석을 위해 주변의 실질을 포함하여 특징인자를 추출하는 것이 CT 복셀 세트를 증가시킬 수 있으며, CT 스캐너와 매개 변수에 대한 컴퓨터 보조진단도구의 유연성을 확보함으로써 진단 성능을 개선할 수 있다. 나이브 베이스와 SVM 약분류기를 이용하는 아다부스트 학습을 통해 304개의 특징인자로부터 유효한 특징인자를 결정하였으며, 제안한 방법을 COPDGene 데이터에 적용한 결과 100%의 정확도, 민감도 및 특이도의 결과를 획득하여 컴퓨터 보조진단에 유용하게 사용될 수 있음을 보였다.
The mucoepidermoid carcinoma is a rare tumor in the lung for less than 1% of all pulmonary neoplasm. It is mostly presented as a solitary pulmonary nodule at a large bronchial tree on bronchoscope. But more peripheral located tumor that not accessible to the bronchoscope are rarely reported on literature. The cytologic findings of these tumors are discribed as a mixture of squamous cells, mucous cells, and intermediate cells with overlapped cellular clusters. We experienced a case of peripheral mucoepidermoid carcinoma of lung diagnosed by fine needle aspiration cytology. The smear showed many cellular clusters on mucoid background. They consisted of many intermediate cells with occasional mucus-secreting cells, but malignant squamous cells were not present.
Pulmonary blastoma is rare malignant neoplasm usually situated peripherally in lung and composed of epithelial and mesenchymal components that resemble the lung at about 3 months gestation. Fortysix year-old man had no symptoms and signs, but solitary pulmonary nodule was found accidentally in a routine simple chest X-ray occupying right upper lung field. Preoperative chest CT-guided aspiration cytology revealed adenocarcinoma. We had done right upper lobectomy with Iymph node dissecton therapeutically. Final postoperative pathology reported this tumor as double pulmonary blastomas(well differentiated fetal adenocarcinoma). We report a case of pulmonary blastoma with review of literature.
A 46 years old male showed radiologically a single cavitary nodular lesion in right upper lung, which extended to the regional chest wall. This finding has to be made into differential diagnosis of numerous pulmonary diseases including infections such as mycobacterial, fungal or bacterial, granulomatous diseases, and neoplasms. For the definite diagnosis, fine needle aspiration biopsy guided by biplane fluoroscopy was performed. The aspirates contained several sulfur granules, in the center of which many gram positive, filamentous organisms were compactly intermingled. Such a findings was compatible with pulmonary actinomycosis. Now the lesions is cleared out by medical treatment with amoxicillin for 3 months.
Metastasis is one of the hallmarks of malignant neoplasms and is the leading cause of death in many cancer patients. A major challenge in cancer treatment is to find better ways to specifically target tumor metastasis. In this study, the anti-metastatic potential of the methanolic extract of Rhizophora apiculata (R.apiculata) was evaluated using the B16F-10 melanoma induced lung metastasis model in C57BL/6 mice. Metastasis was induced in C57BL/6 mice by injecting highly metastatic B16F-10 melanoma cells through the lateral tail vein. Simultaneous treatment with R.apiculata extract (10 mg/kg b.wt (intraperitoneal) significantly (p<0.01) inhibited pulmonary tumor nodule formation (41.1 %) and also increased the life span (survival rate) 107.3 % of metastatic tumor bearing animals. The administration of R.apiculata extract significantly (p<0.01) reduced biochemical parameters such as lung collagen hydroxyproline, hexosamine, uronic acid content, serum nitric oxide (NO), ${\gamma}$-glutamyl transpeptidase (GGT) and sialic acid levels when compared to metastasis controls. These results correlated with lung histopathology analysis of R.apiculata extract treated mice showing reduction in lung metastasis and tumor masses. Taken together, our findings support that R.apiculata extract could be used as a potential anti-metastasis agent against lung cancer.
A 22-year old female visited CNUH due to palpable neck mass. Cytologic examination of a fine needle aspiration was performed and the result was Pap class II. Routine chest x-ray shows solitary pulmonary nodule. For rule-out malignancy, FNA at neck mass was repeated and pathologic finding was dysplasia. She was admitted to MI department for evaluation of solitary pulmonary nodule and percutaneous needle aspiration was done. Pathologic diagnosis was adenoid cystic carcinoma. Thereafter, the lesions were treated by excisional biopsy of submandibular gland mass with left supraomohyoid neck dissection and wedge resection of right lower lobe at ENT department and thoracic and cardiovascular surgery department, respectively. Final diagnosis was adenoid cystic carcinoma arising in submandibular gland with solitary lung metastasis. According to TMN staging system, surgical staging is stage IV of T2N0M1. Clinical follow-up to postoperative 13 months in this case showed that she is alive and well without evidence of recurrence.
