Effects of Group Rehabilitation Gymnastics for Stroke Patients

  • Objective: This study is an experimental study to investigate the effect of group rehabilitation gymnastics on stroke patients. Design: A randomized controlled trial. Methods: From August 1, 2017, to February 30, 2018, 49 subjects hospitalized after being diagnosed with a stroke at K Rehabilitation Hospital had at least 8 weeks (more than 3 times a week) in the group rehabilitation exercise experimental group (25 patients), and the control group (24 patients) did not attend. Data analysis was performed using paired t-test for dependent variables before and after the experiment using SPSS 25. Results: In the experimental group, daily activities were statistically significantly increased from 59.04±25.19 points before treatment to 66.96±24.35 points after treatment (t=2.24, p=0.035). Lung capacity also significantly increased from 280.00±86.99 points to 334.40±93.23 points (t=4.21, p<0.001), and hospital reuse intention also significantly increased from 8.04±1.57 points to 8.88±1.16 points (t=2.67, p=0.013). In the case of the control group, although the MBI, lung capacity, stress, hospital reuse intention, and hospital use satisfaction increased after the experiment compared to before the experiment, there was no significant difference. Conclusions: In conclusion, group rehabilitation gymnastics for stroke patients is effective for daily living movements, lung capacity, and hospital re-use intention. Therefore, if it is continuously applied to stroke patients, it can be used as an intervention to improve the quality of nursing by enhancing physical function.

정상 성인의 신체조성과 폐 기능의 연관성 (Correlation between Body Composition and Lung Function in Healthy Adults)

  • Purpose : We investigated the correlation between body composition and lung function in healthy adults. Methods : This study included 204 healthy adults in whom all measurements were obtained once, and all data were analyzed using the SPSS software for Windows, version 22.0. Pearson's correlation analysis was performed to determine the correlation between body composition (represented by the total body water, protein mass, soft lean mass, mineral mass, basal metabolic rate, fat-free mass, skeletal muscle mass, and body fat percentage) and lung function (represented by the forced vital capacity [FVC], forced expiratory volume in 1 second [FEV1], the FEV1/FVC ratio, maximum voluntary ventilation [MVV], maximum expiratory pressure [MEP], and the maximum inspiratory pressure [MIP]). All measurements were obtained by two investigators to improve reliability. A significance level of α=.05 was used to verify statistical significance. Results : Among the lung function measurements obtained in both men and women, the FVC, FEV1, MVV, and MIP were positively correlated with the total body water, protein mass, soft lean mass, mineral mass, basal metabolic rate, fat-free mass, and skeletal muscle mass in men (p<.05). The FEV1/FVC ratio was negatively correlated with the total body water, soft lean mass, mineral mass, basal metabolic rate, fat-free mass and the body fat percentage (p<.05). Notably, the FVC, FEV1, and MVV were positively correlated with the total body water, protein mass, soft lean mass, mineral mass, basal metabolic rate, fat-free mass, and skeletal muscle mass in women (p<.05). Conclusion : This study showed a significant correlation between body composition and lung function in healthy adults. In combination with future studies on lung function, our results can provide objective evidence regarding the importance of prevention of lung disease, and our data can be utilized in rehabilitation programs for patients with respiratory diseases.

숲 체험 프로그램이 지역아동의 폐활량과 건강체력, 감성지능, 심리적 안녕감에 미치는 효과 (The Effect of Forest Experience Program on the Lung Capacity, Health & Fitness, Emotional Intelligence, and Psychological Well-being of Local Children)

  • 본 연구의 목적은 숲 체험 프로그램이 지역아동의 폐활량, 건강체력, 정서지능, 심리적 안녕감에 미치는 효과를 규명하는 것입니다. 본 연구는 D시 지역아동센터에 2022년 7월 10일부터 9월 30일까지 12주간 초등 3, 4학년을 대상으로 진행되었으며, 실험군과 대조군의 폐활량, 건강체력, 정서지능, 심리적 안녕감의 변화를 t-test를 이용하여 분석하고 검증하였다. 폐활량과 건강체력의 변화는 폐활량에서 대조군과 실험군 간에 통계적으로 유의한 차이가 있었으며(t=24.56, p<.05), 건강체력의 요소 중 심폐지구력에서도 두 집단 간에 통계적으로 유의한 차이가 있었습니다(t=16.64, p<.05). 정서지능과 심리적 안녕감의 변화는 정서지능(t=2.31, p<.05)과 심리적 안녕감(t=3.21, p<.05)에서 실험군과 대조군 간에 통계적으로 유의한 차이가 있었습니다. 본 연구를 통해 숲 체험 프로그램의 긍정적인 효과를 확인하였고 지역의 특성에 맞게 숲 체험 교육 공간의 확충과 맞춤형 숲 체험 프로그램의 개발로 아동들의 참여 여건이 개선되어 많은 아동들이 경험할 수 있도록 제도적 장치가 필요하다고 사료된다.

큰 폐 공기집의 주변 폐 감염 후 혹은 자연적 소실 (Regression of Large Lung Bullae after Peribullous Pneumonia or Spontaneously)

  • Background: A lung bulla may rarely shrink as a result of an inflammation within the bulla or a closing of a bronchus involved in the inflammation process, which is termed 'autobullectomy'. The purpose of this study was to describe clinical features of patients with regressions of bullae during follow-up. Methods: We retrospectively reviewed the cases and individuals who showed unequivocal evidence of interval regressions in a pre-existing bulla. A total of 477 cases with a bulla >5 cm in diameter were screened manually. Thirty cases with bullae that showed regression during follow-up were selected. Results: Regressions of large bullae occurred in 30 of 477 cases (6.3%). The median age of those patients was 61 (range, 53~66) years and 87% of those patients were men. The main cause of a bulla was emphysema (80%). Among 30 cases, 16 cases had pneumonia in the lung parenchyma of the peribullous area. Another 7 cases had a regressed bulla accompanied by an air-fluid level within the bulla. The remaining 7 cases showed a spontaneous regression of the bulla without such events. Complete regression of a bulla occurred in 25 cases. A follow-up chest-X ray showed that in all cases except one, the bulla remained in a collapsed state after 24 months. Forced expiratory volume in one second ($FEV_1$) improved in 3 cases and the other 2 cases had increased forced vital capacity (FVC). In addition, total lung capacity (TLC) and residual volume (RV) decreased in another 2 cases. Conclusion: Regression of a lung bulla occurred not only after pneumonia or the presence of air-fluid level within the bulla, but also without such episodes. The clinical course of regression of a lung bulla varied. After regression of a bulla, lung function could be improved in some cases.

Lung Function Profiles among Individuals with Nonmalignant Asbestos-related Disorders

  • Background: Inhalation of asbestos fibers can lead to adverse health effects on the lungs. This study describes lung function profiles among individuals with nonmalignant asbestos-related disorders (ARDs). Methods: The study population was from the Workers' Compensation (Dust Diseases) Board of New South Wales, Sydney, Australia. Lung function measurements were conducted in males with asbestosis (n = 26), diffuse pleural thickening (DPT; n = 129), asbestosis and DPT (n = 14), pleural plaques only (n = 160) and also apparently healthy individuals with a history of asbestos exposure (n = 248). Standardized spirometric and single-breath diffusing capacity for carbon monoxide ($DL_{CO}$) measurements were used. Results: Mean age [standard deviation (SD)] was 66.7 (10.3) years for all participants. Current and ex-smokers among all participants comprised about 9.0% and 54.8%, respectively. Median pack-years (SD) of smoking for ex- and current-smokers were 22.7 (19.9). Overall 222 participants (38.6%) and 139 participants (24.2%) had forced expiratory volume in 1 second ($FEV_1$) and forced vital capacity (FVC) measurements < 80% predicted, and 217 participants (37.7%) had $FEV_1/FVC$ results < 70%. A total of 249 individuals (43.8%) had DLCO values < 80% predicted and only 75 (13.2%) had DLCO/VA results < 80% predicted. A total of 147 participants (25.6%) had peak expiratory flow (PEF) measurements < 80% predicted. The presence of ARDs lowered the lung function measurements compared to those of healthy individuals exposed to asbestos. Conclusion: Lung function measurement differs in individuals with different ARDs. Monitoring of lung function among asbestos-exposed populations is a simple means of facilitating earlier interventions.

폐절제술을 받은 환자의 호흡운동중재가 폐기능에 미치는 효과 (The Effect of a Breathing Exercise Intervention on Pulmonary Function after Lung Lobectomy)

  • Purpose: The aim of this study was to evaluate the effect of a breathing exercise intervention by measuring pulmonary function test (PFT) three times; preoperative, 3rd and 5th day after operation. Methods: This study was designed as a non-equivalent control group pretest-posttest design. A total of 55 patients with lung cancer were recruited from a Chonnam university hospital in Hawsun-gun, Korea from January to December 2008. Results: 'Forced Vital Capacity (FVC)' and 'Forced Expiratory Volume in 1 second $(FEV_1)$' were significantly improved in the experimental group than those in the control group (p<.05). Conclusion: Breathing exercise intervention was found to be effective in improving pulmonary function among lung cancer patients underwent lung lobectomy. Thus, the breathing exercise can be applied in hospitals and communities for patients with lung cancer as one of the nursing intervention modalities for their better postoperative rehabilitation.

20대 성인 흡연자의 조깅운동이 심장호흡기능에 미치는 영향 (Influence of Smokers's Jogging Exercises on Cardiopulmonary Function of Normal Adults in Their Twenties)

  • Background and Purpose : Effects of smokers's jogging exercises on oxygen uptake, ventilation, heart rate, and vital-capacity of normal adults have not been studied in details on. In this study, people that had completed the established jogging exercises was selected for the study. We could examine the effects of imposed jogging in smokers who might be considered by an experienced physical therapist to be well practiced and proficient in this exercises. Subjects and Methods : A total number of 24 young healthy subjects participated in a randomized crossover controlled study and completed the study. The subjects of this study were divided into jogging and controlled groups which were composed 14 people respectively. The members of exercise group had jogging exercises 50 minutes a day, five times a week, for 5 weeks in same conditions, otherwise controlled group did not. Results : The results of this study were as follows. The jogging group demonstrated significant increases in cardiopulmonary function, vital-capacity and exercise capacity, compared with control group, which demonstrated no change in baseline measurements of 5 weeks. Conclusion : Jogging exercises can increase cardiopulmonary function in people after they have undergone successful PA. These results suggested that it was improved on cardiopulmonary function and lung capacity, since jogging exercise had. The results of this study could be applied for valuable criterion cardiopulmonary function. It is necessary to selects in various groups of the purpose of enhancing confidence in of this research.

스쿼트 운동, 브레이싱 운동, 그리고 유산소 운동이 폐기능에 미치는 영향 비교 (Comparison of the Effects of Squat Exercise, Bracing Exercise, and Aerobic Exercise on Lung Function)

  • Purpose : The purpose of this study is to investigate the effect of squat, bracing and aerobic exercise on lung function, which is known to be effective for strength training, on lung function. Methods : The study was conducted with 33 students from Busan K university. Eleven students were assigned to squats, bracing, and aerobic exercise, six weeks three times a week. In order to measure lung activity, pony Fx manufactured the change amount of FVC (forced vital capacity), FEV1 (Forced expiratory volume at one second), and FEV1/FVC % (forced vital capacity/forced expiratory volume at one second) was analyzed after inputting the information of experimental group A and B controls. As a method of measurement, the difference between the three groups was analyzed using repeated ANOVA. Results : As a result of analyzing the effects of squat, bracing, and aerobic exercise for 6 weeks, all values of FVC, FEV1, FEV1/FVC % were increased from 0 weeks to 6 weeks except FEV1/FVC %. There was no significant difference in FVC from week 3 to week 6. In the squat, bracing, and aerobic exercise, the changes in spirometry showed that the FVC, FEV1, and FEV1/FVC % values in bracing exercise were significantly increased with time than before exercise. As a result of analyzing the changes in the spirometry of squat, bracing, and aerobic exercise, the FVC, FEV1, FEV1/FVC % values in the squat exercise showed statistically significant difference according to the period, but the lowest increase among the three groups. Conclusion : In conclusion, aerobic, bracing and squat exercises all had a significant impact on improving lung function. Therefore, even without aerobic exercise, squat or bracing exercise alone can be expected to improve lung function.

Oscillometry-Defined Small Airway Dysfunction in Patients with Chronic Obstructive Pulmonary Disease

  • Background: The prevalence of small airway dysfunction (SAD) in patients with chronic obstructive pulmonary disease (COPD) across different ethnicities is poorly understood. This study aimed to estimate the prevalence of SAD in stable COPD patients. Methods: We conducted a cross-sectional study of 196 consecutive stable COPD patients. We measured pre- and post-bronchodilator (BD) lung function and respiratory impedance. The severity of COPD and lung function abnormalities was graded in accordance with the Global Initiative for Chronic Obstructive Lung Disease (GOLD) guidelines. SAD was defined as either difference in whole-breath resistance at 5 and 19 Hz > upper limit of normal or respiratory system reactance at 5 Hz < lower limit of normal. Results: The cohort consisted of 95.9% men, with an average age of 66.3 years. The mean forced expiratory volume 1 second (FEV1) % predicted was 56.4%. The median COPD assessment test (CAT) scores were 14. The prevalence of post-BD SAD across the GOLD grades 1 to 4 was 14.3%, 51.1%, 91%, and 100%, respectively. The post-BD SAD and expiratory flow limitation at tidal breath (EFLT) were present in 62.8% (95% confidence interval [CI], 56.1 to 69.9) and 28.1% (95% CI, 21.9 to 34.2), respectively. COPD patients with SAD had higher CAT scores (15.5 vs. 12.8, p<0.01); poor lung function (FEV1% predicted 46.6% vs. 72.8%, p<0.01); lower diffusion capacity for CO (4.8 mmol/min/kPa vs. 5.6 mmol/min/kPa, p<0.01); hyperinflation (ratio of residual volume to total lung capacity % predicted: 159.7% vs. 129%, p<0.01), and shorter 6-minute walk distance (367.5 m vs. 390 m, p=0.02). Conclusion: SAD is present across all severities of COPD. The prevalence of SAD increases with disease severity. SAD is associated with poor lung function and higher symptom burden. Severe SAD is indicated by the presence of EFLT.

Comet Assay를 이용한 케일, 명일엽, 당근, 돌미나리 녹즙의 Chinese Hamster Lung 세포 DNA 손상 보호 효과 (Protective Effect of Yellow-Green Vegetable Juices on DNA Damage in Chinese Hamster Lung Cell Using Comet Assay)

  • The present study was attempted to investigate the antioxidant capacity of popular yellow-green vegetable juices (kale, Angelica keishei, carrot, small water dropwort) and to investigate the effect of vegetable juices on protecting oxidative damage to DNA in cultured Chinese hamster lung (CHL) cells. Antioxidant capacity was analyzed by TRAP assay (Total radical-trapping antioxidant potential). Cellular DNA dmamage was measured by SCGE (single-cell gel electrophoresis, also known as comet assay. Cells incubated in medium with PBS (negative control) or with various concentration of the freeze dried green juices (25, 50, 100, 250 $\mu\textrm{g}$/$m\ell$) resuspended in PBS were treated with $H_2O_2$ (200 ${\mu}{\textrm}{m}$) as an oxidative stimulus for 5 min at 4$^{\circ}C$. The physiological function of each vegetable juice on oxidative DNA damage was analyzed and expressed as tail moment (tail length X percentage migrated DNA in tail) . Kale juice had the highest TRAP value suggesting that kale has the highest antioxidant capacity followed by Angelica keishei, small water dropwort and carrot. Cells treated with $H_2O_2$ had extensive DNA damage compared with cells treated with PBS or pre-treated with vegetable juice extracts. All green juices inhibited $H_2O_2$-induced DNA damage with kale being the most effective juice among the tested juices. These results indicate that green juice supplementation to CHL cells followed by oxidative stimulus inhibited damage to cellular DNA, supporting a protective effect against oxidative damage induced by reactive oxygen species.
