The Effects of Lumbar Stabilizing Exercise on Increasing of the Strength of Lumbar Extensor Muscle in Normal Subjects (요부안정화 운동이 정상인의 요부 신전력 증진에 미치는 영향)

  • The purpose of this study was to find the effects of the lumbar stabilizing exercise on the isometric lumbar extension strength and the range of motion of lumbar extension. 16 healthy subjects were recruited (9 males, 7 females, mean age 25${\pm}$3.2). They performed 5 types of exercises 5 times per week for 4 weeks. We evaluated isometric trunk extension strength (0, 12, 24, 36, 48, 60, $72^{\circ}$) by MedX(Ocala, FL) and measured range of motion of lumbar extension. All measurements were measured at pre-exercise and 4 weeks post-exercise. The results of this study were summarized as follows; After 4 weeks of exercise, isometric lumbar extension strength was generally increased but there were only significant differences at $12^{\circ},\;24^{\circ},\;48^{\circ}$. The range of motion of lumbar extension was increased (p<.05). The isometric lumbar extension strength correlated with the range of motion of lumbar extension. In conclusion, our results have shown that lumbar stabilizing exercise influenced both lumbar extension strength and range of motion of lumbar extension.

Comparing the Immediate Effectiveness of Lumbar Flexion and Extension Exercise With Regards to Pain, Range of Motion, Pelvic Tilt, and Functional Gait Ability in Patients With Lumbar Spinal Stenosis

  • Background: In patients with lumbar spinal stenosis (LSS), lumbar flexion exercise (LFE) is considered a standard therapeutic exercise that widens the space between the spinal canal and intervertebral foramen. However, some researchers have reported that lumbar extension exercise (LEE) may improve lumbar pain and functional ability in patients with LSS. Although exercise intervention methods for patients with LSS have been widely applied in clinical settings, few studies have conducted comparative analysis of these exercise methods. Objects: This study aimed to compare the effects of LFE, LEE, and lumbar flexion combined with lumbar flexion-extension exercise (LFEE) on pain, range of motion (ROM), pelvic tilt angle, and functional gait ability in patients with LSS. Methods: A total of 30 patients with LSS, LFE (n1=10), LEE (n2=10), and LFEE (n3=10) were assigned to each of the three exercise groups. The numerical pain rating scale (NPRS), modified-modified schober test (MMST)-flexion, MMST-extension, pelvic tilt inclinometer, and 6-minute walking test (6MWT) were measured. Results: After the intervention, statistically significant differences were observed in the NPRS (p=.043), MMST-flexion (p<.001), MMST-extension (p<.001), and 6MWT (p=.005) between groups. According to the post hoc test, the NPRS was statistically significant difference between the LFEE and LEE groups (p=.034). The MMST-flexion was statistically significantly different between the LFE and LEE (p=.000), LFE and LFEE (p=.001), and LEE and LFEE (p=.001) groups. The MMST-extension was statistically significantly different between the LFE and LEE (p<.001), LFE and LFEE (p=.002), and LEE and LFEE (p=.008) groups. The 6MWT was statistically significantly different between the LFE and LFEE (p=.042) and the LEE and LFEE (p=.004) groups. Conclusion: This study suggested that LFEE was the most effective exercise for pain and functional gait ability in patients with LSS, LFE was the most effective exercise for lumbar flexion ROM, and LEE was the most effective exercise for lumbar extension ROM.

Effects of Obesity on Lumber Strength and Visual Analogue of Back Pain in Disc Surgical Operated Patients after Rehabilitation (디스크 수술환자의 재활운동 프로그램 적용 후 비만이 요부신전근력 향상 및 요통완화에 미치는 영향)

  • The purpose of this study were to compare the differences of isometric lumber extension strength and subjective pain degrees between obesity patients group and normal body fat group in disc surgical operated patients. The research purposed to indicate how isometric lumbar extension exercise for 12 weeks affected to lumbar strength and visual analogue scale of patients suffered by chronic back pain. The subjects were 65 low back pain patients(male 30, female 35)who had disease on lumbar in W hospital. The lumbar extension strength was measured at seven degrees of angles, which were $0^{\circ}$ $12^{\circ}$ $24^{\circ}$ $36^{\circ}$ $48^{\circ}$ $60^{\circ}$ and $72^{\circ}$ before and after the exercise program. We got the results of subjective pain degree using the modified visual analogue scale(VAS) of Lawlis et al(1989) and measured the maximal isometric lumbar strength of all subjects using MedX lumbar extension machine. Results were as follows; After the exercise, the lumbar extension strength of normal body fat patients groups included males and female were greater than that of the obesity patients groups in all angles(p<.05). The visual analogue scale of chronic back pain patients was decreased significantly after the exercise(p<.05). The results showed the significance between the lumbar extension strength and the visual analogue scale of chronic back pain patients and showed that the isometric lumbar extension exercise decreased the subjective pain degrees of visual analogue scale with and increased lumbar extension strength. The correlation between the visual analogue scale and the %body fat of chronic back pain patients was no significant after exercise. Therefore, the lumbar extension strength exercise is needed for improvement of back strength, decrease of %body fat.

Effect of Lumbar Stabilization and Isometric Lumbar Strengthening Exercises on Lumbar Strength and Extension Range in Healthy Young Adults (등척성 요부 근력강화 운동과 요부 안정화운동이 요부 근력과 신전 범위에 미치는 영향)

  • Purpose : The purpose of this study was to identify the effect of lumbar stabilization exercise (LSE) and isometric lumbar strengthening exercise (ILSE) on lumbar strength and lumbar extension range in healthy young adults. Method : Ten healthy young adults (six males and four females) volunteered to carry out the LSE and ILSE program for two weeks. The subjects were randomly allocated to two groups: the LSE group (n=5) and the LSE plus ILSE group (n=5). The LSE consisted of 20 minutes of exercise related to lumbar stabilization, and the ILSE was composed of five minutes of isometric stabilization exercise with a specific device. Each group exercised three times per week for two weeks. Assessment tools were made using the subjects' isometric lumbar strength and lumbar extension range before and after the interventions. Results : Statistical analysis revealed significant differences in isometric lumbar strength and the lumbar extension range between before and after the interventions in each group (p<.05). Also, the lumbar extension range improved significantly in the LSE plus ILSE group compared to the LSE group; however, there was no statistically significant difference in the two groups' isometric lumbar strength (p>.05). Conclusion : The findings suggest that ILSE might be feasible in clinical settings by offering benefits for lumbar function. Future studies will be continued.

The Effect on Chronic Low Back Pain Patient Through Lumbar Extension Muscular Strength Training (만성 요통환자의 요추부 신전강화에 의한 효과)

  • The number of the subjects of this study were all sixteen including 8 male and 8 female. They were chronic low back pain patients and treated at hospital out patient. The patients took lumbar extension strength training. The results of examination and analysis isometric lumbar extension muscular strength before and after the training are as follows; 1. TFT increased after lumbar extension muscular strength exercise than before the exercise. Inspire of difference of each angle. The reciprocal action didn't happen between training and angle. 2. NMT increased after lumbar extension muscular strength exercise than the before the exercise and the difference of each angle didn't happen. The reciprocal action also didn't happen between the training and angle. 3. Comparing presented standard point with individual point, 13persons' muscle strength of first step increased, on the other and 3 person's muscle strength a little increased in a same category. 4. The grade of low back pain decreased for all subjects. Concluding this study, lumbar extension muscular strength exercise through isokinetic device decreases the grade of low back pain and increase lumbar extension muscular strength. It is that equal exercise effect happen in full range of motion, for the exercise effect doesn't show the difference of each angle.

A Study on Effect by Lumbar Extension Exercise (요부신전운동에 의한 효과에 관한 연구)

  • The purpose of this study is to improve the effect of Lumbar extensor exercise program and develop the Lumbar extensor exercise program which will be suitable to the patients characteristics. In this study the experimental group was made up of 38 subjects. They are the patients with low back pain using the Lumbar extension exercise program in C hospital. The lumbar extension exercise program was given two times a week for 8 weeks. The results were estimated by Lumbar extensor strength by diagnosis agent. The results were measured three times, one time pre-treatment and two times post-treatment at 4weeks and 8weeks by lumbar extensor curve angle(0, 12, 24, 36, 48, 60, 72). The results were compared at pre-test 4weeks and 8weeks. The muscle strength measured at both 4weeks and 8weeks with the greater strength was shown at 4weeks. The muscle strength of patients with M.strain, Laminectomy and HNP was increased at all angles except for patients with stenosis. The results of this study indicated that diagnosis influenced the muscle strength in Lumbar extensor exercise program.

Effect of Lumbar Stabilizing Exercise SEBT Training on Isometric Lumbar Strength, Dynamic Balance Ability and Range of Motion in Open Laser Lumbar Microdisectomy Patients (미세 현미경 레이저 요추 디스크 절제술 환자의 요부안정화운동과 SEBT 트레이닝이 등척성 요부근력과 동적균형능력, 관절가동범위에 미치는 영향)

  • This study examines the effects on isometric lumbar extension strength, dynamic balance ability, and range of motion, after administering 8 weeks SEBT exercise (dynamic balance exercise) and Lumbar Stabilizing exercise, to open laser lumbar microdiscectomy patients. Totally, 14 patients who underwent preservation treatment for 6 weeks after undergoing open laser lumbar microdiscectomy, were enrolled for the study. Patients were randomly assigned to the Lumbar Stabilizing Exercise Group (LSG, n=7) and SEBT Exercise + Lumbar Stabilizing Exercise Group (SGLS, n=7). Results indicate that isometric lumbar extension strength ratio significantly decreases after 8 weeks rehabilitation exercise in both the LSG (p=0.007) as well as SLSG (p=0.024) groups. Normalized reach distance of the three directions in the Y-balance test to examine the dynamic balance capability showed a significant increase in both groups. The dynamic balance capability showed significant increase to the left (LSG, p=0.010; SLSG, p=0.002) and right (LSG, p=0.002; SLSG, p=0.002). Moreover, significant increase was also obtained in the range of joint operation, in both LSG (p=0.006) and SLSG (p=0.017) groups. These results indicate that both groups of rehabilitation exercise achieve positive outcomes on the isometric lumbar extension strength, range of motion, and dynamic balance ability. However, some results suggest that the SEBT program is likely to have a better efficacy.

The Isoinertial Assessment of Lumbar Function in Patients with Chronic Low Back Pain (등저항성삼축동력계(Isostation B-200)를 이용한 만성 요통 환자들의 요추부 기능)

  • Objective : This study obtained normative values for variable parameters of lumbar function with the isoinertial triaxial dynamometer in patients with chronic low back pain. Subjects and Methods : 30 patients(male 15, female 15) with chronic low back pain in this study. Variable parameters that were measured with the Isostation B-200 were lumbar range of motion, isometric maximum torques, and maximum velocities in three axis. Results : In patient male group mean R.O.M. was $82.9{\pm}12.5$ degrees in lumbar rotation, $76.5{\pm}17.1$ degrees in lumbar flexion/extension, and $64.3{\pm}14.5$ degrees in lumbar lateral flexion. In patient female group mean R.O.M. was $78.4{\pm}18.5$ degrees in lumbar rotation, $71.7{\pm}20.4$ degrees in lumbar flexion/extension, and $63.2{\pm}14.4$ degrees in lumbar lateral flexion. In patient male group mean isometric maximum torques was $64.7{\pm}23.8ft-lbs$ in lumbar rotation, $81.1{\pm}42.0ft-lbs$ in lumbar flexion, $122.2{\pm}43.6ft-lbs$ in lumbar extension, and $101.0{\pm}37.0ft-lbs$ in lumbar lateral flexion. In patient female group mean isometric maximum torques was $41.9{\pm}9.2ft-lbs$ in lumbar rotation, $49.9{\pm}23.9ft-lbs$ in lumbar flexion, $90.1{\pm}26.8ft-lbs$ in lumbar extension, and $62.0{\pm}16.7ft-lbs$ in lumbar lateral flexion. In patient male group mean maximum velocity of isoinertial exercise with low (25%) resistance was $102.4{\pm}28.8deg/sec$ in lumbar rotation, $108.9{\pm}32.2deg/sec$ in lumbar flexion/extension, and $103.5{\pm}30.4deg/sec$ in lumbar lateral flexion. In patient female group mean maximum velocity of isoinertial exercise with low (25%) resistance was $84.1{\pm}24.4deg/sec$ in lumbar rotation, $93.2{\pm}32.9deg/sec$ in lumbar flexion/extension, and $98.5{\pm}33.7deg/sec$ in lumbar lateral flexion. In patient male group mean maximum velocity of isoinertial exercise with high (50%) resistance was $74.0{\pm}20.9deg/sec$ in lumbar rotation, $98.7{\pm}32.8deg/sec$ in lumbar flexion/extension, and $85.0{\pm}25.8deg/sec$ in lumbar lateral flexion. In patient female group mean maximum velocity of isoinertial exercise with high (50%) resistance was $67.3{\pm}26.4deg/sec$ in lumbar rotation, $82.5{\pm}31.0deg/sec$ in lumbar flexion/extension, and $79.7{\pm}23.9deg/sec$ in lumbar lateral flexion. Conclusion : Maximum isoinertial velocities were more reliable and more significant than isometric maximum torque for the objective assessment of chronic low hack pain.

The Effect of Proprioceptive Position Sense by Lumbar Flexors and Extensors

  • Purpose: Muscle fatigue affects proprioception, and it causes problems in spinal stability. The purpose of this study was to examine the effect on the accuracy of reproducing the lumbar angles before lumbar exercise and after fatiguing isokinetic lumbar exercise. Methods: Thirty healthy adults participated in this study. Before induction of fatigue by exercise, the proprioception was measured by Biodex. Lumbar positions were passively maintained on stimulation position ($25^{\circ}$ flexion and $25^{\circ}$ extension), and back to the starting position. Subjects actively repositioned the remembered stimulation position, and error degrees between the stimulation position and reposition were measured. Using an isokinetic device at $120^{\circ}$/sec of velocity of angle lumbar flexion/extension exercise resulted in muscle fatigue. The post-fatigue proprioceptive position sense was used in the same way as in pre-fatigue measurement. Results: Means of position sense of pre-fatigue were $2.19{\pm}1.97$ on flexion angle, and $5.04{\pm}2.84$ on extension angle. After exercise induced fatigue, means of position sense were $2.37{\pm}1.83$ on flexion angle, and $4.93{\pm}2.57$ on extension angle. Results of this study showed significant differences of lumbar proprioceptive position sense between pre- and post-fatigue. Conclusion: Lumbar proprioception sense in active repositioning in flexion and extension was affected in the presence of muscle fatigue. Therefore, it should be noted that therapeutic exercise for patients with abnormal proprioceptive sense or elderly people must be performed with care because muscle fatigue can cause secondary damage.

The effect of MedX exercise program on lumbar extensor strength in the elderly with chronic LBP (메덱스 척추강화운동 프로그램이 만성 요통 노인 환자들의 요부 신전 근력에 미치는 영향)

  • The purpose of this study was to compare the lumbar extensor strength between before exercise and after 8 weeks exercise in the elderly. To evaluate lumbar extensor strength of total 18 patients (above 59 age) with low back pain or have experienced surgical operation. Lumbar extensor strength was measured in 8 male patients and 10 female patients (9 op group and 9 non-op group) by Medx lumbar extension machine. The data were analyzed with paired t-test compared to muscle strength, with repeated measured ANOVA compared to amount increase of muscle strength, using SPSS ver. 10.0 program. The results of this study were as follows: 1. There were significantly increased total lumbar extension strength was appear $6918.7{\pm}2802.77$ft-lbs in before exercise, $10432.83{\pm}2802.773397.1$ft-lbs in after 8 weeks exercise (p<. 05). 2. There were significantly increased total lumbar extension strength was increased to compared before exercise with after 8 weeks exercise in sex (p<. 05), op/non-op group (p<. 05).
