추나요법을 동반한 한방복합치료로 호전된 요추 전만각 1례: 증례보고 (Lumbar Lordotic Angle Improvement by Combining Korean Medicine Treatment with Chuna Manual Therapy: Case Report)

  • Objectives To introduce a case of a traffic accident trauma in a patient with lumbar kyphosis and straightened lumbar curvature, where the Korean medicine treatment with Chuna manual therapy (CMT) improved lumbar lordotic angle(LLA) and lower back pain. Methods Chuna manual therapy and other Korean medicine treatments; were provided to patient with a flat back posture and low back pain due to the trauma from a traffic accident. Cobb's angle to evaluate LLA and numeric rating scale (NRS) to evaluate pain were compared before and after the treatment. Results There was a significant increase in LLA to reach the normal range and a decrease in low back pain NRS after Korean medicine treatment with CMT for 9 days of admission treatment and 3 weeks of outpatient treatment. Conclusions This study suggests that Korean medicine treatment, including CMT, may improve low back pain as well as LLA of straightened lumbar curvatures in patients.

A Surgical Option for Multilevel Anterior Lumbar Interbody Fusion with Ponte Osteotomy to Achieve Optimal Lumbar Lordosis and Sagittal Balance

  • Objective : To document lumbar lordosis (LL) of the spine and its change during surgeries with the different height but the same angle setting of the anterior cage. Additionally, we attempted to determine if sufficient LL is achieved at different cage heights and to quantify the change in LL during multi-level anterior lumbar interbody fusion (ALIF). Methods : The medical records and radiographs of 42 patients who underwent more than 2 level ALIFs between 2008 and 2009 were retrospectively reviewed. We evaluated 3 parameters seen on lateral whole spine radiographs : LL, pelvic incidence (PI), and sagittal vertical axis (SVA). The mean follow-up time was 28.1 months and the final follow-up radiographs of all patients were reviewed at least 2 years after surgery. Statistical analysis was performed using the paired t-tests. Results : Lumbar lordosis had changed up to 30 degrees immediately and 2 years after surgery (preoperative mean LL, SVA : 22.45 degrees, 112.31 mm; immediate postoperative mean LL, SVA : 54.45 degrees, 37.36 mm; final follow-up mean LL, SVA : 49.56 degrees, 26.95 mm). Our goal of LL is to obtain as much PI as possible, preoperative mean PI value was $55.38{\pm}3.35$. The pre-operative and two year post-surgery follow-up mean of the Japanese Orthopedic Association score were $9.2{\pm}0.6$ and $13.2{\pm}0.6$ (favorable outcome rate : 95%), respectively. In addition, we were able to obtain good clinical outcomes and sagittal balance with a subsidence rate of 22.7%. Conclusion : We were able to achieve sufficient LL, such that it was similar to the PI, utilizing multi-level ALIF with the use of a tall cage with the same angle setting of the cage. We have found out that achieving sufficient lumbar lordosis and sagittal balance require an anterior lumbar cage with high angle and height.

허리 디스크탈출증 환자의 재위치 감각과 근 피로도에 미치는 안정화운동 프로그램의 영향 (The Effects of Lumbar Repositioning Sense and Muscle Fatigue after Stabilization Exercise Program in Disc Disease Patients)

  • Background: The purpose of this study was designed to find out the effectiveness of reposition sense, muscle fatigue response on lumbar spine after apply lumbosacral stabilization exercise program to 4 patients with chronic low back pain and for 12 weeks. Method: In this study the reposition sense was measured in 3 angle(60, 30, 12) of the lumbar spine motion with blind by MedX test machine and the difference of instability to lumbar vertebra segments in flexion, extension test of standing position and spinal load test Mattress Test by Spinal Mouse. The stabilization exercise program was applied 2 times a week for 12 weeks in hospital and 2 times a day for 20 minutes at home. Result: The results of the present study were that the repositioning sense was appeared the most error in 12 angles of lumbar flexion and Men was appeared to decrease an error more than female in average value of 4 angles after 12 weeks. And average error of male was decrease more than female. Thus the effects of lumbosacral stabilization exercise was improved repositioning sense of prorioceptor. Fatigue response test(FRT) results, in male, was raised muscle fatigue rate during increase weight, on the other hand female appeared lower than male. Conclusion: As a results, lumbosacral stabilization exercise was aided to improvement of lumbar spine repositioning sense and vertebra segments stabilization. It was showed the rate of decrease in typically 12 degree angle point of each 3 angle(60, 36, 12). Especially, that spine instability patients will have a risk when in lifting a load or working with slight flexion posture around 12 degree during the daily of living life and it is probably to increase recurrence rate. Thus, not only lumbar extension muscle strength but also stability of vertebra segments in lumbar spine may be very important.

누운 자세에서 호흡에 따른 요추분절의 운동학적 분석 (Kinematics Analysis of Lumbar Spine during Breathing in Lying Position)

  • Purpose: The purpose of this study was to describe the influence of respiration on the segmental motion of the lumbar spine in the lying position. Methods: Twelve healthy females without a history of low back pain participated. Lumbosacral lordosis, intervertebral body angles, intervertebral body displacements, and anterior heights of the intervertebral disc of the lumbar spine were measured at inspiration, expiration and forced expiration in the supine and prone positions via fluoroscopy. Results: The results of lumbar kinematic analysis in the supine position according to respiration pattern were as follows. The L4/5 intervertebral body angle was significantly higher at forced expiration than at expiration (p<0.05). The L3/4 anterior height of the intervertebral disc was significantly higher at expiration than at forced inspiration and the L5/S1 anterior height of the intervertebral disc was significantly higher at inspiration than at forced expiration (p<0.05). There were no significant differences in the intervertebral body displacements and lumbosacral lordosis in the supine position (p>0.05). The results of lumbar kinematic analysis in the prone position according to respiration pattern were as follows. The L5/S1 anterior height of the intervertebral disc was significantly higher at inspiration than at forced expiration (p<0.05). However, there was no significant difference in the intervertebral body angle, the intervertebral body displacements, and the lumbosacral lordosis (p>0.05). Conclusion: These findings suggested that respiration can affect the intervertebral body angle and anterior height of the intervertebral disc in some segments. The results from this study serve as a step in the development of guidelines for lumbar kinematic analysis for lumbar breathing training.

방사선 일반 정면검사에서 허리뼈 추간판 계측 값에 따른 입사각 적용 (Application of Incidence Angle on Lumbar Spine Anteroposterior General Radiography Image according to Measured Intervertebral Disc Angle)

  • 정상인을 대상으로 체질량지수, 전만각도에 따른 L-spine 3~4, L-spine 4~5, L-spine 5~Sacrum 1의 각각의 추간판각도를 비교 분석하여 입사각을 제시하였다. 체질량지수의 L-spine 3~4, L-spine 4~5, L-spine 5~Sacrum 1의 정면 입사각도는 머리 쪽 방향으로 5.66도, 13.23도, 29.13도였으며, 전만각도의 L-spine 3~4, L-spine 4~5, L-spine 5~Sacrum 1의 정면 입사각도는 머리 쪽 방향으로 6.32도, 16.09도, 35.36도였다. 체질량지수, 전만각도에 따른 정면 입사각도에 따라 적용한 팬텀의 일반 전후방향 영상의 왜곡정도를 면적비율로 비교한 결과 L-spine 4~5, L-spine 5~Sacrum 1의 추간판각도에서 유의미한 차이가 있었고(p<0.05), 전만각도와 추간판각도는 양의 상관관계를 보였다(p<0.05). 팬텀의 전만각도에 따른 추간판각도 계측 값을 머리 쪽 방향으로 입사각을 L4는 11도, L5는 26도로 적용하여 영상의 유용성을 평가한 결과, 왜곡비율면적은 L4에서 14.90%에서 12.11%로 줄어들었고, L5에서는 15.25%에서 13.72%로 줄어들었다. 계측된 추간판 각도에 따른 입사각을 적용한 허리뼈 일반 전후방향 영상에서 허리뼈 4번, 5번을 목적으로 하는 허리뼈 정면 영상에서 왜곡을 줄여, 정확한 구조의 정면 추체상과 인접하는 관절 간 영상을 얻을 수 있었고, 목적하는 부위의 영상의 질과 진단적 정보를 향상시킬 수 있었다.

만성 허리통증 환자의 엉덩허리근 단축과 허리 불안정성 검사 간에 관련성 연구 (A Study on the Relationship Between the Results of Shortening of the Iliopsoas Muscles and the Lumbar Instability Tests in Patients with Chronic Low Back Pain)

  • PURPOSE: This study examined the relationship between lumbar instability and shortening of the iliopsoas muscles in patients with chronic low back pain. METHODS: Forty-nine patients with chronic low back pain participated in this study. The modified Thomas test was used to examine the shortening state of the iliopsoas muscle. The length of the iliopsoas muscle was measured using the hip flexion angle, and those with a flexion angle of 10° or more were classified as positive. Five subtests were used for the lumbar instability test: testing for prone lumbar instability, passive lumbar extension, anterior-posterior mobility, passive straight leg raise, and age. Those who tested positive for at least three of these tests were classified as positive for the lumbar segment instability test. RESULTS: There was a significant association between the results of the lumbar instability test and the shortening of the iliopsoas test (p < .05). After analyzing the association between the iliopsoas length test and the five lumbar instability subtests, the results of the prone lumbar instability test (p < .001) and the anterior-posterior mobility test (p < .05) showed a significant association with the iliopsoas length test. CONCLUSION: The association between lumbar instability and shortening of the iliopsoas muscles was examined in 49 patients with chronic low back pain. Patients with shortened iliopsoas muscles tested positive in the lumbar instability tests more often. Hence, the length test of the iliopsoas muscle can be used to determine lumbar instability in patients with chronic low back pain.

치료적 운동이 허리통증을 가진 여자 청소년 골프선수의 통증, 근기능 및 방사선학적 평가에 미치는 영향: 증례보고 (The Effect of Exercise Therapy on Pain, Muscle Function and Radiological Evaluation in a Female Youth Golf Player with Low Back Pain: Case Report)

  • PURPOSE: The purpose of this study was to determine the effect of exercise therapy on low back pain (LBP), the function of paraspinal and abdominis muscles, and the sacrohorizontal angle as seen on the radiographs of the lumbar spine in a young female golf player with LBP. METHODS: This case report describes an 11-year-old female golfer who presented with LBP. The exercise therapy program comprised lumbar joint mobilization, lumbar spine flexion distraction, abdominal bridge, plank, side plank, and single-leg extensions from a 4-point kneeling position for 40 min/day; this was done twice a week for 8-weeks. LBP [visual analog scale (VAS) and Oswestry disability index (ODI)] and function of paraspinal and abdominis muscles [Ito test, curl-up test, $90^{\circ}$ stop test, squat test, opened eye one leg stance test (OEOL), and closed eye one leg stance test (CEOL)] were measured before and after 4 and 8 weeks of exercise therapy. The radiographs were analyzed for the lumbar Cobb's angle and sacrohorizontal angle before and after 8 weeks of exercise therapy. RESULTS: After 4 and/or 8 weeks of exercise therapy, VAS and ODI scores decreased; results for the Ito test, curl-up test, $90^{\circ}$ stop test, squat test, and OEOL and CEOL of muscle function improved; and the lumbar Cobb's angle and sacrohorizontal angle improved. CONCLUSION: These results suggest that exercise therapy improves LBP, muscle function, and radiographic parameters associated with LBP in young golf players. These findings have clinical implications for exercise therapy in young female golf players who have LBP.

요통환자의 작업자세에 따른 요천추부 각도의 비교 (Difference in the Lumbosacral Region Angle according to Working Posture of patients with Low Back Pain)

  • Low back pain is significant problem in today's society, with lifetime incidence rate reported between 50% and 90%. Many factors associated with LBP are reported. The purpose of this studies were to be evaluated static standing posture aberrations in chronic LBP in comparison with healthy individuals. The samples including 80 subjects recruited to the following two groups:patients and control(normal) Questionnaires were completed by 40 LBP patients and 40 controls at the department of Physical Therapy, Saejong neurosurgical clinic in Taegu city from October 1, 1999 to March 30, 2000. The angle of lumbar lordosis was measured on lateral x-ray films with standing position. In LBP groups. the mean degree of lumbar lordosis, sacral inclination, and lumbosacral joint angle were 29.9 ${\pm}$ 9.3, 34.8 ${\pm}$ 8.2, and 12.7 ${\pm}$ 5.7 respectively. Control groups, the mean degree of lumbar lordosis, sacral inclination and lumbosacral joint angle were 35.3 ${\pm}$ 7.8, 34.9 ${\pm}$ 6.4 and 12.5 ${\pm}$ 4.3 respectively. there were significantly decreaseds in lumbar lordosis in Low back pain group. lumbar lordosis on the working posture had significant differences among groups(sitting position patients 31.4 ${\pm}$ 9.3, standing position patients 29.4 ${\pm}$ 9.3, sitting position control 35.0 ${\pm}$ 6.4, standing position control 35.5 ${\pm}$ 8.8, respectively) (p=0.034). sacral inclination on the working posture had differences among groups(sitting position patients 35.9 ${\pm}$ 8.7.standing position patients 33.6 ${\pm}$ 7.6, sitting position control 33.9 ${\pm}$ 5.9. standing position control 35.6 ${\pm}$ 6.8, respectively). lumbersacral joint angle on the working Posture had differences among groups(sitting position patients 12.0 ${\pm}$ 5.6, standing position patients 13.4 ${\pm}$ 5.9, sitting position control 11.2 ${\pm}$ 3.0. standing position control 13.4$^{\circ}$, respectively).

Effects of Standing Aids on Lumbar Spine Posture and Muscle Activity in the Lumbar Spine and Hip during Prolonged Standing

  • PURPOSE: This study was conducted to compare different standing postures with the use of standing aids for lumbar spine posture and muscle activity, and to identify the most desirable standing posture. METHODS: The lumbopelvic angle was assessed based on static radiographic measurement on the sagittal plane. Lumbar lordosis, lumbosacral lordosis, and the intervertebral joint angle at L1/L5 and L5/S1 were measured using radiography in three standing postures (standing on level ground, standing with one foot on a platform, and standing on a sloped surface). In addition, muscle activity was measured using surface electromyography to examine the co-contraction of the lumbar and hip muscles. RESULTS: Lumbar lordosis, lumbosacral lordosis, and L5/S1 intervertebral joint flexion occurred with one foot on the platform. No significant differences were found between standing on a sloped surface and standing on level ground. However, muscle co-contraction was reduced with the use of standing aids. CONCLUSION: This study demonstrated that standing with a foot on a platform induced lumbar lordosis, but that there was no significant difference between standing on a sloped surface and standing on level ground. However, muscle co-contraction was reduced with the use of standing aids. Based on the motor control pattern as a predictor of LBP, the use of standing aids would help workers during prolonged standing.
