Object : This study was planned to show the influences of MET(muscle energy techniques) treatment and acupuncture treatment on iliopsoas muscles of sprain and strain of lumbar spine patients. Method : This study was carried out on the 39 patients who had been treated for their sprain and strain of lumbar spine from March 2 to October 15, 2010 in the department of oriental rehabilitaion medicine, Dongsuwon oriental medicine hospital. And 22 out of 39 patients had a tenderness and pain in their iliopsoas muscles. And we divided those 22 patients into 2 groups by blocked randomization; group A took near acupuncture point needling treatment on the iliopsoas muscles, group B took MET(muscle energy techniques) treatment on the iliopsoas muscles. To evaluate the efficiency of each treatment, all patients were examined with VAS(visual analogue scale) before treatment and after the 3rd, 5th treatment. Result : On the VAS, it was decreased significantly after treatment in all groups. And it didn't show the clear difference between two groups. Conclusion : In this study, it did not show statistical difference in VAS between two groups.
Objectives The purpose of this study was to investigate the characteristics of inpatients who were admitted to Korean medicine hospital due to low back pain. Methods The current study was designed as a retrospective chart review to investigate descriptive characteristics of LBP patients. The clinical records of 601 patients who were hospitalized for treatment of LBP in Cheonan Korean Medicine Hospital, Daejeon University from 1st, January, 2013 to 31th, December, 2013 were analyzed. Results 1. Most frequently given diagnosis was sprain and strain of lumbar (45.93%). 2. Female outnumbered male patients in all disease groups except fracture of lumbar spine. 3. In distribution according to age, sprain and strain of lumbar, HNP of L-spine and lumbago with sciatica were most frequent at 30s, fracture of lumbar spine was most frequent at 50s and spinal stenosis was most frequent at 70s. 4. The most frequently motive for low back pain was traffic accident (35.4%) 5. Patients with no related medical history were 76.95% 6. 0~1week interval between onset and visit to Korean Medicine Hospital was most frequent in all disease groups. 7. 50.85% of patients went through treatment at medical institutions before the admitting to Korean Medicine Hospital. 8. The average admission days of female was higher than male's. And age goes up, average duration of admission was longer. 9. In most (74.59%) of the patients, symptoms were more than improved. 10. Most frequently prescribed examination was X-ray (65.13%). 11. Most frequently prescribed herb medicine was whal-lak-tang (Huoluo-tang). Conclusions In most (74.59%) of the patients, symptoms were more than improved, especially in sprain and strain of lumbar and lumbago with sciatica. But Patients with a local hospital statistic is not be representative of the incidence of the population. In order to obtain more accurate statistics, it is necessary to compare analysis collect statistics from other medical hospitals.

Objective : The goal of this study was to establish the benefit and prognostic factors of lumbar medial branch block[MBB] for low back pain. Methods : A retrospective analysis was based on the data obtained from 281 patients with low back pain, who visited our hospital between January 2001 and November 2004. Pain relief was evaluated at 2 weeks, 1 month and 3 months. The authors analyzed the results of MBB according to the patient's age, sex, symptom duration, pathologic condition, and presence of radiating pain. Results : Two hundred eighty one patients had sprain [151]. lumbar fracture [27], spinal stenosis [50], herniated lumbar disc [24] acute post-operative pain [8], and chronic post-operative pain [21] with success rate of 63.6%, 59.3%, 26.0%, 25.0%, 87.5% and 42.9%, respectively. The effects of MBB in sprain, lumbar fracture, and acute post-operative pain were significantly better than those in stenosis, herniated lumbar disc and chronic post-operative pain patients. The patients in young age group [<60 years], with short symptom duration [<6 months] and without radiating pain showed good response to lumbar MBB. Conclusion : The lumbar MBB appears to be safe and effective for low back pain in certain selected patients. Good prognostic factors were low back pain without surgical conditions and radiating pain, with short symptom duration [<6 months], and in relatively young age [<60 years] group.

The purpose of this study was to analysis of influence between of private health insurance and length of stay with cervical and lumbar simple sprain patients at Ubiquitous medical environment. They were diagnosed as having sprain of cervical and lumbar spine without neurological symptoms in hospitals and clinics from July 1, 2010 to February 29, 2012. This study was t-test, ANOVA and multiple regression analysis. The results were when the number of private health insurance was input as an independent variable, the subjects had one or two accidental insurances(${\beta}$=2.731, P=0.013), length of stay in beds was longer than they had no accidental insurance and when they had more than three ones(${\beta}$=4.991, P=0.000), length of stay was significantly longer. This study has a meaning in that it is domestic practical study which identified relationships between entry of private health insurance and length of stay.
Objectives This study was designed to compare the effects Gwibi-tang herbal-medication with Danggwisusan herbal-medication for patients with acute cervical and lumbar sprains due to a traffic accident. Methods This study was carried out on 119 acute cervical & lumbar sprain patients by traffic accident. We divided these patients into 2 groups. Group A was treated with Gwibi-tang and Group B was treated with Danggwisusan three times a day for three days. Each patient was treated with acupuncture, physical therapy and two types of herbal-medication. We measured the numerical rating scale (NRS) at two time points : pre-treatment (Tx). and post-Tx. Results 1. The NRS scores significantly decreased after treatment in all patients. 2. The changed NRS scores of nuchal pain showed a borderline significant difference between Group A and Group B (p=0.067). 3. In sub-group analysis, Gwibi-tang showed a greater effect on NRS scores of lumbago pain in female patients (p=0.068) and patients under 30s (p=0.054) compared to Danggwisusan. Conclusions In this study, both types of herbal-medication; Gwibi-tang and Danggwisusan were highly effective on acute cervical and lumbar sprain patients. Especially Gwibi-tang (Group A) was more effective than Danggwisusan (Group B) in reducing the initial nuchal pain. Also Gwibi-tang was more effective for lumbago pain in female and patients under 30s.

Objectives : The purpose of this study was to investigate the effect of chuna treatment(manipulation) on lumbar sprain caused by traffic accident in early stage, by analysis of the Heart Rate Variability(HRV) and Visual Analogue Scale(VAS). Methods : This study carried out on 20 patients who have received hospital treatment in Daejeon Univ. cheonan Oriental Hospital. Non-chuna group got acupuncture-therapy, herbal medication, physical therapy and Chuna group got all the therapies and chuna treatment. We measured HRV and VAS on 2nd, 4th day. Results : After being treated by our methods, Chuna group showed the inclination to balance between the sympathetic and parasympathetic nerve. In chuna group as compared with non-chuna group, an autonomic nerve activity and stress resistance showed the inclination to increase(p<0.05). And chuna treatment reduced the rate of stress increase. Conclusions : The results suggest that chuna treatment help traffic accident patients in early stage to reduce pain. Refer to autonomic nerve system, chuna treatment seem to do positive effect but Further long term study in a large scale is needed.
Objectives : To demonstrate the need for differential diagnosis between discogenic pain and lumbar sprain and strain in acute low-back pain patients. Methods : Outpatients who made their first visits during May 1, 2009 to Oct. 30, 2009(n=53) were examined by history taking, physical examination, X-rays, and CT imaging. Disorders found on lumbosacral spine X-ray cuts and those on lumbosacral spine CT images were separately recorded. The relationship between treatment period, disc space narrowing and disc degeneration on X-rays, and HIVD on CT images was examined. Results : 1. Correlation between disc space narrowing on X-rays and HIVD found on CT images was analyzed. 21(72.41%) out of 29 patients having disc space narrowing on X-rays and HIVD on CT at the same level required treatment for over 8 weeks. 2. 2(50%) out of 4 Lawrence classification grade I patients, 8(66.67%) out of 12 grade II patients, and 14(70%) out of 20 grade III patients needed treatment for over 8 weeks. Conclusions : Disc space narrowing on X-ray and HIVD on CT at the same level, or disc space narrowing and disc degeneration on X-ray image alone indicate a tendency for treatment periods over 8 weeks, which is longer than the conventional treatment period for lumbar strain and sprain.

Background: In Korea, private health insurance has neglected to induce externality on national health insurance by moral hazard. Therefore, we conducted this study in order to explore the influence of private health insurance on unnecessary medical utilization among patients with cervical or lumbar sprain. Method: The study examined a population of 449 patients (admission, 384; out-patient; 85) diagnosed with simple cervical or lumbar sprain without neurological symptoms at 20 small hospitals or clinics in Gwangju and Jeollanam provinces from Jul. 1 to Aug. 31 2008. The data were collected using structured, self-administrated questionnaire which collected information such as whether or not the patient was admitted (as a dependent variable), whether or not they had private health insurance (as a independent variable), and covariates such as socio-demographic characteristics, the factors related to the sprain, and characteristics of the insurance provider. Results: From hierarchical multiple logistic regression analysis, it was found that the admission rate of patient with private health insurance was higher than that those without it (Odds ratio=3.31, 95% Confidence interval; 1.14-9.58), meaning that private health insurance was an independent factor influencing the admission of patients with these conditions. Other determinants of admission were patient age and physician referral. Conclusions: This study is the first empirical study to explore the influence of private health insurance on inducing moral hazard in admission services, specifically among patients with cervical or lumbar sprain. Regulation of benefits provided by private health insurance may be necessary, as the effect of this moral hazard may mean existence of externality.
