The Effects of Joint Mobilization and Stretching on the Muscle Activity and Internal Rotation of Shoulder Joint in Patients With Impingement Syndrome With Posterior Shoulder Tightness (관절가동술과 스트레칭이 어깨후방뻣뻣함을 가진 충돌증후군의 어깨관절 근활성도와 안쪽돌림에 미치는 영향)

  • Background: Posterior shoulder tightness, which is a problem mainly seen in patients with shoulder impingement syndrome, disrupts the scapulohumeral rhythm between the humerus and scapulae. Objects: The aim of this study was to compare the effects of joint mobilization and stretching on shoulder muscle activity and internal rotation range of motion (ROM) of the glenohumeral joint in patients with impingement syndrome with posterior shoulder tightness. Methods: The research subjects included 22 in-patients with impingement syndrome with posterior shoulder tightness. They were randomly divided into two groups: one group (12 patients) was treated with joint mobilization and the other group (10 patients) was treated with stretching for the posterior shoulder tightness. Each treatment was performed five times a week for two weeks, and there were 15 sessions for each treatment. The ROM of the internal rotation and muscle activities of shoulder joint were evaluated pretest and posttest in each group. Electromyography data were collected from the upper, middle, and lower trapezius and serratus anterior during shoulder abduction of 90°, 120°, and 150°. Results: Both the joint mobilization and stretching groups showed significant decreases in muscle activity in the upper, middle, and lower trapezius on the posttest (p < 0.05). There was a significant difference in serratus anterior at 150° (p < 0.05), but there was no significant difference between group in post-hoc analysis (p > 0.025). The internal rotation ROM was significantly increased in the stretching group compared to that in the joint mobilization group (p < 0.025). Conclusion: This study found that both joint mobilization and stretching for posterior shoulder tightness were effective in muscle activity during arm abduction, also in order to increase internal rotation ROM of shoulder joint, stretching was effective in patients with impingement syndrome with posterior tightness.

Relation between Pressure Pain Threshold and Functional Activity in Masticatory and Cervical Muscles (저작근 및 경부근에서 압력통각역치와 기능시 근활성도 간의 관계)

  • This study aimed at evaluating tile relationship between pressure pain threshold(PPT) and electromyographic(EMG) activity in 30 healthy volunteers and 30 patients with temporomandibular disorders. PPTs were determined with electric pressure algometry over masseter, temporalis anterior, sternocleidomastoid, and trapezius muscle during resting and clenching. To obtain more reliable result, two examiners measured PPTs two times on each muscles, randomly. Resting and clenching EMG activity and tenderness of all muscles were measured to evaluate the relationship with PPTs. The collected data were processed by SAS/STAT program. The obtained results were as follows : 1. Pressure pain thresholds in tendered muscles were lower than those in non-tendered muscles. This tendency was more prominant in masticatory muscles than in cervical muscles and in clenching than in resting. 2. There ware a tendency of higher PPTs in unaffected side than in affected side, but there were no significant difference statistically. 3. There wert no difference of PPTs between in preferred side and in ipsilateral side, significantly. 4. There was a significantly positive correlation between resting and clenching PPTs, but there were no correlation between PPTs and EMG activities. 5. Comparison of EMG activity between in higher group and in lower group by resting PPTs didn't show any significant difference. But resting EMG activity in the higher group of clenching PPTs were lower, on the contrary, clenching activity were higher than those in lower groups of clenching PPTs. From this results, the author concluded that the clenching PPTs was more important than resting PPTs in diagnosis of muscle dysfunction. And the author recommanded that all of three following test were performed for the proper diagnosis and treatment of orofacial Pain : muscle tenderness, EMG activity, and PPT.

Effects of Acupuncture on Upper Back Myofascial Pain and Pain Pressure Threshold (경견부 근막통증증후군에 대한 침치료 효과와 압통역치의 변화)

  • Objective : This study was performed to confirm the effects of acupuncture on myofascial pain syndrome(MPS) through the change of visual analogue scale(VAS) and pain pressure threshold(PPT) and the usefulness of pressure algometer on the evaluation of pain. Methods : We perfomed this study with 20 outpatients complaining of upper back pain. Before acupuncture therapy(AT), immediately after AT and 2-3 days after AT, we respectively checked visual analogue scale(VAS) and pain pressure threshold(PPT) through pressure algometer, with patients seated and relaxed. The PPT was checked at major trigger point of upper trapezius, levator scapulae, supraspinatus, infraspinatus, rhomboideus minor. and the patients were needled at the same points and maintained for 15 minutes. Results : VAS of immediately after AT was mild higher than that of before AT, but not significantly different. and VAS of 2-3 days after AT was significantly lower than before AT and immediately after AT. PPT of immediately after AT was lower than before PT, but not significantly different. PPT of 2-3 days after AT was significantly higher than that of before AT and immediately after AT. Also PPT was significantly correlated with VAS. Conclusion : PPT of omen was signicantly lower than that of men. and there was no significant difference by age. PPT was increased according to pain duration. Effectiveness of acupuncture on myofascial pain syndrome through PPT and VAS is showed at 2-3 days after AT rather than immediatly after AT. and pressure algometer is useful for the evaluation of Acupuncture therapy on myofascial pain syndrome.

  • Objectives : The purpose of this study is to compare interrelations between stress and muscles of neck through stress response inventory(SRI), 7 zone diagnostic system and surface electromyography(sEMG). Methods : This study was carried out with the data from SRI, 7 zone diagnostic system and sEMG. First subjects were divided into two group according to the SRI points. subjects in group A had points of SRI in which lower than 30 points. Subjects in group B had points of SRI in which higher than 30 points. And subjects were divided into nonstress group(Group C) and stress grouop(Group D) according to the result of 7 zone diagnostic system. Then we investigated how to differentiate the muscle contraction, fatigue, recovery and asymmetry ratio on sEMG for each groups. Results : In this study, the muscle contraction of both upper trapezius muscle and left sternocleidomastoid muscle and left scalene muscle in stress group were higher than nonstress group significantly. And the muscle recovery of left sternocleidomastoid muscle and left scalene muscle in stress group were higher than nonstress group significantly. Conclusions : This results show that the stress was associated with muscle condition.

Development of a Remote Shoulder Rehabilitation Exercise Device and Monitoring System (원격 어깨재활 운동 디바이스 및 모니터링 시스템 개발)

  • In this paper, we developed a shoulder rehabilitation exercise device and monitoring system to remotely provide rehabilitation system for the ones who need shoulder exercises including the patients with rotator cuff rupture. In order to evaluate the severity of shoulder muscle injury, a total of 4 shoulder rehabilitation exercises ((3) shoulder abduction, (2) shoulder flexion, (3) shoulder abduction with elbow flexion, (4) shoulder extension with elbow flexion) were selected and instructed to be performed with a 3 kg dumbbell for 5 times. For EMG (electromyogram) signal analysis, each subject's maximum voluntary contraction (MVC) was measured. EMG signals reflect the activation level of contracting muscles during dynamic exercises. Six participants' muscle activation levels in posterior deltoid, middle deltoid, upper trapezius, and infraspinatus were measured and compared. The mean power spectrum values in the time and frequency domains were compared between two age-matched groups (20s and 50s). The results showed lower muscle activation in the elderly subjects (n=3) compared to that of the ones in their twenties (n=3).

Pressure Pain Threshold Measurement Using a Pressure Algometer in Myofascial Pain Syndromes (근근막 통증 증후군에서 Pressure Algometer를 이용한 골격근 압통 역치에 관한 연구)

  • Background: There is no reliable objective test for the diagnosis of myofascial pain syndromes. The aim of this study was to evaluate the usefulness of a pressure algometer for the diagnosis of the trigger points and for the evaluation of the treatment in myofascial pain syndromes (MPS). Methods: Twenty female patients with clinical MPS of shoulder were included in this study. Pressure pain thresholds were measured by a pressure algometer at three different sites including the trapezius, supraspinatus and infraspinatus before, and then the 1st, 3rd and 7th days after TPI. Results: Mean pressure pain thresholds were lower in patients with MPS in than normal volunteers in all the examined skeletal muscles. Mean pressure pain thresholds in patients with MPS were increased significantly after TPI in all the examined skeletal muscles. Conclusions: Pressure algometer can be used as relatively objective diagnostic tool for locating trigger points and to quantify the effect of TPI in MPS. However, more investigation is necessary.

The Study of Isometric Endurance Time by Task Type and Maximum Voluntary Contraction (작업형태 및 최대 수의적 수축에 따른 등척성 근지구력에 관한 연구)

  • This study was performed to investigate the isometric endurance time as percentages of maximum voluntary contraction. Electromyogram(EMG) and Borg's CR-I0 value were measured by push-pull-up-down tasks for 10 healthy males. The normalized EMG value and the MPF(mean power frequency) were used to estimate the muscle recruitment pattern and the development of muscle fatigue. The subjects exerted and maintained 5 levels of %MVC(maximum voluntary contraction) in $90^{\circ}$ shoulder flexion/ 180oelbow extension at sitting posture. The up-task showed the lower endurance time and higher Borg's CR-I0 value than the other task types. Comparing Rohmert's curve with the endurance time of task types. Rohmert's curve overestimated the endurance time of up-task and underestimated the endurance time of push-pull-down tasks. The normalized EMG value showed that muscles recruitment patterns were different from task types. The 4 muscles(biceps brachii muscle, tricep brachii muscle. middle deltoid muscle. trapezius muscle) recruitment patterns of up-task were higher than those of other tasks. The MPF value decreased with the endurance time, and the shift of MPF at up-task was larger than that of the other task types.

The Effect of Sling Exercise Therapy with Vibration Balls on Upper Limb Muscle Activity for Paraplegia-Spinal Cord Injury

  • Objective: The purpose of the present study is to investigate the effect of a muscle activity by applying the complex exercise method of sling in accordance with the provision of various vibration intensities for paraplegia-spinal cord injury. Method: The subjects of the study were 15 men in their 40s and 50s with lower limb disabilities and low potential risk, who were randomly divided into a sling exercise group (SG n=4), a sling with low frequency vibration group (SLVG n=4), a sling with mid-frequency vibration group (SMVG n=4), and a sling with high frequency group (SHVG n=4) in accordance with the provision of slings and vibration stimuli. The vibratory intensity provided was divided into low frequency (30 Hz), mid-frequency (50 Hz), and high frequency (70 Hz). The anterior deltoid (AD), the posterior deltoid (PD), the pectoralis major (PM), the upper trapezius (UT), the latissimus dorsi (LD), and the multifidus (MF) were measured to compare and analyze muscle activity. Results: The closed kinetic chain (CKC) exercise to the shoulder joint showed higher muscle activity in most muscles for the SMVG, and statistically significant differences in the anterior deltoid (AD), the pectoralis major (PM), and the multifidus (MF) in particular. Conclusion: The intermediate frequency (50 Hz) string vibration was the effective vibration stimuli for Closed kinetic chain (CKC) exercises.

Pilot Study of Nurses with VDT Work to Prevent Musculosketal Disorders and Back-Pain Problems (VDT 작업 간호사들의 근골격계질환과 요통 문제예방을 위한 기초연구)

  • Back pain is painful for people at a lower lumber(L4/L5 or L5/S1). It is experienced as a most frequent disaster 70% of workers have suffered from back-pain. Especially, the occurrence rate of back pain for nurses is very high. Therefore, this study investigates female nurses who worked as a part of a medical cost management team, which mainly deal with the medical insurance in a general hospital at Seoul area. These nurses had different job tasks, which used computers for 50% of their time, so it can be treated as VDT workers. As a first step of this project, a muscle fatigue was measured for these special VDT workers. First, survey for nurses was conducted to figure out what is a real problem of them. Second, to evaluate an experimental data, a medical cost management team was chosen for subjects. Areas of measurement were 3 places that were the trapezius (TR), the medial deltoid (MD), and the erector spine muscle (ES: L4/L5). These areas are most frequently used, so they were chosen for this study. Measurements were taken before work and after work. From these measurements, it was revealed that a fatigue really comes from their main VDT task excluding natural fatigue after work, so their work environment need to be corrected.

Analysis of Electromyography in Accordance with Abduction Angle of Shoulder Joint (어깨관절 벌림 시 부하 형태에 따른 근활성도 분석)

  • Purpose : This study according to the angle at the shoulder joint abduction compare muscle activity by analyzing abduction in normal depending on the angle of the shoulder joint which muscles are activated exactly know what its purpose is. Methods : 15 students with a healthy shoulder abduction angles ($45^{\circ}$, $90^{\circ}$) according to the trapezius (upper, midder, lower), infrasupinatus, deltoid, pectoralis major, serratus anterior, latissimus dorsi muscle activity of the were analyzed. How the% MVIC EMG activity of each muscle EMG signals were standardized. Results : The mean age of the study subjects 23.6 years old, and is a key 175.6Cm, weight 70.66Kg respectively. $45^{\circ}$ non-load Pectoralis major, load Deltoid, $90^{\circ}$ non-load Deltoid, Latissimus dorsi load showed the most activity. Conclusion : The purpose of this study the muscle activity of the muscles in order to mobilize the comparison of the active muscles, but the experimenter with a range of individual differences that every time I was able to find the average. Based on these results will be helpful in future studies.
