Early Treatment of Class III Malocclusion (3급 부정교합의 초기치료)

  • The Class III malocclusion classified in two types of Skeletal Class III and Pseudo Class III. In the case of the maxillary deficiency, the protraction H-G(facemask) with Bonded RPE can be used. For children with A-P and vertical maxillary deficiency, the preferred treatment is to move the maxilla into a more anterior and inferior position, which also increases its size as bone is added at the posterior and superior sutures. Successful forward repositioning of the maxilla can be accomplished before age 8. To resist tooth movement as much as possible, the maxillary teeth should be splinted together as a single unit. The maxillary appliance must have hooks for attachment to the facemask that are located in the canine-primary molar area above the occlusal plane. The facemask usually worn until a positive overjet of 2-5mm is achieved interincisally. Occipital chin cup is successful in those patients who can bring their incisors close to an edge-to-edge position when in centric relation. This treatment is particularly useful in patients who begin treatment with a short lower anterior facial height, as this type of treatment can lead to an increase in lower anterior facial height. If the pull of the chin cup is directed below the condyle, the force of the appliance may lead to a downward and backward rotation of the mandible.

A study of Korean Norm about tooth size and ratio in Korean adults with normal occlusion (한국성인 정상교합자의 치아크기와 비율에 관한 연구)

  • The purpose of this study was to measure the average tooth size of Korean with normal occlusion. According to the study, the average tooth ratios between the upper and lower teeth which could assure the proper ovebite, overjet and good interdigitation were calculated. The normal occluson sample of this study consisted of 43 Korean male adults and 51 Korean female adults. Among them, 22 Korean male adults and 51 Korean female adults were from KAO(Korean Association of Orthodontists), 21 Korean male adults were from Department of Orthodontics, College of Dentistry, Yonsei University. The results from this study were as follows : 1 The average tooth size of Korean Norm classified by male and female was measured. 2. The average tooth size of Korean male adults with normal occlusion was significantly larger than that of Korean female adults except upper and lower first molars. (p<0.05) 3. The tooth ratio which could Predict the proper overbite and overjet in anterior teeth and proper occlusion in posterior teeth was calculated. 1) Sum of inciosrs = 4:2.97 2) Neff's anterior coefficient = 1.22 3) Bolton's anterior ratio = 78.29%, overall ratio = 91.14% 4. A positive correlation was observed between the sum of lower anterior incisors and the sum of unilateral canine and premolars In each jaw. Based this correlation, the regression equation was made which could Predict the sum of unerupted unilateral canine and premolars in mired dentition. 1) Sum of unilateral unerupted upper canine and premolars' width = 10.435018 + 0.513346 ${\times}$ (sum or lower 4 incisors' width) 2) Sum of unilateral unerupted lower canine and premolars' width = 9.654002 + 0.502565 ${\times}$ (sum of lower 4 incisors' width)

  • This study was undertaken to evaluate the cephalometric changes of the soft tissue and skeletal profile subsequent 10 the rapid palatal expansion in 25 Angle's Class III cases, ranging in age from six to fifteen years, with cross-bite of the anterior teeth, underdevelopment of maxilla and facial disharmony Following results were obtained: 1. ANS moved downward, Point A presented forward & downward movement increasing SNA and Point B presented backward & downward movement decreasing SNB. 2. Mandible was rotated to backward & forward and maxilla moved forward & downward with the bite opening and improvement of anterior teeth cross-bite. 3. Soft tissue on mandible was rotated to backward & forward following hard tissue changes causing the decrease of facial convexity angle and backward & downward rotation of Point B', Pog'. 4. Response of the upper lip was more significant in downward than forward direction, and correlated with the upper central incisor and mandible rotation. 5. Response of the lower lip was more significant in downward than backward direction, and correlated with the mandible rotation. 6. There was a rather high degree of correlation between skeletal profile and soft-tissue profile, 1 : LS, $\bar{1}$:Pog', Pog:LS, Pog:LI, Pog:Pog' in horizontal measurements and $\bar{1}$:Pog', Pog:LI, Pog:Pog' in vertical measurements.

  • This study was designed to investigate the difference between craniofacial characteristics of the normal occlusion and those of Class II Div. 1 malocclusion. The sample was divided into 2 groups, the 50 subjects of Normal occlusion, the 50 subjects of Class II Div. 1 malocclusion in both sexes. Both groups aged from 11 to 14 years. The results of this study were as follows; 1. No significant difference was observed in cranial base shape between both groups, but anterior cranial base size of Class II Div. 1 malocclusion group was larger than that of normal group. 2. No significant difference in antero-posterior position of Maxilla to cranial base was founded between both groups. 3. No difference in Mandibular shapes and Mandibular plane angles to the cranial base was observed between Class II Div. 1 malocclusion and normal occlusion, but Mandibular position in Class II Div. 1 malocclusion was posterior to that of normal group. 4. Antero-posterior relationship of Maxilla and Mandible was significant difference between both groups, but vertical relationship of those was no difference. 5. Maxillary incisor position to cranial base of Class II Div. 1 malocclusion was anteior to normal occlusion, and Maxillary posterior teeth was posterior. Mandibular incisor and mandibular posterior teeth position was no difference. 6. Upper and lower lip position to esthetic line of Class II Div. 1 malocclusion was anterior to normal occlusion.

Comparison of soft tissue changes between incisor tipping and translation after premolar extraction

  • Objective: This study compared soft tissue changes after extraction of the four premolars followed by maximum retraction of the anterior teeth according to the type of anterior teeth movement: tipping and translation. Methods: Patients who had undergone orthodontic treatment involving the extraction of four premolars were retrospectively selected and divided into either the tipping (n = 27) or translation (n = 26) groups based on the retraction of the incisor root apex and the axis changes of the incisors during the treatment period. Lateral pre- and post-treatment cephalograms were analyzed. Results: There were no significant differences between the tipping and translation groups before treatment. The retraction amounts of the root apex of the upper and lower incisors in the tipping group were 0.33 and 0.26 mm, respectively, and 5.02 and 5.31 mm, respectively, in the translation group (p < 0.001). The posterior movements of soft tissue points A and B in the tipping group were 0.61 and 1.25 mm, respectively, and 1.10 and 3.25 mm, respectively, in the translation group (p < 0.01). The mentolabial sulcus angle increased by 5.89° in the tipping group, whereas it decreased by 8.13° in the translation group (p < 0.001). Conclusions: An increased amount of retraction of the incisor root apex led to the increased posterior movement of soft tissue points A and B, and this appeared more distinct in cases involving the lower incisor and lower lip.

  • Patients who want to reduce their lip protrusion usually estimate the severity of the lip protrusion on the frontal aspect. Most orthodontists have a perplexed experience of a reduced thin line of vermilion border on the frontal aspect as incisors we retracted, even though the lip protrusion is thought to be reduced favorably on the sagittal aspect. Some patients also look older after orthodontic treatment because of severe lip thinning. This unaesthetic reduction of vermilion border urges us to study the vertical lip change during orthodontic procedure. The purpose of this study was to evaluate the vertical lip and perioral soft tissue changes in respect to incisor retraction in an effort to analyze which factors might be responsible for their vertical changes, using the multiple regression analysis. The results were as follows. 1. Upper and lower lip philtrum length(SnLs, LiB') were increased after retraction of anterior teeth, where as upper and lower vermilion height(LsSuls, StmiLi), and vermilion length(LsLi) were decreased. 2. Upper and lower lip length(SnStms, StmiB'), and soft tissue lower anterior facial height(SnMe') did not show any significant difference after treatment. 3. The increase of the upper lip philtrum length was mainly influenced by the extrusion of upper anterior teeth(${\Delta}U1V$), and the increase of the lower lip philtrum length was mainly influenced by the initial overjet before treatment. 4. The decrease of the upper and lower lip vermilion height was mainly influenced by the decrease of upper lip thickness.

  • The purpose of this investigation was to make a comprehensive study and evaluation of the oral hygiene status by considering dental plaque index in 60 children age 7-9. The obtained results were as follows 1) Average plaque index of total mandibular teeth was higher than that of maxillary teeth. (Mandible; 2.14, Maxilla; 1.98) 2) In average plaque index per tooth surface, plaque index of facial surface was higher than that of lingual surface in maxilla and lower in mandible. 3) In mixed dentition, dental plaque occur most frequently and in greater quantity on the buccal surfaces of the maxillary permanent 1st molars and the lingual surfaces of the mandibular anterior permanent incisors.

A Study of the Effect of Fluoride to Dental Hard Tissue (불소가 치아 경조직에 미치는 영향에 관한 연구)

  • Caries free anterior teeth were rinsed for 10 minutes with 1%, 5% sodium fluoride and 1% stannous fluoride solution three times a day for the period of experiment. And the author measured the concentration of enamel fluoride by means of Mc Cann's Technique. And the results were as follows : 1. There were little difference of enamel fluoride concentration between the teeth treated with 5% sodium fluoride solution. 2. The enamel fluoride concentration shows a tendency of direct proportion in lower concentration of fluoride solution for the period of 10 days. 3. The Enamel fluoride concentration in the group treated with stannous fluoride solution reveals acute elevation in the 5th day of experiment but the trend was not direct proportional in the 10th day of experiment.

  • An unfavorable tipping movement can occur during the retraction of anterior teeth because orthodontic force is loaded by brackets positioned far from the center of resistance. To avoid this unfavorable movement, a compensating curved wire or lingual root torque wire is used. The purpose of this study is to investigate, using photoelastic material, the distribution of initial stress associated with the retraction of the incisors according to the degree of the compensating curve, to model changes associated with tooth ud alveolar bone structure. The following results were obtained by analysis of the polarizing plate of the effects of initial stress resulting from retraction of the anterior teeth: 1. When the incisors were retracted using combination archwire or sliding mechanics, the maximal polarizing pattern of the apical area decreased as the degree of the compensating owe increased from 0 to 15 to 30. 2. When the incisors were retracted by the combination archwire or sliding mechanics, the maximal polarizing pattern of the canine and premolar area increased as the degree of the compensating curve increased from 0to 15to 30. 3. A lower degree of polarizing patterns were associated with the combination archwire technique than the sliding mechanics technique at a given force. The above results indicate that there is no significant difference between the combination loop archwire technique and sliding mechanics, for the retraction of maxillary anterior teeth with decreased lingual tipping tendency by a compensating curve on the arch wire. However, the use of sliding mechanics is more effective for the prevention of lingual inclination of the anterior teeth, because the hook used in sliding mechanics is closer to the center of resistance of the maxillary anterior teeth.

A case report of Angle's Class 3 malocclusion (Angle씨 제3급 부정교합의 치험예)

  • 11 years 7 months old female had a reverse object of anterior teeth and class III molar relationship. Maxillary right and left canines were erupted labially and lower left lateral incisor was congenitally missed. She was treated by rapid palatal expansion and full band technique with extraction of maxillary and mandibular second molars. The following results were obtained. 1. Maxilary arch was expanded. 2. Anterior cross-bite and crowding was corrected. 3. Molar relationship was corrected. 4. Profile was somewhat improved.
