저강도 초음파 조사 부위의 골 소실 억제 효과 -골 밀도 및 형태학적 특성 평가 (Suppression of Osteoporotic bone loss on the site to which low Intensity Ultrasound is Irradiated - In vivo test on BMD and Morphological Characteristics)

  • Purpose : The aim of this study is to evaluate a possibility of clinical application for the effects of low intensity ultrasound stimulation (LIUS) in morphological characteristics (i.e., structure, bone mineral density) of bone on osteoporotic fracturesprevention. Materials and Methods : Eight virgin 14-week-old ICR mice (approximate weight 25g) were used and ovariectomized (OVX) to induce osteoporosis. Right tibia (US) for each mouse served as the LIUS (1.5MHz frequency, 1.0 kHz pulse repetition on frequency, $30mW/cm^2$ intensity, $200{\mu}s$ pulse length, and stimulation for 20 minutes a day and 5 days a week over a 6-week period). Left tibia (CON) for each mouse served as the non-stimulated controls. Structural parameters and bone mineral density ($g/cm^3$) on trabecular bone of tibiae were calculated and measured from images derived in-vivo micro computed tomography (micro-CT) at 0 week and after 6weeks. Results : The BV/TV and Tb.N in US group were significantly bigger than those in CON group. The Tb.Pf in US group, moreover, was significantly smaller than that in CON group (p<0.05).For the others structural parameters and BMD, however, there were no significant difference between US group and CON group (p>0.05). Conclusion : The LIUS might prevent bone loss and keep bone connectivity in osteoporotic bones. Therefore, the LIUS might prevent effectively the osteoporotic fractures in clinics.

생화학적 표식자에 따른 성인들의 골밀도 분석 (Analysis of Bone Mineral Density According to the Biochemical Variable Markers in Adults)

  • 성인들의 생화학적 표식자를 검사해 골밀도에 직접 관여할 수 있는 중요한 유전적 또는 생화학적 인자들을 규명하고 성인들의 골다공증 병태생리에 관련된 정보를 제공하고 골다공증의 위험인자 등과의 관련요인을 분석하였다. 2007년 9월 12일부터 2008년 3월 18일까지 대학병원 건강증진센터에 내원한 남성 568명 여성 430명으로 총 998명 수진자를 대상으로 양 에너지 엑스선 골밀도 측정기를 이용하여 대퇴경부 및 척추의 골밀도를 측정하였다. 골밀도에 직접 관여하는 혈액의 생화학적 표식자는 인, 칼슘, 관절염, 헤모글로빈, 글루코오스, 간 기능검사의 GOT, GPT, $\gamma$-GTP를 측정하여 성인들의 골다공증 병태생리에 관련된 정보를 제공하여, 골다공증의 진단과 치료 및 예방에 생화학적 위험인자를 분석하여 관련요인을 조사하였다. 남성은 대퇴경부 및 척추의 골밀도는 20대에서 가장 높았고, 60대에서 대퇴경부와 척추의 골밀도가 가장 낮았다. 여성은 대퇴경부의 골밀도가 20대에서 가장 높았으나 척추는 40대에서 골밀도가 가장 높았고, 60대에서 대퇴경부와 척추의 골밀도가 가장 낮았다. 여성들 중 폐경 후의 여성들보다 폐경 전의 여성들이 골밀도가 유의하게 높았으며, 남성의 생화학적 표식자 인에서 정상 기준치 이상 군에서 대퇴경부와 척추에서 골밀도가 높았으며 여성은 정상치군일 때가 대퇴경부와 척추에서 골밀도가 높게 나타났다. 나이가 많아짐에 따라 골밀도는 낮아지고 체중이 높아지면 골밀도는 높아졌다.

난소 적출 동물모델에서 고흥산 석류 농축액의 갱년기 증상 개선 효과 (Pomegranate Extract Improves Menopausal Syndrome in Ovariectomized Rats)

  • 본 연구는 난소 적출 동물 모델을 이용하여 고흥산 석류 농축액 시험군과 함께 이란산 석류 농축액을 주성분으로 하는 시판 제품의 혈청 생화학적 지표, 골밀도 등 몇 가지 지표를 분석하여 갱년기 증상 개선 효과를 비교하였다. 고흥산 석류 농축액과 시판 제품은 난소 절제 동물 모델의 골밀도(bone mineral density, BMD) 및 percent bone volume(bone volume/tissue volume), bone surface density(bone surface/tissue volume)를 증가시켰으며, 특히 고용량의 고흥산 석류 농축액(PE3)은 통계적으로 유의하게 골밀도 및 골볼륨을 증가시켰다. 시판 제품을 투여한 군도 골밀도를 증가시키는 양상을 보였으나 통계적 유의성은 없어 고흥산 석류 농축액이 이란산 석류 제품보다 골밀도 등 뼈 건강에 관련한 갱년기 증상 개선에 더 효과적이라고 사료된다. 고흥산 석류 농축액은 난소 절제에 의한 체중 증가 및 복부/내장지방 축적을 개선하였으며, 농도 의존적으로 HDL-C(high density lipoprotein cholesterol)는 증가시키고 low density lipoprotein cholesterol은 감소시켰다. 또한 고농도 석류 농축액(PE3)의 HDL-C/total cholesterol이 통계적으로 유의하게 증가하여 고흥산 석류 농축액이 갱년기 여성의 비만 및 혈중지질을 개선할 수 있을 것으로 기대된다. 불안장애 실험에서는 고흥산 석류 농축액 투여로 인해 불안장애가 개선되는 경향을 보였으나 통계적 유의성이 없었고, 이란산 시판 제품 투여 시에는 대조군에 비해 유의하게 개선시키는 것으로 나타났다. 우울증 또한 시판 제품 투여군에서는 대조군에 비해 유의하게 개선 효과를 나타내었으나 고흥산 석류 농축액 투여군에서는 우울증 개선 효과가 확인되지 않았다. 이상과 같은 결과를 통해 고흥산 석류 농축액은 난소를 적출한 rat의 골 소실을 현저히 개선하고 복부/내장지방 및 혈중지질을 농도 의존적으로 개선하여 골다공증, 비만 및 심혈관 질환과 같은 갱년기 증상에 효과가 있음을 확인하였다. 그러나 고흥산 석류 농축액 투여군에서 대표적인 갱년기 증상인 우울증과 불안장애 개선에 유의성 있는 효과가 나타나지 않는 것은 동물실험 모델에서 사람에게 나타나는 다양한 갱년기 증상을 모두 평가하기에 한계가 있는 바, 향후 추가적인 연구를 통해 고흥산 석류 농축액이 갱년기 여성에게 나타나는 여러 가지 증상 중 특정 증상에 대해 선택적으로 효과를 나타내는지에 대해 임상시험을 통해 검증할 필요가 있겠다.

성인초기 여성의 골건강 증진을 위한 스마트폰 앱 개발 및 적용: 예비연구 (Development of a Smartphone Application for Promoting Bone Health in Korean Young Adult Women: A Pilot Study)

  • Purpose: The purpose of this study was to develop a smartphone application and evaluate it's effectiveness and usability for promoting bone health in Korean young adult women. Methods: A smartphone application was developed covering exercise, nutrition, and health habits related to bone health. A pilot testing was conducted using a one-group pre/posttest design. A total of 22 female college students with low bone mass participated in the 8-week use of a pilot testing. Results: The "Strong bone Fit body (SbFb)" application was developed using Android platform. In the pilot testing, bone mineral density, serum CTX, and 25 (OH)Vit.D were significantly decreased. There is no significant change in serum osteocalcin and calcium. Serum phosphorus concentration, nutrient intakes (fiber, VitB6, and pantotenic acid), and all bone composition profiles were significantly increased. Bone-health knowledge was significantly increased but no change in health belief and self-efficacy. Conclusion: The results of this pilot testing suggest the effectiveness of the SbFb application to enhance and promote self-managing abilities for bone health. It can provide continuous feedback and high-quality bone health information. However, some contents and working methods of the application need to be modified and its effectiveness on bone health requires further investigations.

Association of Interleukin 10 Haplotype with Low Bone Mineral Density in Korean Postmenopausal Women

  • Osteoporosis is a disease characterized by exaggerated loss of bone mass, with as much as 50 to 85% of the variation in bone mineral density (BMD) commonly accepted as being genetically determined. Although intensive studies have attempted to elucidate the genetic effects of polymorphisms on BMD and/or osteoporosis in several genes, the genes involved are still largely unknown. The possible associations of genetic variants in five-candidate genes (IL10, CCR3, MCP1, MCP2 and GC) with spinal BMD were investigated in Korean postmenopausal women (n = 370). Fourteen SNPs in five candidate genes were genotyped, and the haplotypes of each gene constructed. The associations of adjusted spinal BMD by age, year since menopause (YSM) and body mass index (BMI), with genetic polymorphisms, were analyzed using multiple regression models. Genetic association analysis of Korean postmenopausal women revealed that IL10 -592A > C and/or IL10 ht2 were associated with decreased bone mass, whereas no significant associations were observed with all polymorphisms in other genes. The levels of spinal BMD in individuals bearing the IL10 -592CC genotype were lower ($0.78{\pm}0.16$) than those in others ($0.85{\pm}0.17$) (P = 0.02), and the BMD of IL10 ht2 bearing individuals were also lower ($0.82{\pm}0.15$) than those in others ($0.85{\pm}0.17$) (P = 0.04). Our results suggest that variants of IL10 might play a role in the decreased BMD, although additional study might need to be followed-up in a more powerful cohort.

생쥐의 골격성장과 Ca대사에 미치는 식이 Ca과 칼슘조절 호르몬의 영향 (Bone Growth and Calcium Metabolism in Mouse Affected by Dietary Calcium and Calcium-regulating Hormone Administration)

  • This study has dealt the effect of Ca regulating hormones and dietary Ca levels on Ca metabolism. Animals(BALB/c mice) were divided into three dietary groups(high and medium Ca and Ca free) and hormones including parathyroid hormone(PTH), calcitonin(CT), cholecalciferol(Vit D) were i.p. injected. After feeding experimental diets for five weeks, mice were anaethetized and sacrificed by heart puncture. We found that femur growth of mouse was slightly increased by high dietary Ca without showing statistical significance comparing to low dietary Ca group. The combination of PTH and CT showed the same effect when dietary Ca was high. At the same time, total mineral retention in bone was most affected by dietary Ca. In general, high Ca diet elevated Ca level in the serum. When dietary Ca was low, PTH stimulated Ca release from the bone into the serum, which was shown to be inhibited by CT treatment. Comparing to the control, PTH, Vit D and CT together tended to inhibit serum Ca level at high and medium dietary Ca. PTH and Vit D inhibited Ca reserve in the liver at all dietary levels of Ca. Both PTH and Vit D stimulated bone Ca retention when dietary Ca was low, but this effect was reversed when dietary Ca was high. When PTH, Vit D and CT were administered together, bone Ca level was greatly enhanced at low dietary Ca than at high dietary Ca, which suggests that these hormonal cooperation is needed for proper bone density maintenance especially when dietary minerals are not sufficient.

여성의 개인적 특성과 생활양식요인을 이용한 골량감소 예측모형 (Prediction Model for Reduced Bone mass in Women using Individual Characteristics & Life Style Factors)

  • This study was carried out to identify the Important modifiable risk factors for reduced bone mass and to construct prediction model which can classify women with either low or high bone mass. Through the literature review, individual characteristics such as age, body weight, height, education level, family history, age of menarche, postmenopausal period, gravity, parity, menopausal status, and breast feeding period were identified and factors of life style such as past milk consumption, past physical activity, present daily activity, present calcium intake, alcohol intake, cigarette smoking, coffee consumption were identified as influencing factors of reduced bone mass in women. Four hundred and eighty women aged between 28 and 76 who had given measurement bone mineral density by dual energy x-ray absortiometry in lumbar vertebrae and femur from July to October, 1997 at 4 general hospitals in Seoul and Pusan were selected for this study. Women were excluded if they had a history of any chronic illness such as rheumatoid arthritis, diabetes mellitus, hyperthroidism, & gastrointestinal disorder and any medication such as calcium supplements, calcitonin, estrogen, thyroxine, antacids, & corticosteroids known affect bone. As a result of these exclusion criteria, four hundred and seventeen women were used for analysis. Multiple logistic regression model was developed for estimating the likelihood of the presence or absence of reduced bone mass. A SAS procedure was used to estimate risk factor coefficient. The results are as follows For lumbar spine, the variables significant were age, body weight, menopause status, daily activity, past milk consumption, and past physical activity(p<0.01), while for femoral Ward's triangle, age, body weight, level of education, past milk consumption, past physical activity(p<0.001). Past physical activity, present daily activity and past milk consumption are the most powerful modifiable predictors in vertebrae and femur among the predictors. When the model performance was evaluated by comparing the observed outcome with predicted outcome, the model correctly identified 74.1% of persons with reduced bone mass and 84.5% of persons with normal bone mass in the lumbar vertebrae and 82.9% of persons with reduced bone mass and 75.0% of persons with normal bone mass in the femoral Ward's triangle. On the basis of these results, a number of recommendations for the management of reduced bone mass may be made : First, those woman who are classified as high risk group of the reduced bone mass in the prediction model should examine the bone mineral density to further examine the usefulness of this model. Second, the optimal amount of milk consumption and a regular weight bearing exercise in childhood, adolescence, and early adult should be ensured.

The effect of low-dose intravenous bisphosphonate treatment on osteoporosis in children with quadriplegic cerebral palsy

  • Purpose: Quadriplegic children with cerebral palsy are more susceptible to osteoporosis because of various risk factors that interfere with bone metabolism. Pamidronate is effective for pediatric osteoporosis, but there are no guidelines for optimal dosage or duration of treatment in quadriplegic children with osteoporosis. We aimed to evaluate the efficacy of low-dose pamidronate treatment in these patients. Methods: Ten quadriplegic patients on antiepileptic drugs (6 male, 4 female patients; mean age, $10.9{\pm}5.76years$), with osteoporosis and gross motor function classification system level V, were treated with pamidronate (0.5-1.0 mg/kg/day, 2 consecutive days) every 3-4 months in a single institution. The patients received oral supplements of calcium and vitamin D before and during treatment. The lumbar spine bone mineral density (BMD) z score and biochemical markers of bone metabolism were measured regularly during treatment. Results: The main underlying disorder was perinatal hypoxic brain damage (40%, 4 of 10). The mean cumulative dose of pamidronate was $4.49{\pm}2.22mg/kg/yr$, and the mean treatment period was $10.8{\pm}3.32months$. The BMD z score of the lumbar spine showed a significant increase from $-4.22{\pm}1.24$ before treatment to $-2.61{\pm}1.69$ during treatment (P=0.008). Alkaline phosphatase decreased during treatment (P=0.037). Significant adverse drug reactions and new fractures were not reported. Conclusion: Low-dose pamidronate treatment for quadriplegic children with cerebral palsy increased lumbar BMD and reduced the incidence of fracture.

Can denosumab be a substitute, competitor, or complement to bisphosphonates?

  • Osteoblasts, originating from mesenchymal cells, make the receptor activator of the nuclear factor kappa B ligand (RANKL) and osteoprotegerin (OPG) in order to control differentiation of activated osteoclasts, originating from hematopoietic stem cells. When the RANKL binds to the RANK of the pre-osteoclasts or mature osteoclasts, bone resorption increases. On the contrary, when OPG binds to the RANK, bone resorption decreases. Denosumab (AMG 162), like OPG (a decoy receptor), binds to the RANKL, and reduces binding between the RANK and the RANKL resulting in inhibition of osteoclastogenesis and reduction of bone resorption. Bisphosphonates (BPs), which bind to the bone mineral and occupy the site of resorption performed by activated osteoclasts, are still the drugs of choice to prevent and treat osteoporosis. The merits of denosumab are reversibility targeting the RANKL, lack of adverse gastrointestinal events, improved adherence due to convenient biannual subcutaneous administration, and potential use with impaired renal function. The known adverse reactions are musculoskeletal pain, increased infections with adverse dermatologic reactions, osteonecrosis of the jaw, hypersensitivity reaction, and hypocalcemia. Treatment with 60 mg of denosumab reduces the bone resorption marker, serum type 1 C-telopeptide, by 3 days, with maximum reduction occurring by 1 month. The mean time to maximum denosumab concentration is 10 days with a mean half-life of 25.4 days. In conclusion, the convenient biannual subcutaneous administration of 60 mg of denosumab can be considered as a first-line treatment for osteoporosis in cases of low compliance with BPs due to gastrointestinal trouble and impaired renal function.

Prevalence of osteoporosis according to nutrient and food group intake levels in Korean postmenopausal women: using the 2010 Korea National Health and Nutrition Examination Survey Data

  • BACKGROUND/OBJECTIVES: Women's bone health status is closely related with environmental factors and lifestyle factors. The purpose of this study is to evaluate the dietary risks of osteoporosis and osteopenia for Korean postmenopausal women. SUBJECTS/METHODS: The data from 1,433 subjects from the 2010 KNHANES were used and divided into three groups: normal, osteopenia, and osteoporosis group using bone mineral density (BMD). Nutrient intakes and food intake frequency were evaluated. Logistic regression analysis was applied to determine the odds ratios for osteoporosis and osteopenia. RESULTS: The RNI percentage of each nutrient and food intake frequency from 12 food groups decreased as bone mineral density status deteriorated. Risk for osteoporosis of low calcium (Ca) intake, under the EAR, showed an odds ratio of 2.13(95% CI; 1.26-3.61, P < 0.05). Higher intake frequency showed preventive effect from osteoporosis compared to lower intake frequency in such food group as dairy products (ORs 0.40, CI 0.21-0.75), beans (ORs 0.49, CI 0.29-0.83), seaweeds (ORs 0.55, CI 0.32-0.94), fish (ORs 0.56, CI 0.32-0.98), and fruits (ORs 0.42, CI 0.23-0.79) after adjusting for age. CONCLUSION: To prevent osteoporosis in later life, sufficient Ca intake and more frequent intakes of foods containing Ca such as dairy products, beans, fish, seaweeds, and fruits, which help in Ca absorption, should be stressed for Korean postmenopausal women.
