We know that social psychologic factors important to make a decision on characteristic of back pain and prognosis. These facters have difficult to diagnosis of back pain and being protract the treatment session and which bring about a social problem caused the impairment compensation. Clinically, twenty low back pain patients, 8 organic pain group and 12 functional pain group have completed the MMPI(Minnesota Multiphasic Personality Inventory) and the results were as follows: l)Mean MMPI T-score of Hypochondriasis was 56.60(12.39, that of Depression was 57.95(7.71, and that of Hysteria was 57.50(7.15. and those of other scales were within normal range near to 50. 2)there was no significant difference between male and female groups on MMPI scales 3)The MMPI T-score of organic group was significantly higher than that of functional group on Hypochondriasis and Hysteria scale.
Objectives : The purpose of this study is to know that Beck Depression Inventory(BDI) score to effect on improvement degree of low back pain after oriental medical treatment. Methods : We analyzed charts of 33 inpatients who were treated in Ja-Seng Hospital of Oriental Medicine and Bu-Cheon Ja-seng Hospital of Oriental Medicine. Subject is divided into Group A(BDI<10) and Group B(BDI${\geqq}$10). Numerical Rating Scale(NRS) and Oswestry Disability Index(ODI) were checked on admission, after 1 week, NRS and ODI were rechecked to estimate the efficacy of treatments. Results : 1. In both groups, NRS was significantly decreased(p<0.05). Analyzed result of the interaction between the two groups didn't show significant difference(p=0.197). 2. In Group A, ODI was significantly decresed(p<0.05). In Group B, ODI wasn't significantly decreased(p=0.058). The interaction analysis between the two groups didn't show significant difference(p=0.173). 3. Analyzing patients with chronic back pain longer than 6 months by the presence of depression tendency and NRS, ODI decrease after 1 week of ambulation, while the NRS reduction showed relevancy by the depression tendency (p<0.05), the ODI reduction didn't(p=0.272). Conclusions : 1. Although improvement of pain before and after the treatment and degree of functional recovery in high-level BDI group was less than low-level BDI group, but it was stastically insignificant. 2. High BDI score of patients with chronic low back pain is the factor of disturbing pain improvement.

Objectives : The purpose of this study is to compare the effects of Pharmacopuncture Treatment with Chuna Treatments for Low Back Pain caused by Traffic accidents. Methods : This study was carried out on 81 patients with low back pain caused by Traffic accidents. The patients were divided into 2 group ; The pharmacopuncture(A) group was treated by pharmacopuncture treatment. The chuna(B) group was treated by chuna treatment. We measured Oswestry Disability Index(ODI) and Visual Analog Scale(VAS) on the patients before the treatment and on the 2nd, and 4th weeks after the treatments. Results : After being treated by our methods, the ODI and VAS score's were improved after the 4th weeks treatment in both groups. There is no significant difference on ODI and VAS score after the 4th weeks treatment in both groups. In the early stages(from pre to 2 weeks worth of treatment), Group A showed a decreasing VAS score compared to Group B. In the final stages(from 2 weeks to 4 weeks worth of treatment), Group B showed a higher decreasing amount compared to Group A in VAS score. Conclusions : The results suggest that both pharmacopuncture treatment and chuna treatment is considered to be effective and useful on low back pain caused by traffic accidents. There is no significant difference between pharmacopuncture treatment and chuna treatment for low back pain caused by traffic accidents, however the early stages of treatments(from pre to 2 weeks treatement) show that pharmocopouncture treatment is more effective than chuna treatment for low back pain by traffic accidents. In the final stages(from 2 weeks to 4 weeks treatment), chuna treatment is more effective than pharmacopuncture treatment for low back pain by traffic accident.
Objective : This study is designed in order to evaluate the therapeutic effect of venesection on acute low back pain. Methods : From Nov. 1st 2003 to Apr. 30th 2004, 40 cases of low back pain patients at acute stage were divided into 2 groups; one group(control group) took only acupuncture therapy, and the other group(test group) took acupuncture and venesection therapy. Pain rating score(PRS) was used to assess the pain for its intensity, frequency, duration, and aggravating factors. And visual analog scale(VAS) was used, too. PRS and VAS was checked twice, before the treatment and after the treatment. Treatment was done twice a day. Results : Both acupuncture therapy and acupuncture with venesection therapy showed good effect on low back pain of acute stage. And the latter group showed better effect on decreasing pain than former. It was proved by the difference between PRS and VAS checked before treatment and what checked after treatment. But it was not proved statistically. Conclusion : Only acupuncture and acupuncture with venesection can be recommended as a useful therapy to treat low back pain of acute stage.
Purpose : The purpose of this study was to find out The effect of Low back pain on the total pattens of Proprioceptive neuromuscular facilitation(PNF). Methods : The subjects of the study were 62 low back pain patients. They were divided into two groups: 31 in the experimental group and 31 in the control group. the experimental group performed modalities and PNF total patterns three times for a week. the control group performed only modalities and ROM exercise three times for a week. Back muscle strength was measured by a back muscle strength measuring machine, the intensity of pain was measured by the Visual Analogue Scale (VAS), and the level of disability was measured by Oswestry low back pain disability index. Study measurements were compared before and after 6 weeks exercise program. The two groups of subjects were assessed by utilizing two different balance measurement: Static standing balance was measured by balance performance monitor (BPM) and Dynamic standing balance was measured by one leg standing(OLS). The scale for static standing balance was measured by using, sway area, sway path, max velocity. Results : The results of this study were as follow: 1) The score on visual analogue scale shows statistically significant increase on PNF group of post test(p<.05). 2) The score on Oswestry low back pain disability index. shows statistically significant decrease on PNF group of post test(p<.05). 3) The change sway area was statistically significant on pre-test and post-test(p<.05). 4) The change sway path was statistically significant on pre-test and post-test(p<.05). 5) The change max velocity was statistically significant on pre-test and post-test(p<.05). 6) The score on one leg standing shows statistically significant increase on PNF group of post test(p<.05). Conclusion : These results of this study indicated that Proprioceptive Neuromuscular Facilitation Total patterns which performed for six weeks had a statistically significant influence on low back pain. If the exercise for muscle strength is performed along with therapeutic stabilizing exercise, a better effect can be expected on low back pain. We hope that this study will provide a basic data for further research with a bigger group and on a long-term effect.
Background: To evaluate the effects of Australian stabilization exercise for chronic low back pain. Methods: Interventions consisted of exercises aimed at recovery of Multifidus in cocontraction with Transverse abdominis through neural control retraining. Results: After exercise, there were significant improvement(p<.05) in pain and disability score. Conclusions: Neural control stabilization exercise can be effective intervention for chronic Low Back Pain patients.
Purpose: The purpose of this study was to identify the uses of complementary and alternative therapies(CAT), pain, and quality of life(QOL) in patients with chronic back pain. Method: A descriptive survey was conducted using convenient sample. One hundred sixty two patients with chronic back pain participated in this study. Types and satisfaction of CAT, pattern of back pain, and QOL were investigated. Result: 83.3% of all participants reported in the use of CAT. The most common types of CAT used were acupuncture and half-bath. Yoga were the most high satisfaction among the types. The mean score of back pain was 4.12, QOL was 3.10, that was mostly low score. There was no significant difference back pain and QOL between CAT user and nonuser. Correlation with satisfaction of CAT according the most using types were a negative correlation with back pain and a positive correlation with QOL. Conclusion: These result indicate that the mostly patients with back pain feels the satisfaction of CAT use. It is important that provide the obvious information about CAT which can be helpful to patient.

Background: Insomnia is becoming increasingly recognized as a clinically important symptom in patients with chronic low back pain (CLBP). In this retrospective study, we have determined risk factors associated with clinical insomnia in CLBP patients in a university hospital in Korea. Methods: Data from four-hundred and eighty one CLBP patients was analyzed in this study. The Insomnia Severity Index (ISI) was used to determine the presence of clinical insomnia (ISI score ${\geq}15$). Patients' demographics and pain-related factors were evaluated by logistic regression analysis to identify risk factors of clinical insomnia in CLBP. Results: It was found that 43% of patients reported mild to severe insomnia after the development of back pain. In addition, 20% of patients met the criteria for clinically significant insomnia (ISI score ${\geq}15$). In a stepwise multivariate analysis, high pain intensity, the presence of comorbid musculoskeletal pain and neuropathic pain components, and high level of depression were strongly associated with clinical insomnia in CLBP. Among these factors, the presence of comorbid musculoskeletal pain other than back pain was the strongest determinant, with the highest odds ratio of 8.074 (95% CI 4.250 to 15.339) for predicting clinical insomnia. Conclusions: Insomnia should be addressed as an integral part of pain management in CLBP patients with these risk factors, especially in patients suffering from CLBP with comorbid musculoskeletal pain.
The purpose of this study was to examine the effect of low back pain relieving program on back muscle strength, intensity of pain, low back disability level in elementary school teachers who have low back pain. subjects were elementary school women teachers who worked at eight elementary school located in Seoul. Intended subjects size were seventy consist of thirty-four experimental group(three schools) and thirty-six control group(five schools), but actual subjects size was forty-four. Among the forty-four patients subjects, twenty-three were experimental group receiving health education about right postures, etiologies of low back pain, diagnosis of low back pain and exercise program composed of muscle strengthening exercise, stretching exercises and twenty-one were control group. During the 8 weeks program, the subjects were received two times education and six times group exercise practices in 1st week and three times per week group exercise practices, two times education in other 7 weeks. This study as carried out from April 1, 1999 to June 30, 1999. Back muscle strength was measured by back muscle strength measuring machine and the intensity of pain were measured by the Visual Analogue Scale(VAS), and level of disability was measured by Oswestry low back pain disability scale. Study measurements were taken before and after 8 week exercise program. Data were analyzed using paired t-test, and ANCOVA. The results were summarized as follows. 1. After low back relieving program, back muscle strength was increased significantly(p=0.000) and there was significant difference in back muscle strength change between experimental group and control group(p=0.002). 2. After low back pain relieving program, pain on anterior bending, pain on posterior bending were decreased significantly than measurements before the program(p=0.000 p=0.000) and there was significant difference in pain on anterior bending and posterior bending change between experimental group and control group(p=0.000, p=0.000). 3. After low back pain relieving program, Oswestry disability scale scores were decreased significantly(p=0.000, p=0.000) but there was no significant difference in Oswestry disability score change between experimental group and control group.
Recently, some authors reported that discogenic low back pain should be regarded as a referred pain in respect of neural pathway. The afferent pathways of discogenic low back pain is transmitted mainly by sympathetic afferent fibres from the sinuvertebral nerves in the second lumbar nerve root. This pain arises from the lumbar intervertebral discs, and it had been transmitted mainly through the sympathetic afferent fibres contained in the second lumbar spinal nerve root. Second lumbar dermatome corresponds to the low back area. We experienced a case of low back pain which could not be controlled by conventional therapy and progressed wax and wane. The CT finding showed bulging disc between $L_4$ and $L_5$ and spinal stenosis in $L_4$ area. And epiduroscopic feature showed severe adhesion in $L_4$, $L_5$ and $S_1$. After we blocked $L_2$ root, pain score decreased 10 to 2. Therefore, the $L_2$ root block may be a useful diagnostic procedure as well as provide therapeutic value.
