Low-dose intravenous ketamine versus intravenous ketorolac in pain control in patients with acute renal colic in an emergency setting: a double-blind randomized clinical trial

  • Background: This study was conducted to compare the effectiveness of low-dose ketamine versus ketorolac in pain control in patients with acute renal colic presenting to the emergency department (ED). Methods: This is a double-blind randomized clinical trial. The initial pain severity was assessed using the numerical rating scale (NRS). Then, ketamine or ketorolac was administered intravenously at a dose of 0.6 mg/kg and 30 mg respectively. The pain severity and adverse drug reactions were recorded 5, 15, 30, 60, and 120 min thereafter. Results: The data of 62 subjects in the ketamine group and 64 patients in the ketorolac group were analyzed. The mean age of the patients was $34.2{\pm}9.9$ and $37.9{\pm}10.6\;years$ in the ketamine and ketorolac group, respectively. There was no significant difference in the mean NRS scores at each time point, except for the 5 min, between the two groups. Despite a marked decrease in pain severity in the ketamine group from drug administration at the 5 min, a slight increase in pain was observed from the 5 min to the 15 min. The rate of adverse drug reactions, including dizziness (P = 0.001), agitation (P = 0.002), increased systolic blood pressure (> 140 mmHg), and diastolic blood pressure (> 90 mmHg) was higher in the ketamine group. Conclusions: Low dose ketamine is as effective as ketorolac in pain management in patients with renal colic presenting to the ED. However, it is associated with a higher rate of adverse drug reactions.

저 선량 베타선의 조사에 의한 식물의 생장에 미치는 영향 (Influences of the Plant Growth under Beta-Rays Irradiation at Low Dose)

  • 본 연구는 저 선량 베타선의 장기간(31일) 조사에 의한 춘향이 열무와 알타리 무의 생장에 미치는 영향을 분석하는데 목적이 있다. 실험은 알타리 무와 춘향이 열무에 대하여 각각 1개 샘플의 대조군과 11개 샘플의 실험군으로 구성하였다. 대조군과 실험군의 종자들은 각각의 샘플에서 8개씩 선택되어 같은 조건의 배양토에 파종하였다. 실험군 샘플의 누적선량은 31일 동안 매일 같은 시간에 측정하였다. 생장과정과 발아율은 매주 2회 같은 시간에 측정하였다. 잎사귀 수, 초장의 길이와 생체중은 파종 후 20일과 25일에 각각 측정하여 평균값을 얻었다. 25일 동안 측정된 실험군의 결과에서, 알타리 무의 길이와 무게는 누적선량 0.01 Gy에서 대조군보다 각각 5%와 36%로 증가하였다. 또한 춘향이 열무에서 길이는 누적선량 0.01~0.08 Gy와 0.3 Gy에서 대조군보다 각각 13~17%와 1%, 무게는 누적선량 0.05 Gy와 0.23 Gy에서 대조군보다 각각 36%와 2%로 증가하였다. 잎 수에 대하여, 알타리 무는 누적선량 0.01~0.32 Gy에서 대조군보다 0~50%로 증가하였다. 그리고 춘향이 열무는 0.01~0.0.62 Gy에서 대조군보다 0~67%로 증가하였다. 이 결과로부터, 알타리 무와 춘향이 열무는 대체적으로 낮은 누적선량영역(0.01~0.2 Gy)에서 길이와 무게 그리고 잎 수가 증가하였다. 그리고 실험군의 세포의 크기, 핵의 위치와 세포의 조밀도는 대조군과 거의 유사하게 현미경으로 관찰되었다. 결론적으로, 베타선에 조사된 알타리 무와 춘향이 열무는 방사선 호메시스 이론과 부합되어 낮은 누적선량 영역에서 더 많은 생장을 한 것으로 추정된다. 추후 연구에서 방사선 호메시스와 식물생장의 관련성을 확증하기 위한 추가적인 실험이 필요할 것으로 생각된다.

딥러닝 알고리즘을 이용한 저선량 디지털 유방 촬영 영상의 복원: 예비 연구 (Radiation Dose Reduction in Digital Mammography by Deep-Learning Algorithm Image Reconstruction: A Preliminary Study)

  • 목적 깊은 컨볼루션 신경망 기법을 결합한 영상 잡음 제거 알고리즘을 개발하고 이를 응용하여 저선량 유방 촬영 영상으로 유방암을 진단하는 데 그 효능을 조사하고자 한다. 대상과 방법 6명의 유방 영상 전문의가 전향적 연구에 참여하였다. 모든 영상 전문의는 병변 감지를 위해 저선량 영상을 독립적으로 평가하고 정성적 척도를 사용하여 진단 품질을 평가하였다. 영상 잡음 제거 알고리즘을 적용한 후, 동일한 영상 전문의가 병변 감지 가능성과 영상 품질에 대한 평가를 하였다. 임상 적용을 위해 동일한 영상 전문의가 병변 유형과 위치에 대한 합의 결정 후, 저선량 영상, 재구성된 영상, 기존 선량 영상을 무작위 순서로 제시하여 평가하였다. 결과 전 절제 표본의 저선량 영상을 참조로 40% 재구성된 영상에서 병변이 더 잘 인식되었다. 임상 적용단계에서 40% 재구성된 영상과 비교하여, 기존 선량 영상이 해상도(p < 0.001), 석회에 대한 진단 품질(p < 0.001), 유방 종괴, 비대칭, 구조왜곡의 진단 품질(p = 0.037)에 대해 더 높은 평균값을 보였다. 40% 재구성된 영상은 100% 영상과 비교 시 전반적 화질(p = 0.547), 병변의 가시성(p = 0.120), 대조도(p = 0.083)에서 비슷한 성적을 보였으며 유의미한 차이도 보이지 않았다. 결론 깊은 컨볼루션 신경망 기법을 결합한 효과적인 잡음 제거 및 영상 재구성 처리 알고리즘은 유방 촬영의 상당한 선량 감소를 위한 길을 열어 유방암 진단을 가능하게 할 것이다.

Reduced Ovarian Cancer Incidence in Women Exposed to Low Dose Ionizing Background Radiation or Radiation to the Ovaries after Treatment for Breast Cancer or Rectosigmoid Cancer

  • Background: High dose ionizing radiation can induce ovarian cancer, but the effect of low dose radiation on the development of ovarian cancer has not been extensively studied. We evaluated the effect of low dose radiation and total background radiation, and the radiation delivered to the ovaries during the treatment of rectosigmoid cancer and breast cancer on ovarian cancer incidence. Materials and Methods: Background radiation measurements are from Assessment of Variations in Radiation Exposure in the United States, 2011. Ovarian cancer incidence data are from the Centers for Disease Control and Prevention. Standardized incidence ratios (SIR) of ovarian cancer following breast cancer and rectosigmoid cancer are from Surveillance, Epidemiology, and End Results (SEER) data. Obesity data by US state are from the Centers for Disease Control and Prevention. Mean ages of US state populations are from the United States Census Bureau. Results: We calculated standardized incidence ratios (SIR) from Surveillance, Epidemiology, and End Results (SEER) data, which reveal that in 194,042 cases of breast cancer treated with beam radiation, there were 796 cases of ovarian cancer by 120+ months of treatment (0.41%); in 283, 875 cases of breast cancer not treated with radiation, there were 1,531 cases of ovarian cancer by 120+ months (0.54%). The difference in ovarian cancer incidence in the two groups was significant (p < 0.001, two tailed Fisher exact test). The small dose of scattered ovarian radiation (about 3.09 cGy) from beam radiation to the breast appears to have reduced the risk of ovarian cancer by 24%. In 13,099 cases of rectal or rectosigmoid junction cancer treated with beam radiation in the SEER data, there were 20 cases of ovarian cancer by 120+ months of treatment (0.15%). In 33,305 cases of rectal or rectosigmoid junction cancer not treated with radiation, there were 91 cases of ovarian cancer by 120+ months (0.27%). The difference in ovarian cancer incidence in the two groups was significant (p = 0.017, two tailed Fisher exact test). In other words, the beam radiation to rectum and rectosigmoid that also reached the ovaries reduced the risk of ovarian cancer by 44%. In addition, there was a significant inverse relationship between ovarian cancer in white women and radon background radiation (r = - 0.465. p = 0.002) and total background radiation (r = -0.456, p = 0.002). Because increasing age and obesity are risk factors for ovarian cancer, multivariate linear regression was performed. The inverse relationship between ovarian cancer incidence and radon background was significant (${\beta}=-0.463$, p = 0.002) but unrelated to age (${\beta}=-0.080$, p = 0.570) or obesity (${\beta}=-0.180$, p = 0.208). Conclusions: The reduction of ovarian cancer risk following low dose radiation may be the result of radiation hormesis. Hormesis is a favorable biological response to low toxin exposure. A pollutant or toxin demonstrating hormesis has the opposite effect in small doses as in large doses. In the case of radiation, large doses are carcinogenic. However, lower overall cancer rates are found in U.S. states with high impact radiation. Moreover, there is reduced lung cancer incidence in high radiation background US states where nuclear weapons testing was done. Women at increased risk of ovarian cancer have two choices. They may be closely followed (surveillance) or undergo immediate prophylactic bilateral salpingo-oophorectomy. However, the efficacy of surveillance is questionable. Bilateral salpingo-oophorectomy is considered preferable, although it carries the risk of surgical complications. The data analysis above suggests that low-dose pelvic irradiation might be a good third choice to reduce ovarian cancer risk. Further studies would be worthwhile to establish the lowest optimum radiation dose.

Small sized lung SBRT 치료시 폐 실질 조직에서의 계획선량 전달 정확성 평가 (Evaluation of beam delivery accuracy for Small sized lung SBRT in low density lung tissue)

  • 목 적: 본 연구에서는 lung SBRT가 적용되는 작은 계획 표적 용적(PTV)에 처방 선량이 정확히 전달되는지 실험을 통하여 알아보고자 한다. 치료계획 시스템에서 계산된 선량분포와 실험을 통하여 필름에 측정된 선량분포를 비교 분석하여 정확성을 평가해보고, 폐 실질 조직 내에서 계획 표적 용적의 margin 유용성 평가를 하고자 한다. 대상 및 방법: CT 촬영으로 얻은 Rando phantom 3D 영상의 우측 폐에 직경 2, 3, 4, 5 cm인 가상의 구 표적을 만들어 계획 표적 용적에 처방선량의 95 %가 전달될 수 있도록 6MV-FFF VMAT Arc 2개로 치료계획을 수립하였으며, Eclipse TPS와 동일한 위치에서 선량 비교하기 위해서, 필름을 가상 표적의 회전중심점에 횡단면 방향으로 삽입하고 방사선을 조사하였다. Dose profile을 Eclipse에서 획득하고, 측정값과 계산값을 비교하기 위해 Center point에서의 절대 선량값을 계산하였으며, off-axis 선량 분포를 얻어 RMSE, Coverage ratio 등 비교 인자를 통해 상대 선량 및 선량분포를 비교 분석하였다. 결 과: 직경 2, 3, 4, 5 cm 크기별로 center point에서의 %difference 값은 직경 2 cm에서 -4.65 %로 가장 차이가 큰 값을 보였고, 직경 5 cm일 때 -1.46 %로 가장 차이가 작은 값을 보였다. RMSE값은 직경 2 cm일 때 3.43으로 가장 큰 값을 보였고. 직경 5 cm일 때 2.85로써 가장 작은 값을 보였다. 표적 커버리지를 비교하기 위해 처방선량 95 %가 들어가는 용적의 길이($D_{95}$)를 구하였고, 직경 2 cm일 때, TPS와 필름에서 각각 2.02 cm, 1.86 cm로 커버리지 비율이 92 %로 나타났고 가장 큰 차이를 보였다. 또한 계획 표적 용적 100% 이내에 들어가는 평균선량($D_{mean}$)을 비교했을 때, 직경 2 cm 인 경우 측정 평균선량이 95.72 %로 가장 낮은 값을 보였다. 결 론: 본 연구에서는 실험을 통하여 작은 계획 표적 용적에 처방 선량이 충분히 전달되는지 알아보았다. 실험 결과 모든 비교 인자에서 직경 2 cm인 용적이 가장 큰 차이를 보였다. 이는 표적 용적 중심에서의 선량 감소가 주요인이라 판단된다. 따라서 선량계산 시스템에서 저밀도 조직 내의 작은 용적 치료 계획시 2 mm 이상의 마진(margin)을 더 두거나, 치료 계획 최적화(optimization)시 최대선량을 제한하지 않는 방법으로 표적 내 중심 선량을 높일 수 있을 것이라 사료된다.

Risk Assessment from Heterogeneous Energy Deposition in Tissue. The Problem of Effects from Low Doses of Ionizing Radiation

  • Low doses of ionizing radiation from external or internal sources cause heterogeneous distribution of energy deposition events in the exposed biological system. With the cell being the individual element of the tissue system, the fraction of cells hit, the dose received by the hit, and the biological response of the cell to the dose received eventually determine the effect in tissue. The hit cell may experience detriment, such as change in its DNA leading to a malignant transformation, or it may derive benefit in terms of an adaptive response such as a temporary improvement of DNA repair or temporary prevention of effects from intracellular radicals through enhanced radical detoxification. These responses are protective also to toxic substances that are generated during normal metabolism. Within a multicellular system, the probability of detriment must be weighed against the probability of benefit through adaptive responses with protection against various toxic agents including those produced by normal metabolism. Because irradiation can principally induce both, detriment and adaptive responses, one type of affected cells may not be simply summed up at the expense of cells with other types of effects, in assessing risk to tissue. An inventory of various types of effects in the blood forming system of mammals, even with large ranges of uncertainty, uncovers the possibility of benefit to the system from exposure to low doses of low LET radiation. This experimental approach may complement epidemiological data on individuals exposed to low doses of ionizing radiation and may lead to a more rational appraisal of risk.

저 선량 전산화단층촬영의 관전압과 적응식 통계적 반복 재구성법 적용에 따른 영상평가 및 피폭선량 (Image Evaluation and Exposure Dose with the Application of Tube Voltage and Adaptive Statistical Iterative Reconstruction of Low Dose Computed Tomography)

  • 저 선량 흉부 전산화단층촬영(low dose computed tomography; LDCT)검사 시 기존의 검사방법인 필터보정역투영법인 FBP(filted back projection)와 적응식 통계적 반복 재구성법인 ASIR(adaptive statistical iterative reconstruction)의 적용 및 관전압 변화에 따른 영상의 화질과 피폭선량을 비교 평가해 보고자 하였다. 흉부 phantom을 이용하여 재구성방법에 따라 FBP와 ASIR적용(10%, 20%)을 하였고, 관전압(100kVp, 120kVp)에 변화를 주어 실험을 하였다. 화질평가를 위해 back-ground noise와 signal-noise ratio(SNR), contrast-noise ratio(CNR)를 구하였으며, 선량평가를 위해 CTDIvol과 DLP를 구하였다. 화질평가에 있어 kVp에 따른 ascending aorta(AA) SNR과 inpraspinatus muscle(IM) SNR은 AA SNR과 IM SNR은 유의한 차이가 있었다(p < 0.05). 선량평가에 있어 CTDIvol과 DLP는 유의한 차이가 있었으며(p < 0.05), CTDIvol은 120 kVp, FBP가 2.6 mGy, 120 kVp, 10%-ASIR가 2.38 mGy, 120kVp, 20%-ASIR가 2.17 mGy로 0.43 mGy 감소하였고, 100 kVp, FBP가 1.61 mGy, 100 kVp, 10%-ASIR가 1.48 mGy, 100 kVp, 20%-ASIR가 1.34 mGy로 0.27 mGy 감소하였다. 또한 DLP에서는 120 kVp, FBP가 $103.21mGy{\cdot}cm$, 120 kVp, 10%-ASIR가 $94.57mGy{\cdot}cm$, 120 kVp, 20%-ASIR가 $85.94mGy{\cdot}cm$$17.27mGy{\cdot}cm$(16.7%) 감소하였고, 100 kVp, FBP가 $63.87mGy{\cdot}cm$, 100 kVp, 10%-ASIR가 $58.54mGy{\cdot}cm$, 100 kVp, 20%-ASIR가 $53.25mGy{\cdot}cm$$10.62mGy{\cdot}cm$(16.7%)로 감소하였다. 재구성방법에 따른 FBP와 ASIR 10%, 20%에서는 화질의 변화 없이 선량을 줄일 수 있어 흉부 low dose CT검사 시 ASIR 20%적용하여 검사하는 것이 좋으며, 관전압 변화에 따른 120 kVp와 100 kVp에서는 선량은 크게 줄어들었지만, noise가 증가하여 화질이 떨어지는 것으로 나타났다.

Toxicity of Novel Solubilizer of Paclitaxel, Aceporol 330, in Beagle Dogs

  • In order to develop an improved paclitaxel formulation vehicle, a micelle forming solubilizer, Aceporol 330 was synthesized. It was previously reported that Aceporol 330 provided the linearity of paclitaxel plasma pharmacokinetics. In this study, the single dose toxicity test and 2-week repeated dose toxicity test of Aceporol 330 was performed in beagle dogs after intravenous administration. Single dose and 2-week repeated dose toxicity test of Aceporol 330 showed fever/generalized erythema, severe vomiting, and diarrhea in beagle dogs. However, those toxicities were less severe than those of Cremophor EL. Blood chemistry analysis of 2-week repeatedly treated beagle dogs with Aceporol 330 showed significant elevation of total cholesterol (TCHO) and triglyceride (TG) compared to that of control group. Cremophor EL also significantly increased total cholesterol (TCHO) and triglyceride (TG) as much as Aceporol 330. Results from this study indicated that Aceporol 330 was less toxic than Cremophor EL. Based on the pharmacokinetic advantages and the low toxicity of Aceporol 330 in single dose and 2-week repeated dose toxicity test, Aceporol 330 has a potential for use as a safer solubilizer for paclitaxel than Cremophor EL.

전자선을 이용한 하수처리장 방류수내 대장균군 살균 (Disinfection of Total Coliforms in Sewage Treatment Effluent using Electron Beam)

  • The use of electron beam irradiation was investigated to disinfect total coliforms in the secondary sewage treatment effluent. Unchlorinated secondary effluent was irradiated at different dose of 0.2~1.0 kGy by 1 MeV, ELV-4 Model electron beam accelerator. It is interesting to note that a 100 % reduction in total coliforms and total colonies were achieved until a dose of approximately 0.8 kGy. Even at low dose of 0.2 kGy, the total coliforms and total colonies were successfully inactivated to the level of satisfying the new effluent discharge guideline. Besides disinfection of total coliforms, approximately a 50% removal in biochemical oxygen demand was pronounced at a dose of 0.2 kGy. More than 20 % removal in suspended solids and turbidity was also observed at a dose of 1.0 kGy. The application of electron beam irradiation appeared to be one of options to reuse sewage treatment effluent as agricultural or industrial water.

Effect of Micronization on the Extent of Drug Absorption from Suspensions in Humans

  • A microscopic mass balance approach has hsown that the initial saturation (Is), absorption number (An), dose number (Do), and dissolution number (Dn) are four fundamental dimensionless parameters that can be used to estimate the fraction dose absorbed (F)l of suspensions of poorly soluble drugs in humans. The dissolution number of a drug increases with decreasing its particle size. The effect of micronization on F for suspensions was investigated in terms of Dn. About 90% of maximal F can be achieved at $Dn{\approx}2$. Increasing the solubility of a drug results in better oral absorption through increasing Dn and decreasing the solubility of a drug results in better oral absorption through increasing Dn and decreasing Do. The fractions dose absorbed of digoxin, griseofulvin, and benoxaprofen agree with predicted F values sorbed by reducing particle size, while absorption of drugs with high Do and low Dn is limited by solubility and requires higher solubility to enhance the fraction dose absorbed in addition to micronization. Solubility at the physiological pH should be used for the estimation of the fraction dose absorbed.
