합병증이 병발된 슬관절 치환술에 시행한 관절경술의 효용성 (The Effectiveness of Arthroscopy in Complicated Knee Arthroplasty)

  • 목적: 슬관절 치환술 후에 발생한 합병증의 치료를 위해 시행한 관절경술의 결과를 알아보고 그 효용성을 확인하고자 하였다. 대상 및 방법: 1992년 5월부터 2008년 6월까지 본원에서 슬관절 치환술을 시행받은 후 합병증으로 인하여 관절경적 치료를 받았던 25예를 연구 대상으로 하였다. 25예 중 슬관절 전치환술을 시행받았던 경우가 19예, 부분치환술을 시행받았던 예가 6예였다. 모든 환자에서 관절경을 시행하기 전에 합병증의 원인을 밝히기 위해서 이학적 검사와 방사선 검사를 시행하였으며, 감염이 의심되는 경우 관절액 천자 및 혈액학적 검사를 추가로 시행하였다. 결과: 관절경술 시행 시 진단으로는 전치환술을 시행한 예에서는 감염 11예, 관절의 유착 및 섬유화로 인한 운동 제한 6예, 연부 조직의 충돌 2예 등이 있었으며, 부분 치환술을 시행한 예에서는 반월상 연골 파열, 유동성 치환물의 아탈구, 혈관절증, 관절 내 시멘트 유리체, 연부 조직 충돌 및 슬관절 강직으로 인한 운동 제한이 각각 1예 씩 총 6예가 있었다. 감염이 있던 11예 중 9예에서 관절경술만으로 치료가 되었고, 관절 강직이 있던 7예는 평균 슬관절 운동 범위가 술 전 $65^{\circ}$에서 최종 추시 시 $105^{\circ}$로 향상 되었으며, 나머지 예에서도 관절경술로 성공적인 치료 결과를 보였다. 결론: 슬관절 치환술 후 발생한 합병증의 치료로 시행한 관절경술은 적절한 환자의 선택 시 안전하고 효과적인 방법임을 확인할 수 있었다.

관절외 배부 폐쇄 쐐기 절골술을 이용한 Freiberg병의 치료 결과 (Outcome of Extraarticular Dorsal Closing Wedge Osteotomy for Freiberg's Disease)

  • Purpose: The aim of this study was to evaluate the result of extraarticular dorsal closing wedge osteotomy in Freiberg's disease. Materials and Methods: Between February 2012 and July 2014, total 10 patients who underwent dorsal closing wedge osteotomy and followed up more than 1 year were selected for inclusion. Average age was 16.3 years, and average follow-up period was 15.5 months. The diagnosis was made using magnetic resonance imaging of those with a limitation in walking or usual activity due to pain in the metatarsal head. During operation, we removed loose body, and synovectomy was done. Osteotomy at the metatarsal neck and fixation with Kirschner wire were performed. X-ray was taken to check shortening of 2nd metatarsal and bone union. Moreover, we checked the active range of motion of 2nd metatarsophalangeal joint before and after surgery. At the last follow-up, the shortening of metatarsal, American Orthopaedic Foot and Ankle Society (AOFAS) score, visual analogue scale (VAS), and patient's subjective satisfaction were evaluated. Results: According to the Smillie's stage, there were 3 cases of stage II, 4 cases of stage III, and 3 cases of stage IV. Average bone union time on the osteotomy site was 8 weeks. Average shortening of metatarsal was 2.53 mm. Average AOFAS score improved significantly from 56.9 to 82.8 points at final follow-up (p<0.05), and average VAS score also improved significantly from 6.4 to 1.4 points at final follow-up (p<0.05). Average active range of motion at metatarsophalangeal joint improved from $28.0^{\circ}$ preoperatively to $46.5^{\circ}$ at the final follow-up. Other complications, such as metatarsalgia and arthritis, were not found; however, there was 1 case of delayed union with no symptom. Conclusion: In Freiberg's disease, dorsal closing wedge osteotomy is recommended for the improvement of clinical symptoms and range of motion.

  • The use of materials for modern lightweight auto-bodies is becoming more complex than hitherto assemblies. The high strength materials nowadays frequently used for more specific fields such as the front and rear sub frames, seat belts and seats are mounted to the assembled body structure using bolt joints. It is desirable to use nuts attached to the assembled sheets by projection welding to decrease the number of loose parts which improves the quality. In this study, nut projection welding was carried out between a nut of both boron steel and carbon steel and ultra-high strength hot-stamped steel sheets. Then, the joints were characterized by optical and scanning electron microscope. The mechanical properties of the joints were evaluated by microhardness measurements and pullout tests. An indigenously designed sample fixture set-up was used for the pull-out tests to induce a tensile load in the weld. The fractography analysis revealed the dominant interfacial fracture between boron steel nut weld which is related to the shrinkage cavity and small size fusion zone. A non-interfacial fracture was observed in carbon steel nut weld, the lower hardness of HAZ caused the initiation of failure and allowed the pull-out failure which have higher in tensile strengths and superior weldability. Hence, the fracture load and failure mode characteristics can be considered as an indication of the weldability of materials in nut projection welding.

족근 관절 연부조직 충돌 증후군에서 MRI의 진단적 의의 및 관절경적 치료 결과 (Diagnostic efficacy of specialized MRI & clinical results of arthroscopic treatment in ankle soft tissue impingement syndrome)

  • Introduction: Soft-tissue impingement syndrome is now increasingly recognized as a significant cause of the chronic ankle pain. As a method to detect soft-tissue ankle impingement, a characteristic history and physical examination, routine MR imaging, and direct MR arthrography were used. The efficacy of routine MR imaging has been controversial for usefulness because of low sensitivity and specificity. Direct MR artrhography was recommaned for diagnosis because of the highest sensitivity, specificity and accuracy, but it requires an invasive procedure. The purpose of this study is to investigate the diagnostic accuracy of Fat suppressed, contrast enhanced, three-dimensional fast gradient recalled acquisition in the steady state with rediofrequency spoiling magnetic resonance imaging(CE 3D-FSPGR MRI) and to evaluate the clinical outcome of the arthroscopic treatment in assessing soft-tissue impingement associated with trauma of the ankle. Materials and Methods: We reviewed 38 patients who had arthroscopic evaluations and preoperative magnetic resonance imaging studies(3D-FSPGR MRI) for post-traumatic chronic ankle pain between January 2000 and August 2002. Among them, 24 patients had osteochondral lesion, lateral instability, loose body, malunion of lateral malleoli, and peroneal tendon dislocation. The patient group consisted of 23 men and 15 women with the average age of 34 years(16-81 years). The mean time interval from the initial trauma to the operation was 15.5 months(3 to 40 months), The mean follow-up duration of the assessment was 15.6months(12-48 months). MRI was simultaneously reviewed by two radiologists blinded to the clinical diagnosis. The sensitivity, specificity and accuracy of MRI was obtained from radiologic and arthroscopic finding. Arthroscopic debridement and additional operation for associated disease were performed. We used a standard protocol to evaluate patients before the operation and at follow-up which includes American Orthopedic Foot and Ankle Society Ankle-Hindfoot Score. Results: For the assessment of the synovitis and soft tissue impingement, fat suppressed CE 3D-FSPGR MR imaging had the sensitivity of 91.9%, the specificity of 84.4 and the accuracy of 87.5%. AOFAS Ankle-Hindfoot Score of preoperative state was 69.2, and the mean score of the last follow-up was 89.1. These were assessed as having 50% excellent(90-100) and 50% good(75-89). The presence of other associated disease didn't show the statistically significant difference(>0.05). Conclusion: Fat suppressed CE 3D-FSPGR MR imaging is useful method comparable to MR arthrography for diagnosis of synovitis or soft-tissue impingement, and arthroscopic debridement results in good clinical outcome.

주관절 병변이 있는 운동 선수에서 관절경적 처치 (Arthroscopic Management in the Athletic Elbow Lesion)

  • 목적 : 본연구는주관절동통또는강직을호소하는운동선수에대하여관절경적처치를시행하여그효능을평가하고자하였다. 대상및방법 : 주관절운동이기능적으로제한되며최소6개월동안의물리치료및다른보존적인치료에반응하지않은 13례의운동선수를대상으로하였으며, 증상발현후부터수술시까지의기간은최소8개월에서최장 16개월로다양한분포를가지고있었다. 발견된병변에따라유리체제거술, 구축된 관절낭유리술, 골극제거술등을시행하였다. 결과판정은 Broberg와Morrey 계측방법으로평가하였다. 결과 : 전례에서중등도이상의활액막염을관찰할수있었고, 관절내유리체는9례, 주두및주상돌기골극은2례에서관찰되었다. 전례에서술후관절운동범위의호전및운동시동통의감소를보였고 Broberg와Morrey 계측방법으로술전평균65점에비해술후평균89.4점으로양호한결과를보였다. 결론 : 운동선수의주관절의동통및강직에대하여 관절경적 처치는원인병변의정확한평가가용이하며술후관절주변조직의손상을최소화하여술후빠른재활이가능한효과적인술식중한방법으로사료되었다.

관절경을 이용한 대퇴골과 박리성 골연골염에 대한 치료 (Arthscopic Treatment of Osteochondritis Dissecans in Femoral Condyle)

  • 목적 : 관절경 소견을 기준으로 슬관절 대퇴골과의 박리성 골연골염에 대한 치료를 시행하여 그 결과를 임상적 및 방사선학적으로 분석 하였다. 대상 및 방법 : 대퇴골과의 박리성 골연골염 환자 17면 19례에 대해 관절경을 시행하고 그 소견을 토대로 치료방법을 결정하였다. 관절경 관찰후에 특별한 조작을 가하지 않은 경우를 1군, 다발성 천공술을 시행한 경우를 2군, Herbert 나사로 고정을 시도한 경 우를 3군, 유리체 제거나 기저부 소파술을 시행 한 경우를 4군으로 분류하여 그 결과를 Hughston등의 방법에 의한 분석으로 우수, 양호, 보통, 불량, 실패 등으로 분류하였다. 결과 : 총 19례 중 14례$(74\%)$에서 양호 이상의 결과를 얻었다. 치료 방법에 따른 분류상 1군 중 3례(3/4)에서, 2군 중 3례(3/4)에서, 3군 중 7례(7/8)에서, 4군 중 1례(1/3)에서 양호 이상의 결과를 얻었다. Herbert 나사 고정군에서 다른 군보다 더 좋은 결과를 나타냈다. 결론 : 대퇴골과의 박리성 골연골염의 치료 방법 결정에 있어서 관절경 소견은 믿을만 한 지표로 판단되었으며, 이를 토대로 골성장판이 완전 폐쇄되기 전의 환자에서 Herbert 나사 고정술과 같이 병변부에 대하여 적극적인 재건을 시도하여 만족 할 만한 결과를 얻을 수 있었다.

Streptozotocin 유도 당뇨병 생쥐 모델에서 지각 추출물의 항당뇨 효과 (Antidiabetic Effect of Aurantii Fructus Immaturus in Streptozotocin-induced Diabetes Model of Mice)

  • The aim of this study is to evaluate the antidiabetic effect of the water extract of Aurantii fructus immaturus (WAF), in diabetic models using enzyme, cells and mice, and to suggest a putative mechanism explaining its antidiabetic effect. In an enzyme model using the enzyme α-glucosidase, WAF had no significant effect on α-glucosidase, as compared with acarbose, an antidiabetic drug. Nonetheless, WAF was capable of reducing the blood glucose levels during oral sucrose tolerance test and oral glucose tolerance test, implying that there would be other antidiabetic pathways in no relation to inhibition of α-glucosidase. In cell models using RIN-m5f β-cells and L6 myotubes, WAF, at its non-cytotoxic doses, augmented the secretion of insulin in RIN-m5f β-cells stimulated with 5 mM glucose. In addition, it enhanced the cellular uptake of glucose in L6 myotubes stimulated with deprivation of glucose for 12 h. Therefore, it is most likely that WAF may exert its antidiabetic effects, at least in part, by enhancing insulin secretion and glucose uptake. Meanwhile, in diabetic mice induced with peritoneal injection of streptozotocin (STZ), WAF significantly improved fast blood glucose levels, glycosylated hemoglobin levels, body weight loose, blood pressure, and diabetic adverse effects on functions of the kidney and the liver. Taken together, the water extract of Aurantii fructus immaturus may ameliorate diabetes in mice injected with STZ, at least in part, by enhancing insulin secretion and glucose uptake.

카무트밀 효소 식이가 Dextran Sulfate Sodium으로 유발한 대장염 마우스에 미치는 효과 (Effects of Kamut Wheat Enzyme Diet on Dextran Sulfate Sodium-induced Colitis Mice)

  • Objectives : Ulcerative colitis is a chronic recurrent inflammatory disease of the gastrointestinal tract. However, there are some drawbacks to long-term drug therapy such as the risk of opportunistic infections. Recently, there was an increasing interest on the use of khorasan Kamut wheat because of their higher value of selenium and fiber than modern wheat. The present study was aimed to investigate the effect of Kamut brand wheat enzyme (Kamut WE) diet on colon health in dextran sulfate sodium (DSS)-induced colitis mice. Methods : Female C57BL/6J mice were divided into 6 groups. (1) normal (Water and AIN-93G diet), (2) control (1.25% DSS and AIN-93G diet), (3) Kamut WE (1.25% DSS and Kamut WE diet), (4) normal (Water and AIN-93G diet), (5) control (2.50% DSS and AIN-93G diet), (6) Kamut WE (2.50% DSS and Kamut WE diet). Dietary intake, body weight change, disease activity index (DAI), colon length and spleen weight were monitored. Results : Kamut WE group alleviated colitis symptom, including dietary intake loss, DAI (weight loss, loose stools, bleeding), colon length shortening and spleen swelling. Further, Kamut WE diets showed a significant effect against pathological damage by the increased colon length, decreased DAI and spleen weight in DSS 1.25% as well as DSS 2.50%. Conclusions : Our study provides evidence that Kamut WE diet increased colon length, decreased DAI and spleen weight in intestinal inflammation.

과량의 아연에 의한 아프리카 발톱개구리 (Xenopus laevis)의 배발생 이상과 독성 (Embryotoxicity and Teratogenicity of Excess Zinc on Xenopus laevis)

  • 공업적 목적으로 사용된 후의 아연은 육지와 수계로 유입되며, 토양으로 유입된 후의 아연은 부식되어 수계로 유입된다. 과량의 아연은, 수서생물에 대해 중금속으로서 독성을 가지며 수서 동 ${\cdot}$ 식물의 성장억제를 유발하고, 나아가서는 동물의 기형발생을 초래한다. 본 연구에서는 FETAX (frog embryo teratogenetic assay with Xenopus) 방법에 의해 in vivo에서 과량 아연의 독성을 밝혔다. st. 9 시기에 있는 Xenopus 개구리의 배를 $100{\sim}900\;{\mu}M$ $ZnCl_2$용액에 7일간 노출시켰을 때 100 ${\mu}M$ 아연에서는 81%의 개체가 생존하였고 1000 ${\mu}M$ 에서는 25%가 생존하였다. 그 결과 외형적 이상은 복부팽만과 소화관 형성부전이 일반적으로 나타났고 200 ${\mu}M$ 이상의 아연처리 시 모든 개체에서 이러한 현상이 나타났다. 400 ${\mu}M$ 이상의 농도에서는 모든 개체에서 심장탈색을 보였다. 또한 신체의 탈색과 눈의 수정체 헤르니아, 그리고 느슨한 소화관은 100 ${\mu}M$ 이상의 아연처리에서 매우 빈번히 나타났다. 아연처리 된 유생의 파라핀 조직절편에서는 망막의 광수용기의 파괴, 눈의 초자체방 축소, 심장적혈구수의 감소, 비정상적 간조직, 원신관 세포의 팽윤, 근육형성부전, 그리고 구전정의 유두종 형성 등의 이상을 보였다. 이러한 이장은 미토콘드리아의 파괴 $Ca^{2+}$$Zn^{2+}$의 치환에 의한 세포결합의 저해, 그리고 아연에 의한 적혈구내 leghemoglobin의 형성 등이 원인일 것이다. 과량 아연에 의해 유생의 체장은 감소하였고, 통계분석 결과 체장은 대조군의 체장과 비교했을 때, 300 ${\mu}M$ 아연처리 군부터 매우 유의미하게 체장의 감소가 나타났다. 현재 자연상태에서 양서류의 수적감소와 기형발생이 세계적으로 보고되고 있으며, 본 연구의 결과로 고려했을 때, 과잉 아연은 수계를 오염시켜 양서류의 수적감소를 일으키는 중요한 원인물질로 생각되므로 이의 관리에 관심을 두어야 할 것이다.

중국 제복의 상징성에 관한 연구 (A study of Symbolics of Chinese Liturgical Vestments)

  • This thesis was designed to study Symbolics of Chinese Liturgical Vestments. Chinese who regarded the life of human beings as the combination of heaven and earth considered garments as the traditional product of the movement of nature. Accordingly, they thought human beings are the center of the universe composed by heaven and earth and the chief of all things; therefore man only can utilize clothes to distinguish from all of the colours. This views of clothes led to the development of liturgical vestments esteemed courtesy than anything else, especially the thought of courtesy associated with Conficius who regarded courtesy as the highest things and since then the theory of Five Elements and courtesy were inherited by all the adherents of Conficius. Yin and Yang Five Elements in the liturgical vestments was given absolute symbolics in both formative side and in colourful side. results of research studied in this was can be summed up as follows : 1. The crown of rites was made imitating after the system of head, horn, beard, bread of birds and beasts and that form of crown is front-circ-ular and back-rectangular meant to be towards light and dark. That the upper part of faceplace is black represented the way of heaven and lower part of red symbolized the way of earth. 2. Upper vestment of liturgical rites symbolizes heaven and outskirt represented earth. So front of outskirt is YANG and back is Yin. It is why then are going to harmonize positive and negative making front part three width and back part four width. Therefore, emperor who symbolizes heaven made the subjects recognize high and low and wore Dae-gu(大 ), Kon-bok(袞服), Bel-bok, Chui-bok, and Hyonbok according to the object and position of rites so that he may rule the country based on courtesy. 3. As an accessory of liturgical vestments, Bul, Pae-ok, Su, Dae-dai, Hyok-Dai, Kyu, and Hol were used. Before Bul was used man dressed skirt as the first waist-dress in order to conceal intimate part of the body. Pae-ok, as decoration blended with jade was worn by men of virtue, so men of virtue symbolized morality and virtue by Pae-ok. Su began from Yeok, connected with Pae-su , in Chou-dynasty is said to be originated by practical needs and they are divided into large Su and small su, and maintained as decoration to signify the class positions. Dae-dai did the work as not to loose the liturgical vestments and leather belt hang Bul and Su to begin as the function of practical use are in later years it became decoration to symboliz e the class position. Kyu was a jade used when empeor nominated feudal lords and observe ceremony to God and Hol, was held in hands to record everything not to forget. These Kyu and Hol became to offer courtesy during the time of rites and in later years it became used according to class position rather than practical use. 4. As far as colours are concerned, colours based by five colours according to YIN-YANG Five Elements theory and they were divided into a primary colour and a secondary colours. Primary colours corresponded with the theory of Five Elements each other, Blue, Red, Tellow, White, and Black symbolized ive Elements, five hour space, five directions, and five emperors. Secondary colours contradict with Blue, Red, Yellow, White and Black and another as a primary colour and they are Green, Scaret, Indigo, Violet, Hun colour, Chu colour, and Chi colour. This primary colour was used in liturgical vestments, that is, upper-vestments used black colour. This primary colour was used in liturgical vestments, that is, upper-vestments used black colour as primary colour and outskirt was used Hun colour as secondary colours. Thus symbolism in chinese liturgical vestments mainly began with heaven and earth and corresponded with YIN-YANG Five Elements Scool. They were developed as the scholary theory and Conficius and his followers in the later days and continued up to Min-dynasty.
