두경부 굴곡 운동이 만성 경부통 근로자의 통증 및 경장근 단면적에 미치는 영향 (Effect of Craniocervical Flexion Exercise on Pain and Cross Sectional Area of Longus Colli Muscle in Workers with Chronic Neck Pain)

  • This study compared the effects of craniocervical flexion exercise with conservative cervical flexion exercise in workers with chronic neck pain. Subjects were randomly allocated to two groups: control (n=20) and experimental group (n=20), respectively. The conservative exercise for the control group consisted of cervical flexion exercise, whereas the experimental group performed a craniocervical flexion exercise. To compare the two groups, the visual analogue scale (VAS) for a pain, neck disability index (NDI) for a neck disability scale, and cross sectional area (CSA) of longus colli muscle using ultrasound image were assessed on pre-intervention, post-intervention, and 2 weeks follow-up. 2-way repeated ANOVA was used with Bonferroni post-hoc test. (1) There were significant main effects (within and between groups) (p<0.05) and interaction effect (p<0.05) in VAS. Post-hoc test revealed that there were significant differences in all pair-wise comparisons. (2) There were significant main effects (within and between groups) (p<0.05) and interaction effect (p<0.05) in NDI. Post-hoc test revealed that there were significant differences between pre-intervention and post-intervention, pre-intervention and 2 weeks follow-up. (3) There were significant main effects (within and between groups) (p<0.05) and interaction effect (p<0.05) in CSA of longus colli muscle. Post-hoc test revealed that there were significant differences in all pair-wise comparisons. The findings indicate that craniocervical flexion exercise are more effective for improving pain, neck disability, and CSA of longus colli muscle than cervical flexion exercise in workers with chronic neck pain.

Morphometric Study of the Upper Thoracic Sympathetic Ganglia

  • Objective : Morphometric data for the sympathetic ganglia (SG) of the upper thoracic spine was investigated to identify the exact location of the SG in order to reduce normal tissue injury in the thoracic cavity during thoracoscopic sympathectomy. Methods : In 46 specimens from 23 formalin-fixed adult cadavers, the authors measured the shortest distance from the medial margin of the T1, T2 and T3 SG to the most prominent point and medial margin of the corresponding rib heads, and to the lateral margin of the longus colli muscle. In addition, the distance between the most prominent point of the rib head and the lateral margin of longus colli muscle and the width of each SG were measured. Results : The shortest distance from the medial margin of the SG to the prominent point of corresponding rib head was on average 1.9 mm on T1, 4.2 mm, and 4.1 mm on T2, T3. The distance from the medial margin of the SG to the medial margin of the corresponding rib head was 4.2 mm on T1, 5.9 mm, and 6.3 mm on T2, T3. The mean distance from the medial margin of the SG to the lateral margin of the longus colli muscle was 6.7 mm on T1, 8.8 mm, 9.9 and mm on T2, T3. The mean distance between the prominent point of the rib head and the lateral margin of the longus colli muscle was 4.8 mm on T1, 4.6 mm, and 5.9 mm on T2, T3. The mean width of SG was 6.1 mm on T1, 4.1 mm, and 3.1 mm on T2, T3. Conclusion : We present morphometric data to assist in surgical planning and the localization of the upper thoracic SG during thoracoscopic sympathectomy.

건강한 성인의 경장근 두께 측정에 대한 초음파영상법의 신뢰도 (Reliability of Ultrasonography for The Longus Colli in Asymptomatic Subjects)

  • Purpose: The objective of this study was to evaluate the inter- and intra-examiner reliability of ultrasonography (US) for measuring the thicknesses and cross-sectional area of the longus colli muscle (LCM) in healthy subjects. Methods: Twenty-five healthy adults participated in this study as subjects and two examiners attended to the study. LCM size was measured at the level of the thyroid cartilage. All subjects were randomly allocated to one of the three different methods. Method 1 was that the examiner A conducts the first measurement and one hour later conducts the second measurement and twenty four hours later conducts the third measurement, respectively. Method 2 was that the examiner B conducts the measurements in the same way as the examiner A. Method 3 was that the examiners A and B randomly conduct the measurements twice at an interval of one hour. Results: The intra-class correlation coefficient (ICC) for the intra-examiner reliability ranged from 0.78 to 0.98 at rest and 0.57 to 0.98 during contraction for the examiner A. The $ICC_{3,3}$ for the intra-examiner reliability ranged from 0.60 to 0.96 at rest and 0.57 to 0.92 during contraction for the examiner B. The $ICC_{2,3}$ for the inter-examiner reliability ranged from 0.80 to 0.91 at rest and 0.84 to 0.93 during contraction. Conclusion: The findings of this study indicate that US is a reliable method for measuring the LCM size.

Misconceived Retropharyngeal Calcific Tendinitis during Management of Myofascial Neck Pain Syndrome

  • Differential diagnosis of posterior neck pain is very challenging based on symptoms and physical examination only. Retropharyngeal calcific tendinitis is a rare and frequently misdiagnosed entity in various causes of neck pain. It results from calcium hydroxyapatite deposition in the longus colli muscle which is characterized by severe neck pain, painful restriction of neck movement, dysphagia, and odynophagia. We herein report a case of a patient with acute retropharyngeal calcific tendinitis, who complained of posterior neck pain, initially diagnosed and treated as a myofascial neck pain syndrome.

Comparison of the Cross-Sectional Area of Longus Colli and Muscle Activity of Sternocleidomastoid in Subjects With Forward Head Posture on the Two Craniocervical Flexion Methods

  • Background: The craniocervical flexion (CCF) exercise is one of the effective exercise in correcting forward head posture (FHP). However, some people with FHP achieve CCF with compensatory movements, for example, low cervical flexion using superficial neck flexors such as the sternocleidomastoid (SCM) muscle. No study has yet investigated whether a dual­pres ure biofeedback unit (D-PBU) method to prevent low cervical flexion would be helpful in performing pure CCF movement. Objects: The purpose of this study was to compare the effects of the CCF using D-PBU method and the traditional CCF method on the cross-sectional area (CSA) of the longus colli muscle (LCM) and the activity of SCM muscle in subjects with FHP. Methods: Twenty­four FHP subjects (male: 16, female: 8) were recruited for this study. All subjects performed CCF using two different methods: the traditional CCF method and the CCF using D-PBU method. The CSA of the LCM was measured via ultrasound, and surface electromyography was used to measure SCM muscle activity. Results: The change in CSA of the LCM was significantly larger during the CCF using D-PBU method ($1.28{\pm}.09$) compared with the traditional CCF method ($1.19{\pm}.08$) (p<.05). The SCM muscle activity using the CCF using D-PBU method ($2.01{\pm}1.97$ %MVIC) was significantly lower than when using the traditional CCF method ($2.79{\pm}2.32$ %MVIC) (p<.05). Conclusion: The CCF using D-PBU method can be recommended for increasing LCM activation and decreasing SCM muscle activity during CCF movement in subjects with FHP.

Correlation between contraction ratio, endurance, and muscle tone of cervical muscles

  • Objective: The endurance and strength of deep neck flexor (DNF) muscles have a major role on the function and stability of the cervical spine. In recent years, there has been a lack of research that have investigated the muscle tone of the superficial neck muscles. The purpose of this study was to examine the relationship of between contraction ratio of the DNF and sternocleidomastoid (SCM) muscles, DNF endurance, and muscle tension of the neck muscles. Design: Cross-sectional study. Methods: Forty-seven subjects (male=20, female=27) participated in this study. The muscle tone of the upper trapezius (UT), SCM, and suboccipital (SO) muscle was measured using a contact soft tissue tone-measuring instrument. For the contraction ratio of the SCM and longus colli muscle, the thickness of the relaxation and maximum contraction state of the SCM and longus colli muscles were assessed using a diagnostic ultrasound measuring instrument and a pressure biofeedback unit. The deep neck flexor endurance test (DNFET) was performed in a cranio-cervical flexion posture. The correlations between the measured variables were investigated. Results: The relationship between the DNFET and SO tone showed a significant negative correlation (p<0.05). The relationship between the DNFET and contraction ratio showed a significant positive correlation (p<0.01). There was no significant correlation between the DNFET and SCM and UT tone. Conclusions: This study confirmed that there is a relationship between DNF endurance, DNF activation, and SO tone. The information on the results of this study may be used as a reference that can be actively applied in the clinical environment.

턱 당김 운동이 무릎 굽힘폄의 근 수행력에 미치는 영향 (The Effect of Muscle performance on knee flexion / extension by chin in exercise)

  • Background: The purpose of the present study is to report the effect of muscle performance on knee flexion and extension by chin in exercise. Methods: This study was Participated in 10 healthy subjects. For conducting the chin-in exercise, subjects are lay down with supine position. Using the rounded towel, subjects was performed contraction of longus colli and longus capatis muscle by push the towel. Chin-in exercise was conducted 3 times a day, 4 times a week for 4 weeks. By using the Biodex system4, We measured absolute muscle strength, relative muscle strength, total exercise quantity and average rate of production in knee flexion and extension. The data was analyzed by the repeated-measure ANOVA for comparing before, after exercise 2 weeks, after exercise 4 weeks changes of factors. Results: After chin-in exercise, there was significant difference of before, after 2 weeks and 4weeks results in absolute muscle strength, relative muscle strength, total exercise quantity, average rate of production, agonist/antagonist ratio(p<.05). Conclusion: As a results of this study, chin-in exercise may help to improve muscle ability of knee joint activation and knee joint action performance.

목통경험 유무에 따른 초음파 영상에서 측정된 심부 목근육 크기와 근지구력 시간에 대한 비교 연구 (Comparing Persons with Neck Pain Experience to Persons without Neck Pain Experience in Deep Neck Muscle Size Using Ultrasonography Images and Neck Muscle Endurance Time)

  • 본 연구의 목적은 깊은 목굽힘근과 깊은 목폄근의 두께를 목근육 지구력 검사 동안 측정하여 목통증 경험 유무에 따라 깊은 목근육들의 크기에 차이가 있는 지 관찰하였다. 컴퓨터 사용이 많은 대학생 65명이 참여하였으며, 목통증이나 두통으로 인하여 일상생활을 수행하는데 제약이 없고 현재 척추증상이 없는 자를 대상으로 하였다. 근육의 두께는 근지구력검사를 수행하는 중 근이완기, 수축기와 제한시점에서 각각 측정하였다. 목통증과 기능제한에 대한 정보는 설문지를 통하여 수집하였다. 목통증 경험이 없는 대상자들 (그룹1)의 근지구력 시간이 목통증 경험이 있는 대상자들(그룹2)보다 통계적으로 유의하게 길었다 (p<0.01). 그룹1 대상자의 왼쪽 긴목근이 그룹2 대상자보다 근이완기에서 두껍게 관찰되었으며(p=0.02), 오른쪽 긴머리근은 목통증 경험이 없는 대상자들이 근수축에서 제한시점까지의 변화가 목통증 경험이 있는 대상자보다 크게 관찰되었다(p=<0.01). 목통증과 연관하여 특정 근육의 기능파악이 중요하며, 임상 대상자들의 목수축 패턴과 근기능에 대한 연구가 더 필요하다.

젊은 성인에서 머리-목 굽힘근 운동 시 시선과 압력이 목긴근과 목빗근의 근두께에 미치는 영향 (The Effect of Gaze Directions and Pressure Levels on longus colli and Sternocleidomastoid Thickness during Cranio-cervical flexor Exercise in Young Adults)

  • 이 연구는 정상 성인에서 머리-목 굽힘 운동 시 시선의 각도와 압력이 깊은목굽힘근의 두께변화에 미치는 영향을 알아보고자 하였다. 본 연구에는 27명이 참여하였다. 머리-목굽힘 운동하는 동안 4가지(0°, 20°, 40°, 60°)의 시선과 5가지(20mmHg, 22mmHg, 24mmHg, 26mmHg, 28mmHg)의 압력에서 목긴근과 목빗근의 근 두께는 초음파 영상을 이용해 측정하였다. 시선 각도 및 압력변화에 따른 목긴근과 목빗근의 두께 변화를 비교 분석하기 위하여 반복측정분산분석을 수행하였다. 본 연구결과, 목긴근은 20과 22mmHg에서 0°와 20°, 0°와 40°, 0°와 60°은 유의한 차이를 보였으며, 20°에서 가장 근활성도가 높았다(p<.05). 목빗근은 28mmHg에서 0°와 20°, 20°와 40°, 40°와 60°은 유의한 차이를 보였으며, 20°에서 가장 낮은 근활성도를 보였다(p<.05). 본 연구에서 머리-목 굽힘 운동 시에는 시선의 방향을 20°로 설정하는 것이 목긴근의 활성화를 높이고, 목빗근의 근활성도를 낮추어 운동의 효과를 얻을 수 있다는 것을 찾았다. 본 연구의 결과를 바탕으로 임상환경에서 머리-목 굽힘 운동시에 시선 방향을 설정하여 운동의 효과를 증대시키길 바란다.

기능성 라텍스 베개가 만성 경부통 환자의 경부근의 근두께, 근긴장도 및 근피로도에 미치는 영향 (Effect of Functional Latex Pillow on Muscle Thickness, Muscle Tonicity and Muscle Fatigue of Cervical Muscle in Patients with Chronic Cervical Pain)

  • PURPOSE: This study examined the effect of muscle thickness of the deep cervical flexor muscle, muscle tonicity, and muscle fatigue of the superficial cervical flexor muscle by applying a functional latex pillow to patients with chronic cervical pain. METHODS: An experimental group using a functional latex pillow and a control group using a general pillow were assigned randomly to 30 people. Each pillow was applied in a comfortable lying position in the experimental group and control group. The deep cervical flexor muscle thickness was measured in the longus colli and longus capitus using ultrasonography. The muscle tonicity and muscle fatigue of the superficial cervical flexor muscle were measured separately in the sternocleidomastoid muscle using a myotonometer and electromyography. RESULTS: In the experimental group(functional latex pillow), the muscle tonicity of the superficial cervical flexor muscle like the sternocleidomastoid muscle was significantly lower than that of the control group (general pillow)(p < .01). CONCLUSION: This study suggests that the functional latex pillow may effectively reduce the muscle tonicity of the sternocleidomastoid muscle, which is the superficial cervical muscle, in patients with chronic cervical pain. On the other hand, it was not effective on the muscle thickness of the deep cervical flexor muscle and muscle fatigue of the superficial cervical flexor muscles.
