Importance and Performances of Visiting Nurse Services Provided under the Long Term Care Insurance System for the Elderly (노인장기요양보험 제도 하에 제공되는 방문간호 서비스의 중요도와 성과도)

  • Purpose: This study was attempted to provide quality improvement and revitalization of visiting nurse services by Importance-Performance Analysis of persons being provided with the services. Methods: The subjects were 350 people being provided with visiting nurse services from a long term home care institution in the kangwon Province during the period of data collection between Aug. 16 and Sep. 15, 2011. Data analysis was conducted through t-test, ANOVA, and IPA by using the SPSS/WIN 12.0 program. Results: As a result of the Importance-Performance Analysis of visiting nurse services, it turned out that the strength on reliability, assurance, and empathy should be maintained with good work and the aspects of responsiveness appeared to be intensively improved, and the tangibility proved to be subject to improvement. Conclusion: For the activation of visiting nurse services, methods for improving the performance in the intensive care area and management strategy establishment which highlights the advantages of strength maintenance area are needed.

The Development of Client-centered and Integrated Home Nursing Care Model in Rural Areas (농어촌 지역의 대상자 중심 통합방문형 간호서비스 모형 개발)

  • Purpose: This study aimed to develop a client-centered integrated home nursing care model for rural areas by analyzing public health nursing, hospital-based home care services, and long-term nursing care in Korea. Methods: The literature review performed included data from the National Assembly Library, DBpia, RISS, and KISS, Google Scholar, the Ministry of Government Legislation, Statistics Korea, and the Ministry of Health and Welfare. Results: The client-centered and integrated home nursing care model in a rural area was opened as the Home Nursing Care Center in a public health center operating directly or on consignment. This model provides both a hospital-based home care services as well as long-term care, in accordance with the health status of the client and difficulty of nursing services. Moreover, the nurse who worked in a sub-organization (Centers for Supporting Healthy Living, Public Health Units, and etc.) of the public health center as care coordinator and case manager facilitates to connect home nursing care services and social welfare services. Conclusions: Our data indicates that the client-centered integrated home nursing care model in rural areas effectively combines professional services, regional accessibility, and social welfare services.

Performance and Requirements of Visiting Nursing Care in Long-Term Care Insurance Using the OMAHA System (노인장기요양보험 방문간호서비스 수행도와 필요도 : 오마하시스템 문제분류체계를 이용하여)

  • Purpose: The aim of this study was to compare between performance and requirements of visiting nursing care in long-term care insurance using the OMAHA system. Methods: The subjects were 72 nurses who had worked in a visiting nursing care center in long-term care insurance. Data were collected from December 5, 2016 to January 31, 2017 using self-recorded questionnaires. The collected data were analyzed using descriptive statistics and paired t-tests. Results: Four dimensions of the OMAHA system showed statistically significant differences between performance and requirements of visiting nursing care in long-term care insurance. The requirements of visiting nursing care were higher than was performance on all 40 items of the OMAHA system. The greatest difference was in environmental domain and then the psychosocial domain. Conclusion: Based on the results, we found that the environmental and psychosocial domains were the largest gap areas. Therefore, with the reality of elderly people living alone and the increase in elderly couples, active intervention connected with the community is needed in residential areas. Further, we suggest that the OMAHA system can be utilized as an integrated conceptual framework for developing and enhancing visiting nursing care in long-term care insurance.

Multi-level Analysis of Factors related to Quality of Services in Long-term Care Hospitals (다수준 분석을 이용한 요양병원 서비스 질에 영향을 미치는 요인 분석)

  • Purpose: In this research multi-level analysis was done to identify factors related to quality of services. Patient characteristics and organizational factors were considered. Methods: The data were collected from the Health Insurance Review and Assessment Service(HIRA) data base. The sample was selected from 17,234 patients who had been admitted between January 2007 and May 2008 to one of 253 long-term care hospitals located in Seoul, six other metropolitan cities or nine provinces The data were analyzed with SAS 9.1 using multi-level analysis. Results: The results indicated that individual level variables related to quality of service were age, cognitive ability, patient classification, and initial quality scores. The organizational level variables related to quality of service were ownership, number of beds, and turnover rate. The explanatory power of variables related to organizational level variances in quality of service was 23.72%. Conclusion: The results of this study indicate that differences in the quality of services were related to organizational factors. It is necessary to consider not only individual factors but also higher-level organizational factors such as nurse' welfare and facility standards if quality of service in long term care hospitals is to be improved.

A study on dental professionals' recognition on a system of long-term care insurance for the elderly (노인장기요양보험제도에 관한 치과종사자의 인식도 연구)

  • This study grasped recognition on a system of long-term care insurance for the elderly targeting dental professionals who are working at dental hospitals and clinics where are located in Daejeon Metropolitan City. It developed and utilized materials available for educating the care staff in a system of long-term care insurance for the elderly and the dental professionals who participate in the oral hygiene service. It carried out the effective duty performance for the elderly in a situation of long-term care protection. Thus, the purpose of this study was to contribute to the early settlement in a system of long-term care insurance for the elderly. The following conclusions were obtained as a result of having carried out self-administered questionnaire research targeting 238 people from August 1 to August 30 in 2008. 1. In the general characteristics of the research subject, the present working place was indicated to be 22.7% for dental hospital, 71.8% for dental clinic, and 5.5% for others. As for the main duty field, the medical treatment & cooperative duty was indicated to be the highest with 61.8%, and was statistically significant(p=0.000). 2. The necessity for a system of long-term care insurance for the elderly was indicated to be 77.7% for 'necessary' and 1.7% for 'unnecessary,' and was statistically significant(p=0.016). 3. In the item of dividing the service of long-term care insurance for the elderly, the dental hygienists showed higher recognition than non-dental hygienists, and indicated significant difference(p=0.010). 4. As for recognition on a system of long-term care insurance for the elderly in dental professionals who responded as saying of 'knowing name and contents' about a system of long-term care insurance for the elderly, the recognition level was high in recognition of subjects' age(p=0.000), division in services(p=0.012), contents in at-home care service(p=0.000), execution in oral-hygiene service(p=0.004), procedure of using the long-term care insurance for the elderly(p=0.016), item of judging grade of long-term care insurance for the elderly(p=0.013), medical charge by service according to judging grade of long-term care insurance for the elderly(p=0.015), burden of cost for a system of long-term care insurance for the elderly(p=0.011), qualification of care staff(p=0.002), and contents of oral-hygiene service(p=0.027), and showed significant difference. 5. The service of long-term care insurance for the elderly and the oral-hygiene service indicated the statistically significant correlation. Accordingly, all of dental professionals need to make a desperate effort to improve dental professionals' knowledge on a system of long-term care insurance for the elderly enough to be required a system of long-term care insurance for the elderly. The more systematic and standardized professional education and materials are thought to be needed to be developed aiming at the success in oral-hygiene service within a system of long-term care insurance for the elderly, by strengthening professionalism in dentists and dental hygienists.

In-depth interview with the center managers for home visit dental hygiene services in the long-term care insurance (노인장기요양보험 구강위생서비스의 활성화를 위한 재가장기요양기관 관리책임자 심층면담)

  • Objectives: This study was carried out to identify plans to activate home visit oral care services by dental hygienists in Long-term care insurance. Methods: In-depth interviews were carried out with 21 Long-term Home Care Center Managers as target. A total of 21 (27%) Home Care Centers were selected through convenience sampling among 78 Home Care Centers that are located in Won-ju city. The Managers were presented with questions and answered in 20-30 minutes in accordance with the interview instructions. The interview results were analyzed through content analysis, and their experiences and perceptions were classified into two themes and categorized again into four components. Results: The Home Care Center Managers suggested that dental hygienists should activate home-visit oral hygiene services. It is necessary to improve the management process and awareness of the elderly. Conclusions: To activate oral hygiene services, it is necessary to improve the service guideline and enhance the efficiency of the service process. This should be acceptable both to the elderly who need the services and the dental hygienists who provide them.

A Comparative Study on the Care Services According to Care Levels in the Korean and German Home Care System (한국과 독일 방문요양의 등급별 급여 비교 연구)

  • Purpose: This study aims to classify multiple forms of home care services from a service user's perspective. By comparing the German system to the Korean one, this study seeks to find better ways of enhancing client-centered care. Methods: The data of 121 home care users were collected from six home care centers that had under the management of nursing managers for more than five years. The researcher used a German instrument to estimate the level of care. Results: High correlation was found between German and Korean assessment criteria (r=0.81, p<.001). However, compared to their German counterparts, Korean home care providers allocated more time towards provision of domestic help per daily visit. German home care providers allocated more time towards services relating to provision of support for physical activities, including personal hygiene. It was confirmed that the level of care in Korea does not correlate with either the categories of home care services and duration. Conclusion: Therefore, care services should be offered according to long-term care needs. This study suggests a need for systematic care aimed at strengthening the client's autonomy which should be properly planned, implemented, and managed.

Convergence study of long-term care insurance system for elderly of awareness of middle-aged people and demand for dental services in the system (한국 중년층의 노인장기요양보험제도에 대한 인식과 구강보건진료에 대한 요구의 융합 연구)

  • The purpose of this study was to awareness of dental care services in the elderly long-term care insurance system and retirement preparation of middle-aged people. The survey respondents had a 56.1% saving for old age life, And showed a tendency to they don't like depend on one's family(41.8%), elderly care was intended to use facility services and nursing care. 36.7% of respondents answered "I know a little" about the system. Also, the need for dental care services within the system was high (98.5%), If the service is provided, 92.3% of the respondents said that they would participate, otherwise unrecognized rate of dental care services in long-term care insurance was 85.2%. Therefore, it is necessary to expand the awareness of the part of dental care service in the system. The results of this study can be used as basic data for the improvement of the system of dental care services in long-term care insurance for elderly.

Person-centered Care and Nursing Service Quality of Nurses in Long-term Care Hospitals (요양병원 간호사의 인간중심돌봄과 간호서비스 질)

  • Purpose: This study investigated the correlation between person-centered care (PCC) and nursing service quality of nurses in long-term care hospitals. Methods: The subjects were 114 nurses working in 8 long-term care hospitals. Instruments for evaluating PCC and nursing service quality were used. The data were analyzed by descriptive statistics, two samples-test, one-way ANOVA, Pearson's correlation and Multiple regression. Results: The mean of PCC was $3.25{\pm}0.45$ out of 5 and the nursing service quality was $3.87{\pm}0.40$. There were significant differences in PCC in terms of age and income satisfaction, the application of their opinions, the satisfaction of hospital managers, administrators and nurse managers. There were significant differences in nursing service quality according to age, position, the satisfaction of hospital managers, administrators and nurse managers. Nurses' PCC showed a significant positive correlation with nursing service quality. Factors influencing nursing service quality included PCC, their position and age and the most influencing one was PCC. Conclusion: This study suggests that the PCC is the strongest affecting element to the quality of nursing service in long-term care hospitals. Therefore, the strategies to improve the practice of person-centered care should be carried out to enhance the quality of nursing service.

Long-Term Care Needs Assessment of the Disabled Workers After an Industrial Injury (산재장해인의 장기요양서비스 요구 분석)

  • Purpose: The objective of this study was to evaluate long-term care needs using RAI MDS-HC and MI-CHOICE among the disabled workers. Methods: Data were obtained from 45 personal care recipients with the disability of mental and nervous system, and analyzed using SAS 9.1 by applying t-test, ${\chi}^2$ test, or fisher's exact test. Results: Only 'bed mobility' and 'indoor ambulation' items of ADL and problem activity were statistically significant factors by the level of personal care benefit. By MICHOICE grouping, 20.0 percent of subjects belonged to nursing home group, 51.5 percent were home care service, 28.9 percent were intermittent personal care. Conclusion: Personal care services in industrial accident compensation insurance have been categorized with two groups according to level of disability. But our results could contribute to provide personal care service according to the long term care needs.
