Hand Hygiene Compliance among Visitors at a Long-term Care Hospital in Korea: A Covert Observation Study

  • Purpose: This study aimed to assess hand hygiene (HH) compliance among visitors at a long-term care hospital in South Korea. Methods: The study was conducted at a 502-bed long-term care hospital located in Gyeonggi-do Province. From July 1 to August 15, 2017, including more than 6 weekends and one holiday, a trained observer covertly assessed visitors' HH at all five units (360 beds in total) of the study hospital building until the completion of 1,000 HH opportunities (i.e., 200 opportunities per unit). The modified World Health Organization (WHO) HH observation form was used. Instead of professional categories and the "before clean/aseptic procedure" moment, the estimated age range for each visitor were recorded in four categories: children (<14 years old), adolescents (14~18), adults (19~64), and the elderly (${\geq}65$). The collected data were analyzed using SPSS 22.0. Results: A total of 1,000 HH opportunities were observed from 766 visitors (an average of 1.31 per visitor) and the overall HH compliance rate was 20.3%. Overall, 53.7% of the HH cases were performed with soap and water. Among the 4 HH moments, the "after body fluid exposure risk" moment showed the highest compliance rate (83.5%); 93.9% used soap and water. The most commonly exposed potential body fluid among visitors was saliva (48.1%). Conclusion: For hospital visitors in long-term care hospitals, HH education programs including HH moments need to be developed and implemented. Further studies are necessary to evaluate visitors' HH compliance in various hospital settings and find the related variables influencing visitors' HH.

  • 본 연구는 종합병원, 요양병원, 한방병원 이용자의 선택요인과 고객만족, 재이용의도에 미치는 영향을 비교 분석하였다. 본 연구는 2019년 11월 1일부터 2020년 4월 30일까지 4개 종합병원, 6개 요양병원, 3개 한방병원을 대상으로 설문조사를 실시하였다. 종합병원, 요양병원, 한방병원은 인적 요인, 효율성 요인이 고객만족에 양(+)의 영향력이 있었다. 요양병원은 소득이 높을수록 고객만족도가 높았으며, 효율적 요인이 재이용의도에 양(+)의 영향력이 있었다. 한방병원은 대외이미지 요인이 고객만족과 재이용의도에 중요한 요인이었다. 이러한 연구결과를 바탕으로 경쟁 시대에 접어든 병원들이 고객만족도와 재이용의도를 높이고 효율적 경영을 위한 유용한 자료가 될 것을 기대된다.

A Study of Care Workers Knowledge on Pressure Ulcer

  • The purpose of this study was to examine the degree of knowledge of care workers working at long term care hospitals and nursing homes on pressure ulcer. A total of 81 care workers including 34 at long term care hospitals and 47 at nursing homes were surveyed. 24 questions were used to evaluate their degree of knowledge on pressure ulcer. Their knowledge on pressure ulcer scored 12.84 out of the total score of 24 points(SD=3.40), which was equal to 53.50 (SD=24.23) out of 100 points. Their knowledge on the prevention of pressure ulcer was highest among the subareas of evaluation. Their knowledge on pressure ulcer statistically significantly differed according to education on pressure ulcer(P<.05). A pressure ulcer is a skin disorder that may be prevented and cured. At this point when long term care facilities are rapidly increasing, care workers highlevel knowledge on and good management of pressure ulcer is very important. Practically educating them on pressure ulcer including the provision of recent, updated relevant knowledge will be necessary.

일부 노인 장기요양보호기관 종사자간의 서비스연계 조사 (The Analysis of Factors on the Service-Linkage of Long-term Care Workers for the Elderly)

  • Purpose: This study was to analyze the factors that affect the service relationship of long term care workers for the elderly and to provide basic resource for the successful connection of long term care services. Methods: 259 subjects who were engaged in long term care units completed a self-administered questionnaire that measured the extent of service linkage among one another. The Cronbach's ${\alpha}$ score determined the internal consistency of the acquired data and the discriminated validity was estimated by Pearson's correlation coefficient. Multiple regression analysis was conducted to investigate the influence of the known factors on the service linkage. Results: Acceptance and participation negatively influenced on the service linkage. Reliance, comprehension, recognition on service, and frequent contact with others positively activated the service linkage of long term care workers. Conclusion: The establishments of systemic training courses providing education that emphasizes reliability and recognizes other services, including work environment to contact easily are needed to improve the service-linkage of long-term care workers for the elderly.

노인장기요양 방문간호서비스의 소요시간별 방문당 원가 분석 (Estimation of Nursing Costs Based on Nurse Visit Time for Long-Term Care Services)

  • Purpose: The purpose of this study was to estimate nursing costs and to establish appropriate nursing fees for long-term care services for community elders. Methods: Seven nurses participated in data collection related to visiting time by nurses for 1,100 elders. Data on material costs and management costs were collected from 5 visiting nursing agencies. The nursing costs were classified into 3 groups based on the nurse's visit time under the current reimbursement system of long-term care insurance. Results: The average nursing cost per minute was 246 won. The material costs were 3,214 won, management costs, 10,707 won, transportation costs, 7,605 won, and capital costs, 5,635 won per visit. As a result, the average cost of nursing services per visit by classification of nursing time were 41,036 won (care time <30 min), 46,005 won (care time 30-59 min), and 57,321 won (care time over 60 min). Conclusion: The results of the study indicate that the fees for nurse visits currently being charged for long-term care insurance should be increased. Also these results will contribute to baseline data for establishing appropriate nursing fees for long-term care services to maintain quality nursing and management in visiting nursing agencies.

요양병원 간호사의 임파워먼트, 감염관리 조직문화, 감염관리 인지도가 감염관리 수행도에 미치는 영향 (Effects of Empowerment, Infection Control Organizational Culture and Infection Control Awareness on Performance among Nurses in Long-Term Care Hospitals)

  • Purpose: The purpose of this study was to examine the influence of empowerment, infection control organizational culture, and infection control awareness on infection control performance among nurses in long-term care hospitals. Methods: A descriptive survey study was conducted with 125 nurses as subjects who had been working for at least six months in four long-term care hospitals located in Busan metropolitan city and Gyeongsangnam-do Province. Data were collected from September 30 to October 28, 2021 and analyzed using t-test, one-way ANOVA, Pearson's correlation coefficients, and stepwise multiple regression with SPSS/WIN 26.0. Results: The results showed that infection control performance had significant correlations with empowerment (r=.36, p<.001), infection control organizational culture (r=.51, p<.001), and infection control awareness (r=.75, p<.001). Multiple regression analysis for infection control performance revealed that the most powerful predictor was infection control awareness (β=.70, p<.001). Empowerment, infection control awareness and awareness of infection control guidelines explained approximately 60.0% of the variance in infection control performance. Conclusion: Findings indicated that various factors are related to the infection control performance among nurses in long-term care hospital. Based on the results of this study, further development and application of the programs to enhance empowerment and infection control awareness are needed in order to improve the infection control performance of nurses in long-term care hospitals.

요양병원 간호사의 건강증진행위에 미치는 영향요인 (Factors Influencing health Promoting Behaviors for Long-term care Hospital Nurses)

  • 본 연구는 요양병원 간호사의 자기효능감, 사회적 지지가 간호사 자신의 건강증진행위에 미치는 영향요인을 파악하기 위한 서술적 조사연구이다. D시 3개 요양병원에서 6개월 이상 근무한 간호사 180명을 대상으로 2021년 3월1일부터 4월 30일까지 시행되었으며, 자료분석은 SPSS 21.0으로 기술분석, 피어슨 상관관계, 다중회귀분석을 시행하였다. 연구결과, 요양병원 간호사의 건강증진행위와 자기효능감(r=.35, p<.001)과 사회적 지지(r=.34, p<.001)의 상관이 있었다. 요양병원 간호사의 건강증진행위에 영향을 미치는 요인으로 자기효능감(β=.290, p=.041), 사회적 지지(β=.189, p=.004)가 유의한 결과를 보였으며 17.2%의 설명력을 나타냈다. 즉, 요양병원 간호사의 건강증진행위는 간호사의 자기효능감 증진과 사회적 지지에 따라 증가할 수 있으며 요양병원 간호사의 자기효능감 증진 프로그램과 사회적 지지가 이루어질 수 있는 환경조성이 필요할 것으로 생각된다.

중년기 요양 환자의 건강사정도구 개발 (Development of Health Assessment Tool for Middle-aged Adults in Long-term Care Settings)

  • Purpose: This was a methodological study to develop a valid and reliable health assessment tool for middle-aged adults in long-term care settings (HATMAL). Methods: At the first stage of this study, initial 56-items HATMAL were derived via literature review, survey for needs assessment, and focus group interviews with experts. At the second stage, 54 items HATMAL were finalized via content validity test with 5 experts and a pilot study with 30 nurses. At the third stage, validity and reliability tests were conducted with 100 middle-aged patients in a long-term care hospital. Data were analyzed by factor analysis using SPSS 18.0 program. Results: Exploratory factor analysis resulted in 3 factors; functional assessment, symptom assessment, and comfort assessment. These 3 factors explained 83.8% of total variance and construct validity was confirmed. Test-retest reliability was .67 for appetite changes, .76 for regulating for behavior problems, and 1.0 for other items. Conclusion: Results indicate that HATMAL is valid and reliable to assess the health for middle-aged patients in long-term care settings. This study would contribute to provide more effective personalized treatment and care for middle-aged patients in long-term care settings considering their own characteristics.

요양병원 간호사의 간호역량과 근무환경이 간호서비스 질에 미치는 영향 (Effect of Competence and Work Environment of Nurses on Nursing Service Quality in Long-term Care Hospitals)

  • Purpose: This study sought to determine the effect of the competence of nurses and their, work environment on the quality of nursing service in long-term care hospitals using the Donabedian model as a theoretical framework. Methods: This descriptive investigative study analyzed nursing competency, the work environment, and nursing service quality in a group of 182 nurses directly in charge of patient care at long-term care hospitals in special cities, metropolitan cities, and small and medium-size cities. The data were analyzed using IBM SPSS/WIN 27.0 version. Results: In long-term care hospitals, nurses' competence (r=.674, p<.001) and work environment (r=.444, p<.001) were correlated with quality of nursing service, and the nurses' competence was correlated with the work environment (r=.443, p<.001). The factors affecting the quality of nursing service in long-term care hospitals were competence, the work environment, and the age of nurses. Conclusion: In this study, both competence and the work environment of nurses were observed to be important factors in improving nursing service quality in long-term care hospitals. Therefore, efforts aimed at enhancing these factors are necessary to ensure the high quality of nursing service in these hospitals.

  • 본 연구는 요양병원 간호사의 간호근무환경, 도덕적 고뇌가 인간중심돌봄의 관계를 확인하고, 인간중심돌봄에 미치는 영향을 파악하기 위한 서술적 조사연구이다. D시와 G도 소재 요양병원에서 현재 근무하는 간호사를 대상으로 시행되었다. 자료수집은 2020년 5월 1일부터 31일까지로 총 154명을 SPSS 25.0로 분석하였다. 간호근무환경은 2.42점, 도덕적 고뇌는 3.27점, 인간중심돌봄은 3.60점이었다. 인간중심돌봄의 영향요인은 간호근무환경, 도덕적 고뇌로 나타났다. 이는 요양병원 간호사의 인간중심돌봄을 증진시키기 위한 방안으로 관리자들의 지속적인 관심으로 요양병원 간호사의 근무환경 개선과 도덕적 고뇌의 감소를 위한 실질적인 방안 마련이 필요해 보인다.
