Development of Dementia Care Model in a Community (지역사회 치매관리 모형 개발 : 광명시의 경우)

  • There has been a dramatic increase in public awareness regarding dementia during recent years. However, dementia remains a family affair and patients do not receive adequate care in Korea. This study aims to assist patients and their caregivers by establishing Home and Community based Long-Term Care in a city. The data collected for analysis include five main categories: dementia prevalence, limitations of daily activities of patients, burden of caregivers, the services that patient's family want to utilize, the resources that handle dementia in the community. Major findings can be summarized as follows: 1)The prevalence rate of dementia for elderly people is 13.1 per 100 persons. Alzheimer's disease amount to 38.9% of dementia patients and vascular dementia account for 36.7% of them 2)Eight out of ten patients have mild dementia. Almost all patients have normal ADL. IADL, however, shows different picture. In every items of IADL, about 60% of patients reveals some limitations. 3)The proportion of patients who had medical diagnosis is as low as 20%. Families of patients think dementia as normal aging process and medical doctors in the community do not give special concern to dementia patients. 4)Caregivers does not have proper social support. They suffer from long care time, experience large obstacles in respect of health, daily living, and social activity. 5)Health center and Community welfare center have launched some programs-consultation, home-visiting nursing, day care center, voluntary force mobilization and so on-for dementia patients. But they do not perform expected roles and functions because of lack of skilled personnels and inadequate coordination of relevant organizations for dementia care. 6)Families of dementia patients prefer home helper and home-visiting nurse to hospitalization. For the future, however, demand for institution-based long-term services will increase. We develope community dementia care model based on above findings as follows: 1)Health center execute community cardiovascular control program for the prevention of vascular dementia. 2)Refer to epidemiologic characteristics of patients and preference of family, the most urgent task for dementia care in this city is to expand and organize Home and Community based Long-Term Care. 3)For the continuous and comprehensive care, care plan for a patient must be prepared. Case management team should be builded to prepare this plan and coordinate relevant resources. 4)Special long-term care unit for dementia will be needed in a near future. This unit should have multiple functions, such as day-care center, short stay facility, training center for relevant personnels, besides long-term nursing home considering effective care of dementia and efficient operation of the facility. 5)Voluntary workers deserve their due efforts. Incentive mechanisms must be developed to activate voluntary activities.

  • Korea faces new social needs of community-based service for senior citizen as aging is getting fasten. Targeting elderly people who are beneficiaries of long-term care insurance and caring service, this paper aims to explore decent and proper services for senior citizens in terms of rehabilitation, health improvement and quality of life. Especially this research approaches from the perspective of service providers, including service facilities and care-service givers. Using FGI methods with care-providers and in-depth interviews with community social workers as well as nurses, convergence approaches are explored. As a result, welfare field and nursing practices are clearly divided in the community, so that the convergence education and team work tool would be developed in the near future.

Current Status and Factors Related to Physical Activity of Older Nursing Home Residents with and without Dementia: Effects of Individual and Institutional Characteristics (장기요양시설 거주노인의 치매 여부에 따른 신체활동수준과 관련 요인: 개인 및 기관 특성의 영향)

  • Background: Physical activity (PA) is critical for maintaining the health and well-being of older people in community and also institutional settings. The purpose of this study was to examine the current status of PA and related individual and organizational factors among older nursing home residents with and without dementia. Methods: This is a secondary data analysis study of a nationwide survey of 92 long-term care facilities in Korea, and the study sample includes older residents with dementia (n= 753) and without dementia (n= 480). The level of PA was measured by PA time and whether or not residents had an outdoor activity over the past 3 days. Multi-variate, multi-level analyses were conducted. Results: More than half of the sample in both groups had no or less than 1 hour of PA. About one out of four older people in our sample had no outdoor activity over the past 3 days regardless of whether or not they had dementia. Among the people without dementia, several individual-level factors were associated with PA, including dependency for activities of daily living, social activity participation, and caregivers' belief in the functional improvement of residents. Unlike the non-dementia group, individual- and organizationallevel factors including staffing level and having the relevant equipment for PA were associated with PA among those with dementia. Conclusion: Study findings provide evidence on the lack of PA among older nursing home residents, and the importance of institutional capacity with regard to human resources and physical equipment for promoting PA among people with dementia, in particular. Policies and supports are needed to promote the implementation of healthy aging programs including PA for older nursing home residents. Such programs should be person-centered with consideration to the physical and cognitive status of individual residents.

The effects of long term endurance or resistance exercise training on anabolic and catabolic pathway in skeletal muscle of middle-aged rats (장기간의 지구성 운동 또는 저항성 운동이 중년 흰쥐의 골격근 내 단백질 동화 및 이화기전에 미치는 영향)

  • The purpose of this study was to investigate the effects of long-term endurance exercise or resistance exercise training on muscle anabolic/catabolic pathway. 50wks-old male Wistar rats(n=30) were randomly assigned for 3 groups (sedentary, endurance exercise, resistance exercise group). After 12-week of training, plantaris muscles were dissect to measure protein level. Akt/mTOR signal-related proteins were significantly increased only after resistance exercise training, but catabolic signal-related proteins, FoxO1 and MuRF1, were significantly decreased after resistance and endurance exercise training. After endurance exercise training, AMPK and PGC-1α protein levels were significantly increased. Therefore, the endurance exercise training has been shown to affect the protein balance of aging muscle through inhibition of muscle protein catabolism. The present results suggest the possibility that not only resistance exercise but also endurance exercise will be affectable to keep or increase muscle volume and capacity of middle-aged people.

IDENTIFICATION AND ASSESSMENT OF AGING-RELATED DEGRADATION OCCURRENCES IN NUCLEAR POWER PLANTS

  • Aging-related degradation of nuclear power plant components is an important aspect to consider in securing the long term safety of the plant, especially the seismic safety, since the degradation of the components affects not only their seismic capacity but their response. This can cause a change in the seismic margin of a component and the overall seismic safety of a system. To better understand the status and characteristics of degradation of components in Nuclear Power Plants (NPPs), the degradation occurrences of components in the U.S. NPPs were identified by reviewing recent publicly available information sources and the characteristics of these occurrences were evaluated and compared to observations from the past. Ten categories of components that are of high risk significance in Korean NPPs were identified, comprising anchorage, concrete, containment, exchanger, filter, piping systems, reactor pressure vessels, structural steel, tanks, and vessels. Software tools were developed to expedite the review process. Results from this review effort were compared to previous data in the literature to characterize the overall degradation trends.

  • Recently, concern over climate change and global politics associated with traditional fossil fuel energy sources has driven significant increase in wind energy utilization over the past decade around world. Generally, life-time of wind turbine system should be guaranteed for twenty years. Therefore, performance deterioration of wind turbine system occurs owing to aging effects for long term operation. In this work, a new type of auto tuning algorithm for overcoming the problem of performance deterioration is proposed. Furthermore, various simulations are carried out to verify the feasibility of the proposed scheme.

A Comparison of the Voice Differences of Patients with Idiopathic Parkinson's Disease and a Normal-Aging Group (파킨슨병 환자와 정상 노인의 음성비교)

  • In view of the hypothesis that the effects of Parkinson disease on voice production can be detected before pharmacological intervention, the voice differences of patients with Idiopathic Parkinson's disease and a healthy aging group were diagnostically analyzed with the long term object of establishing, for clinical purposes, early disease-progression biomarkers. Fifteen patients with Idopathic Parkinson's disease (prior to pharmacological intervention) and a healthy control group of 15 were selected and every voice was recorded three times using praat (ver. 5022) with a headset mic. Relevant parameters - acoustic measure of /a/ phonation, F0 related parameters, MPT related parameters, articulatory ratio, VOT - were then analyzed by MANOVA. Significant differences were found in the F0 related (low F0, high F0, F0 range) and MPT related parameters. There were also significant differences in acoustic measurements (intensity, shimmer, HNR, jitter), AMR (/$t{\Lambda}$/,/$k{\Lambda}$/) and VOT (/ta/), The findings indicated that the voice production of patients with Idiopathic Parkinson's disease have normal pitch but bad quality. In particular, with slow articulatory ratios and VOT values, the tongue tip functioning of patients was lower than for the healthy group.

  • As a result of rapid aging speed in our society, many problems related to elderly people have happened in many parts of our society. Among them, supply for elderly housing is one of the biggest problems. To solve these problems, 'long-term care insurance' has been put in operation from July 2008. By the time of the insurance operation, Ministry of Health and Welfare is increasing facilities every year according to '10-year expending plan of Care service infra' from 2002. As a result, the supply rate of elderly facilities has been raised. But the differences of facility supply rate between regions are very high in some cases. Therefore older people who need care sometimes cannot get proper care services in some areas. In that case, the frail older people have to use other care facilities of other regions. This is not a proper situation from the point of "Aging in Place". In order to prevent that case, it is necessary to set up proper 'Daily Living Spheres' and establish elderly care plan for it. Considering the points above, this study proposes the size of 'Daily Living Spheres' for the elderly, the kind and amount of elderly care facilities in it for the construction of Community Based Elderly Care System.

  • Since our country is rapidly progressing from an aging society to an advanced age society, it already puts in effect basic old-age pension system, hurries training human resources like care-giver, and has ahead offering the aged long-term care insurance. But our country still needs to develop a systematic curriculum in order to train professionals that can fundamentally deal with an aging society problems. In this paper, therefore, we describe the development of a curriculum for training silver-care professionals improving qualities of old person life in order to suit to an aged society and prepare for an advanced age society. It includes a network model of a silver-care curriculum based on subject classifications according to fields and levels.

A Study on Elderly Entrepreneurial Intention in the Hospitality and Tourism Industry in China

  • Elderly entrepreneurship is becoming increasingly important as a response to the impact of the aging population and the resulting demand on government support systems, as well as a means of ensuring long-term economic and social development. The purpose of this study is to investigate the elderly's entrepreneurial intentions in the hotel and tourism sector in Ma'anshan City, Eastern China. The researcher used an online survey of older people aged 50 to 64 in a tourism destination that is approaching an aging society stage to see if the Theory of Planned Behavior can explain the entrepreneurial ambitions of the elderly in the hospitality and tourism industry. There were 391 questionnaires gathered in all, 367 of which were valid. The data was analyzed using descriptive statistics and regression analysis. The study reveals that personal attitudes toward entrepreneurial behavior and perceived behavior control are highly influenced by societal norms. It also shows that personal attitude and perceived behavior control are antecedents of the elderly's entrepreneurial intent in this particular industry. This research adds to the research on geriatric entrepreneurship in hospitality and tourism, as well as human resource development for seniors in China, helping to alleviate the country's aging demographic concerns.
