트레드밀 운동이 뇌졸중 환자의 폐기능과 보행능력에 미치는 효과 : 메타분석 (Effects of Treadmill Exercise on Pulmonary Function and Gait Capacity in Stroke Patients : A meta-analysis)

  • Purpose : This study aimed to conduct a comprehensive meta-analysis to evaluate the effect size for pulmonary function and gait capacity of treadmill exercise in stroke patients. In addition, we aimed to examine the current status of treadmill interventions and compare the effect sizes of treadmill training to provide evidence-based practice for future development and application. Methods : The meta-analysis study was conducted using research articles that applied treadmill exercise to stroke patients and were published between January 2005 and February 2020. For a systematic meta-analysis, we conducted a search using the PICOS framework and selected 25 domestic stroke- and treadmill-related studies. The Comprehensive Meta-Analysis program was used to calculate the effect sizes for lung function and gait capacity (6-minute walk test and 10 m walk test). As Cohen's d has a tendency to overestimate the effect size, we used Hedge's g to increase the accuracy in computing the effect size. Results : Based on the results of the meta-analysis, the total effect size of treadmill exercise was 0.535, which was statistically significant, with a medium effect size (p<0.05). The effects of treadmill exercise in stroke patients were divided into dependent variables, namely pulmonary function (0.372) and gait capacity (0.584). In addition, for gait capacity, the effect sizes were evaluated for both the 6-minute walk test (0.756) and the 10 m walk test (0.514). Conclusion : This study provides objective evidence of the effectiveness of treadmill-based gait training programs. We hope that the results of this study will help support the development and implementation of treadmill-based gait training in stroke patients. Treadmill training is expected to improve not only pulmonary function, but also the gait capacity of stroke patients. Long-term investigations on the effects of treadmill training in stroke patients are necessary.

  • 뇌졸중 질환은 전세계적으로 가장 중요한 사망원인 중 하나이며, 특히 고령자에게 장애의 원인이 되는 가장 중요한 질환이다. 뇌졸중 질환이 발생하면 사망 또는 심각한 장애를 유발하기 때문에, 적극적인 일차 예방과 전조증상의 빠른 발견이 매우 중요하다. 특히, 일상생활에서의 뇌졸중 전조증상 발병을 감지 및 정확히 예측하여 전문가의 신속한 진단을 유도할 수 있어야 한다. 최근까지의 연구에서는 뇌졸중 환자의 전조증상을 예측하는 방법론으로 CT(Computed Tomography)나 MRI(Magnetic Resonance Imaging)와 같은 영상 분석이 대부분이었으나, 이러한 접근에는 오랜 검사 시간과 높은 검사 비용 등에 대한 한계점을 가지고 있다. 본 논문에서는 고령자의 뇌졸중 질환 발병이 보행 시 족압(Foot Pressure)에 어떤 영향을 미치는지 임상 데이터를 이용해 실험하였다. 실험 결과, 보행 중에 뇌졸중 고령자와 일반 고령자 간에 12개의 셀에서 * p < .05 인 유의미한 차가 있음을 분석 및 검증하였다. 결과적으로 고령의 뇌졸중 환자와 일반 고령자의 일상생활의 보행 패턴에 유의미한 차이를 발견했다는 것에 그 의미가 크다고 할 수 있다.

장기간 한약 복용한 뇌졸중 환자의 간 및 신장 기능 수치 변화에 대한 증례 보고 (A Case Report of Liver and Renal Function Changes in Stroke Patients Taking Long-term Herbal Medicine)

  • Objectives: This case report describes a patient who had suffered a stroke and who was treated for a long period with herbal medicine in combination with a Western medicine. Methods: Changes in liver function and renal function laboratory tests were measured while the patient took herbal medicine during the 1190 days of hospitalization. Results: During the hospitalization period, the liver function test (LFT) levels were elevated two fold, but after taking the herbal medicine, the LFT levels were stabilized and have remained stable since. Conclusions: The high risk of prescribing herbal medicine by an unlicensed person and the possibility of long-term herbal medicine usage for stabilizing liver function test were shown.

  • In this paper, a novel planar type magnetic levitation system without other assistant devices is proposed and it can move with 6 degree of freedom (X, Y, Z, ${\theta}_X$, ${\theta}_Y$, ${\theta}_Z$) in wafer size as well as in nano scale positioning.The mover is composed with 2-D Halbach permanent magnet array and the stator is composed with $10{\times}10$ coil arrays.It was composed in laboratory and tested with short stroke (4 [mm]) and long stroke (160 [mm])movements. Activity recognition of stroke-affected people using wearable sensor

  • Stroke is one of the leading causes of long-term disability worldwide, placing huge burdens on individuals and society. Further, automatic human activity recognition is a challenging task that is vital to the future of healthcare and physical therapy. Using a baseline long short-term memory recurrent neural network, this study provides a novel dataset of stretching, upward stretching, flinging motions, hand-to-mouth movements, swiping gestures, and pouring motions for improved model training and testing of stroke-affected patients. A MATLAB application is used to output textual and audible prediction results. A wearable sensor with a triaxial accelerometer is used to collect preprocessed real-time data. The model is trained with features extracted from the actual patient to recognize new actions, and the recognition accuracy provided by multiple datasets is compared based on the same baseline model. When training and testing using the new dataset, the baseline model shows recognition accuracy that is 11% higher than the Activity Daily Living dataset, 22% higher than the Activity Recognition Single Chest-Mounted Accelerometer dataset, and 10% higher than another real-world dataset.

  • Nowadays, the axial stiffness of crankshaft of long-stroke diesel engine is low compared to that of the old types of engine by increasing stroke/bore ratio and major critical speed might occur within engine operation speed. An axial damper needs to be installed in order to reduce the axial vibration of the crankshaft in the event of reduced or stopped axial damper function are discussed.

  • The scavenging efficiency has a great influence on the performance of a diesel engine, especially slow two-stroke diesel engines which are usually used as a marine propulsion power plant. And this is greatly affected by the conditions in the cylinder, scavenging manifold and exhaust manifold during the gas exchange process. There are many factors to affect on the scavenging efficiency and these factors interact each other very complicatedly. Therefore the simulation program of the gas exchange process is very useful to improve and predict the scavenging efficiency, due to the high costs associated with redesign and testing. In this paper, a three-zone scavenging model for two-stroke uniflow engines was developed to link a control-volume-type engine simulation program for performance prediction of long-stroke marine engines. In this model it was attempted to simulate the three different regions perceived to exist inside the cylinder during scavenging, namely the air, mixing and combystion products regions, by modeling each region as a seperate control volume. Finally the scavenging efficiency was compared with three type of scavenging modes, that is, pure displacement, partial mixing and prefect mixing.

중풍환자의 불면에 대한 산조인 단미(안심산)의 유효성 및 적응증 평가 (Effectiveness of Zizyphus seed (Ansim-san) for Insomnia in Stroke Patients)

  • Objectives : Zizyphus seed has long been used in hypnotics and sedatives in oriental medicine, and it is reported that the Zizyphus seed elicited a variety of pharmacological actions besides CNS depression. We did a clinical study on the effectiveness of Zizyphus seed (Ansim-san) for insomnia in stroke patients. Methods : To 41 stroke patients suffering from insomnia, we administered the extract of Zizyphus seed (Ansim-san). Outcome measures were subjective in nature and included a morning questionnaire, visual analog scales, and observation forms by study personnel. We also, to find the applicable symptom-complex of Zizyphus seed (Ansim-san), examined accompanying symptoms. Results : 1. Zizyphus seed extract use meant that sleep latency was shorter, total sleep time was longer, number of wakenings was lower, wake time during sleep was shorter, sleep quality was improved, condition upon awakening was improved, ability to concentrate was improved, and the patients fell asleep more easily (P<0.001). There were no differences in morning sleepiness. 2. Among accompanying symptoms, an oppressed feeling in the chest was significant (P=0.035) in the patients who improved sleep quality. Conclusions : The results of this clinical study suggest that in stroke patients, Zizyphus seed (Ansim-san) can be effective for insomnia, and significantly effective in patients who have an oppressed feeling in the chest.

뇌졸중 환자를 위한 전문요양시설의 필요성 (The Necessity of Skilled Nursing Facilities for Stroke patients)

  • The patients with a central nervous impediment and a sequela caused by a Stroke are continuously increasing, and the burden of family and society for Stroke patients are also increasing. Since Stroke patients are required to receive a long time medical treatment and care. It adds a economical burden as well as a mental and physical burden of patient's family. In addition, it is a primary factor which deepens the disparity of medical resources in social aspect. But the social concern for these patients are very insufficient and are entirely left to the patient's family The mediation of Skilled Nursing Facility for Stroke patients enable to receive more special and proper medical treatment and recuperation service in the middle of time when they return to home from hospital. It also contributes to the settlement of the charge of patient's family and the serious disparity of medical resources. Therefore, this study will summon the social understanding of the necessity of Skilled Nursing Facilities through the above debate.

뇌 저산소증 영상 (Brain Hypoxia Imaging)

  • The measurement of pathologically low levels of tissue $pO_2$ is an important diagnostic goal for determining the prognosis of many clinically important diseases including cardiovascular insufficiency, stroke and cancer. The target tissues nowaday have mostly been tumors or the myocardium, with less attention centered on the brain. Radiolabelled nitroimidazole or derivatives may be useful in identifying the hypoxic cells in cerebrovascular disease or traumatic brain injury, and hypoxic-ischemic encephalopathy. In acute stroke, the target of therapy is the severely hypoxic but salvageable tissue. $^{18}F-MISO$ PET and $^{99}mTc-EC-metronidazole$ SPECT in patients with acute ischemic stroke identified hypoxic tissues and ischemic penumbra, and predicted its outcome. A study using $^{123}I-IAZA$ in patient with closed head injury detected the hypoxic tissues after head injury. Up till now these radiopharmaceuticals have drawbacks due to its relatively low concentration with hypoxic tissues associated with/without low blood-brain barrier permeability and the necessity to wait a long time to achieve acceptable target to background ratios for imaging in acute ischemic stroke. It is needed to develop new hypoxic marker exhibiting more rapid localization in the hypoxic region in the brain. And then, the hypoxic brain imaging with imidazoles or non-imidazoles may be very useful in detecting the hypoxic tissues, determining therapeutic strategies and developing therapeutic drugs in several neurological disease, especially, in acute ischemic stroke.
