Concept Analysis of the Rehabilitation in Elderly with Stroke Disease (노인 뇌졸중 질환자의 재활에 대한 개념분석)

  • Purpose: The goal of this study was to define and clarify the concept of 'rehabilitation' of stroke patients in the elderly. Method: For analyzing the concept of rehabilitation, this study used the hybrid model, which involves an in depth interview six stroke patients. Result: The concept of rehabilitation of patients with a stroke condition can be categorized with two dimensions; overcoming, new recovery aspects. Besides, three attributes and twelve indicators were analyzed. The concept of rehabilitation of patients with a stroke condition can be expressed as one of the following processes; It can be said that it is a process of accepting and overcoming one's physical limitations in order to overcome, and to grow resilience by making a leap from crisis to recovery with inner enforcement in dimension of new recovery. Conclusion: The rehabilitation is important for stroke conditioned patients in order to control their disease in the long term. The attributes and indicators drawn by this study could be used for tool development and useful resources of rehabilitation competence for stroke conditioned patients.

A study on the calculation of Synthesized torsional vibration for the marine diesel engine shafting by the modal analysis method (모오드 해석법에 의한 박용디젤기관 추진축계의 합성 비틀림 진동계산에 관한 연구)

  • The calculation of torsional vibration for marine diesel engine propulsion shafting is normally carried out by equalizing exciting energy to damping energy, or using the dynamic magnifier. But, with these methods, the vibration amplitudes are calculated only for resonance points and vibration amplitudes of other running speeds of engine are determined by the estimation. Recently, many energy-saving ships have been built and on these ships, two-stroke, supercharged, super-long stroke diesel engines which have a small number of cylinders are usually installed. In these cases, the first order critical-torsional vibrations of these engine shaftings appear ordinarily near the MCR speed and the stress amplitudes of their vibration skirts exceed the limit stress defined by the rules of classification society. To predict the above condition in the design stage, the synthesized vibration amplitudes of all orders which are summed up according to their phase angles must be calculated from the drawings of propulsion shaft systems. In this study, a theoretical method to fulfill the above calculation is derived and a computer program is developed according to the derived method. And a shafting system of two-stroke, super-long stroke diesel engine which was installed in a bulk carrier is analyzed with this method. The measured values of this engine shafting are compared with those of calculated results and they show a fairly good agreement.

High Precision Hybrid Milling Machine Using Dual-Stage (듀얼스테이지를 이용한 고정밀도의 하이브리드 밀링머신)

  • High precision machining technology has become one of the important parts in the development of a precision machine. Such a machine requires high speed on a large workspace as well as high precision positioning. For machining systems having a long stroke with ultra precision, a dual-stage system including a global stage (coarse stage) and a micro stage (fine stage) is designed in this paper. Though linear motors have a long stroke and high precision feed drivers, they have some limitations for submicron positioning. Piezo-actuators with high precision also have severe disadvantage for the travel range, and the stroke is limited to a few microns. In the milling experiments, the positional accuracy has been readily achieved within 0.2 micron over the typical 20 mm stroke, and the path error over 2 micron was reduced within 0.2 micron. Therefore, this technique can be applied to develop high precision positioning and machining in the micro manufacturing and machining system.

Two cases of Combination Therapy of Acupuncture, Herbal medication and Speech Therapy for Aphasic Stroke Patients (중풍 후유증으로 인한 실어증 환자에 한방치료와 언어치료를 병행한 경험2례)

  • Aphasia is frequent in stroke patients and most patients with aphasia exhibit spontaneous progressive improvement in language abilities over time, but few recover completely. Neurological variables, especially initial severity of aphasia and time post-onset, appear to have influence on improvement. Effect of speech therapy and pharmacotherapy has been studied and some drugs, like amphetamine are proved to be benefit for recovery of aphasia following stroke. But there has been few evidence to facilitate recovery from aphasia by acupuncture or herbal medication therapy. So we report two cases of aphasic stroke patients who treated by combination therapy of acupuncture, herbal medication(Cheongsinhaeo-tang) and speech therapy over 6 months and improved in language abilities. Further clinical studies will be needed to explore the effects of acupuncture and herbal medication therapy for aphasia. Researchers should examine the long term effect of these treatment, and whether it is more effective than speech therapy and western pharmacotherapy or not..

Effectiveness of Bee Venom Acupuncture on Shoulder Pain after Stroke (뇌졸중 환자의 견관절 통증에 대한 봉독약침의 임상효과)

  • Objectives : Hemiplegic shoulder pain is one of the common sequelae of stroke. Although many different methods of treatment are applied, none have yet been proved to be effective. In this study we assessed the effectiveness of bee venom in patients with shoulder pain after stroke. Methods : This study was a double blinded, randomized, placebo-controlled clinical trial. All subjects received either 0.6 ml bee venom injection (venom:saline = 1:10000) or 0.6ml physiological saline solution (placebo) at three acupoints. The effectiveness was assessed by visual analogue scale, pain rating score, Fugl-Meyer assessment and passive external rotation. Results : There were 24 patients in the venom group and 22 patients in the placebo group. The venom group5,hewed more effectiveness in VAS and PRS than the placebo group. Conclusions : This study suggests that bee venom injection has significant analgesic effect on hemiplegic shoulder pain. Further study based on multi-centers, larger population, and long term 1311ow-up is needed to confirm this suggestion.

Paradigm Shift in Intra-Arterial Mechanical Thrombectomy for Acute Ischemic Stroke : A Review of Randomized Controlled Trials after 2015

  • Three randomized control trials (RCTs), published in 2013, investigated efficacy of mechanical thrombectomy in large vessel occlusions and did not show better results compared to intravenous (IV) recombinant tissue-type plasminogen activator (tPA) alone. However, most clinicians treating stroke consider mechanical thrombectomy as the standard treatment rather than using IV tPA alone. This paradigm shift was based on five RCTs investigating efficacy of mechanical thrombectomy in acute ischemic stroke conducted from 2010 to 2015. They demonstrated that mechanical thrombectomy was effective and safe in acute ischemic stroke with anterior circulation occlusion when performed within 6 hours of stroke onset. There are four reasons underlying the different results observed between the trials conducted in 2013 and 2015. First, the three RCTs of 2013 used low-efficiency thrombectomy devices. Second, the three RCTs used insufficient image selection criteria. Third, following the initial presentation at the hospital, reperfusion treatment required a long time. Fourth, the three RCTs showed a low rate of successful recanalization. Time is the most important factor in the treatment of acute ischemic stroke. However, current trends utilize advanced imaging techniques, such as diffusion-weighted imaging and multi-channel computer tomographic perfusion, to facilitate the detection of core infarction, penumbra, and collateral flows. These efforts demonstrate that patient selection may overcome the barriers of time in specific cases.

A Study on Manually and Continuously Variable Impact Force Control Device Development for Hydraulic Breakers (유압브레이커의 수동 무단 타격력 제어기구 개발에 대한 연구)

  • In this paper, the development of a manually and continuously variable impact force control mechanism for hydraulic breakers was studied. Generally, a hydraulic breaker has one or two piston strokes. Hydraulic breakers, which have two strokes, have two valve-switching ports and make short and long piston strokes. The piston stroke valve controls the piston stroke by opening and closing a short stroke-switching port. The short piston stroke mode is used to break soft rock, concrete, or asphalt. This stroke control valve system is not popular for small hydraulic breakers mounted on 1 to 14-ton excavators. To preserve the carrier-like excavator, proper breaking force is needed, and it can be easily controlled by multiple piston stroke control valves. The easiest way to control these breakers is to use several switching ports and valves but they are not easy to install in small hydraulic breakers and are expensive. To use only one switching port and valve, a method can be used to change the open area of the switching port to delay valve switching. This method provides multiple piston strokes.

Relationships of Activities of Daily Living and Body Image with Quality of Life in Stroke Patients: Mediating Effects of Interpersonal Relations (뇌졸중 환자의 일상생활 수행능력, 신체상이 삶의 질에 미치는 영향: 대인관계 매개효과 중심)

  • Purpose: This study was conducted to identify factors associated with stroke patients' quality of life (QOL) and examine the mediating effects of interpersonal relations in the relationships of activities of daily living (ADL) and body image with QOL. Methods: In this study, 160 stroke patients were recruited from an outpatient clinic of a university hospital and rehabilitation clinic of a long-term care hospital. Participants completed a questionnaire which included sociodemographic characteristics, ADL, body image, interpersonal relation, and QOL. Descriptive statistics, t-tests, ANOVA, Pearson's correlation coefficients, multiple regression analysis, and process macro mediation analysis were conducted using SPSS. Results: The mean scores were 91.01 (range 6~100) for ADL, 61.19 (range 40~79) for body image, 87.53 (range 29~123) for interpersonal relations, and 186.67 (range 71~243) for QOL. Multiple regression analyses showed that ADL, body image, interpersonal relations, and participation in group activities after stroke were significantly associated with QOL among stroke patients (p<.05). There were no mediating effects of interpersonal relations in the relationships of ADL and body image with QOL (p>.05). Conclusion: This study showed that there is a need for physical, psychological, and social recovery to improve the QOL of stroke patients.

A Study on Performance Simulation of an Reciprocating Engine for Small Long Endurance Unmanned Aerial Vehicles (소형 장기체공 무인기용 왕복엔진 성능 예측 시뮬레이션 연구)

  • Development of an engine with good fuel economy is very important for successful implementation of long endurance miniature UAVs (unmanned aerial vehicles). In the study, a 4-stroke glow-plug engine was modified to a gasoline-fueled spark-ignition engine. Engine tests measuring performance and friction losses were conducted to tune a simulation program for performance prediction. It has been found that excessive friction losses are caused by insufficient lubrication at high speeds. The simulation program predicts that engine power and fuel economy get worse with high altitude due to increasing portion of friction losses. The simulation results suggest quantitative guidelines for further development of a practical engine.

  • Recently, it was very difficult for hydraulic governor to regulate the speed of high power engine with long stroke at low speed and low load, because of the jiggling phenomena by rough fluctuation of rotating torque and the hunting phenomena by long dead time occurred in fuel combustion process in the engine cylinder. In this paper, the influence of engine dead time is investigated by Nickels chart, and hybrid controller selected advantages of PID and fuzzy logic controller is provided to improve the performance of speed control of a low speed and long stroke diesel engine.
