Home Healthcare Service Awareness Survey for Korean Medicine Doctors: a survey study

  • Objectives: Discussions regarding "medical blind spots" in Korea's "aging society" are continuously rising. In addition, the demand for medical attention and care for the elderly and vulnerable populations continues to increase. Given this, the government is promoting the "home healthcare service" project. This study aims to lay the foundation for promoting this project by investigating the perception of clinical Korean Medicine (KM) doctors in the "community health care" project. Methods: With the cooperation of the Association of Korean Medicine, we sent a questionnaire to all KM doctors through e-mail. The survey included personal information, awareness, appropriate disease and intervention, proper visit location, and pros and cons. Results: A total of 602 responses were collected and analyzed. Approximately 20% of the doctors answered that they were well aware of the service, while 55% responded that they did not know about it. For a visit, a KM doctor selected the appropriate diseases in the order of stroke, dementia and Parkinson's disease, osteoarthritis, and chronic diseases. Among treatments, acupuncture, moxibustion, and herbal medicine exhibited similar results. The most common opinion was that KM doctors should schedule their visits once a week for 6-12 months, which was the most prolonged period among the given options. More than 80% (84.1%) of the doctors replied that care projects were highly essential, and about 63.8% expressed their willingness to participate in these projects. Conclusion: To provide appropriate home health care, we must raise awareness among Korean medicine doctors. In addition, the healthcare budget must be increased to provide the required support.

스마트 환경에서의 사용자 상황인지 기반 지식 필터링을 이용한 콘텐츠 추천 시스템 (Content Recommendation System Using User Context-aware based Knowledge Filtering in Smart Environments)

  • 스마트 환경에서는 센서, 디스플레이, 스마트폰 등 각종 장치들이 존재하며, 이러한 장치들을 이용하여 다양한 콘텐츠가 제공될 수 있다. 그러나 방대한 양의 콘텐츠가 다수의 사용자들에게 제공되고 있지만, 대부분의 환경에서 사용자에 대한 고려가 없거나 위치, 시간 등의 간단한 요소만을 고려하고 있어 사용자를 위한 유의미한 콘텐츠 제공에 한계가 있다. 이에 본 논문에서는 사용자에게 맞춤형 콘텐츠를 제공하기 위해 사용자, 장치, 콘텐츠가 가진 상황 정보를 인지하여 콘텐츠를 추천할 수 있는 시스템인 상황인지 기반 콘텐츠 추천 시스템을 제시한다. 상황인지 기반 콘텐츠 추천 시스템은 스마트 환경의 컨텍스트를 추론하고 사용자와 콘텐츠의 정보를 이용하여 사용자의 콘텐츠별 선호도를 산출하고 사용자에게 콘텐츠를 추천한다. 이러한 시스템의 프로세스를 구축하기 위해 도메인 지식을 온톨로지 모델로 구축하고, 콘텐츠 추천 시스템을 설계 및 구현하기 위한 방법을 제시한다. 그리고 부산의 센텀시티를 도메인으로 하여 사례 연구를 진행하며 산출된 0.8730의 평균 절대값 오차를 이용하여 제시한 시스템의 콘텐츠 추천 성능의 우수성을 검증하였다.

Shape and anatomical relationship of the mental foramen to the mandibular premolars in an Indian sub-population: a retrospective CBCT analysis

  • Objectives: This study assessed the shape and anatomical relationship of the mental foramen (MF) to mandibular posterior teeth in an Indian sub-population. Materials and Methods: In total, 475 existing cone-beam computed tomography records exhibiting 950 MFs and including the bilateral presence of mandibular premolars and first molars were assessed. Images were evaluated 3-dimensionally to ascertain the position, shape, and anatomical proximity of MFs to mandibular teeth. The position and shape of MFs were measured and calculated. The Pythagorean theorem was used to calculate the distance between the root apex of the mandibular teeth and the MF. Results: MFs exhibited a predominantly round shape (left: 67% and right: 65%) followed by oval (left: 30% and right: 31%) in both males and females and in different age groups. The root apices of mandibular second premolars (left: 71% and right: 62%) were closest to the MF, followed by distal to the first premolars and mesial to the second premolars. The mean vertical distance between the MF and the nearest tooth apex calculated on sagittal sections was 2.20 mm on the right side and 2.32 mm on the left side; no significant difference was found according to sex or age. The distance between the apices of the teeth and the MF was ≥ 4 mm (left; 4.09 ± 1.27 mm and right; 4.01 ± 1.15 mm). Conclusions: These findings highlight the need for clinicians to be aware of the location of the MF in treatment planning and while performing non-surgical and surgical endodontic procedures.

Hemorrhagic Complications Following Ultrasound-Guided Breast Biopsy: A Prospective Patient-Centered Study

  • Objective: We aimed to evaluate the clinical and imaging factors associated with hemorrhagic complications and patient discomfort following ultrasound (US)-guided breast biopsy. Materials and Methods: We prospectively enrolled 94 patients who were referred to our hospital between June 2022 and December 2022 for US-guided breast biopsy. After obtaining informed consent, two breast radiologists independently performed US-guided breast biopsy and evaluated the imaging findings. A hemorrhagic complication was defined as the presence of bleeding or hematoma on US. The patients rated symptoms of pain, febrile sensation, swelling at the biopsy site, and dyspnea immediately, 20 minutes, and 2 weeks after the procedure on a visual analog scale, with 0 for none and 10 for the most severe symptoms. Additional details recorded included those of nausea, vomiting, bleeding, bruising, and overall satisfaction score. We compared the clinical symptoms, imaging characteristics, and procedural features between patients with and those without hemorrhagic complications. Results: Of 94 patients, 7 (7%) developed hemorrhagic complications, while 87 (93%) did not. The complication resolved with 20 minutes of manual compression, and no further intervention was required. Vascularity on Doppler examination (P = 0.008), needle type (P = 0.043), and lesion location (P < 0.001) were significantly different between the groups. Patients with hemorrhagic complications reported more frequent nausea or vomiting than those without hemorrhagic complications (29% [2/7] vs. 2% [2/87], respectively; P = 0.027). The overall satisfaction scores did not differ between the two groups (P = 0.396). After 2 weeks, all symptoms subsided, except bruising (50% 2/4 in the complication group and 25% [16/65] in the no-complication group). Conclusion: US-guided breast biopsy is a safe procedure with a low complication rate. Radiologists should be aware of hemorrhagic complications, patient discomfort, and overall satisfaction related to this procedure.

불균질부 방사선치료 시 계획 선량의 정확성 평가 (Evaluation of Planning Dose Accuracy in Case of Radiation Treatment on Inhomogeneous Organ Structure)

  • 목 적: 불균질부를 포함하고 있는 치료부위의 치료계획 시 불균질 경계면에서의 TPS상 선량분포와 phantom을 이용하여 측정된 실제 선량분포를 비교하여 그 차이를 알아보고자 한다. 대상 및 방법: 4 cm 두께의 solid water phantom 사이에 폐와 유사한 밀도를 가진 8 cm 두께의 cork (density: 0.23 $g/cm^2$)를 위치시켜 phantom을 제작하여 CT 영상을 획득하였으며, 본원에서 사용하고 있는 Pinnacle 치료계획 시스템의 Collapsed-cone(CC) convolution 선량계산 알고리즘을 이용하여 6/15 MV 광자선으로 치료 계획된 선량분포와 실제 phantom에 EBT2 필름을 삽입해 측정한 선량을 비교 평가하였다. 또한 실제 폐암 환자와 유사한 치료계획을 비교하기 위해 Phantom 내부에 치료하고자 하는 종양부위(target volume)로 가정한 파라핀($3{\times}3{\times}3$ cm)을 Location "A" (일반조직과 떨어져있는 가상의 종양: 섬모델)와 Location "B" (일반조직과 붙어있는 가상의 종양: 반도모델)에 삽입하여 CT scan 후 치료계획을 시행하였다. 선량계획과 동일한 조건으로 Phantom을 set-up 후 Phantom의 paraffin target volume 경계면 A (Ant방향), B (Rt방향), C (Post 방향) point에 필름을 삽입하고 방사선을 조사하여 측정된 선량을 TPS선량과 비교평가 하였다. 결 과: 불균질 phantom을 이용한 계획선량과 측정선량과의 차이는 solid water와 cork 경계면을 제외한 부분에서 선량차이가 크지 않았지만 밀도가 급격히 변화하는 첫 번째 구간과 두 번째 구간에서 -5.4%~-12.6%의 선량감소를 보였다. 또한 paraffin target을 삽입한 실험에서는 Location "A"의 경우 실제 측정선량이 A, B, C point에서 각각 -2.5~-4.7%, -2.3~-2.8%, -4.5~-8.8%의 낮은 선량을 나타냈으며, Location "B"의 경우에도 A, B, C point에서 각각 0.08~5.27%, -3.17~-4.74%, -7.86~-11.56%의 선량 차이를 나타내었다. 결 론: 이번 연구의 결과 불균질부 내에서의 치료계획 시스템의 계획된 선량과 실제 측정된 선량에 오차의 가능성이 확인되었다. 급속도로 발전하고 있는 방사선 치료기술과 그만큼 정밀함을 요하는 치료계획 시 이러한 가능성에 대해 인지하고 선량검증에 대한 여러 방법들을 연구하고 개발하는 것이 치료의 발전과 필수적으로 동반되어야 할 것이며 본원에서도 이번 연구를 통해 치료계획 시 발생할 수 있는 변수에 대해서 더 주의 깊게 판단하고 적용할 수 있을 것으로 사료된다.

U-마켓에서의 사용자 정보보호를 위한 매장 추천방법 (A Store Recommendation Procedure in Ubiquitous Market for User Privacy)

  • Recently, as the information communication technology develops, the discussion regarding the ubiquitous environment is occurring in diverse perspectives. Ubiquitous environment is an environment that could transfer data through networks regardless of the physical space, virtual space, time or location. In order to realize the ubiquitous environment, the Pervasive Sensing technology that enables the recognition of users' data without the border between physical and virtual space is required. In addition, the latest and diversified technologies such as Context-Awareness technology are necessary to construct the context around the user by sharing the data accessed through the Pervasive Sensing technology and linkage technology that is to prevent information loss through the wired, wireless networking and database. Especially, Pervasive Sensing technology is taken as an essential technology that enables user oriented services by recognizing the needs of the users even before the users inquire. There are lots of characteristics of ubiquitous environment through the technologies mentioned above such as ubiquity, abundance of data, mutuality, high information density, individualization and customization. Among them, information density directs the accessible amount and quality of the information and it is stored in bulk with ensured quality through Pervasive Sensing technology. Using this, in the companies, the personalized contents(or information) providing became possible for a target customer. Most of all, there are an increasing number of researches with respect to recommender systems that provide what customers need even when the customers do not explicitly ask something for their needs. Recommender systems are well renowned for its affirmative effect that enlarges the selling opportunities and reduces the searching cost of customers since it finds and provides information according to the customers' traits and preference in advance, in a commerce environment. Recommender systems have proved its usability through several methodologies and experiments conducted upon many different fields from the mid-1990s. Most of the researches related with the recommender systems until now take the products or information of internet or mobile context as its object, but there is not enough research concerned with recommending adequate store to customers in a ubiquitous environment. It is possible to track customers' behaviors in a ubiquitous environment, the same way it is implemented in an online market space even when customers are purchasing in an offline marketplace. Unlike existing internet space, in ubiquitous environment, the interest toward the stores is increasing that provides information according to the traffic line of the customers. In other words, the same product can be purchased in several different stores and the preferred store can be different from the customers by personal preference such as traffic line between stores, location, atmosphere, quality, and price. Krulwich(1997) has developed Lifestyle Finder which recommends a product and a store by using the demographical information and purchasing information generated in the internet commerce. Also, Fano(1998) has created a Shopper's Eye which is an information proving system. The information regarding the closest store from the customers' present location is shown when the customer has sent a to-buy list, Sadeh(2003) developed MyCampus that recommends appropriate information and a store in accordance with the schedule saved in a customers' mobile. Moreover, Keegan and O'Hare(2004) came up with EasiShop that provides the suitable tore information including price, after service, and accessibility after analyzing the to-buy list and the current location of customers. However, Krulwich(1997) does not indicate the characteristics of physical space based on the online commerce context and Keegan and O'Hare(2004) only provides information about store related to a product, while Fano(1998) does not fully consider the relationship between the preference toward the stores and the store itself. The most recent research by Sedah(2003), experimented on campus by suggesting recommender systems that reflect situation and preference information besides the characteristics of the physical space. Yet, there is a potential problem since the researches are based on location and preference information of customers which is connected to the invasion of privacy. The primary beginning point of controversy is an invasion of privacy and individual information in a ubiquitous environment according to researches conducted by Al-Muhtadi(2002), Beresford and Stajano(2003), and Ren(2006). Additionally, individuals want to be left anonymous to protect their own personal information, mentioned in Srivastava(2000). Therefore, in this paper, we suggest a methodology to recommend stores in U-market on the basis of ubiquitous environment not using personal information in order to protect individual information and privacy. The main idea behind our suggested methodology is based on Feature Matrices model (FM model, Shahabi and Banaei-Kashani, 2003) that uses clusters of customers' similar transaction data, which is similar to the Collaborative Filtering. However unlike Collaborative Filtering, this methodology overcomes the problems of personal information and privacy since it is not aware of the customer, exactly who they are, The methodology is compared with single trait model(vector model) such as visitor logs, while looking at the actual improvements of the recommendation when the context information is used. It is not easy to find real U-market data, so we experimented with factual data from a real department store with context information. The recommendation procedure of U-market proposed in this paper is divided into four major phases. First phase is collecting and preprocessing data for analysis of shopping patterns of customers. The traits of shopping patterns are expressed as feature matrices of N dimension. On second phase, the similar shopping patterns are grouped into clusters and the representative pattern of each cluster is derived. The distance between shopping patterns is calculated by Projected Pure Euclidean Distance (Shahabi and Banaei-Kashani, 2003). Third phase finds a representative pattern that is similar to a target customer, and at the same time, the shopping information of the customer is traced and saved dynamically. Fourth, the next store is recommended based on the physical distance between stores of representative patterns and the present location of target customer. In this research, we have evaluated the accuracy of recommendation method based on a factual data derived from a department store. There are technological difficulties of tracking on a real-time basis so we extracted purchasing related information and we added on context information on each transaction. As a result, recommendation based on FM model that applies purchasing and context information is more stable and accurate compared to that of vector model. Additionally, we could find more precise recommendation result as more shopping information is accumulated. Realistically, because of the limitation of ubiquitous environment realization, we were not able to reflect on all different kinds of context but more explicit analysis is expected to be attainable in the future after practical system is embodied.

유통업체의 위치기반 모바일 쇼핑서비스 제공에 대한 소비자 반응 : PAD 감정모델과 정보의 상황관련성을 중심으로 (Consumer Responses to Retailer's Location-based Mobile Shopping Service : Focusing on PAD Emotional State Model and Information Relevance)

  • 본 연구는 소비자가 지각하는 유통업체의 위치기반 모바일 쇼핑정보 서비스에 대한 정보의 상황관련성과 정보자극에 대한 PAD 감정변수들(환기, 지배력, 즐거움) 간의 상호 인과관계와 이용의도에 대한 이들의 효과를 실증 연구 하였다. 미국 내 모바일 이용자를 대상으로 무작위 표본추출법에 근거하여 추출되었고, 총 335명의 사용가능한 응답이 수거되었다. 분석결과, 환기와 상황관련성은 즐거움에 정(+)의 영향을 주었으나 지배력은 즐거움에 유의한 영향력을 나타내지 않았다. 즐거움은 이용의도에 정(+)의 영향을 주었다. 본 연구를 통해 위치기반 모바일 서비스에 대한 소비자의 인지적 반응과 감정적 반응을 통합적으로 살펴보았으며, PAD 감정차원간의 체계적인 관계를 규명하였다. 연구결과를 바탕으로 모바일 쇼핑서비스 개발자, 유통업체, 그리고 마케팅 실무자를 위한 시사점을 논의하였으며, 연구의 한계점과 더불어 향후 연구 방향을 제시하였다.

  • 최근 지능화된 사물들이 연결되는 네트워크를 통해 사람과 사물, 사물과 사물 간에 상호 소통하는 응용프로그램 및 하드웨어가 잇달아 소개되고 있다. 이런 추세에 더불어, 상황인식 기반의 지식이 결합되어 인공지능 서비스를 제공하는 사물인터넷(IoT : Internet of Things) 환경이 급속도로 발전하고 있다. 사물인터넷을 활용한 산업 중 하나로 자동차 산업을 들 수 있다. 최근에는 연료 효율과 원활한 교통 환경뿐만 아니라 운전자와 승객의 안전을 최우선으로 하는 자율 주행 자동차가 화두가 되고 있다. 이전부터 센서, 라이다, 카메라, 레이더 기술 등을 이용하여 자율 주행 자동차를 위한 주위 환경 인식에 대한 연구가 활발히 진행돼 왔다. 현대에는 차세대 무선통신 기술인 WAVE를 기반으로 차량과 차량, 차량과 주변의 교통 인프라와의 통신을 통한 네트워킹을 형성하고 주변 환경에 대한 정보를 공유하는 등 사물인터넷을 활용한 자율 주행 자동차 연구가 활발히 진행되고 있다. 본 논문에서는 자율 주행 자동차의 주위 환경 인식 기술의 일환으로 고속도로 교통 표지판 및 전방 잔여거리 인식에 관한 연구를 진행하였다. 본 연구는, 도로 교통 표지판이 설치 규정에 의하여 정해진 규격과 지정된 설치 위치를 갖고 있다는 특성을 이용하였다. 궁극적으로, 고속도로 주행 중 촬영한 영상을 이용하여 해당 비디오 영상 내에서 도로 교통 표지판을 인식한 뒤 추가적으로 표지판에 씌어 있는 문자 정보를 인식하고 이를 운전자 및 승객이 인지하도록 하는 이론 학습과 해당실험 결과를 제시하였다.

Incidence of Fever Following Endobronchial Ultrasound-Guided Transbronchial Needle Aspiration

  • Background: Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a minimally invasive diagnostic method for mediastinal and hilar lymphadenopathy. This study aimed to investigate the incidence of fever following EBUS-TBNA. Methods: A total of 684 patients who underwent EBUS-TBNA from May 2010 to July 2012 at Seoul National University Hospital were retrospectively reviewed. The patients were evaluated for fever by a physician every 6-8 hours during the first 24 hours following EBUS-TBNA. Fever was defined as an increase in axillary body temperature over $37.8^{\circ}C$. Results: Fever after EBUS-TBNA developed in 110 of 552 patients (20%). The median onset time and duration of fever was 7 hours (range, 0.5-32 hours) after EBUS-TBNA and 7 hours (range, 1-52 hours), respectively, and the median peak body temperature was $38.3^{\circ}C$ (range, $137.8-39.9^{\circ}C$). In most patients, fever subsided within 24 hours; however, six cases (1.1%) developed fever lasting longer than 24 hours. Infectious complications developed in three cases (0.54%) (pneumonia, 2; mediastinal abscess, 1), and all three patients had diabetes mellitus. The number or location of sampled lymph nodes and necrosis of lymph node were not associated with fever after EBUS-TBNA. Multiple logistic regression analysis did not reveal any risk factors for developing fever after EBUS-TBNA. Conclusion: Fever is relatively common after EBUS-TBNA, but is transient in most patients. However, clinicians should be aware of the possibility of infectious complications among patients with diabetes mellitus.

실체적 인터페이스 디자인 시스템에 관한 연구 - 텐저블즈의 설계 및 프로토타입 구현을 중심으로 - (A Study on the Tangible Interface Design System -With Emphasis on the Prototyping & Design Methods of Tangibles -)

  • 기술의 발전과 HCI에 대한 연구 등으로 예전에는 생각할 수 없을 만큼 컴퓨터는 손쉽게 접근할 수 있게 되었고, 이러한 변화는 컴퓨팅을 우리가 살고 있는 실세계로 옮겨오게 되었으며, 실체적 인터페이스에 대한 관심이 높아지고 있다. 실체적 사용자 인터페이스(TUI)는 빛, 소리, 공기 및 물의 흐름과 같은 것을 미디어로 이용하여 인간 지각의 주변에 있는 물체를 사용자가 인지할 수 있게끔 한다. 실체적 사용자 인터페이스 시스템은 디지털 비트에 구현된 실체화된 물리적 물체이며, 이것은 물론 만져지는 인터페이스(Graspable UI)의 특징을 가지고 있다. 그리고 다양한 물리적 물체를 입력에 사용하고 있다. 그러나 여기서 실체적 인터페이스의 핵심을 이루는 텐저블즈는 단순한 입력 장치만이 아니라 시스템의 모델을 구체적으로 형상화하는 전체 시스템을 의미한다. 텐저블즈의 물리적형상은 단순히 손으로 잡기 위한 실체가 아니라 시스템의 개념적 모델을 형상화한 실체이다. 사용자는 시스템의 물리적 형태를 통하여 시스템을 이용하고 이를 통하여 입력을 위한 액션을 취한다. 이러한 사용자의 액션에 대한 반응에 대해 시스템은 리액션을 취하게 되는데 사용자는 디지털적인 반응(사운드, 그래픽 정보)뿐만 아니라 텐저블즈의 형태, 위치 등의 물리적 속성 역시 리액션(Reaction)으로 받아들이게 된다. 본 연구는 텐저블즈의 물리적 형상과 물리적 속성 등, 인터랙션 과정에 영향을 미치게 되는 요소와 사용자 반응과 시스템의 리액션에 대한 연구를 다루고 있다.
