Concurrent Chemoradiotherapy in Nasopharyngeal Carcinoma (비인강암의 동시 항암화학방사선치료)

  • Purpose:This is a retrospective study to evaluate the results of concurrent chemoradiotherapy in nasopharyngeal carcinoma. Material and Methods:From Mar 2000 to June 2005, 18 patients with nasopharyngeal carcinoma completed planned concurrent chemoradiotherapy. Stages were I in 1 patients, II in 2 patients, III in 7 patients and IV in 8 patients. Pathologic type was squamous cell carcinoma(WHO type 1) in 2 patients, non-keratinizing type(WHO type 2) in 8 patients and undifferetiated carcinoma(WHO type 3) in 8 patients. The follow up period ranged from 30 months to 95 months with a median of 56 months. Follow up was possible in all patients. Results:Response to concurrent chemoradiation therapy was a complete response in all patients. Patterns of failure were as follows:local recurrence in only one patient(5.6%) and distant metastases in three patients with N3 diseases(16.7%). The overall 5 year survival rates were 88.5%, the 5 year disease free survival rate was 77% and these were very good results. There were no significant differences in the local control and survival rates between the clinical stages and pathologic types. Conclusion:The outcome of the nasopharyngeal carcinoma treated with concurrent chemoradiotherapy was very good, even though most of the patients(15/18=83.3%) were in stage III and IV diseases. We concluded that concurrrent chemoradiotherapy in nasopharyngeal carcinoma showed the good local control and survival rates without significant complications. In the patients with N3 disease, we have to consider the more effective and strong chemotherapeutic regimens to prevent distant metastases.

  • Brachytherapy is a method of radiotherapy in advantage to achieve better local control with minimum radiation toxicity in comparison with external irradiation because radiation dose is distributed according to the inverse square low of gamma-ray emitted from the implanted sources. The main characteristics of brachytherapy are delivering of higher dose to target volume shortening of total treatment period and sparing of normal tissue. Recent development of iridium ribbons for low dose rate implant provides improvement of technology of brachytherapy in terms of safety and efficiency. High dose rate method. on the other hand, is effective to avoid unnecessary expoure of medical personnel.

  • An epidemic infectious disease model consists of six compartments viz. Susceptible, Exposed, Infected, Quarantine, Recovered, and Virus with nonlinear saturation incidence rate is proposed to know the viral disease dynamics. There exist two biological equilibrium points for the model system. The system's local and global stability is done through Lyapunov's direct method about equilibrium points. The sensitivity analysis has been performed for the basic reproduction number and equilibrium points through the normalized forward sensitivity index. Sensitivity analysis shows that virus growth and quarantine rates are more sensitive parameters. In support of mathematical conclusions, numerical experimentation has been shown.

  • The WiBro to start June 2006 is mobile internet service with 60km/h, 3Mbps. And current WiBro service is used in Seoul, the metropolitan area and the local major cities. This study suggests WiBro technology and analyzes wireless communication technology to vehicle-to-infrastructure communications for the unmanned vehicle control. We were driving vehicle with two laptop computer's WiBro unit for V2I communication environment. So each RAS(Radio Access Station) on center is tested coverage area, transmission rate, latency rate, lost rate and transmission rate to parking area environment.

Postoperative Radiation Therapy in the Soft-tissue Sarcoma (연부 조직 육종의 수술 후 방사선 치료 결과)

  • Purpose : The major goal of the therapy in the soft tissue sarcoma is to control both local and distant tumor. However, the technique of obtaining local control has changed significantly over the past few decades from more aggressive surgery to combined therapy including conservative surgery and radiation and/or chemotherapy. We retrospectively analyzed the treatment results of the postoperative radiation therapy of soft tissue sarcoma and its prognostic factor. Materials and Methods : Between March 1983 and June 1994, 50 patients with soft tissue sarcoma were treated with surgery and postoperative radiation therapy at Kang-Nam St. Mary's hospital. Complete follow up was possible for all patients with median follow up duration 50 months (range 6-162 months). There were 28 male and 32 female patients. Their age ranged from 6 to 83 with a median of 44 years. Extremity (58$\%$) was the most frequent site of occurrence followed by trunk (20$\%$) and head and neck (12$\%$). Histologically malignant fibrous histiocytoma (23$\%$), liposarcoma (17$\%$), malignant schwannoma (12$\%$) constitute 52$\%$ of the patients. Daily radiation therapy designed to treat all areas at a risk for tumor spread upto dose of 4500-5000 cGy. A shrinking field technique was then used and total 55-65 Gy was delivered to tumor bed. Twenty-five patients (42$\%$) received chemotherapy with various regimen in the postoperative period. Results : Total 41 patients failed either with local recurrence or with distant metastasis. There were 29 patients (48$\%$) of local recurrence. Four patients (7$\%$) developed simultaneous local recurrence and distant metastasis and 8 patients (13$\%$) developed only distant metastasis. Local recurrence rate was rather higher than of other reported series. This study included patients of gross residual, recurrent cases after previous operation, trunk and head and neck Primary This feature is likely explanation for the decreased local control rate. Five of 29 Patients who failed only locally were salvaged by re-excision and/or re-irradiation and remained free of disease. Factors affecting local control include histologic type, grade, stage, extent of operation and surgical margin involvement, lymph node metastasis (p<0.05). All 21 patients who failed distantly are dead with progressive disease at the time of this report. Our overall survival results are similar to those of larger series. Actuarial 5 year overall survival and disease free survival were 60.4 $\%$, 30.6$\%$ respectively. Grade, stage (being close association with grade), residual disease (negative margin, microscopic, gross) were significant as a predictor of survival in our series (p<0.05). Conclusion : Combined surgery and postoperative radiation therapy obtained 5 year survival rate comparable to that of radical surgery.

Effect of Brewery Waste Replacement of Concentrate on the Performance of Local and Crossbred Growing Muscovy Ducks

  • Two experiments were carried out to evaluate the effects of brewery waste (BW) replacement of concentrate (C) in growing duck diets. In Exp. 1, which was carried out on-station, 300 ducklings were allocated in a $2{\times}5$ factorial experiment: Two breeds (local Muscovy and crosses of French and local Muscovy)${\times}5$ levels of C and with BW offered ad libitum. Concentrate only ad libitum as the control diet (C100), and levels of 75% (C75), 50% (C50), 25% (C25) and 0% (C0) of the amount of the control diet consumed, and with BW ad libitum. In Exp. 2, 200 ducklings were allocated in a $2{\times}2$ factorial experiment on five smallholdings: two breeds (local and crossbred Muscovy ducks)${\times}2$ diets (the C100 and C50 diets from Exp.1). In Exp.1 total dry matter (DM), BW, crude fiber (CF) and crude protein (CP) intakes were highest on the C0 diet and ME and lysine intakes lowest (p<0.001). Daily live weight gains were higher for the crossbred ducks than for the local Muscovies (p<0.05) and were highest for treatments C100 and C50, and lowest for treatment C0 (p<0.05). Weights of breast muscle, liver and abdominal fat were significantly higher for the crossbred ducks. Breast and thigh muscle and abdominal fat weights were significantly higher for the C100, C75 and C50 diets, while gizzard weights were highest for the C25 and C0 treatments. Net profits were higher for the crosses, and for treatments C50 and C25. In Exp. 2 total DM, CF and CP intakes were significantly higher for the C50 diet, and ME intakes lower (p<0.001). Daily gains of the crosses were significantly higher than those of the local Muscovy ducks, and were similar for the C100 and C50 diets. The highest net profits were from the crosses and ducks fed the C50 diet. It was concluded that BW can replace 50% of the concentrate in growing Muscovy duck diets without reducing daily live weight gains and with improved economic benefits.

A CLINICAL STUDY OF MALIGNANT TUMORS OF THE MAXILLA (상악 악성종양의 임상적 연구)

  • Malignant tumors of the maxilla represent about 3-4% of head and neck malignant neoplasm and the origins are maxillary gingiva, hard palate and maxillary sinus. We are to investigate clinical features and results according to various treatment modalities of maxillary malignant tumor to get information for better treatment results. Sixty patients with malignant maxillary tumors treated at Yonsei medical center from 1992 to 1997 were studied retrospectively. They are evaluated according to clinical signs & symptoms, stages at first diagnosis, primary site, histopathologic features, treatment method, recurrence and survival rate with clinical records, biopsy results and CT radiograph. The most common primary site was maxillary sinus and most common histopathology was squamous cell carcinoma. The local recurrence rate was 18% at 5 years, neck failure was 3% and distant metastasis was 18%. Overall survival rate was 78% at 2 years and 69% at 5 years. Failure at primary site is the main problem in the curative treatment. So, to improve survival in these patients efforts should be directed toward improvement of local control.

  • We investigated the cell growth rate according to the change of temperature of the Thermo-rod used for the local hyperthermia therapy. For this study, we fabricated the Thermo-rods (TR) using Duplex Stainless Steels having magnetic properties as well as non magnetic properties. To evaluate cell growth rates up to 15 days, we conducted cell proliferation test using cell counting methods. For the tests, the CAPEs and Osteoblats were seeded on the 6-we11 plates with the induction heated thermo-rods 30 mins a day for 15 days with 2 days interval and without induction heated thermo-rods as control group respectively. We calculated cell growth rates, 6 hours after heating. From the results, in case of CAPEs and Osteobalsts seeded groups, the cell growth rates in all groups increased drastically for 6 days after seeding, but decreased irregularly after 6 days. In conclusion, the cell growth rates showed no significant difference among all groups and it indicated that there were no effects of temperate ($41^{\circ}C$) on cell growth rates.

  • In this paper, we designed radio data transceiver to control the machine or equipments for automation in local areas. The designed module can transmit data with 153.6Kbps and uses 424MHz RF carrier to transmit data in the ISM band regulation. It has a processor, CPLD chip of the Altera Company, to control the data in transmitting and receiving. The processor is implemented by programming with VHDL. We will make this module with compact in dimension and higher data rate and apply to RFID technology.

Motion Analysis with Time Delay Neural Network (시간 지연 신경망을 이용한 동작 분석)

  • A novel motion analysis system is presented in this paper. The proposed system is inspired by processing functions observed in the fly visual system, which detects changes in input light intensities, determines motion on both the local and the wide-field levels. The system has several differences from conventional motion analysis system. First, conventional systems usually focused on matching similar feature or optical flow, but neural network is applied in this system. Back propagation is used by learning method, and Tine Delay Neural Network (TDNN) is also used as analysis method. Second, while conventional systems usually limited on only two frames of sequence, the proposed system accept multiple frames of sequence. The experimental results showed a 94.7% correct rate with a speed of 71.47 milli seconds for real and synthetic images.
