The Role of Modern Radiotherapy Technology in the Treatment of Esophageal Cancer

  • Radiation therapy (RT) has improved patient outcomes, but treatment-related complication rates remain high. In the conventional 2-dimensional and 3-dimensional conformal RT (3D-CRT) era, there was little room for toxicity reduction because of the need to balance the estimated toxicity to organs at risk (OARs), derived from dose-volume histogram data for organs including the lung, heart, spinal cord, and liver, with the planning target volume (PTV) dose. Intensity-modulated RT (IMRT) is an advanced form of conformal RT that utilizes computer-controlled linear accelerators to deliver precise radiation doses to the PTV. The dosimetric advantages of IMRT enable better sparing of normal tissues and OARs than is possible with 3D-CRT. A major breakthrough in the treatment of esophageal cancer (EC), whether early or locally advanced, is the use of proton beam therapy (PBT). Protons deposit their highest dose of radiation at the tumor, while leaving none behind; the resulting effective dose reduction to healthy tissues and OARs considerably reduces acute and delayed RT-related toxicity. In recent studies, PBT has been found to alleviate severe lymphopenia resulting from combined chemo-radiation, opening up the possibility of reducing immune suppression, which might be associated with a poor prognosis in cases of locally advanced EC.

  • The search for an ideal radioisotope for radiotherapy continues. As a generator-produced radioisotope emitting both beta and gamma rays with a short physical half-life of 16.9 hr, $^{188}Re$ is an excellent candidate for radiotherapy. Its applications Include the irradiation of coronary artery to prevent restenosis, treatment of rheumatoid arthritis, treatment of peritoneal effusion, palliation of metastatic bone pain, and treatment of liver cancer.

흉복부 방사선 치료 시 압력 기반 복부압박장치 적용에 따른 치료 간 재현성 평가 (Reproducibility evaluation of the use of pressure conserving abdominal compressor in lung and liver volumetric modulated arc therapy)

  • 목 적: 폐암과 간암 환자에 대해 압력 기반 복부압박장치를 적용하여 입체적 세기변조 회전 방사선치료(VMAT)를 시행하였을 때의 치료 간(interfraction) 위치 재현성과 호흡 재현성을 평가함으로써 그 유용성을 평가하고자 한다. 대상 및 방법: 압력 기반 복부압박장치를 사용해 입체적 세기변조 회전 방사선치료를 받은 6명의 폐암 환자와 3명의 간암 환자를 대상으로 하였다. 치료 간 위치 재현성 평가를 위해 모의치료 CT 영상과 매일 획득한 CBCT 영상 174건을 비교하여 이미지 정합의 이동 값을 비교, 분석하였다. 치료 간 호흡 재현성 평가를 위해 모의치료 4DCT 영상과 매주 획득한 4D CBCT 영상 54건을 비교하여 호흡량 오차를 구하였다. 결 과: 수평수직 세 방향의 3D 벡터값인 전체 위치변화(Overall position variation, Overall VP)값은 폐와 간에서 각각 평균 1.1 ± 1.4 mm, 4.5 ± 2.8 mm로 나타났다. 호흡 변화(respiratory variation, Vr)값은 폐에서 평균 0.7 mm ± 3.4 mm (p=0.195), 간에서 평균 3.6 mm ± 2.6 mm (p<0.05)로 나타났다. 결 론: 흉복부 방사선 치료 시 압력 기반 복부압박장치의 적용은 복부 압박 재현을 통해 치료 간 종양의 호흡 변이뿐만 아니라 위치 변이를 안정적으로 조절하는 데에 유용한 것으로 사료된다. 보다 안정적인 재현성을 위해 치료 계획 시 적절한 PTV 여유가 고려되어야 하고, 매 치료 전 영상 유도에 따른 표적의 위치와 호흡 검증이 필요하다.

간암 진료가이드라인의 현재와 전망 (Clinical Practice Guidelines for Hepatocellular Carcinoma: Current and Future Perspectives)

  • 간세포암종은 만성 간질환과 간경변증을 동반하는 경우가 많고, 예후에 종양 인자 이외에도 잔존 간 기능이 주요한 영향을 미친다. 또한, 간세포암종에 대한 고위험인자를 가진 경우 특정한 영상 소견(예: 고혈관성)을 보이면, 조직검사 없이도 비침습적인 진단이 가능하다. 다른 고형암에서와 마찬가지로 수술적 절제, 방사선치료, 항암치료 등의 치료가 시행되기도 하지만, 간세포암종에서만 이루어진다고도 할 수 있는 간이식, 경동맥화학색전술, 고주파열치료술과 같은 치료 방법들이 시행되기도 한다. 종양의 다양성, 치료 방법의 다양성, 사회적 여건(의료 보건 체계, 의료 자원의 가용성 등) 등을 반영하며 여러 간세포암종 가이드라인들이 발표되어 왔으며, 각 가이드라인들은 여러 측면에서 유사하면서도 서로 상이하기도 하다. 본고에서는 다음의 간세포암종 진료 가이드라인들을 살펴보고, 가이드라인들의 특징과 앞으로 가이드라인에서 다루어야 할 부분 등에 대하여 논하고자 한다.

간암환자에 대한 세기조절방사선치료에서의 2차 산란선량평가 (Estimation of Secondary Scattered Dose from Intensity-modulated Radiotherapy for Liver Cancer Cases)

  • 간암 환자에 대한 세기조절방사선치료(IMRT, intensity modulated radiotherapy) 및 세기조절회전방사선치료(VMAT, volumetric arc therapy)와 나선식토모치료(TOMO, Helical Tomotherapy)에서 2차 암의 원인이 될 수 있는 산란 및 누출선량률을 평가하였다. 5명의 간암 환자에 대해 IMRT와 VMAT, TOMO 치료계획을 실시하여 등중심(iso-center)으로부터 20, 40, 60, 80 cm 위치에서 유리선량계(RPLGD, radiophotoluminescence glass dosimeter)를 이용하여 선량을 측정하였다. 계획표적체적(Planning Target Volume, PTV)에 조사된 단위 선량(Gy)당 측정된 산란 및 누출선량은 IMRT의 경우, 최소 0.01에서 최대 3.13 Gy로 측정 되었고 VMAT에 대해서는 최소 0.03에서 최대 2.35 Gy까지, TOMO에 대해서는 최소 0.04에서 최대 1.30 Gy 까지 측정 되었다. 각 치료법에 대한 평균장기등가선량은 세기조절방사선치료에 대해 세기조절회전방사선 치료와 나선식단층토모치료가 각각 갑상선에서 75%와 51%, 대장에서 75%와 41%, 직장에서 72%와 48%, 전립선에서 76%와 50%로 나왔다. 본 측정을 통하여 산란 및 누출선량은 치료 중심으로부터의 거리에 따라 감소함을 보았으며 TOMO 치료의 경우, 환자치료를 위해 사용하는 모니터단위(MU, monitor unit)가 타 치료법에 비해 상대적으로 큼에도 불구하고 산란 및 누출선량은 크지 않는 것으로 평가되었다.

Intensity-modulated Radiotherapy Combined with Endocrine Therapy for Intermediate and Advanced Prostate Cancer: Long-term Outcome of Chinese Patients

  • Aim: The aim of this study was to evaluate acute adverse events and efficacy of three-dimensional intensitymodulated radiotherapy (IMRT) combined with endocrine therapy for intermediate and advanced prostate cancer. Methods: Sixty-seven patients were treated with three-dimensional IMRT combined with maximum androgen blockade. The correlation between radiation-induced rectal injury and clinical factors was further analyzed. Results: After treatment, 21 patients had complete remission (CR), 37 had partial remission (PR), and nine had stable disease (SD), with an overall response rate of 86.5%. The follow-up period ranged from 12.5 to 99.6 months. Thirty-nine patients had a follow-up time of ${\geq}$ five years. In this group, three-year and five-year overall survival rates were 89% and 89.5%, respectively; three-year and five-year progression-free survival rates were 72% and 63%. In univariate analyses, gross tumor volume was found to be prognostic for survival ($X^2$ = 5.70, P = 0.037). Rates of leucopenia and anemia were 91.1% and 89.5%, respectively. Two patients developed acute liver injury, and a majority of patients developed acute radiation proctitis and cystitis, mainly grade 1/2. Tumor volume before treatment was the only prognostic factor influencing the severity of acute radiation proctitis (P < 0.05). Conclusions: IMRT combined with endocrine therapy demonstrated promising efficacy and was well tolerated in patients with intermediate and advanced prostate cancer.

카페시타빈과 통합의학 프로그램 치료 병용을 통해 호전된 진행성 유방암 1례 (Case of Remission of Progressive Breast Cancer Treated by Capecitabine with Integrative Medicine Therapy)

  • This report is aimed to investigate the effect of Integrative Medicine Therapy (IMT) in treating breast cancer metastasized to several portion. A 55-year-old woman who was diagnosed breast cancer and treated by breast conservation surgery (BCS) IN 2002. (pT2M0N0), adjuvant chemotherapy and radiotherapy till Jan. in 2013. But cancer had metastasized to sacrum (2009), liver (2012) and ribs (2013). Finally cause of several side effects of Xeloda(neuralgia and vommiting), she gave up to be treated by Xeloda and started to receive IMT including Abnoba Viscum, Vitamin C therapy Xeloda again, Korean herbal medicine and pharmacopuncture from Jul. in 2013. The effect was evaluated with Positron Emission Tomography and Computed Tomography (PET-CT) and Abdomen Computed Tomography (CT). The metastatic tumor in liver was disappeared and cancers to ribs, their size decreased after 12 months and pain of sacrum maintained VAS 2 continually. These results suggest that IMT is a therapeutic method to treat metastatic tumor originated from breast cancer.

Measurement of Variation in Water Equivalent Path Length by Respiratory Organ Movement

  • In particle radiotherapy, a shape of the beam to conform the irradiation field is statically defined by the compensator, collimator and potal devices at the outside of the patient body. However the target such as lung or liver cancer moves along with respiration. This increases the irradiated volume of normal tissue. Prior discussions about organ motions along with respiration have been mainly focused on inferior-superior movement that was usually perpendicular to beam axis. On the other hand, the change of the target depth along the beam axis is very important especially in particle radiotherapy, because the range end of beam (Bragg peak) is so sharp as to be matched to distal edge of the target. In treatment planning, the range of the particle beam inside the body is calculated using a calibration curve relating CT number and water equivalent path length (WEL) to correct the inhomogeneities of tissues. The variation in CT number along the beam path would cause the uncertainties of range calculation at treatment planning for particle radiotherapy. To estimate the uncertainties of the range calculation associated with patient breathing, we proposed the method using sequential CT images with respiration waveform, and analyzed organ motions and WELs at patients that had lung or liver cancer. The variation of the depth along the beam path was presented in WEL rather than geometrical length. In analyzed cases, WELs around the diaphragm were remarkably changed depending on the respiration, and the magnitude of these WEL variations was almost comparable to inferior-superior movement of diaphragm. The variation of WEL around the lung was influenced by heartbeat.

속발성 간암으로 인한 협통(脇痛) 치험(治驗) 1례(例) (One Case on Hypochondriae Pain developed by Metastatic Liver Cancer)

  • This study was performed on the basis of clinical consideration about patient who has hepatocholangiocarcinoma, and pancreatic cancer. Cancer has gradually increased morbidity and mortality all over the world including Korea. Western medicine has treated with surgical therapy, chemotherapy, radiotherapy and immunotherapy, but has not obtained outstanding results and has a difficulty due to side effects by those therapies. In this study, we recognized that symptom including hypochondriac pain by cancer are rapidly decreased with herbal medication treatment and acupuncture therapy. Oriental medicine treatment is applied throughly on the basis of the oriental medicine principle and the prescriptions were used Naeso-oksultang(內消沃雪湯). According to the above results, we can know this patient has improvement of patient's symptom on administration of oriental medicine treatment, it is helpful in decresing symptoms of patient and in improving quality of life.

수술 불가능한 췌장암 환자에서 방사선 치료의 역할 (Role of Radiotherapy in Unresectable Pancreatic Carcinoma)

  • From 1988 to 1991, nineteen patients with unresectable localized pancreatic carcinoma were treated with radiotherapy and/or hyperthermia or in combination with chemotherapy. Radiation dose of 4500-5000 cGy with or without additional 500-1000 cGy was administered over 5 to 6 weeks to the pancreatic tumor area using 10 MV linear accelerator. Five of 19 patients were given chemotherapy, either neoadjuvant or maintenance setting with FAM regimen (5-FU, adriamycin and mitomycin C), which was repeated every 4 weeks for one year or until progression. Symptomatic palliation was achieved in 17 among 19 patients ($89{\%}$) and objective response (complete or partial response in CT finding) was achieved in 5 among 11 patients ($45{\%}$). The median survival time was 9 months and one-year survival rate, $32{\%}$. Local-regional failure was documented in 10 among 13 patients ($77{\%}$) and distant failures were found in the liver (3 patients) and carcinomatosis (2 patients). Prognostic significance of various factors such as age, sex, performance status, tumor location, stage, etc. were assessed. Any factors did not have the prognostic significance in univariate analysis. Treatment was well tolerated in most of the patients with only mild to moderate toxicity.
