악성 흉막액을 동반한 원발성 종격동 지방육종 1예 (The Primary Mediastinal Liposarcoma with Effusion)

  • Primary liposarcoma of the mediastinum is a very rare and relatively slow growing tumor. Since the original description by Pallase and Roubier in 1916, there have been about 55 reported cases in world literature until 1985. Recently, we experienced one case of a primary mediastinal liposarcoma with malignant effusion. A 51-year-old man complained of dyspnea and chest discomfort. The chest plain films and computerized tomogram showed a huge mass of the posterior mediastinal space. The needle aspiration biopsy was done in the huge mass and the histologic examination revealed mediastinal round-cell type liposarcoma. Patient refused surgery or chemotherapy after establishing the diagnosis. About 6 months later, the metastatic pleural effusion was noted. After discharge, he was lost to follow up since then. The clinical and therapeutic features of the previously reported cases of primary liposarcoma arising in the mediastinum have been reviewed. Surgery may served to establish a tissue diagnosis, to relieved the patient's symptoms and result occasionally in a cure but radiotherapy or chemotherapy is ineffctive.

  • Mammalian physiology and behavior are regulated by an internal time-keeping system, referred to as circadian rhythm. The circadian timing system has a hierarchical organization composed of the master clock in the suprachiasmatic nucleus (SCN) and local clocks in extra-SCN brain regions and peripheral organs. The circadian clock molecular mechanism involves a network of transcription-translation feedback loops. In addition to the clinical association between circadian rhythm disruption and mood disorders, recent studies have suggested a molecular link between mood regulation and circadian rhythm. Specifically, genetic deletion of the circadian nuclear receptor Rev-$erb{\alpha}$ induces mania-like behavior caused by increased midbrain dopaminergic (DAergic) tone at dusk. The association between circadian rhythm and emotion-related behaviors can be applied to pathological conditions, including neurodegenerative diseases. In Parkinson's disease (PD), DAergic neurons in the substantia nigra pars compacta progressively degenerate leading to motor dysfunction. Patients with PD also exhibit non-motor symptoms, including sleep disorder and neuropsychiatric disorders. Thus, it is important to understand the mechanisms that link the molecular circadian clock and brain machinery in the regulation of emotional behaviors and related midbrain DAergic neuronal circuits in healthy and pathological states. This review summarizes the current literature regarding the association between circadian rhythm and mood regulation from a chronobiological perspective, and may provide insight into therapeutic approaches to target psychiatric symptoms in neurodegenerative diseases involving circadian rhythm dysfunction.

  • Matrix metalloproteinases (MMPs) are enzymes that can degrade collagen in hybrid layer and reduce the longevity of adhesive restorations. As scientific understanding of the MMPs has advanced, useful strategies focusing on preventing these enzymes' actions by MMP inhibitors have quickly developed in many medical fields. However, in restorative dentistry, it is still not well established. This paper is an overview of the strategies to inhibit MMPs that can achieve a long-lasting material-tooth adhesion. Literature search was performed comprehensively using the electronic databases: PubMed, ScienceDirect and Scopus including articles from May 2007 to December 2019 and the main search terms were "matrix metalloproteinases", "collagen", and "dentin" and "hybrid layer". MMPs typical structure consists of several distinct domains. MMP inhibitors can be divided into 2 main groups: synthetic (synthetic-peptides, non-peptide molecules and compounds, tetracyclines, metallic ions, and others) and natural bioactive inhibitors mainly flavonoids. Selective inhibitors of MMPs promise to be the future for specific targeting of preventing dentin proteolysis. The knowledge about MMPs functionality should be considered to synthesize drugs capable to efficiently and selectively block MMPs chemical routes targeting their inactivation in order to overcome the current limitations of the therapeutic use of MMPs inhibitors, i.e., easy clinical application and long-lasting effect.

지역거점공공병원의 지속가능한 외부공간에 대한 연구 (A Study on Sustainable Outdoor Space of Regional Public Hospitals)

  • Purpose: Since hospitals have a big impact on the natural and social environment, hospitals' sustainability must be considered in various aspects including environmental, social, and economical aspects. So far, research on sustainable hospitals has mainly been conducted on indoor environment, but outdoor space of hospital is as important as well. The purpose of this research is to study the sustainability of hospital outdoor space and to make proposals for improving sustainable regional public hospitals. Methods: Through preceding research analysis and literature review, the concept of sustainable outdoor space of hospital was defined. Also, the evaluation contents of outdoor space were extracted from domestic and foreign green building certification criteria. With this analytic frame, we visited 4 regional public hospitals in Korea, interviewed the facility managers, and analyzed sustainability of hospital outdoor space. Results: Analysis of 4 regional public hospitals reveals that there is a lack of consideration for sustainability in all 4 categories; urban and network, natural environment, community, and user. In terms of community, sustainability has rarely been applied to all four hospitals. The site selection and terrain of the site were essential to sustainability. Implications: Establishing a master plan considering future expansion is critical. Also more attempt to show regional characteristics is needed. It is necessary to design a comprehensive outdoor space that considers sustainability and also healing environment. Level difference in the site can be used as a design element.

  • Oxidative stress to proteins, lipids, or DNA is higher in human autopsy tissue and in rodent models of a number of neurodegenerative conditions, including Alzheimer's and Parkinson's disease. On the basis of this information, we established a screening system using N18-RE-105 cells to identify therapeutic agents that can protect cells from glutamate toxicity. During the course of our screening program, we recently isolated the active compound 9-hydroxy-$\alpha$-tocopherone ($\alpha$-TP), which prevents glutamate-induced cell death, from Viola mandshurica. The chemical structure of $\alpha$-TP was identified using spectroscopic methods and by comparison with literature values. Antioxidant activity and protective effects of $\alpha$-TP were evaluated by DPPH radical-scavenging assay, morphological assay, MTT reduction assay, and lactate dehydrogenase (LDH) release assay. These results suggest that $\alpha$-TP could be a new potential chemotherapeutic agent against neuronal diseases.

좌측 하폐엽에 공급하는 비정상적인 체동맥의 치료 (수술과 색전술의 비교) (Treatment of Systemic Arterial Supply to Lower Lobe of Left Lung (Operation vs. Embolotherapy): Comparison of Two Cases and Literature Review)

  • 폐에 체동맥이 분포하는 선천성 기형은 드문 질환으로 하폐엽의 기저분절에 폐동맥 대신 대동맥에서 1개 이상의 체동맥이 나오는데 정상적인 기관지와 연결 유무에 따라 폐 분획증과 좌측 폐동맥 이상 기시증(Anomalous Origin of Left Pulmonary Artery, AOLPA)으로 구분된다. 한일병원 흉부외과에서는 하행 흉부 대동맥에서 시작되는 비정상적인 체동맥을 가진 두 예의 폐동맥 기형을 경험하고, 1예는 기저분절절제술을, 다른 예는 동맥색전술을 시행하였고, 2년간 관찰한 결과 양호하였다. 이 결과를 바탕으로 외과적 수술이 이 기형의 근본적인 치료법이지만, 전신상태가 불량하거나 폐기능이 저하된 고위험군에서 동맥색전술도 선택적으로 시행할 수 있는 가능한 치료법으로 생각된다.

상악골의 악성 섬유성 조직구종 - 증례보고 - (Malignant Fibrous Histiocytoma of the Maxilla - Report of A Case -)

  • 악성 섬유성 조직구종은 상악골부위에서는 매우 드물게 발생하는 악성 종양으로서, 두경부에 발생되는 예가 전체 악성 섬유성 조직구종의 $7{\%}$ 이며 이들 $7{\%}$$12{\%}$가 상악골에 발생한다고 한다. 상악골의 악성 섬유성 조직구종은 국소재발이나 원격전이가 $55{\%}$의 환자들에서 발생하며 평균 생존기간은 상악골, 하악골 및 구강연부조직의 악성 섬유성 조직구종에 관한 연구에서 30개월로 보고되었다. 상악골의 악성 섬유성 조직구종은 일차적으로 근치적 수술요법이 주 치료방법이다. 방사선치료에 관한 보고들은 대상환자들이 적어서 지금까지 체계적으로 보고되지 않았고, 증례보고에 의한 종양의 퇴행 또는 조직학적 변화가 발표된 바 있으며 일부 저자들은 수술후 방사선치료가 도움이 되지 않으므로 재발성 또는 수술 불가능한 경우에 시행하기를 권하기도 한다. 재발성 또는 전이성 악성 섬유성 조직구종 환자들에서 항암화학요법으로 $33{\%}$의 관해율을 보였다는 보고가 있다. 저자들은 상악골의 악성 섬유성 조직구종 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

하악골 후퇴증의 외과적 치험례 (A Case Report of a Surgical Correction of the Mandibular Retrusion)

  • 심한 하악 후퇴증은 교정치료만으로 성공적인 결과를 얻을 수 없으며 골절단술, 이부성형술등 외과적 교정이 고려되어야 한다. 설골상근의 장력에 의한 회귀현상을 최소화하기 위해 수술시 하악 체부를 전방회전시킴과 동시에 가능한 전방으로 overcorrection한다. 저자등은 구미인에 비해 발생빈도가 적은 하악 후퇴증을 주소로 내원한 환자를 양측성 하악골 시상절단술과 이부 성형술을 통하여 만족할만한 안모개선 및 교합관계 개선을 이루었기에 이에 보고하는 바이다.

파열 간암의 간동맥 항암 화학색전술 후 발생한 유도 폐렴 (Lipiodol-induced pneumonitis following transarterial chemoembolization for ruptured hepatocellular carcinoma)

  • Transarterial chemoembolization (TACE) is a widely accepted nonsurgical modality used for the treatment of multinodular hepatocellular carcinoma (HCC). The careful selection of the candidate is important due to the risk of developing various side effects. Fever, nausea, abdominal pain, and liver enzyme elevation are commonly known side effects of TACE. Hepatic failure, ischemic cholecystitis, and cerebral embolism are also reported, although their incidence might be low. Pulmonary complication after TACE is rare, and the reported cases of lipiodol pneumonitis are even rarer. A 53-year-old man was treated with TACE for ruptured HCC associated with hepatitis B virus infection. On day 19 after the procedure, the patient complained of dyspnea and dry cough. Chest computed tomography showed diffuse ground glass opacities in the whole-lung fields, suggesting lipiodol-induced pneumonitis. After 2 weeks of conservative management, the clinical symptoms and radiologic abnormalities improved. Reported herein is the aforementioned case of lipiodol-induced pnemonitis after TACE, with literature review.

식이장애 환자에서 나타나는 감정표현불능증의 치료적 함의 (The Therapeutic Implications of Alexithymia in Patients with Eating Disorders)

  • 감정표현불능증(alexithymia)은 자신의 감정을 판별하거나 감정과 신체 감각을 구별하지 못하고 감정을 교류하지 못하며 공상이 부족하고 외부적 환경에만 인지적으로 집중하는 특성으로 정의되며, 그 기저에는 감정 처리 과정 상의 지속적인 결함이 존재하는 것으로 알려져 있다. 한편, 식이장애는 비적응적인 식이 관련 행동들로 인하여 신체적, 심리사회적 기능이 저하되는 질환으로서 이러한 비적응적 식이 관련 행동은 감정을 처리하고 조절하는 능력의 저하와 관련이 있다는 보고들이 있다. 식이장애 환자들에게 나타나는 감정표현불능증을 연구한 기존 연구들을 살펴 보면 식이장애 환자들은 정상인들에 비해 높은 수준의 감정표현불능증을 보이며, 감정표현불능증 요소 중 감정을 판별하기 어려움 요소와 감정을 묘사하기 어려움 요소가 정상인들에 비해 높은 수준으로 나타난다. 높은 수준의 감정표현불능증은 유년기 학대 및 우울증과 상호 작용하면서 식이장애 행동의 발생 위험성을 증대시키며 식이장애의 치료 효과 또한 반감시키는 것으로 보인다. 이상의 논의를 종합해 볼 때 감정표현불능증을 포함한 정서 처리 및 조절 능력의 결함에 도움을 줄 수 있는 치료가 식이장애의 치료에도 도움이 될 가능성이 있으므로 이에 대한 추가적인 연구가 필요할 것이다.
