Functional Primary Closure of Unilateral Imcomplete Cleft Lip by Modified Delaire's Technique: Report of a case (Delaire 변법을 이용한 편측 불완전 구순열 환자의 기능적 일차 봉합술: 증례보고)

  • The outcome of primary surgery for cleft lip is judged by its effects on the quality of oro-facial function and development. Many surgical techniques have been tried to obtain better results, however, Delaire introduced a technique of functional closure of the lip and nose, based on the findings of no true hypoplasia in the tissues either side of the cleft. In a seven-month-old Asian male patient with unilateral incomplete cleft lip, we carried out the primary closure by modified Delaire's technique. With no alveolar bone graft, the vertical incision on the nasal base was omitted in this patient because of his acceptable symmetry of nose. Also, a small Z plasty was added on the non-cleft side. The V-shaped incisions, whose notch was located on each side of the red vermilion, were designed and beveled incisions were performed for the rehabilitation of lip length and thickness, considering the postoperative wound contracture. We assured that this modification of Delaire's technique could be applied for various cases of primary closure of incomplete cleft lip.

  • The author presents Bardach' s technique for the residual unilateral cleft lip nasal deformity, The key to a successful and stable correction of the nasal deformity is to lengthen the columella on the cleft side and to mobilize alar cartilage from its surrounding tissue, creating a symmetric shape and length, The major advantages of the technique are lengthening of the cleft columella and creation of a symmetric and well-projected nasal tip.

  • Lipschitz Algebras Lip(X, ${\alpha}$) and lip(X, ${\alpha}$) were first studied by D. R. Sherbert in 1964. B. Pavlovic in 1995 shown that in these algebras, the prime ideals containing a given prime ideal form a chain. In this paper, we show that the above property holds in $Lip^n(X,\;{\alpha})$ and $lip^n(X,\;{\alpha})$, the Lipschitz algebras of finite differentiable functions on a perfect compact place set X.

  • It is well known that screech tones of supersonic jet are generated by a feedback loop driven by the instability waves. Near the nozzle lip where the supersonic jet mixing layer is receptive to external excitation, acoustic disturbances impinging on this area excite the instability waves. This fact implies that the nozzle lip thickness can influence the screech tones of supersonic jet. The objective of the present study is to experimentally investigate the effect of nozzle-lip thickness on screech tones of supersonic jets issuing from a convergent-divergent nozzle. A baffle plate was installed at the nozzle exit to change the nozzle-lip thickness. Detailed acoustic measurement and flow visualization were made to specify the screech tones. The results obtained obviously show that nozzle-lip thickness significantly affects the screech tones of supersonic jet, strongly depending on whether the jet at the nozzle exit is over-expanded or under-expanded.

  • Effect of lip shape on the hole flangeability of high strength steel sheets is investigated. Circular plates of various hole sizes are tested and the variation of lip length as well as the variation of thickness on the sectional views of the finished lip were studied. The conventional hole flanging process is limited to a certain limit hole diameter below which failure will ensue during the hole expansion. The intention of this work is to examine the effect of lip shape on the flangeability of TRIP steel and Ferrite-Bainite duplex steel and find out major parameters which can affect flanging shape of high strength hot rolled steels. Over the ranges of conditions investigated, the minimum hole diameter of F+B steel is better than TRIP steel. while, the lip-shape accuracy of TRIP steel is better than that of F+B steel. although the tensile strength and elongation of %P steel are superior than those of Ferrite-Bainite duplex steel, the flangeability is found to be not so strongly sensitive to the tensile properties but sensitive to displacement on the circumferential direction of hole edge.

  • A General survey on the cleft lip and/or palate patients in Pusan National University Hospital was done. The conclusions were as follow. 1. In gender ratio, male was higher than female. 2. In the cleft type, unilateral cleft lip and palate, cleft lip and alveolus, and bilateral cleft lip and palate in higher ratio order. Left unilateral cleft lip and palate was higher than right in ratio. 3. In first hospital visiting age for dental treatment, the group from 6 years old to 12 years old was most and in that the age group for attending elementary school (about 7-8 years old) showed especially higher ratio. The large majority of patient were born in 1980's and lived in Pusan metropolitan city, KyungSangNamDo. Especially, most of them lived in the neighboring Held of Pusan National University Hospital. 4. Class III skeletal pattern and Angle's Class III molar relationship was most prevalent. Angle's Class II molar relationship showed relative higher ratio because tooth loss and malposition. 5. Primary lip closure in 3 months, secondary lip closure in 6-7 years old, palatal closure in 12-16 months and bone graft in 8-10 years old were operated mostly. Rhinoplasty and scar revision were operated in too early age so the need of infantile orthopedics must be considered. 6. Maxillary expansion and protraction were operated mostly in 8-11 years old but operating in primary dentition must be considered. 7. Tooth alignment were started mostly in mixed dentition and consideration about prosthodontic treatment and retention will be need. 8. In tooth anomaly, tooth malformation and missing were most prevalent.

  • Patients who want to reduce their lip protrusion usually estimate the severity of the lip protrusion on the frontal aspect. Most orthodontists have a perplexed experience of a reduced thin line of vermilion border on the frontal aspect as incisors we retracted, even though the lip protrusion is thought to be reduced favorably on the sagittal aspect. Some patients also look older after orthodontic treatment because of severe lip thinning. This unaesthetic reduction of vermilion border urges us to study the vertical lip change during orthodontic procedure. The purpose of this study was to evaluate the vertical lip and perioral soft tissue changes in respect to incisor retraction in an effort to analyze which factors might be responsible for their vertical changes, using the multiple regression analysis. The results were as follows. 1. Upper and lower lip philtrum length(SnLs, LiB') were increased after retraction of anterior teeth, where as upper and lower vermilion height(LsSuls, StmiLi), and vermilion length(LsLi) were decreased. 2. Upper and lower lip length(SnStms, StmiB'), and soft tissue lower anterior facial height(SnMe') did not show any significant difference after treatment. 3. The increase of the upper lip philtrum length was mainly influenced by the extrusion of upper anterior teeth(${\Delta}U1V$), and the increase of the lower lip philtrum length was mainly influenced by the initial overjet before treatment. 4. The decrease of the upper and lower lip vermilion height was mainly influenced by the decrease of upper lip thickness.

  • Radical paring of the cleft edge during primary cleft lip operation or repeated secondary surgeries can result in tightness of the upper lip. In case, the degree of the resulting side-to-side tension is very severe, the possibility of a lip switch flap must be considered. When the lip tightness accompanies a loss of more than two-thirds of the Cupid's bow, an Abbe flap is an alternative. The disadvantages of Abbe flap are scar formation on the lower lip, design of incision line on the upper lip, disharmony of colors, and the dysfunction of the orbicularis muscle. These problems have been recognized in the literature and extreme discretion has been advised in its application. We experienced four cases of Abbe flap operation which were designed differently to correct the secondary unilateral or bilateral cleft-lip nasal deformities. The Abbe flap operations resulted in removal of the scars and tightness of the upper lip, reconstruction of the Cupid's bow, lengthening of the columella, and therefore secondary cleft-lip nasal deformity could be corrected. It is thought that carefully applied Abbe flap is an appropriate method to relieve horizontal tightness or flattening of the upper lip which occured after primary operation of cleft lip.

A New Method for Creating a Definite Philtrum by the Flipping of an Orbicularis Oris Muscle Flap in a Patient with an Indistinct Philtrum

  • The philtrum plays a key role in the appearance of the upper lip and nostril sill. Therefore, construction of the philtrum is crucial for attaining a natural appearance of the upper lip. We used a flipping myoplasty of the orbicularis oris muscle on a patient with a flat philtrum in order to effectively reconstruct the philtral dimple and column. A 35-year-old female presented to our department with the complaint of a flat upper lip. A superficial layer of the orbicularis oris muscle on the median aspect of the upper lip was vertically incised and elevated to a thickness of 2 mm. Both sides of the elevated muscle flap were then folded to the lateral sides so that the border could be sutured onto the outer portion of the orbicularis oris muscle. The patient was observed for one year postoperatively. Her philtrum deepened by 1.25 mm, with the central angle of her Cupid's bow improving from a preoperative measurement of $146^{\circ}$ to $128^{\circ}$ postoperatively. In a patient with an indistinct philtrum, a flipping orbicularis oris myoplasty was performed to attain a definite philtral column and a philtral dimple. Natural upper lip movement was maintained, and an aesthetically and functionally satisfactory reconstruction was achieved.

On Pattern Kernel with Multi-Resolution Architecture for a Lip Print Recognition (구순문 인식을 위한 복수 해상도 시스템의 패턴 커널에 관한 연구)

  • Biometric systems are forms of technology that use unique human physical characteristics to automatically identify a person. They have sensors to pick up some physical characteristics, convert them into digital patterns, and compare them with patterns stored for individual identification. However, lip-print recognition has been less developed than recognition of other human physical attributes such as the fingerprint, voice patterns, retinal at blood vessel patterns, or the face. The lip print recognition by a CCD camera has the merit of being linked with other recognition systems such as the retinal/iris eye and the face. A new method using multi-resolution architecture is proposed to recognize a lip print from the pattern kernels. A set of pattern kernels is a function of some local lip print masks. This function converts the information from a lip print into digital data. Recognition in the multi-resolution system is more reliable than recognition in the single-resolution system. The multi-resolution architecture allows us to reduce the false recognition rate from 15% to 4.7%. This paper shows that a lip print is sufficiently used by the measurements of biometric systems.
