경구 스테로이드 치료로 호전된 Lewis-Sumner 증후군 1예 (A Case of Lewis-Sumner Syndrome Improved by Oral Steroid Therapy)

  • We present a case with stepwise weakness and sensory involvement of both hands for more than 2 months. His nerve conduction study findings revealed prolonged terminal latencies, decreased motor and sensory conduction velocities and conduction blocks of both ulnar nerves, more severely on left side. And there were other abnormalities manifested with mononeuropathy multiplex. Increased cerebrospinal fluid protein was found. We diagnosed him as Lewis-Sumner syndrome and tried high dose oral steroid therapy for 2 months. He showed improvement of motor functioning with persistent conduction block.

Influence of Lewis Base on the Nonstoichiometry and the Properties of Magnetite Films Prepared by Aqueous Solution Method

  • Lewis bases were employed to control the stoichiometry of ferrite film prepared by light enhanced plating (LEP) technique. When 2,2'-bipyridyl was used as a Lewis base, conversion electron Mosbauer spectroscopy (CEMS) and x-ray powder diffraction (XRD) experiments showed that the main component of the ferrite films was metal-deficient magnetite (Fe3(1-δ)O4). Nonstoichiometry and roughness of LEP films were increased by the addition of 2,2'-bipyridyl. Using ethylenediaminetetraacetate (EDTA) as a Lewis base, produced film that was a mixture of magnetite and Υ-FeO(OH). No low temperature transition (Verwey transition) of magnetite was detected in resistivity and ac-susceptibility measurements for the LEP films. Surface morphology of the LEP films was observed by atomic force microscopy (AFM). The size of dominant particles was about 0.2 μm.

  • 넓적가시거저리[Bolitophagiella pannosa (Lewis)]는 국내에서 처음 보고되는 종으로 성충에 대한 기재문을 제공하였고, 야외관찰과 실내사육을 통해 다음과 같은 간략한 생활사를 밝혔다. 넓적가시거저리는 균식성 곤충으로 활엽수림에 나는 버섯, 특히 흰구멍장이속(Perenniporia)에 속하는 국내미기록종인 P. medulla-panis (Fr.) Donk와 아까시재목버섯[P. frazinea (Fr.) Ryv.]을 선호하였고, 성충과 유충이 혼재된 상태로 숙주버섯의 자실체 속이나 숙주버섯이 붙어있는 나무껍질 밑에서 월동하였으며, 실내사육(25.5${\sim}$$26.1^{\circ}C$; 63.5${\sim}$64.5% RH)한 결과 알에서부터 성충까지 약 54일이 소요되었다.

Hydrogen-bonded clusters in transformed Lewis acid to new Brønsted acid over WOx/SiO2 catalyst

  • The behavior of hydrogen species on the surface of the catalyst during the Lewis acid transformation to form Brønsted acid sites over the spherical silica-supported WOx catalyst was investigated. To understand the structure-activity relationship of Lewis acid transformation and hydrogen bonding interactions, we explore the potential of using the in situ diffuse reflection infrared Fourier transform spectroscopy (DRIFTS) with adsorbed ammonia and hydrogen exposure. From the results of in situ DRIFTS measurements, Lewis acid sites on surface catalysts were transformed into new Brønsted acid sites upon hydrogen exposure. The adsorbed NH3 on Lewis acid sites migrated to Brønsted acid sites forming NH4+. The results show that the dissociated H atoms present on the catalyst surface formed new Si-OH hydroxyl species - the new Brønsted acid site. Besides, the isolated Si-O-W species is the key towards H-bond and Si-OH formation. Additionally, the H atoms adsorbed surrounding the Si-O-W species of mono-oxo O=WO4 and di-oxo (O=)2WO2 species, where the Si-O-W species are the main species presented on the Inc-SSP catalysts than that of the IWI-SSP catalysts.

  • The Lewis acid mediated intramolecular additions of allylsilanes to acetal substrates are described. Excellent regio- and diastereoselectivity are achieved by boron trifluoride promoted cyclization of allysilane-acetals 5 and 8 derived from hydroxy allysilances 3. Cyclizations occur in moderate to high yields, providing directr routes to cis-2, 6-disubstituted 4-methylenetetrahydropyrans 6 and 9.

  • 1-Alkoxymethylphosphonates 4 and 1-alkylthiomethylphosphonates 5 can be prepared by the reaction of actal derivatives and diethyl trimethylsilyl phosphite in the presence of Lewis acid under mild conditions. The dependency of the chemoselectivities with Lewis acid on the reaction of O,S-acetals with phosphites is described.

Ivor Lewis 수술 후 발생되는 문합 부위 합병증을 예방하기 위한 술식 평가 (Experience with a Safe Anastomotic Method for Ivor Lewis Operation)

  • 배경: 식도암 수술에서 문합 부위 합병증은 수술 방법, 문합 위치 등과 관계 있다고 알려져 있으며 수술 후 환자의 삶의 질에도 중요한 부문을 차지한다. 저자들은 식도암 수술에 대한 고전적인 Ivor Lewis수술 중 stapling후 문합부의 조직 긴장 장력을 감소시킴으로써 식도-위 문합 부위 합병증의 발생 빈도를 줄이도록 고안된 술식을 평가하고자 하였다. 대상 및 방법: 2003년 5월부터 2007년 4월까지 식도암으로 Ivor Lewis 수술을 시행 받은 22명 중 외래 추적관찰이 가능한 18명을 대상으로 하였다 평균 연령은 61세$(46{\sim}73)$이었고 남자가 17명(94.4%)이었다. 양성 문합 협착은 술 후 추적 관찰 중 연하곤란을 호소하는 환자들에게 식도조영술을 시행하여 식도내 풍선화장술이 필요한 경우로 정의하였다. 결과: 평균 추적관찰 기간은 17.2개월$(1{\sim}45)$이었다. 수술 후 문합 부위 누출은 없었다. 양성 문합 협착은 1명(5.6%1에서 수술 후 4개월째 발생하였으며 수술 당시 식도 내경이 작아서 25 mm Stapler로 문합했었다. 식도내 풍선확장술을 2차례 시행한 후 증상이 호전되었으며 현재까지 38개월째 외래 추적 관찰 중이다. 결론: Ivor Lewis 수술 후 발생하는 양성 문합 협착 등 합병증을 줄이기 위하여 고안된 본원의 문합 술식은, 다른 저자들과 비교하여 효과적인 방법으로 생각되며 이후 장기적인 연구가 필요할 것으로 생각된다.
