The CCND1 G870A Gene Polymorphism and Leukemia or Non-Hodgkin Lymphoma Risk: a Meta-analysis

  • In recent years, mounting evidence has indicated that the CCND1 G870A gene polymorphism, which impacts the mitotic cell cycle, may influence leukemia or non-Hodgkin lymphoma risk. Unfortunately, the previous results were inconsistent. Therefore, a meta-analysis was performed to obtain a more precise estimation of any association. We conducted a search in PubMed, Embase and CNKI covering all published papers up to March, 2014. A total of 9 publications including 10 case-control studies met the inclusion criteria. Odds ratios (ORs) and their 95% confidence intervals (95%CIs) were applied to assess association. The pooled ORs showed significant association in non-Hodgkin lymphoma (comparison A vs G: OR= 1.114, 95%CI=1.053-1.179, p=0.000; homozygote comparison AA vs GG: OR=1.245, 95%CI=1.110-1.396, p=0.000; heterozygote comparison AG vs GG: OR=1.095, 95%CI=1.000-1.199, p=0.05; dominant model AA/GA vs GG: OR=1.137, 95%CI=1.043-1.239, p=0.003; and recessive model AA vs GA/GG: OR=1.177, 95%CI=1.066-1.301, p=0.001). However, there was no association between the CCND1 G870A polymorphism and leukemia risk. In conclusion, the CCND1 G870A polymorphism may increase risk of non-Hodgkin lymphoma, but not leukemia. However, more primary large scale and well-designed studies are still required to evaluate the interaction of CCND1 G870A polymorphism with leukemia and non-Hodgkin lymphoma risk.

Histopathological classification of the bovine lymphoma caused by bovine leukemia virus in Korea (소 백혈병 바이러스 감염에 의한 소 림프종의 병리조직학적 분류)

  • Several studies have been performed on the bovine leukemia since bovine leukemia virus (BLV) had been detected in 1982 in Korea. We have conducted histopathological study on the bovine lymphoma caused by BLV because only limited results were reported on the pathological characterization of lymphoma. Lymphoma tissues were obtained from cattle necropsied and slaughtered during a designated period. Lymphoma was classified histopathologically according to the National Cancer Institute Working Formulation. Leukotic tissues consisted of fairly uniform sheets of closely packed medium to large lymphocytes without any architectural arrangement in all 30 cases. Twenty five cases belong to diffuse large cell type, while three cases were diffuse mixed cell type, and two cases were immunoblastic large cell type among 30 cases. Follicular type lymphoma was not detected in this study. The mitotic index of tumor cells showed average 2.5 in the field of 400X. Nuclear cleavage was detected in 53% of cases. Multi-nucleated cells were detected among tumor cells in 30% of lymphoma cases. In conclusion, the most common morphologic cell type of bovine lymphoma in Korea was a diffuse large cell type with multinucleated cells and nuclear cleavages.

  • The frequency and distribution of lymphoma caused by bovine leukemia virus (BLV) infection in various organs were investigated. Lymphoma samples were obtained from slaughtered cattle or from cattle submitted to the National Veterinary Research and Quarantine Service, Korea. Thirty female Holstein-Friesian dairy cattle aged over three years with the BLV-associated lymphoma were studied. None of the Korean native cattle (Hanwoo) had lymphoma in this study however. Lymphoma tissues were gray to pink in color, soft in consistency, and bulged from the cut surface. In advanced lymphoma tissues, there was great variety in the appearance of involved structures due to hemorrhage, necrosis, and/or calcification. Neoplastic tissues were observed in lymph nodes in all lymphoma cases. Intestine (96.4%), heart (88.9%), stomach (73.1%), and diaphragm (62.5%) were frequently involved with lymphoma. However, there was no lymphoma detected in liver. Large neoplastic masses, sometimes reaching the size of over 20 cm, were found in the abdominal cavities. It is suggested that metastasis of lymphomas occurs mainly via lymph based on gross observations; neoplasia may have been initiated in the serosal surface of the lung, heart, peritoneum, and numerous hollow organs in the abdominal cavity. Also many organs in the abdominal and thoracic cavity were affected by neoplastic tissues simultaneously. Characteristics observed in this study could be used as criteria to differentiate BLV-associated lymphoma from other nodular lesions in the slaughterhouse and as fundamental data to make clear the mechanism of metastasis or pathogenesis of EBL.

  • Background: Leukemia and lymphoma demonstrate significantly incidence rates throughout the world and particularly in Iran they cause serious mortality and diagnosis and treatment expenditures for both families and the health system. Combined they account for about 11 percent of cancers in Mazandaran province, ranking number 2 in prevalent cancers. The purpose of this study was to provide a first general and specific description of leukemia and lymphoma in Mazandaran province. Materials and Methods: In this descriptive retrospective study, entire patient's data were reviewed which had confirmed diagnosis of leukemia and lymphoma with valid laboratory or pathology reports in the period 2001-2008. The data were collected by Babol health research site related to Tehran University of Medical Science. Incidence rates based on age groups, gender, city of residence and type of malignancy were calculated and analyzed. Results: In Mazandaran province, 1,146 cases of leukemia and lymphoma were encountered, 5.9 in 100,000 persons on average annually. The highest incidence rates were obtained at age of 70 or above (26.4) and the lowest at age of 0-9 (2.3).The incidence rates in males and females were 7.1 and 4.8 respectively with a ratio of 1.5. The highest incidence rate was in Babol (7.3) and the lowest was calculated in Neka and Tonekabon equally (1.5). According to the type of malignancy, non Hodgkin lymphoma, with 2.5/100,000 have the most incidence rate and myeloid leukemia with 1.8 had the lowest. Conclusions: The obtained findings indicate clear differences in incidence rates based on age, gender, residence, and type of malignancy. Therefore it's suggested that in addition to promote data collecting programs, research projects should be programmed to define leukemia and lymphoma risk factors in this province.

Leukoencephalopathy and Disseminated Necrotizing Leukoencephalopathy Following Intrathecal Methotrexate Chemotherapy and Radiation Therapy for Central Nerve System Lymphoma or Leukemia

  • Objective : Intrathecal methotrexate (MTX) therapy combined with whole brain radiotherapy (WBRT) is one of the major treatment modalities for leukemia and lymphoma involving the central nervous system (CNS). The purpose of this study was to retrospectively determine the incidences of leukoencephalopathy and disseminated necrotizing leukoencephalopathy (DNL) following intrathecal MTX therapy for CNS lymphoma or leukemia and to assess the potential risk factors. Methods : Between January 2000 and August 2009, 143 patients with CNS lymphoma or leukemia received intrathecal MTX therapy alone or in combination with WBRT at a single institution. Patients were followed up clinically and radiologically at regular two- or three-month intervals. Medical records were reviewed to obtain information regarding the patients' demographics, medical histories, radiologic characteristics, treatments, and clinical courses. Results : On follow-up MR images, leukoencephalopathy was found in 95 of 143 patients (66.4%). The median time to develop leukoencephalopathy was 6.6 months. Among those with leukoencephalopathy, four patients showed seven extensive white-matter changes with strongly enhancing lesions demonstrating DNL. Histological confirmation was done in six lesions of three patients and radiological diagnosis alone in one patient. Four lesions spontaneously disappeared on MR images without any treatment, with a mean duration of 14 months before disappearance of DNL. Conclusion : Leukoencephalopathy is a common phenomenon that occurs following intrathecal MTX therapy; however, DNL occurs at a very low incidence. For newly developed enhancing lesions, consideration for the occurrence of DNL should be taken to avoid unnecessary invasive procedures or therapies.

Epirubicin Inhibits Soluble CD25 Secretion by Treg Cells Isolated from Diffuse Large B-cell Lymphoma Patients

  • Objective: To investigate the effect of epirubicin on soluble CD25 (sCD25) secretion by CD4+CD25+ regulatory T (Treg) cells isolated from diffuse large B-cell lymphoma (DLBCL) patients. Methods: Treg cells were isolated from the peripheral blood mononuclear cells isolated from the newly diagnosed DBLCL patients. The concentration of sCD25 in the supernatant was determined with a commercial sCD25 (IL-2R) enzyme-linked immunosorbent assay (ELISA) kit. The fluorescence intensity of CD25 was detected by flow cytometry. Results: Cell survival rate was significantly decreased along with the increase of epirubicin concentration after treatment for 24 h. There was also a significant difference in the concentration of sCD25 between the epirubicin group and the control group (P<0.01). A positive correlation between the Treg cells survival rate and the concentration of sCD25 was detected (r=0.993, P<0.01). When equal numbers of CD4+CD25+ Treg cells of the epirubicin group and the control group were cultured for another 24 h without epirubicin the CD25 fluorescence intensity on the surface of Treg cells was obviously higher in the epirubicin group than that in the control group (P<0.01), while the sCD25 concentration in the supernatant in the epirubicin group was significantly lower than that in the control group (P<0.05). Conclusion: Epirubicin may improve the body's immune functions by inhibiting the sCD25 secretion by Treg cells in DLBCL patients.

A fatal case of acute pulmonary embolism caused by right ventricular masses of acute lymphoblastic lymphoma-leukemia in a 13 year old girl

  • We report a case of a 13-year-old girl with acute lymphoblastic lymphoma- leukemia, who presented with a cardiac metastasis in the right ventricle, resulting in a pulmonary embolism. At the time of her leukemia diagnosis, a cardiac mass was incidentally found. The differential diagnosis for this unusual cardiac mass included cardiac tumor, metastasis, vegetation, and thrombus. Empirical treatment was initiated, including anticoagulation and antibiotics. She underwent plasmapheresis and was administered oral prednisolone for her leukemia. Five days later, she experienced sudden hemodynamic collapse and required extracorporeal membrane oxygenation insertion and emergency surgery. These interventions proved futile, and the patient died. Pathology revealed that the cardiac mass comprised an aggregation of small, round, necrotic cells consistent with leukemia. This is the first known case of acute lymphoblastic leukemia presenting as a right ventricular mass, with consequent fatal acute pulmonary embolism. A cardiac mass in a child with acute leukemia merits investigation to rule out every possible etiology, including vegetation, thrombus, and even a mass of leukemic cells, which could result in the fatal complication of pulmonary embolism.

  • Central nervous system[CNS] involvement of acute lymphoblastic leukemia may occur. However, CNS involvement as a first manifestation of leukemia is very rare. An 8-year-old girl complained of a backache after playing in the water. Neurological examination detected progressing paraparesis. Magnetic resonance imaging[MRI] of the thoracolumbar spine showed a well-circumscribed homogeneous posterior extradural mass lesion extending from T7 to T9. MRI of the brain showed diffused fatty marrow replacement of the calvarium and the skull base. We report a patient with epidural Burkitt's lymphoma of the thoracic and lumbar vertebra causing compression of the spinal cord after pathologic evaluation. The tumor consisted mainly of lymphoblastic cells, which were identical to those originally seen in the bone marrow aspiration and biopsy. After decompressive laminectomy she began consolidation chemotherapy.

A literal study of anti-tumor effects of chunghwangsan for leukemia (청황산(靑黃散)의 백혈병(白血病)에 대(對)한 항종양효능(抗腫瘍效能) 연구(硏究))

  • In the literatual study of anti-tumor effects of chunghwangsan for leukemia, the results were as follows. 1. Chunghwangsan is composed of Indigo naturalis and Realgar. The composing rate is 9 : 1 and it is made into capsule or piece. The basic administration is 0.3g per day and could increase the quantity each day. 2. The effects of Chunghwangsan is expelling toxin and colling, colling blood to detumescence, drying wetness and anticancer are. So it can be used to treat AML, CML and lymphoma. 3. The anticancer component of Indigo naturalis is indirubin which has the effects of suppression the transplanted tumor, activating the phagocyte of macrophage, promoting the maturation of myeloblast to improve cure rate of CML. The anticancer component of Realgar is $As_2O_3$ which has the direct cellular toxicity for leukemia cell. 4. In viewpoints of oriental medicine, leukemia is malignant myeloid neoplasia in which pathogen invade to shaoyin(少陰). So Chunghwangsan which is expelling toxin and colling, colling blood to detumescence, drying wetness and anticancer is effective to leukemia. 5. In clinical reports, Chunghwangsan is often used in CML, and also used in AML, lymphoma and so on. 6. Chunghwangsan is cool-natured, so we must carefully pay attention to pregnant women and hematsdthenic patients. The main side effects are nausea, bone marrow pain, diarrhea, polydefecation, hematokezia and purpora. We sometimes take invigorating stomach medicine to prevent the side effects. 7. If we continuously develop Chunghwangsan and therapy for leukemia with syndrome differentiation. we can improve the response and cure rate for leukemia in the future.

Histopathological Features of Lymphoma in Yogyakarta, Indonesia

  • The incidence and prevalence, the second most common lymphoid malignancy after leukemia, are both increasing. The distribution of lymphoma varies among sexes, age groups, and sites. In Indonesia, information about the incidence of lymphoma and its characteristics are insufficient. Therefore, this study was performed to evaluate the incidence of lymphoma and features based on age group, sex, site, clinical diagnosis, and histopathological type in Indonesia. This study is an observational analytical study with a cross-sectional design aimed to evaluate the histopathological profile of lymphoma in Yogyakarta from 2010-2014. It was based on secondary data from Anatomic Pathology Department's medical records from several hospitals and laboratories. The result showed an increased incidence of lymphoma in Yogyakarta in 2010-2014 (p=0.039). Lymphoma mostly occurred in age range 45-64 years (p=0.004), dominated by male with ratio 1.6:1. DLBCL was found to be the most common histopathological type (44.4%). Sex, age, and clinical diagnosis demonstrated statistically significant correlations with the histopathological type (p<0.001). In conclusion, the incidence of lymphoma has significantly increased from 2010-2014. There are statistically significant correlations between gender, age, and clinical diagnosis with the histopathological type of lymphoma.
