  • Purpose: The aim of this retrospective study was to compare the survival rate of oxidized titanium implants and sandblasted large-grit acid etched implants in soft bone. Methods: 201 oxidized titanium implants were inserted in 84 patients between May 1999 and May 2004. 120 sandblasted large-grit acid etched implants were inserted in 74 patients between December 2000 and May 2004. The patients were followed-up 0${\sim}$5 years in ITI group or 0${\sim}$6 years in BRA group, respectively. The following information was collected from the patient records: age, gender, systemic disease, implant type, number, length and diameter of the implants, their location in the jaws, bone quantity, the number of failed implants, the causes of failure, and advanced surgery for bone augmentation. Results: In the oxidized titanium implants, 8 implants showed early failure, and 1 implant showed late failure, respectively. The cumulative survival rate was 95.48%. In the sandblasted large-grit acid etched implants, 1 implant showed late failure and cumulative survival rate was 99.10%. The cumulative survival rate and the survival rates in the case of the advanced procedure during the implant placement were not significantly different in both groups. Conclusions: Oxidized titanium implants and sandblasted large-grit acid etched implants can be used successfully in soft bone regardless of the surgical methods used during the implant placement. • Detrimental effects of inbreeding have been studied by many researchers for a long time. However, only a few studies have shown the occurrence of inbreeding depression due to evolutionary changes as a purging process. In this study, two different populations (inbreeding and outbreeding) of Drosophila melanogaster were compared to assess inbreeding effects on artificial population bottlenecks. For inbreeding conditions, a couple of D. melanogaster (one virgin and one male) were selected from an inbred population and cultured in a vial. For outbreeding conditions, a couple of D. melanogaster were selected from different populations and cultured in a vial. There were significant differences in body lengths of adults, but not in other parameters such as the total number of adults, the rate of survival, and the rate of wing mutants. The mean body length of adults of outbreeding populations was longer than that of inbreeding populations in the first generation (G1; P = 0.004), but not in the second generation (G2; P = 0.066). Although the other three parameters (total number of adults, rate of survival, and rate of wing mutants) showed differences in their mean values between inbreeding and outbreeding populations, these differences were not statistically significant. This might be due to genetic purging. This study demonstrated one additional experimental case related to inbreeding depression in artificial bottlenecked populations. Further studies are necessary to confirm the clear interaction between inbreeding depression and genetic purging using more generations and replicates (or samples) of D. melanogaster.

  • Background The nasolabial flap is ideal for reconstruction of the nasal alar subunit due to its proximity, color and contour match, and well-placed donor scar. When raised as a random-pattern flap, there is a risk of vascular compromise to the tip with increased flap length and aggressive flap thinning. Surgical delay can greatly improve the chances of tip survival, allowing the harvest of longer flaps with greater reach. Methods We describe our technique of lengthening the nasolabial flap through multiple delay procedures. A bipedicled flap was first raised and then transferred as a unipedicled flap with a 6:1 length-to-width ratio. During the delay process, the flap tip was thinned to the subdermal layer. Results In our case series of seven patients, defects as far as the medial canthal area and contralateral ala were reconstructed successfully with no incidence of tip necrosis or flap loss. The resultant flaps were thin enough to be folded over for the reconstruction of alar rim defects. Conclusions We highlight the success of our surgical technique in creating thin and robust nasolabial flaps for the reconstruction of full-thickness defects around the nose.

  • Objectives : Period analysis estimates up-to-date survival rates of cancer patients. In this approach, analysis is restricted to recent time period by left-truncating all observations at the beginning of the period and right-censoring at its end. Here, we applied period analysis to examine changes in 5-year relative survival (RS) by age group for 1997 and for 2002. Methods : Using the National Cancer Incidence Database, 5-year RS was estimated for 1997 and 2002 in four age groups (15-54, 55-64, 65-74, and 75 years old and over) using period analysis. After excluding death certificate-only cases, patients with an unknown date of diagnosis or follow-up length, a total of 813,889 patients diagnosed with a first primary invasive cancer during 1992 2002 were included for analysis. Followup for vital status was included until 31 December 2002. Results : Five-year RS increased from 41.7% for 1997 to 46.7% for 2002. Increases in survival occurred in all age groups except in the 75 and over group. Conclusions : The age gradient in cancer prognosis seems to have widened between 1997 and 2002, a finding that requires further study of prognostic factors, including stage at diagnosis. Period analysis accurately estimates survival rates, especially for cancers with better prognosis.

  • The aims of this study were to evaluate the prognostic predictabilities of the risk classifications (Waterston, Montreal, and Spitz), preoperative treatment of pneumonia and the length of esophageal gap in esophageal atresia. Twenty-five cases of esophageal atresia were reviewed retrospectively. The data was analyzed with Likelihood ratio test for trend and Fisher's exact test. Survival rates were 100.0 % in group A and B, and 60.0 % in group C according to Waters ton classification (p=0.027). By Montreal classification, 95.7 % in group I and 50.0 % in group II (p=0.086). By Spitz classification, 95.5 % in group I and 66.7% in group II (p=0.159). The survival rate was 85.7 % in the patients with pneumonia preoperatively treated and 94.4% in the patients without pneumonia. The difference was not significant (p=0.490). Survival rate in the patients with long gap (${\geq}$3cm) was 50.0 % and 100.0 % in patients with short gap (<3cm) with significant difference (p=0.020). The Waterston classification could be modified into two groups because the prognosis of both group A and B were the same. Preoperative pneumonia does not seem to be a risk factor. The length of the esophageal gap showed prognostic value.

  • Purpose: To evaluate the prognostic value of serum CYFRA21-1, CEA and hemoglobin levels regarding long-term survival of patients with esophageal squamous cell carcinoma (ESCC) treated with concurrent chemoradiotherapy (CRT). Methods: Age, gender, Karnofsky Performance Status (KPS), tumor location, tumor length, T stage, N stage and serum hemoglobin, and CYFRA21-1 and CEA levels before concurrent CRT were retrospectively investigated and related to outcome in 113 patients receiving 5-fluorouracil and cisplatin combined with radiotherapy for ESCC. The Kaplan-Meier method was used to analyze prognosis, the log-rank to compare groups, the Cox proportional hazards model for multivariate analysis, and ROC curve analysis for assessment of predictive performance of biologic markers. Results: The median survival time was 20.1 months and the 1-, 2-, 3-, 5- year overall survival rates were 66.4%, 43.4%, 31.9% and 15.0%, respectively. Univariate analysis showed that factors associated with prognosis were KPS, tumor length, T-stage, N-stage, hemoglobin, CYFRA21-1 and CEA level. Multivariate analysis showed T-stage, N-stage, hemoglobin, CYFRA21-1 and CEA level were independent predictors of prognosis. By ROC curve, CYFRA21-1 and hemoglobin showed better predictive performance for OS than CEA (AUC= 0.791, 0.704, 0.545; P=0.000, 0.000, 0.409). Conclusions: Of all clinicopathological and molecular factors, T stage, N stage, hemoglobin, CYFRA21-1 and CEA level were independent predictors of prognosis for patients with ESCC treated with concurrent CRT. Among biomarkers, CYFRA21-1 and hemoglobin may have a better predictive potential than CEA for long-term outcomes.

  • The accumulation of ammonia, a consequence of intensive aquaculture activity, can damage cultured animals. We analyzed the survival rates, growth, antioxidant responses, and immune responses of abalones, Haliotis discus hannai (mean shell length 70.2 ± 4.9 mm; mean body weight 36.9 ± 3.6 g), biweekly for four weeks, to determine the accumulated concentrations of ammonia (0, 0.1, 0.2, 0.4, 0.8, and 1.6 mg/L). In our biweekly measurements, a survival rate of ≥ 95% was observed for all concentrations of ammonia. The specific growth rate (SGR) decreased at ≥ 0.8 mg/L (p < 0.05), but the daily increment in shell length (DISL) showed no significant change (p < 0.05). Regarding antioxidant responses, the superoxide dismutase (SOD) activity of the hepatopancreas was increased at ammonia concentrations over 0.8 and 0.2 mg/L (p < 0.05) at two and four weeks, respectively, and the SOD activity of the gills increased at concentrations over 0.4 and 1.6 mg/L (p < 0.05) at four weeks. The catalase (CAT) activity of the hepatopancreas and gills increased at ammonia concentrations > 0.8 mg/L (p < 0.05). Phenoloxidase (PO) activity increased at ammonia concentrations over 0.8 mg/L (p < 0.05), and Lysozyme (LZM) increased at concentrations over 0.8 and 0.4 mg/L (p < 0.05). Overall, our findings indicated that ammonia concentrations over 0.8 mg/L in seawater might damage the SGR, antioxidant responses, and immune responses in H. discus hannai. Our findings suggest the necessity of improvement or reinstallation for abalone aquaculture systems and can be used to assess the toxic effects of ammonia on H. hannai.

  • Research has found that the management of the host plant is essential to mistletoe cultivation. A Trunk injection test on the host plant that contained a mixture of indole-3-butryc acid (IBA) and liquid fertilizer was conducted with respect to the improvement of the one year survival rate of mistletoe. As a result, the trunk injection experiments showed the effect of the IBA and liquid fertilizer mixture in all treatments. This mixture was effective to increase the survival rate of mistletoe by 20% with the IBA at 100 mg/L and Hyponex at 100 mg/L. The examination proved that the host plant fertilizer effect was the most effective treatment for organic fertilizer with 60% added NPK (4-2-1). Its effects were higher compared to the control at the length and diameter of one-years-old branches in the host plant, even when the parasitic mistletoe improved its growth in length, diameter, and number of branches. Comparing the control and host plant fertilizer, the latter was the most effective way to process 20 kg per a treatment effect in the experimental site and to process at any time after the inoculation. This treatment is effective to improve the growth of mistletoe by watering the host plant three times per week. Therefore, the management of the host plant is an essential element in the successful cultivation of mistletoe, not only to supply fertilizer and plant hormones to the host plant in the initial inoculation time but also to provide organic fertilizer and irrigation for the host plant.

  • Two edible plants, Allium victorialis var. platyphyllum and Ligularia stenocephala, and one medicinal plant, Panax ginseng, were cultivated in the understory of an artificial Larix kaempferi plantation for ten years. Growth characteristics (number of leaves and flower stalks per plant, and leaf length and width), survival rate, and yield (fresh weight of plants) per unit area (1 ㎡) were investigated one year after planting, and six and ten years following cultivation. P. ginseng and L. stenocephala survived at a high percentage for two years after planting. Results showed that P. ginseng had longer and thicker roots when aged; however, a large number of plants died and the yield was low. In contrast, almost half of A. victorialis var. platyphyllum died within two years of planting. The surviving plants grew well for ten years and the yield was increased. The leaf length and width of L. stenocephala increased; however, the survival rate and the number of leaves per plant decreased as the period of cultivation was extended. In contrast, A. victorialis var. platyphyllum survived at a lower rate (50%) than the two other crops (98% for L. stenocephala and 83% for P. ginseng) during the early cultivation period, with little change in the survival rate over an extended time; however, the yields increased. This species showed an increase in the number of flower stalks and leaves, and as a result, the larger leaves increased the yield. We evaluated the understory cultivation of three crops in a L. kaempferi plantation under natural conditions, with no irrigation or fertilization, and Allium victorialis var. platyphyllum showed the greatest growth potential among the three tested crops.
