수술 중 예상치 못한 과출혈이 발생한 여호와의 증인 환자의 마취 1 예 (Anesthesia for a Jehovah's Witness Patient Experiencing Unexpected Perioperative Hemorrhage - A Case Report -)

  • Jehovah's Witnesses refuse a transfusion of blood or blood products because of religious beliefs; this refusal at times presents a dilemma for the treating physician. We report a case of a 25-year-old Jehovah's Witness patient who underwent a reoperation for a previous proximal humerus shaft fracture and experienced unexpected massive hemorrhage intraoperatively and postoperatively. The postoperative lowest hemoglobin level was 2.9 g/dl. The patient recovered from the severe anemia without any clinical sequala. We review the legal, ethical and religious issues and suggest the best possible medical care that Jehovah's Witness patient would permit.

  • Healthcare applications involve complex structures of interacting processes and professionals that need to exchange information to provide the care services. In this kind of systems many different professional competencies, ethical and sensibility requirements as well as legal frameworks coexist and because of that the information managed inside the system should not be freely accessed, on the contrary. it must be subject to very complex privacy restrictions. This is particularly critical in distributed systems, where additionally, security in remote transmissions must be ensured. In this paper, we address the fundamental security issues that must be considered in design of a distributed healthcare application.

산업보건분야에서의 생성형 AI: ChatGPT 활용과 우려 (Applications and Concerns of Generative AI: ChatGPT in the Field of Occupational Health)

  • As advances in artificial intelligence (AI) increasingly approach areas once relegated to the realm of science fiction, there is growing public interest in using these technologies for practical everyday tasks in both the home and the workplace. This paper explores the applications of and implications for of using ChatGPT, a conversational AI model based on GPT-3.5 and GPT-4.0, in the field of occupational health and safety. After gaining over one million users within five days of its launch, ChatGPT has shown promise in addressing issues ranging from emergency response to chemical exposure to recommending personal protective equipment. However, despite its potential usefulness, the integration of AI into scientific work and professional settings raises several concerns. These concerns include the ethical dimensions of recognizing AI as a co-author in academic publications, the limitations and biases inherent in the data used to train these models, legal responsibilities in professional contexts, and potential shifts in employment following technological advances. This paper aims to provide a comprehensive overview of these issues and to contribute to the ongoing dialogue on the responsible use of AI in occupational health and safety.

Normative Issues in Next Generation Sequencing Gene Testing

  • Despite the commercialization of Next generation sequencing (NGS) gene testing, only a few studies have addressed the various ethical and legal problems associated with NGS testing in Korea Here, we reviewed the normative issues that emerged at each stage of the wet analysis and bioinformatics analysis of NGS gene testing. In particular, it was in mind to apply various international guidelines and the principles of bioethics to actual clinical practice. Considering the characteristics of NGS testing, wet analysis of additional testing can be justified if presumptive consent is recognized. Furthermore, the medical relationship between diseases needs to be established and it should be clear that the patient would have given consent if the patient had been aware of the correlation between genes. At the stage of bioinformatics analysis, the question of unsolicited findings arises. In case of unsolicited and relevant findings, according to American College of Medical Genetics and Genomics (ACMG), a recognized relationship between genes and diseases needs to be established. In case of unsolicited and not-relevant findings, it is almost impossible to determine whether knowing or not knowing the findings is more beneficial to the patient. However, it seems to be certain that the psychological harm an individual may suffer from such information is likely to be greater if the disease is severe and if there is no cure. The list of genes for which the ACMG guidelines impose reporting obligations is a good reference for judgment.

유전체학 시대의 한국 종양 유전 간호의 과제 (Current Issues and Tasks of Genetic Cancer Nursing in Korea)

  • Purpose: The purpose of this review article is to introduce how the Korean Society of Genetic Nursing (KSGN) has evolved and tried to translate genomic knowledge to nursing practice, and then to suggest the future role of genetic nurses in Korea. Methods: A literature review was performed and the current status of genetic counselling in Korea was explored. Then the educational and clinical experiences of the authors were incorporated. Finally, the main activities of Korean nursing for genetics were identified. Results: Two types of genetic counsellor certification have been issued in Korea: one is issued by the Korean Society of Genetic Medicine, another by the Korean Society of Breast Cancer since June 2011. A few Korean nursing researchers have continuously performed research related to genetic nursing and undertook several research projects funded by the government since 2003. In February 2011, KSGN was established and is now trying to establish further international networks. Conclusion: Nursing genetic experts should be trained to integrate all specialties for genetic counselling, so they can provide holistic genetic services including ethical, legal, and social issues (ELSI).

소셜 빅데이터 기반 인공지능 개발윤리 인식 분석 (A Study on the Awareness of Artificial Intelligence Development Ethics based on Social Big Data)

  • Artificial intelligence is a core technology in the era of digital transformation, and as the technology level is advanced and used in various industries, its influence is growing in various fields, including social, ethical and legal issues. Therefore, it is time to raise social awareness on ethics of artificial intelligence as a prevention measure as well as improvement of laws and institutional systems related to artificial intelligence development. In this study, we analyzed unstructured data, typically text, such as online news articles and comments to confirm the degree of social awareness on ethics of artificial intelligence development. The analysis showed that the public intended to concentrate on specific issues such as "Human," "Robot," and "President" in 2018 to 2019, while the public has been interested in the use of personal information and gender conflics in 2020 to 2021.

공공디자인 정책 결정에 ChatGPT의 활용 가능성에 관한연구 (A Study on the Potential Use of ChatGPT in Public Design Policy Decision-Making)

  • 본 연구는 공공디자인 정책 결정에 있어 거대 언어 및 정보 모델인 ChatGPT가 기여할 가능성이 있는지에 대해 공공디자인 가진 특징을 중심으로 연구했다. 공공디자인은 디자인의 원리와 접근법을 사용하여 사회문제를 해결하고, 공공서비스 개선을 목표로 한다. 공공디자인 정책과 계획을 수립하기 위해서는 지역의 일반 현황, 인구 현황, 인프라 현황, 자원 현황, 안전 현황, 정책 현황, 법규 현황, 경관 현황, 공간 현황, 공공디자인 현황, 지역 이슈 등 방대한 자료를 기반으로 한다. 따라서 공공디자인은 방대한 자료와 더불어 방대한 언어를 수록하는 디자인 연구 분야다. 인공지능 기술의 급속한 발전과 공공디자인의 중요성을 고려해 ChatGPT와 같은 거대 언어 및 정보 모델이 공공디자인 정책에 어떻게 기여할 수 있는지 알아보고자 한다. 이와 함께, 공공디자인의 개념 및 원칙, 그리고 정책 개발과 실행에 대한 역할을 검토하고, ChatGPT의 개요 및 특징, 적용 사례나 ChatGPT의 선행 연구를 살펴 공공디자인 정책 결정에 활용할 수 있는지 연구했다. 연구 결과, ChatGPT는 공공디자인 정책 수립과정에서 방대한 언어 정보를 제공하고, 의사결정의 지원 역할이 가능하다는 사실을 밝혔다. 특히, ChatGPT는 정책 수립과정에서 다양한 관점을 제공하고, 정책 결정에 필요한 정보를 신속하게 제공하는 데 유용함이 있었다. 이와 함께 정부 정책 개발에 인공지능을 활용하는 추세라는 것이 여러 논문을 통해 확인되었다. 하지만, ChatGPT의 활용에는 윤리적, 법적, 개인 프라이버시 등의 문제 또한 발견되었다. 무엇보다 윤리적인 문제가 제기되었으며, 편향성과 공정성 관련 문제 또한 나타났다. ChatGPT를 공공디자인의 정책 결정에 실질적으로 활용하려면, 첫째, 정책 개발자와 공공디자인 전문가의 역량을 일정부분 키워 활용해야 한다. 둘째, 가칭 '인공지능 정책 활용에 관한 조례'라는 법령(法令)을 마련해 법률(法律)적 정비가 이뤄지기 전까지 지속해서 보완해가면서 활용하는 것이다. 현재로서는 이 두 가지 방안을 적용해 활용하는 것이 필요하다. 따라서 공공디자인 정책 결정에 있어 ChatGPT와 같은 거대 언어 및 정보 모델의 활용은 방대한 언어를 수록하는 디자인 분야에서는 활용할 가치가 충분하다는 것이다.

한국형 기업의 사회적 책임활동 측정을 위한 척도 개발 연구: 소비자 인식을 중심으로 (Developing a Scale for Measuring the Corporate Social Responsibility Activities of Korea Corporation: Focusing on the Consumers' Awareness)

  • 그동안 기업의 사회적 책임활동에 대한 척도개발이 국·내외적으로 일부 연구자들에서 수행되어져 왔지만, 기업의 사회적 책임활동을 측정하고 있는 구성개념(측정항목)들에 대한 이론적 배경의 결핍과 기업의 네 가지 사회적 책임활동 차원이 과연 서로 이질적인 차원인지에 대한 의문이 제기되었다. 또한, 기업의 사회적 책임활동과 관련된 지난 30년간 발표된 정량적 분석에 의하면, 대부분의 연구들이 측정항목과 방법론 측면에서 많은 문제점을 보이고 있다. 이처럼 기업의 사회적 책임활동 (CSR)에 대한 학문적 관심은 오래 지속되고 있지만, 기업의 사회적 책임차원 (경제적, 법률적, 윤리적, 자선적 책임)에 대하여 소비자들이 어떻게 바라보는지, 혹은 어떻게 인지하는지에 대해 측정할 수 있는 척도가 미흡한 실정이다. 특히 기업의 사회적 책임활동에 대한 척도가 서구사회 위주로 개발되어 국내에서 인용되다보니, 국내 특성에 맞는 기업의 사회적 책임활동을 측정할 수 있는 척도가 미흡하였다. 이에 본 연구는 다양한 이해관계들을 대상을 집단면접(FGI)을 실시하여 국내 상황에 맞는 척도를 개발하였고, 본 연구에서 개발된 새로운 척도가 기존 척도에 비하여 더 높은 예측력을 지니는지를 간접적으로 증명하였다. 본 연구결과를 통해 새롭게 개발된 기업의 사회적 책임활동에 대한 척도는 다음과 같다. 우선, 경제적 책임은 '제품의 품질개선 노력', '고객의 불평처리에 대한 체계구축', '이윤창출을 통한 국가 경제발전 이바지', '고용창출 노력', 법률적 책임으로는 '법적기준 준수', '직원들의 복지추구와 고용 관련법 준수', '명시된 계약적 책임이행', '기업경영 관련법 준수', '소비자보호법 준수', 윤리적 책임으로는 '윤리강령 지침마련', '과장광고나 허위광고', '투명경영', '사업파트너와의 공정한 거래', 자선적 책임으로는 '지역사회와의 협력사업', '스포츠 및 문화활동 지원', '지역사회 봉사', '사회환원' 등이다. 연구결과는 국내 기업들에게 경제적 책임활동의 중요성을 다시 한 번 인지시키고 있으며, 특히 국내 기업들에게 자사의 사회적 책임활동을 진단하는데 중요한 실무적인 시사점을 제시할 것이다.

노인간호학 교과과정에 대한 조사연구 (A Study of Gerontological Nursing Curriculum)

  • To survey the present status of the gerontological nursing course at three year diploma programs, baccalaureate degree programs(BSN), and graduate programs in Korea, and to analyze the contents of the syllabus and gerontological nursing textbooks to provide the basic data in developing a standard model for gerontological nursing curriculum. Method: Data was collected from all the nursing programs in Korea from Nov. 2000 to Feb. 2001 by mail and fax. Result: The gerontological nursing courses has been offered 36 diploma program, 40 BSN, and 17 graduate programs. And the credits of the gerontological nursing course offered by the program were as follows : one credit (10 diploma and 8 BSN), two credits (22 diploma and 29 BSN), and three credits (1 BSN). The contents of curri- culum were analyzed by comparing the core curriculum of NGNA. The majority of the schools included Gerontological Nursing in General, Theory of Aging, Aging Processes, Care Plan Options, and Common Health Problems. The subjects which very few school cover are Legal/ Ethical Issues, Evaluation, Regulatory & Reimbursement Issues, Education Issues, Nursing Research in Gerontology, and Environmental Issues of Older Adults. There were some differences in these results among diploma courses, BSNs, and graduate schools. The gerontological nursing textbooks contained similar contents to those of the diploma and the baccalaureate programs.

유전상담의 제도적인 고찰 (Genetic counseling in Korean health care system)

  • Unprecedented amount of genetic information being generated from the result of Human Genome Project (HGP) and advances in genetic research is already forcing changes in the paradigm of health and disease. The ultimate goal of genetic medicine is to use genetic information and technology to develop new ways of treatment or even prevention of the disease on an individual level for 'personalized medicine'. Genetics is play ing an increasingly important role in the diagnosis, monitoring and management of common multifactorial diseases in addition to rare single-gene disorders. While wide range of genetic testing have provided benefits to patients and family, uncertainties surrounding test interpretation, the current lack of available medical options for the diseases, and risks for discrimination and social stigmatization may remain to be resolved. However an increasing number of genetic tests are becoming commercially available, including direct to consumer genetic testing, yet public is often unaw are of their clinical and social implications. The personal nature of information generated by a genetic test, its power to affect major life decisions and family members, and its potential misuse raise important ethical considerations. Therefore appropriate genetic counseling is needed for patient to be informed with the benefits, limitations and risks of genetic tests, prior to informed consent for the tests. Physician also should be familiar with the legal and ethical issues involved in genetic testing to tell patients how w ell a particular genetic risk factor relates with likelihood of disease, and be able to provide appropriate genetic counseling. Genetic counseling become a mandatory requirement as global standard for many genetic testing such as prenatal diagnosis, presymtomatic DNA diagnostic tests and cancer susceptibility gene test for familial cancer syndrome. In oder to meet the challenge of genetic medicine of 21 century in korean health care system, professional education program and certification board for medical genetics specialist including non-MD genetic counselors should be addressed by medical society and regulatory policy of national health insurance reimbursement for genetic counseling to be in place to promote the implementation of clinical genetic service including genetic counseling for proper genetic testing.
