앉은 자세에서 일어서기(Sit to stand)과제의 동작시간과 지면반발력 분석 (The Analysis of Movement time and Ground Reaction Force during Sit to Stand Task in Healthy)

  • Purpose : Sit-to-stand (STS) is one of the important activities of daily living (ADL) and each of its parameters is used frequently. This study aimed to examine the influence of different seat height on performance time and floor reaction force strength during an STS movement. Methods : Fifteen young-adult male subjects participated in this study. The subjects were divided into three groups based on lower leg length. Subjects performed an STS movement twice from chairs at height adjusted by the lower leg length of each subject. To examine the influence of the chair seat height, ground reaction forces during a STS performed with 3 chair heights adjusted to each subjects lower leg length were compared. Results : Vertical ground reaction force and time during an STS movement were measured to evaluate. Parameters regarding ground reaction force were selected for analyses. Significant differences were found in ground reaction force at G1 was greater than G2 and G3. Conclusion : The STS movement achievement strategy differed since chair seat height changes relatively by the difference in lower leg length. When conducting the ability to achieve STS movement rating test, chair seat height considering each subject's lower leg length may be needed.

Comparison of Hip Extensor Muscles Activities According to Forward Trunk Lean Angles During Single-leg Deadlift

  • Background: Excessive hamstring (HS) activation due to the weakness of the gluteus maximus (GM) causes pain in the hip joint. A single-leg deadlift is a hip extensor exercise, especially GM, that includes functional movements in daily life and complex multi-joint training. In single-leg deadlift, the muscle activity depends on the forward trunk lean angle, and it's necessary to study which muscles are used dominantly depending on the angle. Objects: The purpose of this study was to compare the effect on the muscle activity of the GM and HS during single-leg deadlift according to different forward trunk lean angles and the ratio of the GM vs HS (GM/HS). Methods: Twenty-one healthy female participants were recruited. The muscles activities of the GM, HS and the GM/HS ratio were measured through electromyography during single-leg deadlift according to three condition of forward trunk lean angles (30°, 60°, and 90°). Results: The GM and HS activities significantly differed among three conditions (p < 0.05). GM/HS ratio was significantly higher at 30° and 60° of forward trunk lean compared to 90°. Moreover, the GM activity was significantly higher at 60° of forward trunk lean than at 30° (p < 0.05). Conclusion: The single-leg deadlift at 60° of forward trunk lean is a proper GM muscle strengthening exercise.

  • 스포츠 활동 중 하퇴부 손상 및 동통은 과도한 훈련활동 및 강도 증가, 잘못된 훈련 방법, 훈련 환경의 적용 잘못으로 발생하는 경우가 많으며 주로 달리기, 축구, 롤러브레이드 등 하지를 많이 사용하는 종목에서 발생된다. 급성 손상으로는 급성 구획 증후군, 근육 손상 및 좌상, 근 경련 등이 있으며 만성 손상으로는 만성 활동성 구획 증후군, shin splints, 피로골절, 아킬레스 건염 등이 발생된다. 이러한 손상이 발생되는 원인과 치료 방법에 대해 알아보고 적절한 훈련 방법, 보조구 장착을 하여 하퇴부 손상을 예방할 수 있는 방법이 중요할 것으로 생각된다.

하지혈류제한과 병행한 계단운동이 무릎 폄근에 미치는 영향 (The Effect of Stair Exercise with Restriction Blood Flow on Knee Extensor Muscle)

  • PURPOSE: Low-intensity exercise with restriction of blood flow has been proposed as an alternative exercise to secure the disadvantage of a high-intensity resistance exercise. However, studies of how affects the muscle using functional exercise are lacking. Thus, the purpose of this study was to investigate knee extensor muscle strength during stair exercise of functional exercise with leg blood flow restriction. METHODS: Twenty two healthy young adults with no history of musculoskeletal or neurogical disorder were participated in this study. participant were randomized into either non-restriction group(11 subject) or restriction group (11 subject). The restriction blood cuff attached to the proximal end of the leg. Measurement of knee extensor strength was used by cybex dynamometer. Data analyzed in independent t-test and paired t-test. RESULTS: Knee extensor muscle strength was significantly different between groups. Also, there were significant differences in the strength of knee extensor within the group. CONCLUSION: This study found that stair exercise with restriction of blood flow did influence to knee extensor muscle strength. These results will also be able to promote the effect of increasing the muscle power applied to functional exercise. Henceforth, studies will be made in the intervention method that can be applied to health vulnerable person.

노인의 균형 및 보행과 족관절 근력과의 상관관계 (Correlations between Muscle Strength of the Ankle and Balance and Walking in the Elderly)

  • Purpose: This study was designed to investigate the correlations between muscle strength of the ankle and balance, walking in the elderly. Methods: Thirty-nine subjects were selected from a population of female volunteers. Measurement of balance ability included evaluation of timed "up and go", functional reach, and a one leg standing test. Measurement of walking analysis included evaluation of cadence, stride length, step length, and walking speed. Maximal voluntary isometric contraction (MVIC) of the ankle muscle strength was measured by use of a dynamometer. Results: For balance, there were significant negative correlations between timed "up and go" and the MVIC of the ankle dorsiflexor. There were significant positive correlations between one leg standing with the eyes closed and the MVIC of the ankle dorsiflexor. For walking, there were significant positive correlations between cadence, walking speed and the MVIC of the ankle dorsiflexor. Conclusion: This study showed that there were close relationships between muscle strength of the ankle dorsiflexor and walking and balance in the elderly.

Immediate Effect of Postural Control of the Contra-Lateral Side on Exercise-Induced Fatigue of the Ipsi-Lateral Plantar Flexor Muscle

  • Purpose: The purpose of the current study was to examine the effects of exercise-induced fatigue of the plantar flexor muscle in the dominant ankle on the plantar flexor strength and postural control function of the contra-lateral side. Methods: Twenty-one young adults (male: 10, female: 11) volunteered to participate in this study. An exercise-induced fatigue protocol to induce fatigue was performed in the plantar flexor of the dominant ankle. For the fatigue protocol, the participants were instructed to raise their heels as high as possible in the position with one leg stance of the dominant lower limb, and the heel was then downed after holding for 1 second. The muscle strength of the contra-lateral plantar flexor was measured using a digital muscle strength test device, and the static and dynamic postural control were tested by acquiring the center of gravity velocity while performing one leg standing. A paired t-test was used to identify the differences between the pre- and post, and the data were analyzed using SPSS 12.0 software. Results: Comparison of the pre- and post-test data revealed a significant difference in the plantar flexor strength and dynamic postural control after exercise-induced muscle fatigue in the dominant side. On the other hand, there was no significant difference in the static postural control. Conclusion: These findings have practical implications, suggesting that unilateral muscle fatigue affects the ankle muscle strength and postural ability of the contralateral side.

발목관절 근력과 관절위치감각, 그리고 균형능력에 미치는 고유수용성 신경근 촉진법 중재와 테이핑 중재 비교연구 (A Comparative Study of the Effects of Proprioceptive Neuromuscular Facilitation and Taping Interventions on Balance Ability, Joint Position Sense, and Ankle Joint Strength)

  • Purpose: The aim of the present study was to investigate the effects of proprioceptive neuromuscular facilitation (PNF) and taping interventions on balance ability, joint position sense, and ankle joint strength. Methods: Thirty subjects who had experienced an ankle sprain within the previous 3 months participated in this study. The subjects were randomly assigned to a PNF group (n=15) or a taping group (n=15). Before and after the interventions, ankle dorsi-flexion and plantar-flexion strength, joint position error, and total center of pressure movements in one leg while in a standing position were measured. Results: Regardless of the group allocation, ankle dorsi-flexion and plantar-flexion strength significantly improved after the interventions (p<0.05). Compared to preintervention measurements, joint position errors were significantly reduced postintervention (p<0.05). The PNF intervention significantly decreased the total lateral movement of the center of pressure in the one leg standing condition (p<0.05). Conclusion: Both PNF and taping interventions improved joint position sense and ankle joint strength. In common with the findings of a previous study, the PNF intervention improved balance ability. Further study is required to investigate the effects of various PNF and taping interventions on ankle performance in subjects with chronic ankle sprains.

Relationships among Lower Extremity Muscle Circumference, Proprioception, ROM, Muscle Strength, and Balance Control Ability in Young Adults

  • Purpose: The purpose of this study was to analyze the correlation between balance control ability and leg circumference, proprioception, range of motion (ROM), and muscle strength in young adults. Methods: The subjects of this study were 30 university students who were enrolled in D university in Gyeongbuk province. We measured the dynamic balance and static balance using the Biorescue. The muscular strengths of the hip, knee, and ankle joints were measured using a muscle contraction dynamometer. The ROM and proprioception were measured using an inclinometer. Pearson correlation analysis was used to test the correlations between balance control ability and variables. Results: Sway length was significantly correlated with knee and hip joint muscle strength, ROM, and proprioception of hip and ankle joints (p<0.05). Sway speed was significantly correlated with ROM and proprioception in hip joints (p<0.05). Limit of stability was significantly correlated with muscle strength and ROM in ankle joints, and proprioception in hip, knee, and ankle joints (p<0.05). Conclusion: The sway length was most related to hip extension and ankle joint plantar flexion in the range of motion and ankle joint plantar flexion in proprioception. Overall, balance training for young adults will be of effective help if the treatment focuses on the knee and hip joints, range of motion and the ankle and hip joints' proprioception.

The Effect of Video Game-Based Home Exercise on Balance and Muscle Strength in the Elderly in the COVID-19 Era

  • Objective: This study investigated the effect of a video game-based home exercise program on the improvement of balance and muscle strength in the elderly. Design: Randomized controlled trial Methods: Participants were randomly assigned to two groups: a video game-based home exercise training group (experimental group, n=16) and a control group (n=17). The experimental group trained three times a week for 50 minutes per session for six weeks, and the control group did not perform any exercise. To evaluate postural balance, one leg standing test, Berg balance scale, functional reach test, and timed up and go test were performed, and five times sit-to-stand was performed to evaluate lower extremity muscle strength. Results: In the experimental group, there were statistically significant improvements (p<0.05) in one leg standing test, Berg balance scale, functional reach test, and timed up and go test, and five times sit-to-stand after the intervention, but in the control group, the difference before and after the intervention was not significant. The difference between the groups according to the experimental method showed a significant increase in the experimental group (p<0.05). Conclusions: These results suggest that a video game-based home exercise program effectively improves balance and muscle strength in the elderly. A video game-based exercise program can contribute to the rehabilitation of the elderly as a method of home remote rehabilitation.

낙상예방 프로그램이 양로원 여성노인의 보행, 균형 및 근력에 미치는 영향 (Effect of the Fall Prevention Program(EPP) on gait, balance and muscle strength in elderly women at a nursing home)

  • Purpose: To determine the effect of the Fall Prevention Program(EPP) on gait, balance and muscle strength in elderly women at a nursing home. Method: The subjects of this consisted of 38 elderly women between the ages of 70 to 89 years living at a nursing home located in Seoul. Each of the experimental group and control group was composed of 19 subjects. The subjects in experimental group have participated in FPP for the 8 weeks which consisted of exercise, education and foot care. They started to exercise for 40 minutes per session, 3 sessions a week during the 1st week at 40% of age adjusted maximum heart rate. From the 2nd week to the 4th week, they increased the duration of exercise to 50 minutes per session and the intensity to 60% of age-adjusted maximum heart rate. They participated in 50 minutes at 60% of age-adjusted maximum heart rate from the 5th week to the 8th week. Each exercise session consisted of 10 minutes of warming-up exercise, 30 minutes of conditioning exercise and 10 minutes of cooling-down exercise. They participated in education for 20 minutes per week from the 1st week to the 4th week. Then they participated in a 30-minute foot care program per week from the 5th week to the 8th week. Gait, balance and muscle strength for each subject were measured before and after FPP. Gait was evaluated by step length, step width, gait speed and walking distance. Balance was measured by the duration of standing on one leg with their eyes closed and open each, and a get-up and go test. Grip strength was measured by hand dynamometer. Hip extensor and flexor strength, knee extensor and flexor strength and ankle plantarflexor and dorsiflexor strength were measured by manual muscle tester. Data was analyzed using SPSS form Windows. t-test and Chi square test were utilized as a homogeneity test. Repeated measure ANOVA was used to test the effect of FPP. Result: 1) Step width significantly decreased, and step length, gait speed and walking distance significantly increased in the experimental group compared with the control group after FPP(p<0.005). 2) There was no significant change in standing time on one leg with their eyes closed after FPP. The standing time on leg with their eyes open and the time of "get-up and go" significantly decreased in the experimental group compared with the control group after FPP(p<0.005). 3) Muscle strength-grip strength, hip extensor and flexor strength-significantly increased in the experimental group compared with the control group after FPP(p<0.005). 4) There was no significant difference of frequency of fall between the experimental group and control group during the period of FPP. Conclusion: These results suggest that FPP can increase gait, balance and muscle strength of elderly women at a nursing home.
