건강유지 프로그램이 시설노인의 신체 건강과 정신 건강에 미치는 효과 (The Effect of a Health Maintenance Program on Physical Function and Mental Health of the Elderly in Nursing Homes)

  • Purpose: The purpose of this study was to determine the effects of a Health Maintenance Program on physical functions and mental health of the elderly in nursing homes. Method: Sixty elderly(over 65 years old) in a randomized control study participated in a 16-week group-based intervention including functional exercises and health education. The participants were divided into 3 groups(Health Maintenance Program Group, Supportive Music Exercise Group, and Control Group) of 20 elderly each. Data was collected from Dec. 1st, 2005 to Mar, 30th, 2006. Physical function of lower body strength was assessed using a 30-second chair test, flexibility was assessed using a sit-and-reach test, and static balance was assessed by the ability to balance on one leg with open and closed eyes. Depression was assessed using the Korean Form of the Geriatric Depression Scale and self esteem was assessed using Rosenberg's Self Esteem Questionnaire. Data was analyzed by Chi-square test, One-way ANOVA, and Repeated measure two factor analysis. Results: A Health Maintenance Program significantly increased muscle strength, flexibility and static balance, but depression and self-esteem scores were not significantly changed. Conclusion: Findings demonstrated that a Health Maintenance Program was more effective on physical function than mental health of the elderly in nursing homes.

노인 건강운동 프로그램 개발 (The Development of Aquatic Health Exercise Program for the Old)

  • Purpose: The purpose of this study was to investigate the effects of aquatic health exercise program for the old. Methods: 20 healthy female with an average age of 71years($71{\pm}4.2$), were participated in this study. The aquatic health exercise program that include warm-up, stretching, strengthening exercise, WATSU and cool-down was performed in the pool two times a week for eight weeks(40min per one session). Body composition, strength of the both knee flexors and extensors, balance ability(sway area and path), whole body reaction time and flexibility(forward reaching test in long sitting) were measured before and after exercise. The data was analyzed with paired t-test to determine significant differences of all suggested factors between pre and post-exercise by make use of the SPSS(ver 10.0) package program. Results: The strength of the both knee flexors and extensors were increased significantly(right knee; p<0.01, left knee; p<0.05). Anteroposterior sway area (p<0.01), and sway path (p<0.001) of both leg were reduced significantly with eyes closed and opened. Whole body reaction time by optical stimulation was increased significantly (p<0.01) but whole body reaction time by auditory stimulation was no significant difference. The flexibility was increased significantly (p<0.01). Conclusion: Aquatic health exercise program can improve muscle strength, balance, whole body reaction, and flexibility.

Numerical analysis of tilted angle shear connectors in steel-concrete composite systems

  • This study investigates numerically the behavior of tilted angle shear connectors embedded in solid concrete slabs. Two different tilted angle connectors were used, titled angle with 112.5 and 135 degrees between the angle leg and steel beam flange. A nonlinear finite element model was developed to simulate and validate the experimental push-out tests. Parametric studies were performed to investigate the variations in concrete strength and connector's dimensions. The results indicate that the ultimate strength of a tilted angle shear connector is directly related to the square root of the concrete compressive strength. The effects of variations in the geometry of tilted angle connectors on the shear capacity are discussed in details. Based on the numerical analyses, two equations are proposed to estimate the ultimate capacity of tilted angle shear connectors of 112.5 and 135 degrees in the defined range of parameters.

The Effects of Vibration Exercise after Modified Bröstrom Operation in Soccer Players with Ankle Instability

  • Background: Vibration exercise after ankle surgery improves proprioception and ankle muscle strength through vibration stimulation. Objective: To examine the effects of vibration exercise on the ankle stability. Design: Randomized controlled clinical trial (single blind) Methods: Twenty soccer players were randomly divided into experimental group and control group. The Vibration exercise program was conducted 12 weeks and 3 times a week. Ankle joint proprioceptive sensory test and Isokinetic muscle strength test were performed using Biodex system pro III to measure plantar flexion / dorsiflexion and eversion / inversion motion. Results: The result of isokinetic test of ankle joint is showed significant improvement in all measurement items, such as leg flexion, lateral flexion, external and internal muscle forces, compared to previous ones by performing vibration movements for 12 weeks. However, in the comparison group, plantar flexor ($30^{\circ}$), eversion muscle ($120^{\circ}$), inversion ($30^{\circ}$) of limb muscle strength were significantly improved compared with the previous phase; was no significant difference in dorsi-flexion. There was no significant difference between groups in all the items. Conclusions: In this study, we analyzed the effects of rehabilitation exercise on soccer players who had reconstructed with an ankle joint ligament injury through vibration exercise device. As a result, we could propose an effective exercise method to improve the ability, and confirmed the applicability as an appropriate exercise program to prevent ankle injuries and help quick return.

Restricted Blood Flow Exercise in Sedentary, Overweight African-American Females May Increase Muscle Strength and Decrease Endothelial Function and Vascular Autoregulation

  • Objectives: Exercise with partially restricted blood flow is a low-load, low-intensity resistance training regimen which may have the potential to increase muscle strength in the obese, elderly and frail who are unable to do high-load training. Restricted blood flow exercise has also been shown to affect blood vessel function variably and can, therefore, contribute to blood vessel dysfunction. This pilot study tests the hypothesis that unilateral resistance training of the leg extensors with partially restricted blood flow increases muscle strength and decreases vascular autoregulation. Methods: The subjects were nine normotensive, overweight, young adult African-Americans with low cardiorespiratory fitness who underwent unilateral training of the quadriceps' femoris muscles with partially restricted blood flow at 30% of the 1-repetition maximum (1-RM) load for 3 weeks. The 1-RM load and post-occlusion blood flow to the lower leg (calf) were measured during reactive hyperemia. Results: The 1-RM load increased in the trained legs from $77{\pm}3$ to $84{\pm}4 kg$ (P < 0.05) in the absence of a significant effect on the 1-RM load in the contralateral untrained legs (P > 0.1). Post-occlusion blood flow decreased significantly in the trained legs from $19{\pm}2$ to $13{\pm}2mL{\cdot}min^{-1}{\cdot}dL^{-1}$ (P < 0.05) and marginally in the contralateral untrained legs from $18{\pm}2$ to $16{\pm}1mL{\cdot}min^{-1}{\cdot}dL^{-1}$ (P = 0.09). Changes in post-occlusion blood flow to the skin overlying the trained and the contralateral untrained muscles were not significant. Conclusion: These results demonstrate that restricted blood flow exercise, which results in significant gains in muscle strength, may produce decrements in endothelial dysfunction and vascular autoregulation. Future studies should determine whether pharmacopuncture plays a role in treatments for such blood vessel dysfunction.

시설거주 노인여성을 위한 근력강화 훈련이 요실금과 신체기능에 미치는 효과 (Effect of Muscle Strength Training on Urinary Incontinence and Physical Function: A Randomized Controlled Trial in Long-term Care Facilities)

  • Purpose: This study was done to determine whether muscle strength training programs have an impact on improving symptoms of urinary incontinence (UI) and physical function among elderly women with UI who reside in long-term care facilities. Methods: A randomized controlled trial was conducted. Participants had to be over 65 years, score over 15 score on the mini-mental state examination, and be able to walk alone or with an assistant. Seventy residents were randomly allocated to either the training group (n=35) or control group (n=35). The program consisted of 50 minutes, twice a week for 8 weeks, and included Kegel's exercise, Thera-band training and indoor walking. Main outcomes were UI symptoms, peak vaginal pressure and physical functions measured with timed up and go test (TUG), one leg standing test (OLST), activities of daily living (ADL) and grip strength. Changes in outcome measurements were calculated from baseline to 4 weeks and to 8 weeks using repeated measures ANOVA. Results: There were significant differences in peak vaginal pressure (p<.001), TUG (p<.001), OLST (p=.012) and grip strength (p<.001) in the interaction between groups and time. Conclusion: Future studies are suggested to confirm the effect of muscle strength training in long-term care facilities where elderly women with UI reside.

앞십자인대 재건술 후 엉덩관절 벌림근 강화운동이 근력과 보행에 미치는 영향 (The Effects of Strengthening Exercise of Hip Abductors on Muscle Strength and Ambulation in Patient with ACL Reconstruction)

  • Purpose: The purpose of this study was to investigate the effect of strengthening exercises of hip abductors on muscle strength and ambulation for patients with ACL reconstruction. Methods: The subjects were randomly assigned to the intervention group (general exercise plus strengthening of hip abductors) or the control group (general exercise without the strengthening of hip abductors). Both groups participated in a six-week exercise protocol after the surgery for ACL reconstruction. The knee strength test (quadriceps, hamstring, hip abductor) and gait analysis were performed in pretest and post-test. Results: Muscle strength was measured using the Biodex system III model (Biodex Medical System, Inc, NY, US). The results showed no significant difference in knee strength (quadriceps, hamstring) between the two groups (p>0.05), however, there was a significant difference in hip abductors (p<0.05). The gait analysis was measured with Gaitrite system (CIR System Inc, US). Results of comparison between groups showed a significant increase in the step length of both groups. (p<0.05), and there was a significant difference in the affected leg of the intervention group(p<0.05). Conclusion: Exercise of hip abductors for patients with ACL reconstruction provides significant benefits with respect to improvement of muscle strength in hip abductors, increasing the stride. This result indicates that a training program designed for ACL reconstruction patients should include strengthening of the hip abductor. We think that further study is needed to determine the relationship of hip muscles and knee joint for patients with ACL reconstruction.

Effect of aging on tear strength and cytotoxicity of soft denture lining materials; in vitro

  • PURPOSE. The aim of this in vitro study was to evaluate the effect of aging on the tear strength and cytotoxicity of four soft denture lining materials. MATERIALS AND METHODS. Four commonly used soft denture lining materials, (Coe-Comfort$^{TM}$ GC America Inc., Alsip, IL, USA; Coe-SOFT$^{TM}$ GC America Inc., Alsip, IL, USA; Visco-gel Dentsply Caulk Milford, DE, USA; and Sofreliner Tough M Tokuyama Dental Corporation Tokyo, Japan) were selected. Sixty trouser-leg designed specimens per lining material were fabricated using a stainless steel mold for tear strength testing. The specimens were divided into non-thermocycling and 1000-, and 3000-thermocycling groups. For the cytotoxicity test, twenty-four disk shaped specimens per material were fabricated using a stainless steel mold. The specimens were soaked in normal saline solution for 24 h, 48 h and 72 h. Cytotoxicity was measured by XTT assay in L929 mouse fibroblasts. Data were analyzed by two way analysis of variance and Dunnett's test (P<.05). RESULTS. Before thermocycling, Sofreliner Tough M ($10.36{\pm}1.00N$) had the highest tear strength value while Coe-Comfort$^{TM}$ ($0.46{\pm}0.10N$) had the lowest. After 3000 cycles, Sofreliner Tough M ($9.65{\pm}1.66N$) presented the highest value and Coe-Comfort$^{TM}$ ($0.42{\pm}0.08N$) the lowest. Sofreliner Tough M, in all incubation periods was the least toxic with significant differences compared to all other materials (P<.05). Coe-Comfort$^{TM}$, Coe-$SOFT^{TM}$, and Sofreliner Tough M did not show any significant differences within their material group for all incubation periods. CONCLUSION. This in vitro study revealed that aging can affect both the tear strength and cytotoxicity of soft denture materials depending on the composition.

근력강화운동 프로그램이 만성 뇌졸중 노인의 근력, 일상생활동작, 건강지각 및 우울에 미치는 영향 (Effects of a Muscle Strengthening Exercise Program on Muscle Strength, Activities of Daily Living, Health Perception, and Depression in Post-stroke Elders)

  • Purpose: The purpose of this study was to identify the effects of an exercise program on muscle strength, Activities of Daily Living (ADL), health perception, and depression among post-stroke elders. Methods: Nonequivalent control group design with pretest and posttest was used. A total of 23 post-stroke elders who met inclusion criteria were recruited. Muscle strength in upper and lower extremities, ADL, health perception, and self-rated symptoms of depression were measured. The exercise program consisted of deep breathing, range of motion, and muscle strengthening exercises using Thera-band, 30~40 min/day, three days a week, for 12 weeks. Results: Muscle strength of right shoulder (U=11.50, p=.001), right leg (U=13.50, p=.002), and health perception (U=24.00, p=.010) in the experimental group was significantly higher compared to the control group at 12 weeks of post-intervention. Depression scores were significantly lower in the experimental group compared to the control group at 6 weeks (U=28.00, p=.021), at 12 weeks (U=22.00, p=.006) of an exercise program. Conclusion: By applying this program, post-stroke elders showed increases in muscle strength and a decrease in depression as well as improvement of health perception. Further study is needed to verify the effects of the exercise program on cardiovascular physiologic variables through long-term follow-up.

그룹 재활운동 및 체육 프로그램이 지역사회 뇌졸중 장애인의 보행기능, 균형지수 및 무릎관절 근력에 미치는 영향 (The Effects of Group Rehabilitation Exercise and Physical Education Program on Local Community Stroke Patient's Walking Function, Balance Index and Muscle Strength in the Knee Joint)

  • Purpose : This study aims to present a rehabilitation exercise and physical education program (REPEP) for local community stroke patients by examining an exercise program provided from traditional physical therapeutic perspectives. Methods : The subjects were 40 stroke patients residing in a local community and managed by a public health center (the group-exercise group: 20, the individual-exercise group: 20). In the case of the group-exercise group (GEG), the subjects were divided into small groups composed of five members each to participate in the program. The individual-exercise group (IEG) took part in the program individually. The subjects received a REPEP that included a warm-up exercise, an elastic band exercise, a leg exercise, a balance and gait exercise, an arm and trunk exercise, and a cool-down exercise twice per week for 20 weeks. They had their gait function, balance index, and muscle strength in the bilateral knee joints tested before and after the experiment. Results : Compared to before the experiment, both groups' gait function, balance index, and muscle strength in the knee joints increased after the experiment. After the experiment, the GEG experienced more improvement in their gait function, balance index, and muscle strength in the bilateral knee joints, excluding their gait velocity and $300^{\circ}/sec$ flexion and extension, than the IEG (p<.05). Conclusion : An exercise program provided from the traditional physical therapeutic perspectives may be applied to stroke patients as their REPEP. In addition, a REPEP was more effective in improving their gait function, balance index, and muscle strength in the knee joints when the subjects formed a group and took part in the program than when they partook in it individually.
