A Case Report of Treating Developmental Abnormality Child by Cerebral Palsy with Korean Medical Treatment (뇌성마비로 인한 발육이상 아동 한방치료 1례)

  • Objectives : The purpose of this study is to report one case of Korean medical treatment of patient with developmental abnormality by cerebral palsy whose main symptoms are difference in left and right leg length, foot size and construction of right ankle. Methods : We did retrospective study for one developmental abnormality patient who had been treated by herbal medicine(Samul-tang, Yookmijihwang-tang hap Yijin-tang + Sungjang-hwan), acupuncture, and moxibustion for more than one year. The effects of treatment were evaluated by measurement of leg length, foot size and dorsiflexion ROM. Results : After treatment, length and foot size difference between both leg, ROM difference between both ankle were almost reduced. According to the ratio of the right to the left, leg length was improved from 0.91 to 0.98 and foot size was improved from 0.93 to 0.98. The difference in left and right ankle dorsiflexion ROM was improved from 55° to 27.5°. Conclusions : This clinical case study has demonstrated that Korean medicine may be an safe and effective option for treating developmental abnormality.

Effects of Early Feed Restriction on the Occurrence of Compensatory Growth, Feed Conversion Efficiency, Leg Abnormality and Mortality in Unsexed Broiler Chickens Reared in Cages

  • Two experiments were conducted to evaluate the effect of early feed restriction on growth, feed conversion ratio (FCR) and mortality in unsexed broiler chickens. In Experiment 1, 350 one-day-old broiler chickens were divided into 7 groups. Each treatment group was represented by five replicates of ten broilers each. One group was fed ad libitum as the control group and the other six groups were fed 25% ad libitum (25% multiplied by amount of feed intake of ad libitum chickens at the previous day) for 4 or 6 days, 50% ad libitum for 4 or 6 days, and 75% ad libitum for 4 or 6 days. In experiment 2, 500 broiler chickens were divided into 10 groups. Each treatment group was represented by five replicates of ten broilers each. One group was fed ad libitum as the control group. Three ages of feed restriction initial timing (2, 4 or 6 days of age) and three types of feed restriction (physical restriction, meal feeding and diet dilution) were used ($3{\times}3$) in both experiments. They were feed-restricted for 6 days. Results showed that restricted broilers exhibited compensatory growth in both experiments except for diet dilution groups. In both experiments, FCR of restricted broilers was higher, whereas feed intake of them was lower during restriction period. FCR of restricted broilers was lower upon refeeding. Mortality was inconsistently affected by early feed restriction. Leg abnormality was lower in restricted broilers in both experiments. Level of feed restriction significantly influenced body weight, FCR and feed intake of restricted broilers (p<0.05), but duration of feed restriction had no effect. Type of restriction significantly affected body weight, feed intake and FCR (p<0.05). It appeared that to achieve the best result (complete compensatory growth and better FCR), broilers should be restricted at 25% ad libitum for 6 days (Experiment 1). Meal feeding started at 2 days of age would show the best performance (Experiment 2).

  • Ununited Anconeal Process (UAP) is defined as a disease of young dogs due to developmental abnormality in which there is a seperation of the anconeal process from the diaphysis of the ulna, leading to a front leg lameness and secondary osteoarthritis. A six -year-old German Shepherd dog was presented because of intermittent right front leg lameness. Diagnosis was made of ununited anconeal process with moderate osteoarthritis by radiographic examination. Surgical removal of ununited anconeal process and curettage of osteoarthritic growth within the elbow joint relieved significant clinical signs despite of size and age of the dog.

  • Objectives : This study is performed for the purpose of proving the efficacy of the Oriental medicine of swollen leg. Edema is defined as an increase in the extravascular(interstitial) component of the extracellular fluid volume, which may expand by several liters before the abnormality is recognized. We treated one case of patient with a swollen leg. Methods & results : We examined DITI before treatment and after treatment. We measured the circumference of thigh and calf every morning, so we compared with the values day after day. So we found the fact that leg swelling considerably decreased after treatment for 14 days. We followed up after discharge. Conclusions : Acupuncture, Aquacupuncture and Herbal medicine is possible to be effective for treating patients with a swollen leg.

Effects of Antibiotics(Avoparcin, Nosiheptide, Enramycin) as Supplementary Growth Promoters on the Performance of Broiler Chickens (성장촉진용 항생제(Avoparcin, Nosiheptide, Enramycin)가 육계의 생산성에 미치는 영향)

  • In order to study performance enhancing effects of supplementary antibiotics (avoparcin, nosiheptide and enramycin), two feeding trials were conducted. In Experiment 1, 1, 040 male Arbor Acres were reared in floor pens for 6 wk. Chicks were assigned to one of the following four treatments: Basal diet(B), B+avoparcin 10 ppm, B+nosiheptide 2.5 ppm and B+enramycin 5 ppm. Each treatment had five replications of 52 chicks each. In Experiment 2, two antibiotics (avoparcin and enramycin) were compared in $2{\times}2$ (antibiotics$\times$sex) factorial design. One thousand broiler chicks were reared in floor pens for 6 wk. Each of the 4 treatments had five replications of 50 chicks each. The results of Experiment 1 showed that antibiotic treatments (enramycin, avoparcin and nosiheptide) significantly(P <0.05) improved weight gain. Feed/gain ratio of avoparcin treatment and enramycin treatment tended to he lower than the control but they were not statistically significant. Dressing percentages were high in avoparcin and enramycin treatments compared to the control and nosiheptide treatment. The number of E. coli and Cl. perfringens in ileum and cecal contents were decreased by antibiotic treatments. Moisture contents in excreta of the birds were not significantly affected by the treatments. Leg abnormality and mortality were not significaniy different among treatments. In Experiment 2, weight gain was significantly (P<0.01) different between sexes, but not between antibiotic treatments. Significant effects of antibiotics (P<0.01), sex (P<0.01) and interaction (P<0.05) were shown in feed in take. Feed/gain ratio of avoparcin treatment was significantly (P<0.01) lower than that of enramycin treatment. leg abnormality and mortality were not significantly different among treatments but those in male broiler tended to be higher than in female broilers. It was concluded that nonsystemic antibiotics supplemented to the broiler diets suppress undesirable microorganisms and improve broiler performace in general and avoparcin was most effective in improving feed/gain ratio.

Angiographically Occult Vascular Malformation of the Cauda Equina Presenting Massive Spinal Subdural and Subarachnoid Hematoma

  • We report a case of a non traumatic spinal subdural hematoma or subarachnoid hematoma manifesting as lumbago, leg pain and bladder dysfunction that showed angiographically occult vascular malformation (AOVM). Although the spinal angiogram did not reveal any vascular abnormality, the follow-up magnetic resonance image showed AOVM. Complete surgical removal was performed due to the aggravated bladder dysfunction. This case highlights the need to consider bleeding due to spinal AOVM, even when angiography is negative.

Morphologic Changes of L5 Root at Coronal Source Images of MR Myelography in Cases of Foraminal or Extraforaminal Compression

  • Objective: Two findings easily found at coronal source images of MR myelography (MRM) were evaluated : dorsal root ganglion (DRG) swelling and running course abnormality (RCA) of L5 exiting root at foramen or extraforamen. We tried to find the sensitivity of each finding when root was compressed. Methods: From 2004 July to 2006, one hundred and ten patients underwent one side paraspinal decompression for their L5 root foraminal or extraforaminal compression at L5-S1 level. All kinds of conservative treatments failed to improve leg symptom for several months. Before surgery, MRI, CT and MRM were done. Retrospective radiologic analysis for their preoperative MRM coronal source images was done to specify root compression sites and L5 root morphologic changes. Results: DRG swelling was found in 66 (60%) of 110 patients. DRG swelling has statistically valuable meaning in foraminal root compression (chi-square test, p<0.0001). Seventy-two (66%) in 110 patients showed abnormal alteration of running course. Abnormal running course has statistically valuable meaning in foraminal or extraforaminal root compression (chi-square test, p<0.0001). Conclusion: Three-dimensional MRM provides precise thin sliced coronal images which are most close to real operative views. DRG swelling and running course abnormality of L5 exiting root are two useful findings in diagnosing L5 root compression at L5-S1 foramen or extraforamen. MRM is thought to provide additional diagnostic accuracy expecially in L5-S1 foraminal and extraforaminal area.

  • The amniotic band syndrome is an uncommon congenital fetal abnormality, presumably due to fetal entanglement in strands of ruptured amniotic sac. The defects caused by this syndrome vary from simple limb defects to major visceral and craniofacial defects. We cared for a newborn infant with this syndrome, who showed constriction rings of the right leg and right axilla, right club foot, thoracic scoliosis, polydactyly, absence of the right thumb and aplasia cutis of the scalp. We report this case with a brief review of the literature.

Relative Bio-Availability of Different Phosphorus Supplements in Broiler and Layer Chicken Diets

  • Two experiments on commercial broiler chickens (3-37 d) and WL layers (252-364 d) were conducted to study the relative bioavailability of phosphorus (P) from different P supplements in comparison to tricalcium phosphate (TCP), at constant dietary calcium (Ca):P ratio. The P sources tested were dicalcium phosphate (DCP), steam sterilized bone meal (SSBM), commercial mineral mixture (CMM), phosphoric acid (PA) and a combination of CMM + PA. Fluorine (F) content in CMM and SSBM was 13.12 and 0.14 g/kg, respectively. In commercial broiler diets, DCP, SSBM or PA could be used as supplemental P sources without affecting (p<0.05) weight gain, feed intake, tibia ash and, P and Ca contents in tibia ash when compared to TCP. Severity of leg abnormality and deposition of F in bone were higher (p<0.05) in group fed CMM. P retention and serum inorganic P content was significantly reduced (p<0.05) in CMM fed birds compared to those fed TCP, SSBM, PA or CMM+PA. Among other P sources (TCP, SSBM, PA and CMM+PA) the serum inorganic P levels did not vary significantly (p<0.05). The P retention also significantly reduced in CMM fed groups compared to those fed DCP or PA. The P retention significantly increased from 0.183 to 0.216 units by supplementation of PA to CMM diet. In layers, egg production was not affected by replacing TCP with DCP, SSBM, PA or CMM+PA, but significantly (p<0.05) reduced with CMM. Feed (kg)/kg egg mass, egg weight, shell quality (shell weight and shell thickness) and serum Ca levels were not influenced by dietary variation in P source. The poor performance of both broilers and layers fed on CMM based diets could be attributed to the presence of hlgher levels of F (647.8 and 630.1 mg/kg, respectively) and low P utilization. Based on growth, bone mineralization and P retention it is concluded that DCP, SSBM or PA can be used as alternatives to TCP in broiler diets. In WL layer diets, in addition to above P sources, CMM can also be used as supplemental P source by replacing one half of P from CMM with PA without affecting egg production and shell quality.

Effects of a Water Exercise on the Lower Extremities Coordination during Obstacle Gait in the Female Elderly - Focusing on Training and Detraining Effects - (수중운동이 여성노인 장애물보행 시 하지 협응에 미치는 영향 - 훈련 및 훈련잔여효과 중심으로 -)

  • The purpose of this study was to investigate the training and detraining effects of a 8-week water exercise on lower extremities coordination during obstacle gait in the female elderly. Eight elderly participants (age: $76.58{\pm}4.97$ yrs, height: $148.88{\pm}7.19$ cm, body mass: $56.62{\pm}6.82$ kg, and leg length: $82.36{\pm}2.98$ cm), who stayed at the Seoul K welfare center, were recruited for this study. All participants had no history of orthopedic abnormality within the past 1 year and completed the aquatic exercise program which lasted for 8 weeks. To identify the training and detraining effect of 8 weeks of water exercise, a 3-D motion analysis with 7 infrared cameras and one force plate sampling frequency set at 100 Hz and 1,000 Hz, respectively, was performed. A two-way ANOVA was performed to find training and detraining effects among diferent obstacle heights. In this study significant level was set at .05. Significant training effects of LTS (lead foot thigh and shank) coordination in all obstacle height were found (p<.05). It is also found that the training effect of LTS remained 37%, 58%, and 25% in obstacle height of 30%, 40%, and 50%, respectively. Lead foot showed the greater detraining effect of coordination compared with trail foot, and SF (shank and foot) coordination revealed better detraining effects of coordination compare with TS (thigh and shank) in both feet. Based on the findings, a 8 week water exercise give an positive effects to the elderly in terms of segment cooperation which potentially helps reducing their accident falls. The magnitude of detraining may also help the elderly to find the retraining moment.
