Anger and Alexithymia in Women with Premenstrual Syndrome (월경전기 증후군 여성들에서 분노 및 감정표현불능증)

  • Objectives : Objectives of this study was to explore the relationships between anger, alexithymia and premenstrual syndrome(PMS). Methods The subjects included 125 women with PMS and 58 women with non-PMS assessed by Daily record of Severity of Problems(DRSP) and Shortened Premestrual Assessment Form(SPAE). Anger level was assessed by anger expression scale. The level of alexithymia was assessed by Toronto alexithymia scale. We compared the scores of anger, alexithymia between PMS group and no-PMS group. Results : The mean scores of anger in(p=0.04), anger total(p=0.01) were significantly higher in women with PMS compared to control subjects. and the mean scores of alexithymia(p=0.04) were also significantly higher in women with PMS compared to control subjects. Conclusions : This study showed that the level of anger and alexithymia could be higher in women with PMS compared to control subjects, and that symptoms of PMS could be associated with anger and alexithymia. therefore, It would be better to consider the level of anger and alexithymia in treating and evaluating women with PMS.

Effects of Yoohyangheukho-dan ($R{\check{u}}xi{\bar{a}}ngh{\bar{e}}ih{\check{u}}-d{\bar{a}}n$) on Osteoporosis Induced by Ovariectomy in Rats (유향흑호단(乳香黑虎丹)이 난소적출로 유발된 흰쥐의 골다공증 모델에 미치는 영향)

  • Objectives Osteoporosis is an impending problem in the rapidly aging society. The purpose of this study is to identify the effects of Yoohyangheukho-dan ($R{\check{u}}xi{\bar{a}}ngh{\bar{e}}ih{\check{u}}-d{\bar{a}}n$) on osteoporosis induced by ovariectomy in rats. Methods 24 rats were randomly assigned to a SHAM group, a control group, and a Yoohyangheukho-dan ($R{\check{u}}xi{\bar{a}}ngh{\bar{e}}ih{\check{u}}-d{\bar{a}}n$) group (n=8). Ovaries were extracted and for 8 weeks, the rats were given dry feeds and Yoohyangheukho-dan ($R{\check{u}}xi{\bar{a}}ngh{\bar{e}}ih{\check{u}}-d{\bar{a}}n$) group were given a mixture of dry feeds and Yoohyangheukho-dan ($R{\check{u}}xi{\bar{a}}ngh{\bar{e}}ih{\check{u}}-d{\bar{a}}n$). At 8 weeks, their body weight, femur mass, tibia ash per body ratio, overall surface area and thickness of the trabeculae, overall surface area of the osteoblasts, and the number of osteoclasts were measured and levels of albumin, aspartate transaminase (AST), alanine transaminase (ALT), alkaline phosphatase (ALP), total-cholesterol, calcium and estradiol were recorded. Results There was no significant difference in the weight of the femur and the tibia ash per body ratio. The Yoohyangheukho-dan ($R{\check{u}}xi{\bar{a}}ngh{\bar{e}}ih{\check{u}}-d{\bar{a}}n$) group had significantly thicker trabeculae than the control group and showed a minimal increase of overall surface area of the trabeculae. The overall surface area of the osteoblasts and the number of osteoclasts decreased in the Yoohyangheukho-dan ($R{\check{u}}xi{\bar{a}}ngh{\bar{e}}ih{\check{u}}-d{\bar{a}}n$) group. There were no statistically significant differences in AST, ALT, ALP, total cholesterol, phosphorus, and estradiol levels. On the contrary the Yoohyangheukho-dan ($R{\check{u}}xi{\bar{a}}ngh{\bar{e}}ih{\check{u}}-d{\bar{a}}n$) group had significantly higher levels of Albumin, triglyceride, and calcium. Conclusions It can be concluded that oral intake of Yoohyangheukho-dan ($R{\check{u}}xi{\bar{a}}ngh{\bar{e}}ih{\check{u}}-d{\bar{a}}n$) prevents the thinning process of the trabeculae. Thus, it may have positive effects on the treatment and prevention of osteoporosis in postmenopausal women.

Examination about Utility of Prone Position in PET/CT of Stomach Cancer Patient (위암 환자의 양전자 방출 컴퓨터 단층 검사에서 복와위(伏臥位) 촬영의 유용성에 대한 연구)

  • Purpose: Currently, PET/CT scan has been known to provide useful information to both preoperative and postoperative examination of cancer patients. Contracted stomach by the long fasting could cause difficulties of interpretation because of its size on reconstructed image data. To solve this problem, after the whole body PET/CT scan, patients were administrated in drinking 300 mL of water to expand stomach and performed additional scan on stomach region. Not only PET/CT scan but also CT performs this water-administration, and patients were take oral solution to make stomach expand for stomach cancer. When this scan performed, patients lay supine position. In this study, we evaluated the capacity of stomach through PET/CT scan with drinking water performed in supine and prone position so that we can distinguish exact location of cancer around pylorus and inferior wall of stomach. Furthermore, image data from supine and prone positions were analyzed the difference of volume of stomach through the change of standardized uptake values. Materials and Methods: From July 2009 to January 2010 in severance hospital, 30 patients who were diagnosed as early gastric cancer or advanced gastric cancer were chosen. All patients had PET/CT scan before the operation and have had follow-up PET/CT. The patients fast for at least 8 hours, and had an injection intravenously with $^{18}F$-FDG, 7.4 MBq (0.2 mCi/kg) per kilogram. They were rested for 60 minutes. Before the examination, all patients were administrated to drink water for 300 mL Patients had PET/CT scan with supine position around the region of stomach, whole body, and around the region of stomach with prone position after drinking another 300 mL of water respectively. Results: As a results of comparison between stomach capacity of 30 patients in supine and prone position, the study draw results that average capacity of stomach body was 460.29 $mm^2$ in supine position, and 641.39 $mm^2$ in prone position for 30 patients. The change of capacity shows 41.3% expanded in prone position. And there was no noticeable difference at maximum standardized uptake values in supine position and prone position. Conclusion: As results, stomach would have more expanded capacity in prone position than supine position. For patients who have physical disabilities to move freely, additional scan in prone position will be obstacle to perform. However, if additional scan in supine position add with the scan in prone position, it will be easier to diagnose stomach cancer. Moreover, we believe that this study will help the research for inventing support tools for patients who have physical disabilities in prone position.

  • Objective : To determine the clinical and bacteriologic efficacy and safety of Cefprozil in acute pharyngitis and pharyngotonsilitis caused by Group A beta hemolytic streptococci in pediatric patients. Methods : Any patient of 3 to 14 age who visited the hospitals enrolled in this study with the signs and symptoms of pharyngitis or pharyngotonsilitis since July, 2000 to March, 2001, was taken throat culture and given Cefprozil(15 mg/kg/day, in two divided doses) for 10 days. 138 patients of whom showed positive culture results were followed up for the signs and symptoms during the treatment to determine clinical efficacy. Any undesirable effect was reported to determine the safety of the drug. Follow up cultures were done at the end of the study and bacteriologic efficacy was determined. Results : 138 of 256 patients who visited the hospitals with the signs and symptoms of pharyngitis or pharyngotonsilitis showed positive growth on throat culture. Mean age of the patients was $6.1{\pm}2.5$ and males and females were equally numbered. 129 of them complained fever on the first visit and 112(86.6%) of them were improved at the end of the study. Cervical lymphadenitis was seen in 58 patients and 44(75.9%) of them improved at the end of the study. Exudative pharyngitis was seen in 96 patients and 81(84.3%) of them improved. The overall clinical effcacy based on this results showed that 110(79.7%) of the patients were cured and 17(12.3%) of them improved. On the cultures and bacteriologic efficacy, 24.6% of them showed documented eradication after treatment and 62.3% of them showed presumptive eradication. Sensitivity test was done by agar dilution method and Cefprozil showed 100% sensitivity. Erythromycin, Clarithromycin and azithromycin showed 87%, 85.6 %, 90.6% sensitivity, respectively. Conclusion : Cefprozil is proved to be effective in controlling group A streptococcal pharyngitis and pharyngotonsilitis in children and showed good sensitivity. Cefprozil can be used as an effective oral cephalosporin in the patients showing penicillin hypersensitivity or patients who other drugs have failed.

Age Related Prevalence of Antibodies to Hepatitis A Virus, Performed in Korea in 2005 (국내에서 2005년에 실시한 연령별 A형 간염 바이러스 항체 보유율)

  • Purpose : Hepatitis A viral infections have been continued after re-emerging since mid 1990s in Korea. The incidence of this disease has been increased in young adults younger than 30 years of age since 2000. This study was performed to evaluate the prevalence of antibody to hepatitis A in Korea(two regions; Incheon and Changwon) in 2005, and was compared with the results of similar studies in mid 1990s. Methods : The study was conducted from January 2005 to June 2005, and consisted of 1,301 enrolled subjects, neonates to 50 years old, living in Incheon and Changwon in Korea. All sera were frozen and stored at $-70^{\circ}C$ until assayed. Anti-HAV IgG antibodies were measured by microparticle enzyme immunoassay(HAVAB, Abbott Lab., IL, USA). Results : The prevalence of anti-HAV IgG was 61.1% in infants younger than 1 year old, 30.5% in 1~5 years, 14.6% in 6~10 years, 1.7% in 11~15 years, 6.5% in 16~20 years, 36.6%in 21~30 years, 77.5% in 31~40 years, and 99.8% in 41~50 years. Statistical differences were not found between male and female, but there was statistical difference in 6~10 years old age group between the two areas. Conclusion : Our study indicate that the prevalence of antihepatitis A virus antibody has shifted from children to old adolescents and young adults. This result suggests that the risk of sudden outbreaks or increasing incidence of hepatitis A viral infections in young adults may be expected in our society. The preventive strategies of hepatitis A including vaccination should be prepared.

Penicillin Resistant Distribution and in-vitro Susceptibility of Oral Antibiotics against Streptococcus pneumoniae, isolated from Pediatric Patients with Community-Acquired Respiratory Infections in Korea (급성 호흡기감염 환아에서 분리된 폐구균에 대한 페니실린 내성분포와 경구 항생제에 대한 감수성 연구)

  • Purpose : S. pneumoniae is one of major pathogens of community-acquired respiratory infections. The rate of antibiotic resistance to this organism has increased, and resistance to multiple antimicrobial agents in a single strain of S. pneumoniae may compromise the efficacy of empiric antimicrobial treatment commonly used for respiratory infections. We did this study to find out the penicillin resistant distributions and oral antibiotics susceptibility patterns against S. pneumoniae, isolated from pediatric patients with community-acquired respiratory infections in Korea. Methods : One hundred fifty six pneumococcal isolates obtained from pediatric patients with community-acquired respiratory infections such as acute otitis media(AOM), sinusitis and pneumonia between May 2000 to June 2003. And MICs of penicillin and oral antibiotics(amoxicillin, amoxicillin-clavulanate, cefaclor) were performed by broth microdilution methods according to the NCCLS(2003a). Results : Seventy eight percent of the isolates were resistant to penicillin. The isolates, collected from AOM patients showed the highest penicillin resistance(92.7%). The resistant rates of amoxicillin (16.7%) and amoxicillin-clavulanate(9.6%), based on susceptibility breakpoints established by the NCCLS, were markedly lower than these of penicillin. But, the resistant rate of cefaclor was very high, above 95%. Conclusion : We concluded that pneumococci isolated from study cases may be one of the world's highest penicillin resistant rates. But, amoxicillin and amoxicillin-clavulanate can be used as a first-line antibiotics. Finally, we hope that a continuous surveillance study to monitor resistant patterns of pneumococcal respiratory infections will be needed for the standard guidelines of empiric antibiotic treatment.

Cross-calibration of Bone Mineral Density between Two Different Dual X-ray Absorptiometry Systems: Hologic QDR 4500-A and Lunar EXPERT-XL (서로 다른 이중에너지 방사선흡수기계 기종(Hologic QDR 4500-A와 Lunar EXPERT-XL) 간의 골밀도 교차 보정)

  • Purpose: A cross-calibration equation is needed to compare bone mineral density measured by different dual X-ray absoptiometry systems. We performed this study to establish appropriate cross-calibration equations between two different dual X-ray absorptiometry systems. Materials and Methods: Bone mineral density of anterior-posterior lumbar spine (L2-4 level) and femoral neck were measured in 109 women ($55{\pm}11yr$) using two different dual X-ray absorptiometry systems (Lunar EXPERT-XL and Hologic QDR 4500-A). Bone mineral density values measured by two systems, including area, bone mass content, bone mineral density and percentile of young normals were compared and cross-calibration equations between two systems derived. Results: The bone mineral density values of 109 women measured by Lunar system were $0.958{\pm}0.17g/cm^2$ at L2-4 and $0.768{\pm}0.131g/cm^2$ at femur neck, which were significantly higher ($13{\pm}6%$ at L2-4 and $19{\pm}7%$ at femur neck, p<0.001) than those ($0.851{\pm}0.144 g/cm^2$ at L2-4 and $0.649{\pm}0.108 g/cm^2$ at femur neck) by Hologic system. Bone mineral content and percentile of young normals measured by Lunar system were also significantly higher than those by Hologic system (p<0.001), whereas there was no difference in area (p>0.05). There was a high correlation between bone mineral density values of L2-4 and femoral neck obtained with both dual X-ray absortiometry systems (r=0.96 and 0.95, respectively). Cross-calibration equations relating the bone mineral density were Lunar= 1.1287${\times}$Hologic -0.0027 for L2-4 and Lunar= 1.1556${\times}$Hologic+0.0182 for femoral neck. Conclusion: We obtained cross-calibration equations of bone mineral density between Lunar EXPERT-XL and Hologic QDR 4500-A. These equations can be useful in comparing bone mineral density obtained by different dual X-ray absorptiometry systems.

  • This study analyzed the curriculum and clinical practicum of the department of clinical laboratory science in Korea. The 2017 educational curricula of all 3-year and 4-year universities were analyzed. Clinical practice was conducted by a questionnaire. As a result of curriculum analysis, the 3-year curriculum was able to grasp the curriculum that focused on the national examinations of medical technologists, and the 4-year curriculum was open to a variety of subjects reflecting the changes in the future, but only at a few universities. In addition, the autonomous enrollment application made it possible to take a national examination without enrolling in courses that students find difficult in major courses. In the case of clinical practice, it was difficult to standardize in various practical institutes, practice periods, and practice credits. Therefore, it will be necessary to standardize the composition of the credits according to the duration of clinical practice and the duration of education. Moreover, is necessary to revise the curriculum in consideration of medical technologist job analysis and clinical field, and it will be necessary to standardize the curriculum and clinical practice model through the Medical Technologist Evaluation Center.

Evaluation of Family Adaptability and Cohesion as Risk Factor of Postpartum Depression : Preliminary Study (산후우울증에서 위험인자로서의 가족응집성-적응력 평가 : 예비적 연구)

  • Objectives : Prevalence of postpartum depressive disorders reaches approximately 10-15% of childbearing women. This preliminary study was intended to explore the relationships between postpartum depressive disorders and family adaptability and cohesion. Methods : Childbearing women(n=24) were assessed for risk factors for postpartum depression before and after childbirth prospectively. At gestational age between 36th and 40th week, the questionnaire about various factors before childbirth, Edinburgh postnatal depression scale(EPDS), Family adaptability-cohesion evaluation scale (FACES), and Beck anxiety inventory(BAI) were assessed. After childbirth, for the diagnosis of postpartum depressive disorders, a structured diagnostic interview according to MINI International Neuropsychiatric Interview was performed, furthermore, the questionnaire about various factors after childbirth, EPDS, and BAI were assessed at 4-6 weeks postpartum. Results : Among various factors and scales, family adaptability and cohesion, mood symptom and change during pregnancy were associated with postpartum depressive disorder in comparison with non-postpartum depressive disorder subjects. In addition, low family adaptability and cohesion and mood change during pregnancy were significantly associated factors with postpartum depressive disorder using logistic regression analysis. Conclusion : The results suggest the postpartum depressive disorder could be related with prenatal low family adaptability and cohesion. This preliminary study, however, includes only a few subjects, so that further large-sized study will be needed to replicate our results.

  • Two forms of gonadotropin releasing hormone (GnRH) are identified in the brain of adult mature spotted sea bass (Lateolabrax sp.) by immunohistochemical methods. Salmon GnRH immunoreactive (sGnRH-ir) cell bodies were distributed in the olfactory bulb, ventral telencephalon and preoptic region. Immunoreactive fibers were observed in the vicinity of the brain including the olfactory bulbs, the telencephalon, the optic nerve, the optic tectum, the cerebellum, the medulla oblongata and rostral spinal cord. In most cases, these fibers did not form well defined bundles. However, there was a clear continuum of immunoreactive fibers, extending from the olfactory bulbs to the pituitary. cGnRH-II-ir cell bodies were only found in olfactory bulbs. However, the distribution of cGnRH-II-ir fibers was basically similar to that of sGnRH-ir fibers except for the absence of their continuity between the olfactory bulbs and the pituitary. These data suggest that sGnRH and cGnRH-II are endogenous peptides and indicate the presence of multiple neuroendocrine functions in the brain of the spotted sea bass. It seems that sGnRH not only regulates GTH secretion but also functions as a neurotransmitter, whereas cGnRH-II functions only as a neurotransmitter.
