Videothoracoscopic Treatment of Spontaneous Pneumothorax - A Prospective Study of 30 Patients - (비디오 흉강경을 이용한 자연기흉의 수술치료)

  • 30 patients with spontaneous pneumothorax underwent videothoracoscopic treatment between March and July 1992. The patients ranged in age from 16 years to 62 years (mean age, 30.4 years) and the incidence according to age group was highest as 50 % in the adolescence between 21 and 30 years old. The indications of the therapeutic videothoracoscopy for spontaneous pneumothorax were recurrence (30.8%), persistent air leak (30.8%), visible blebs on the chest X-ray (20.4%), tension pneumothorax (15.4%), and bilaterality (2.6%). Intraoperative scopic findings were as follows; blebs (87.1%), pleural adhesion (45.2%), and pleural effusion (22.6%). The operation was performed under general anesthesia with one lung intubation guided by flexible fiberoptic bronchoscopy. Procedures included bleb and/or wedge resection, tetracycline pleurodesis with mechanical abrasion, and parietal pleurectomy. Successful treatment was obtained in 66.7% (20/30) and the mean postoperative hospital stay of the successful cases was 5 days. Videothoracoscopy also provided the benefits of lesser postoperative pain, rapid recovery, short hospitalization, and smaller scar of wound by reduced trauma on access. The total 13 postoperative complications were occured in 10 patients, which showed somewhat higher rate than that of other reports because of lack of experiences in the earlier period, however, it had decreased progressively as experiences were accumulated and instruments were improved in the later period. The operative mortality was absent. Conclusively, videothoracoscopy is a new interesting modality of surgical treatment of spontaneous pneumothorax and also can be extensively applicable in the diagnosis and treatment of other thoracic surgery.

Thermocompression bonding for wafer level hermetic packaging of RF-MEMS devices (RF-MEMS 소자의 웨이퍼 레벨 밀봉 패키징을 위한 열압축 본딩)

  • In this study, we describe a low-temperature wafer-level thermocompression bonding using electroplated gold seal line and bonding pads by electroplating method for RF-MEMS devices. Silicon wafers, electroplated with gold (Au), were completely bonded at $320^{\circ}C$ for 30 min at a pressure of 2.5 MPa. The through-hole interconnection between the packaged devices and external terminal did not need metal filling process and was made by gold films deposited on the sidewall of the throughhole. This process was low-cost and short in duration. Helium leak rate, which is measured to evaluate the reliability of bonded wafers, was $2.7{\pm}0.614{\times}10^{-10}Pam^{3}/s$. The insertion loss of the CPW packaged was $-0.069{\sim}-0.085\;dB$. The difference of the insertion loss between the unpackaged and packaged CPW was less than -0.03. These values show very good RF characteristics of the packaging. Therefore, gold thermocompression bonding can be applied to high quality hermetic wafer level packaging of RF-MEMS devices.

Model-based and wavelet-based fault detection and diagnosis for biomedical and manufacturing applications: Leading Towards Better Quality of Life

  • In this paper, the analytical fault detection and diagnosis (FDD) is presented using model-based and signal-based methodology with wavelet analysis on signals obtained from sensors and sensor networks. In the model-based FDD, we present the modeling of contact interface found in soft materials, including the biomedical contacts. Fingerprint analysis and signal-based FDD are also presented with an experimental framework consisting of a mechanical pneumatic system typically found in manufacturing automation. This diagnosis system focuses on the signal-based approach which employs multi-resolution wavelet decomposition of various sensor signals such as pressure, flow rate, etc., to determine leak configuration. Pattern recognition technique and analytical vectorized maps are developed to diagnose an unknown leakage based on the established FDD information using the affine mapping. Experimental studies and analysis are presented to illustrate the FDD methodology. Both model-based and wavelet-based FDD applied in contact interface and manufacturing automation have implication towards better quality of life by applying theory and practice to understand how effective diagnosis can be made using intelligent FDD. As an illustration, a model-based contact surface technology an benefit the diabetes with the detection of abnormal contact patterns that may result in ulceration if not detected and treated in time, thus, improving the quality of life of the patients. Ultimately, effective diagnosis using FDD with wavelet analysis, whether it is employed in biomedical applications or manufacturing automation, can have impacts on improving our quality of life.

Biorisk Assessment of Medical Diagnostic Laboratories in Nigeria

  • Background: The aim of this study was to assess public and private medical diagnostic laboratories in Nigeria for the presence of biosafety equipment, devices, and measures. Methods: A total of 80 diagnostic laboratories in biosafety level 3 were assessed for the presence of biosafety equipment, devices, and compliance rate with biosafety practices. A detailed questionnaire and checklist was used to obtain the relevant information from enlisted laboratories. Results: The results showed the presence of an isolated unit for microbiological work, leak-proof working benches, self-closing doors, emergency exits, fire extinguisher(s), autoclaves, and hand washing sinks in 21.3%, 71.3%, 15.0%, 1.3%, 11.3%, 82.5%, and 67.5%, respectively, of all laboratories surveyed. It was observed that public diagnostic laboratories were significantly more likely to have an isolated unit for microbiological work (p = 0.001), hand washing sink (p = 0.003), and an autoclave ($p{\leq}0.001$) than private ones. Routine use of hand gloves, biosafety cabinet, and a first aid box was observed in 35.0%, 20.0%, and 2.5%, respectively, of all laboratories examined. Written standard operating procedures, biosafety manuals, and biohazard signs on door entrances were observed in 6.3%, 1.3%, and 3.8%, respectively, of all audited laboratories. No biosafety officer(s) or records of previous spills, or injuries and accidents, were observed in all diagnostic laboratories studied. Conclusion: In all laboratories (public and private) surveyed, marked deficiencies were observed in the area of administrative control responsible for implementing biosafety. Increased emphasis on provision of biosafety devices and compliance with standard codes of practices issued by relevant authorities is strongly advocated.

Unilateral Extrapedicular Vertebroplasty and Kyphoplasty in Lumbar Compression Fractures : Technique, Anatomy and Preliminary Results

  • Objective : A single balloon extrapedicular kyphoplasty has been introduced as one of the unilateral approaches for thoracic compression fractures; however, the unilateral extrapedicular technique in the lumbar area needs a further understanding of structures in the lumbar area. The purpose of the present study is to describe methods and pitfalls of this procedure based on the anatomy of the lumbar area and to analyze clinical outcome and complications. Methods : Anatomical evaluation was performed with 2 human cadavers. A retrospective review of unilateral extrapedicular approaches yielded 74 vertebral levels in 55 patients that were treated with unilateral extrapedicular vertebroplasty and kyphoplasty. Radiographic assessment included the restoration rate of vertebral height and correction of kyphosis. Results : Anatomical evaluation indicates that the safe needle entry zone of bone for the extrapedicular approach was located in the supero-lateral aspect of the junction between the pedicle and vertebral body. The unilateral extrapedicular procedure achieved adequate pain relief with a mean decreases in pain severity of $7.25{\pm}1.5$ and $2.0{\pm}1.4$, respectively. Complications were 1 retroperitoneal hematoma, 6 unilateral fillings and 3 epidural leak of the polymethylmethacrylate. Conclusion : The method of a unilateral extrapedicular approach in kyphoplasty and vertebroplasty in the lumbar area might be similar to that in thoracic approach using a route via the extrapedicular space. However, different anatomical characteristics of the lumbar area should be considered.

  • Studies on the carbohydrate metabolism of yeast as influenced by gamma-irradiation from cobalt-60 have been carried, then the mechanisms of radiation effect on respiration and fermentation were discussed under considerations of permeable changes of irradiated cell membrane. The cells of baker's yeast (Saccharomyces cerevisiae) which had been gamma-irradiated of 240 k.r. doses for an hour, then were put into aerobic oxidation and anaerobic fermentation without substrate. Total and fractionated carbohydrates of irradiated yeast cells were determined by calorimetric method with anthrone and orcinol reagents, the amounts of total carbohydrate, trehalose, RNA-ribose, PCA-soluble glycogen, alkali-soluble glycogen, acetic acid-soluble glycogen, mannan and glucan were determined according to the course of aerobic oxidation and anaerobic fermentation. It is found that the carbohydrates of irradiated cells leak out and amount of the losses teaches eleven times more than that of control, the volume of losses are seems to be replaced by water, it can be suggested the damage of gamma-irradiation occurs in the site of passive transport of cell membrane. The endogeneous aerobic respiration of irradiated cells are increased much more than control, the synthesis of reserve glycogen, glucan and RNA-ribose promoted much more than control. The anaerobic fermentation of irradiated cells are also increased than that of control, but the breakdown of carbohydrate is less than endogeneous respiration of irradiated cells. The synthetic rate is also less than that of aerobic oxidation. In irradiated yeast cells, trehalose is revealed to be primary substrate for endogeneous carbohydrate metabolism, so it is proved that the enzymic patterns are not changed but the activities of enzymes relating endogeneous respiration and autofermentation is activated. It is to be considerable to distiguish endogeneous respiration and autofermentation from exogeneous respiration and fermentation on irradiation, for membrane permeability changes and loses out carbohydrate by ionizing radiation.

  • In this study, we report that effects of a filler with a high coefficient of thermal expansion on a sealant for high-temperature ($750{\sim}850^{\circ}C$) SOFC. We designed a $SiO_2-BaO-ZnO-B_2O_3-Al_2O_3$ glass system with a softening temperature higher than $750^{\circ}C$. The properties of the glass system show not only low volumetric shrinking but also low swelling. The glass system did not create a crystal phase during along-term heat treatment. We fabricated a seal gasket with 0, 10, 15, and 20 wt% cristobalite added as filler materials with glass powder. The coefficient of thermal expansion of the seal gasket increased according to cristobalite content. During along-term heat treatment, the leak rate decreased by about 5% after a heat treatment in an oxidizing atmosphere at $750^{\circ}C$ for 2000 h, also decreasing by about 6% after a heat treatment in a reducing atmosphere at $750^{\circ}C$ for 1000 h.

The long-term centimeter variability of active galactic nuclei: A new relation between variability timescale and black hole mass

  • We study the long-term radio variability of 43 radio bright AGNs by exploiting the data base of the University of Michigan Radio Astronomy Observatory (UMRAO) monitoring program. The UMRAO database provides high quality lightcurves spanning 25 - 32 years in time at three observing frequencies, 4.8, 8, and 14.5 GHz. We model the periodograms (temporal power spectra) of the observed lightcurves as simple power-law noise (red noise, spectral power $P(f){\propto}f^{-{\beta}}$ using Monte Carlo simulations, taking into account windowing effects (red-noise leak, aliasing). The power spectra of 39 (out of 43) sources are in good agreement with the models, yielding a range in power spectral index (${\beta}$) from ${\approx}1$ to ${\approx}3$. We find a strong anti-correlation between ${\beta}$ and the fractal dimension of the lightcurves, which provides an independent check of the quality of our modelling of power spectra. We fit a Gaussian function to each flare in a given lightcurve to obtain the flare duration. We discover a correlation between ${\beta}$ and the median duration of the flares. We use the derivative of a lightcurve to obtain a characteristic variability timescale which does not depend on the assumed functional form of the flares, incomplete fitting, and so on. We find that, once the effects of relativistic Doppler boosting on the observed timescales are corrected, the variability timescales of our sources are proportional to the black hole mass to the power of ${\alpha}=1.70{\pm}0.49$. We see an indication for AGNs in different regimes of accretion rate, flat spectrum radio quasars and BL Lac objects, having different scaling relations with ${\alpha}{\approx}1$ and ${\approx}2$, respectively. We find that modelling the periodograms of four of our sources requires the assumption of broken powerlaw spectra. From simulating lightcurves as superpositions of exponential flares we conclude that strong overlap of flares leads to featureless simple power-law periodograms of AGNs at radio wavelengths in most cases (The paper is about to be submitted to ApJ).

  • In present work, $La_{0.8}Ca_{0.2}CrO_3$ (LCC) ceramic interconnect layer for SOFC was prepared by using thermal plasma spray coating process. The LCC powders were synthesized by Pechini method and calcined at the temperature of $1000^{\circ}C$. The prepared LCC powder was characterized by x-ray diffraction (XRD), scanning electron microscopy (SEM), particle counter, BET analysis, respectively. In addition, basic and essential properties of LCC layer coated by thermal plasma spray coating process such as the morphology of surface and cross section for coated layer, gas leak rate, and electrical conductivity were analyzed and discussed. Based on these experimental results, it can be concluded that the LCC layer coated by thermal plasma spray coating process can be suitable as a ceramic interconnect of SOFC operated at $800^{\circ}C$.

Monitoring System for the Elderly Living Alone Using the RaspberryPi Sensor (라즈베리파이 센서를 활용한 독거노인 모니터링 시스템)

  • In 2017, Korea has reached 1.3 million elderly people living alone. The government is promoting the basic care service for the elderly by using care workers to check the security of the elderly living alone. However, due to lack of service personnel and service usage rate of elderly care workers, it is difficult to manage. To improve these environmental constraints, this study attempted to construct a monitoring system for elderly people living alone by using sensors such as temperature, humidity, motion detection, and gas leak detection. The sensor periodically collects the current status data of the elderly and sends them to the server, creates a real time graph based on the data, and monitors it through the web. In the monitoring process, when the sensor is out of the range of the specified value, it sends a warning text message to the guardian to inform the current situation, and is designed and implemented so as to support the safety life of the elderly living alone.
