We report a case of double orifice mitral valve complicating a partial atrioventricular septal defect.The preoperative diagnosis was suggested by a characteristic angiographic and echocardiographic findings but the correct diagnosis of double orifice mitral valve itself was proven by open heart surgery. The smaller accessory mitral orifice was located in the left lateral leaflet and was left untouched. The A-V septal defect was closed with Gore-Tex patch and localized tricuspid annuloplasty was done using Gore-Tex tubular graft for correction of severe tricuspid regurgitation. The anterior mitral cleft left intact had mild mitral regurgitation on postoperative echocardiography.Postoperative course was uneventful and the patient was discharged 2 weeks later.

This study was performed to evaluate echocardiographic parameters in dogs with experimental mitral regurgitation subjected to dobutamine stress testing. In 8 beagle dogs, a 4-prong grasping forceps was inserted into the left ventricle through the carotid artery with fluoroscopic guidance. The disruption of chordae or mitral valve leaflet was performed. Echocardiographic protocols included quantitative Doppler echocardiography and M-mode measurement for evaluating left ventricle function. After all measurement was obtained at rest, dobutamine was infused incrementally. In stress testing, all measurement also was performed at rest as the same method. In stress Doppler echocardiography, regurgitant fraction and aortic stroke volume was increased significantly (P<0.001). Effective regurgitant orifice and regurgitant volume was not changed. In M-mode examination, fractional shortening was increased significantly at stress test (P<0.001). From the results obtained in this study, it could be suggested that dobutamine stress echocardiography increase left ventricle performance in non-functional mitral regurgitation and quantitative Doppler echocardiography is non-invasive, accurate method in valvular regurgitation.

We have developed a monoleaflet polymer valve as an inexpensive and viable alternative, especially for short-term use in the ventricular assist device or total artificial heart. The frame and leaflet of the polymer valve were made from polyurethane, To evaluate the hemodynamic performance of the polymer valve a comparative study of flow dynamics past a polymer valve and a St. Jude Medical prosthetic valve under physiological pulsatile flow conditions in vitro was made. Comparisons between the valves were made on the transvalvular pressure drop, regurgitation volume and maximum valve opening area. The polymer valve showed smaller regurgitation volume and transvalvular pressure drop compared to the mechanical valve at higher heart rate. The results showed that the functional characteristics of the polymer valve compared favorably with those of the mechanical valve at higher heart rate.

Calli and leaflets of ginseng (Panax ginseng C.A. Meyer) were cultured on 1/2MS media supplement. with kinetin, 2 iP, NAA, 2,4-D and IBA to assess their capacity to regenerate embryoids and organs. Root calli produced numerous embryoids and shoots in 1/2MS medium supplemented with 2 mg/l NAA and 2mg/s 2iP, and the combination of 2 iP and NAA was more effective than the combination of kinetin and NAA in induction of embryoid and shoot from root calli. Culture of leaflet in the medium supplemented with IBA resulted in profuse root regeneration.

Pectus excavatum occasionally occurs in patients who have underlying cardiac disease, especially Marfan syndrome. This report describes a patient with pectus excavatum who had ascending aortic aneurysm with aortic regurgitation and anterior leaflet prolapse of mitral valve. This patient underwent replacement of aortic valve and ascending aorta with 25 mm SJM valved conduit graft[Bentall operation with Cabrol shunt , and mitral valve replacement with SJM 31 mm, the pectus excavatum was corrected at the time of completion of the intracardiac operation with the modified sternal turnover. This procedure offered excellent operative exposure for the inracardiac operation with prevention of low cardiac output after operation due to depressed sternum and maintained chest wall stability resulting good cosmetic chest wall appearance. This patient recovered and discharged in good postoperative result with minimal temporary peroneal nerve palsy in his left leg.

A new species of Corydalis sect. Corydalis (Fumariaceae), C. misandra B.U.Oh, is described from the provinces of Gyonggi-do and Gangwon-do in Korea. The new species is distinct from its close relative C. namdoensis in having elliptic or linear leaflet shape, entire or slightly dentate bract apex, broad and rhomboidal lower outer petal, 14 papillate stigma, oblong stamen and linear capsule with one rowed seeds, etc.

Human heart valves diseased by congenital heart defects, rheumatic fever, bacterial infection, cancer may cause stenosis or insufficiency in the valves. Treatment may be with medication but often involves valve repair or replacement (insertion of an artificial heart valve). Bileaflet mechanical heart valves (BMHVs) are widely implanted to replace the diseased heart valves, but still suffer from complications such as hemolysis, platelet activation, tissue overgrowth and device failure. These complications are closely related to both flow characteristics through the valves and leaflet dynamics. In this study, the physiological flow interacting with the moving leaflets in a bileaflet mechanical heart valve (BMHV) is simulated with a strongly coupled implicit fluid-structure interaction (FSI) method which is newly organized based on the Arbitrary-Lagrangian-Eulerian (ALE) approach and the dynamic mesh method (remeshing) in FLUENT. The simulated results are in good agreement with previous experimental studies. This study shows the applicability of the present FSI model to the complicated physics interacting between fluid flow and moving boundary.

This study is to examine changes since autonomy of lotting-out price of apartment house and planning factors related to sale of apartment house through leaflet of sale of apartment house. Objects of the study were leaflets of sale of apartment houses through the Donga Il Bo daily newspaper from 2001 to 2003. The results of research can be summarized to three. First, traffic of locational factors in advertisement of sale of apartment house showed the highest frequency and it was found that it was an important planning factor of apartment house. Second, considering that advanced facilities and the highest finishing materials were used, quality of apartment house has been advanced. Third, considering that community space, theme park and green zone showed high occupancy in external space, there has been high increase in external space as well as in internal one since autonomy of lotting-out price.

Double mitral valve orifice is an unusual congenital abnormality, and is rarely of clinical importance. That abnormality not produce hemodynamic deficits, but it has been associated with other congenital heart diseases, especially atrioventricular canal defects, and so it is surgically important condition. After the first report by M.S. Greenfield in 1876, there are only a few reported cases all over the world. We experienced two cases of double mitral valve orifice with atrioventricular canal defect. The small accessory mitral orifice was placed in anterior mitral leaflet as a hole type in both cases. The A-V canal defects are corrected with a one patch technique, but the accessory orifices are left unclosed because there were no mitral regurgitations through the orifice. On the 4th postoperative day, the patients have been performed echocardiography; both had no mitral regurgitation. Both of patients are now being follow-up and we find no specific problems. A small accessory mitral orifice may be left unclosed and rarely produce significant regurgitation, conclusively.
