Purpose: To evaluate the results and effectiveness of distal chevron osteotomy combined with lateral soft tissue release for the correction of hallux valgus deformity. Materials and Methods: 31 patients who underwent distal chevron osteotomy with lateral soft tissue release between July 2002 and June 2003, were analyzed in terms of hallux valgus angle, intermetatarsal angle, subluxation of sesamoid, AOFAS score and the occurrence of avascular necrosis of the first metatarsal head. Results: The mean amount of the correction of hallux valgus angle was $26.3^{\circ}$. The mean amount of the correction of intermetatarsal angle was $6.6^{\circ}$. The mean amount of the correction of sesamoid subluxation was 1.2 points. The mean improvement of AOFAS score was 25.8 points. Avascular necrosis of the first metatarsal head was not found in any cases. Conclusion: For correction of hallux valgus deformity, distal chevron osteotomy with lateral soft tissue release showed improvement of hallux valgus angle, sesamoid subluxation and AOFAS score with no evidence of avascular necrosis of the first metatarsal head.
Purpose: Open lateral release and complete lateral release have been conducted as a surgical method in patients with patellofemoral malalignment. But authors sought to find out the best method by conducting selective release, with minimal excision of the involved lesion, and comparative analyzing the result, as postoperative satisfaction and complication. Materials and Methods: Over the 68 patients of 90 cases who underwent arthroscopic release, among 94 patients of 129 cases who underwent lateral retinacular release, from January 1993 to June 1998 were followed up prospectively. A radiological evaluation of patellar inclination, patellar tilt, congruence angle, and Q-angle and a clinical evaluation of HSS-Knee score and modified patellar score were used for analysis data before operation and data at 1 year and 5 year after operation. Results: According to the radiologic evaluation, the patellar tilt and translation revealed improvement of the results, from $13.4^{\circ}$ and 12.1mm to $3.6^{\circ}$ and 3.8mm with arthroscopic lateral complete release, and from $12.3^{\circ}$ and 11.2mm to $4.8^{\circ}$ and 5.2mm with selective release, and from $13.6^{\circ}$ and 12.3mm to $3.3^{\circ}$ and 3mm with open release. But they were not significantly related to the clinical results. HSS-Knee score was 84.2%(48/57), 81.8%(27/33), 82.1%(32/39) and modified patellar score was 82.5%(47/57), 81.8%(27/33), 82.1%(32/39) respectively, which revealed satisfactory results. And no significant difference among the operative methods were shown. Conclusion: Arthroscopic lateral retinacular release which is one of the surgical method for patellofemoral malalignment enhances rehabilitation and satisfaction of the patient, by releasing the involved retinaculum within lesser surgical extent, compared to open and complete lateral release without complications such as adhesion.

The present study aimed to document the biomechanical findings of soft tissue reconstruction surgeries for the treatment of medial patellar luxation in dogs. Stifle joints (n = 12) from dogs weighing 4.1-8.4 kg were used in this study. The following soft tissue reconstruction techniques used for the treatment of medial patellar luxation were selected for this study: vastus medialis release, medial retinacular release, and capsule release for medial realignment (n = 6), and retinacular imbrication and anti-rotational suture for lateral realignment (n = 6). A 5-kg traction using an electronic scale was applied at $45^{\circ}C$ laterally for medial realignment and medially for lateral realignment. Fluoroscopic imaging was used to measure the length of patellar displacement (LPD) in each technique. Among medial realignment techniques, capsule release had the highest horizontal LPD; vastus medialis release had significantly higher horizontal LPD than medial retinacular release. Vastus medialis release had the smallest increase statistically in vertical LPD, and vertical LPD did not differ significantly between medial retinacular and capsule release. Among lateral realignment techniques, the horizontal LPD was significantly higher in anti-rotational suture with retinacular imbrication than in retinacular imbrication alone, but the vertical LPD did not differ significantly between the two groups. Our findings indicated that vastus medialis release could decrease the medial tension on the patella without inducing patellar instability in dogs. Both medial retinacular and capsule release could increase patellar instability; moreover, medial retinacular release does not decrease the medial tension on the patella. Antirotational suture with retinacular imbrication provides more lateral tension than retinacular imbrication alone.
Purpose: This study seeks to address the results of percutaneous lateral release and medial reefing for patients with recurrent patellar dislocation. Materials and Methods: This paper focuses on 27 cases from 25 patients who had a surgical operation at our hospital in a span of eight years and six months from the time of December 1996 to May 2005. The subjects consist of 11 males and 14 females, and their average age was 22.2 years old. All the patients had their trauma history, and the average frequency of dislocation before surgery was 11.5 times. Results: The congruence angle before operation was 23.3 degree on the average, while the lateral patellofemoral angle was -5.7 degree on average. However, the congruence angle after surgery came to -2.4 degree, while the lateral patellofemoral angle recovered to the normal range of within 11.5 degrees. Four cases showed the recurrent dislocation, and two out of those four cases had -35, -12 degrees of their lateral patellofemoral angle respectively and 59, 14 degrees of their congruence angle respectively. Conclusion: Using percutaneous lateral release with medial reefing can be considered as an effective treatment for recurrent patellar dislocation

The purpose of this study is compare the effects of two therapeutic processes-i) conservative Myofacial Release Technique along and ii) Transcutaneous Electrical Nerve stimuliation alone - on the increase in the range of motion(ROM) and on the decrease in patient's pain with chronic Neck pain. The subjects of the study were student with choronic neck pain in Daegu health university. Half of them (10) took conservative Myofacial Release Technique along and the others (10) took Transcutaneous Electrical Nerve stimuliation alone. I used 3D Motion analysis. The range of motion (ROM) in six areas-flexion, extension, left lateral flexion, right lateral flexion, left rotation, right rotation-to check the effectiveness of the two combinations of methods, measured pain with visual analogue scale (VAS) to see the effect on pain reduce. Assement was conducted to the groups before the treatment began and affer the four weeks treatment ended. This study shows that both groups demonstrated significant improvement in ROM increase and pain reduction. Flexion ROM increases were significant in MFR and TENS after treatment 2 weeks and 4 weeks. Extension ROM increases were significant in MFR and TENS after treatment 2 weeks. Right rotation ROM was no significant differences were found in MFR and TENS after treatment all weeks. Right lateral flexion ROM increases were significant in MFR and TENS after treatment 3 weeks. Left lateral flexion ROM increases were significant in MFR and TENS after treatment 1 weeks. But the others no significant differences were in MFR group and TENS group during 4 weeks.
Purpose: The aim of this study was to evaluate the effects of active mandibular exercise (AME) in patients with limited mouth opening after maxillomandibular fixation (MMF) release. Methods: The study used a quasi-experimental, nonequivalent control group and a pre test-post test design. Sixty-two patients with Maxillomandibular Fixation Release were assigned to the experimental (n=31) or control group (n=31). The AME was performed in the experimental group for 4 weeks. The exercise AME consisted of maximal mouth opening, lateral excursion and protrusive movement. These movements were repeated ten times a day. After the final exercise of the day, the number of tongue blades used for mouth opening was noted. The effect of AME was evaluated after MMF release at different time intervals: a) immediately, b) after 1 week, c) after 2 weeks, d) after 4 weeks, and e) after 12 weeks. The exercise was assessed using the following criteria: a) mandibular movements, b) pain scores associated with maximal mouth opening, c) discomfort scores associated with range of movement, and d) daily life activities that involve opening the mouth. Results: The experimental group showed significant improvement regarding the range of mandibular movements (maximal mouth opening (F=23.60, p<.001), lateral excursion to the right side (F=5.25, p=.002), lateral excursion to the left side (F=5.97, p=.001), protrusive movement (F=5.51, p=.001)), pain score (F=39.59, p<.001), discomfort score (F=9.38, p<.001). Daily life activities that involve opening the mouth were more favorable compared to those in the control group. Conclusion: The AME in patients after MMF release is helpful for increasing mandibular movement range, decreasing pain and discomfort, and improving day life activities that involve opening the mouth. Therefore, AME is highly recommended as an effective nursing intervention.
Purpose: The objective of this study was to assess the clinical outcome of arthroscopic release of the extensor carpi radialis brevis (ECRB) tendon in chronic recalcitrant lateral epicondylitis and tried to determine any prognostic factors. Materials and Methods: A retrospective review of 24 patients with lateral epicondylitis treated by arthroscopic ECRB release was performed. Outcome measures included a patient self rating and visual analog scale (VAS). Functional evaluation was made with Quick-disabilities of the arm, shoulder and hand (DASH) score system to identify preoperative factors that might be associated with the outcomes. Results: Twenty one (88%) elbows received benefit from the procedure. A mean preoperative VAS pain score and the mean Quick-DASH were significantly improved at final follow up. Age, sex, dominant arm and duration of symptom, presence of capsular tear or calcification did not correlate significantly with the clinical outcome. Three of four patients without T2-weighted high signal focus of the ECRB origin on preoperative magnetic resonance imaging (MRI) showed higher Quick-DASH score. Conclusion: Arthroscopic release of the ECRB is an effective option for chronic recalcitrant lateral epicondylitis. Lack of high signal focus on preoperative MRI is significantly associated with a poor surgical outcome and a considerable factor for the proper surgical indication.
Purpose: To evaluate the results of percutaneous lateral release and arthroscopic medial plication for patients with recurrent patella dislocation. Materials and Methods: This study includes the thirty-one cases of patients who have had a surgical operation from March 2001 to March 2008. All the patients had recurrent patella dislocation after trauma history. The average age was 24.2 years old and the average follow up was 47.4 months (minimum 24 months). Results: The preoperative congruence angle, lateral patellofemoral angle, Lysholm score was $24.8^{\circ}$, $-6.2^{\circ}$, 75 points on average respectively. However, the postoperative congruence angle, lateral patellofemoral angle, Lysholm score was improved to normal range: $-2.8^{\circ}$, $10.2^{\circ}$, 95 points on average. Five cases showed the recurrent dislocation after operation. Among them, three cases showed femoral trochlear dysplasia. Conclusion: Percutaneous lateral release and arthroscopic medial plication showed satisfactory results for recurrent patella dislocation with low complication rate. However, in cases with femoral trochlear dysplasia, another surgical treatment may be considered.
This study analyzed kinematic variables about the cross step, the delivery and the release for women's javelin athletics recorded over 50m in the 2004 Busan International Athletics Competition. It was used the Kwon3D Motion Analysis Package Ver. 3.1 Program(Kwon, 2000) for analysing the kinematic variables about the distance, the velocity, and the angle, then we had the results as follows; 1 In the Cross step phase, the COG velocity was low because their step length was short. To keep the CM velocity from the approach to the last cross over step contact, the athletes have to keep the longer step length within about 130% of the athletics' height. 2. In the Delivery phase, the athletics' COG height was gradually lower, and the deceleration of the COG was going up. As the same in the cross step, Therefore the athletes have to increase the step length within about 100% of their height, in order to increasing the COG velocity. And it was shown they have to make small angle of the elbow as possible from the right foot contact to the left foot contact in order to being the big acceleration of the upperarm at the release phase. 3. In the release phase, it was shown to being low position of the release point as the COG was low and then the release velocity of the upperarm was low. Specially when the shoulder lean lateral angle is big at the release phase, it was shown they have a excessive release angle. And, when it was shown the high rotation angle of the shoulder, the shoulder was opened forward bigger than the trunk was opened forward. So the transmission of velocity from the proximal segments was a fast change.
