Cultivation of Sprout by Highly Concentrated Oxygen Water Soaking (고농도 산소수 침지를 통한 새싹 재배)

  • In order to compare the germination and growth rate of the sprouts soaked in highly concentrated oxygen water, it with specific amounts of oxygen dissolved was produced in a high pressure reactor by pressuring oxygen. The sprouts were observed after being soaked in $20^{\circ}C$ oxygen water with 20, 30, 40, 50 ppm of oxygen dissolved each. Results of ten days later indicate that the final germination rate of the sprout soaked in 50 ppm oxygen water was $24.6{\sim}28.6%$ higher than that of the sprout soaked in distilled water. The final growth length also measured 6-7 mm higher than the sprout soaked in distilled water, demonstrating that enough supply of oxygen to the sprout induces stability and efficiency in its growth.

  • Guillain-Barre syndrome, or acute inflammatory demyelinating Polyradiculoneuropathy, is frequently accompanied by cardiac and autonomic dysfunction. We report a patient who had tachycardia, orthostatic hypotension, hypertension, pronounced blood pressure fluctuations, abnormal sweating, constipation and urinary frequency as well as qudriparesis. We thought that the GBS was incurred by Damp-Heat, used Heat-Clearing and Dampness-Transforming decoction(Chongjoo-tang) in the early stage. In the later stage, fortifying the Spleen and Boosting Qi plus Supplementing the Kidney decoction(Palmultang+chongawon) was used to remove low back pain and boost recovery. The patient reached the nadir 14 days after onset. He became bed-bound and autonomic dysfunction was very severe. From 3rd week, abnormalities of autonomic function and paresis impoved gradually and he could walk above 5m without walker or equivalent support at the 5th week after onset.

Aspiration Pneumonia caused by Povidone-iodine (Betadine) in Cleft Palate Patient (구개열 환자에서 포비돈 아이오다인에 의한 흡인성 폐렴)

  • Povidone-iodine (PVP-I, Betadine) has many advantages as a disinfectant solution for preoperative preparations, especially for oral surgery. It is widely used because of its low toxicity and excellent bactericidal effect. The authors report a case of pneumonia secondary due to the aspiration of PVP-I, which was used as an oral disinfectant. We present a case of aspiration pneumonia that resulted from the careless use of PVP-I during preoperative preparation. The patient was a 10-month-old female scheduled for elective surgery to correct a complete cleft of secondary palate. During the preparation of the oral cavity for that cleft palate patient, PVP-I was incidentally aspirated to the airway. The lung resistance was noted with positive pressure and pneumonic infiltration on chest radiograph was seen. The operation was postponed and the patient underwent respiratory care. Three months later, elective palatoplasty under general anesthesia was performed without complication. Based on the experiences of this case, the authors advise that throat-packing should be placed before oral preparation to prevent serious pulmonary complications.

Effects of Plank Exercise on Abdominal Muscle Thickness and Disability in Subjects With Mild Chronic Low Back Pain (플랭크 운동이 경한 만성 요통 대상자의 복부 근육 두께와 장애에 미치는 영향)

  • Background: Chronic low back pain (CLBP) causes morphological changes in muscles, reduces muscle strength, endurance and flexibility, negatively affects lumbar stability, and limits functional activity. Plank exercise strengthens core muscles, activates abdominal muscles, and improves intra-abdominal pressure to stabilize the trunk in patients with CLBP. Objects: We investigated the effect of plank exercise on abdominal muscle thickness and disability in patients with CLBP. Methods: We classified 33 subjects into 2 groups: An experimental (n1=17) and a control group (n2=16). Patients in the experimental group participated in plank exercise and those in the control group participated in stretching exercise. Patients in both groups attended 20-minute exercise sessions thrice a week for 4 weeks. Abdominal muscle thickness in each subject was evaluated ultrasonographically, and disabilities were assessed using the Oswestry disability index (ODI). Results: Four weeks later, abdominal muscle thickness showed a significant increase over baseline values in both groups (p<.05). Patients in the experimental group reported a more significant increase in the thickness of the external oblique muscle than that in the control group (p<.05). ODI scores in the experimental group were significantly lower after intervention than before intervention (p<.05). Conclusion: Plank exercise increases the thickness of the external oblique muscle and reduces disability secondary to mild CLBP. Therefore, plank exercise is needed to improve lumbar stability and functional activity in patients with mild CLBP.

Effect of the pulmonary recruitment maneuver on pain after laparoscopic gynecological oncologic surgery: a prospective randomized trial

  • Objective: To evaluate the effectiveness of the pulmonary recruitment maneuver (PRM) at the end of the operation to decrease laparoscopy-induced abdominal or shoulder pain after gynecological oncologic surgery. Methods: In total, 113 women undergoing laparoscopic surgery for malignant or premalignant gynecological lesions were assigned randomly to two groups: the PRM group (the patient was placed in the Trendelenburg position ($30^{\circ}$) and the PRM, consisting of two manual pulmonary inflations to a maximum pressure of $40cmH_2O$) (n=54) and the control group (n=52). Postoperative shoulder and abdominal pain was assessed 12, 24, and 48 hours later using a visual analog scale (0-10). In addition, the incidence of post-discharge nausea and vomiting was recorded until 48 hours after discharge. Results: Postoperative shoulder pain at 12 and 24 hours was significantly less severe in the PRM group ($2.2{\pm}0.5$ and $2.0{\pm}0.4$) than in the control group ($4.0{\pm}0.5$ and $3.9{\pm}0.4$; both p<0.001). The PRM significantly reduced the severity of upper abdominal pain at 12 and 24 h compared with the control group ($3.1{\pm}0.4$ and $2.9{\pm}0.4$ vs. $2.9{\pm}0.5$ and $4.9{\pm}0.5$; both p<0.001). The analgesic requirement during the postoperative period was similar in the two groups (control group, 78.8%; PRM group, 75.9%; p=0.719). Conclusion: The PRM effectively and safely reduced postoperative shoulder and upper abdominal pain levels in patients undergoing laparoscopic gynecological oncologic surgery. Research on hysteretic characteristics of EBIMFCW under different axial compression ratios

  • Energy-saving block and invisible multiribbed frame composite wall (EBIMFCW) is an important shear wall, which is composed of energy-saving blocks, steel bars and concrete. This paper conducted seismic performance tests on six 1/2-scale EBIMFCW specimens, analyzed their failure process under horizontal reciprocating load, and studied the effect of axial compression ratio on the wall's hysteresis curve and skeleton curve, ductility, energy dissipation capacity, stiffness degradation, bearing capacity degradation. A formula for calculating the peak bearing capacity of such walls was proposed. Results showed that the EBIMFCW had experienced a long time deformation from cracking to failure and exhibited signs of failure. The three seismic fortification lines of the energy-saving block, internal multiribbed frame, and outer multiribbed frame sequentially played important roles. With the increase in axial compression ratio, the peak bearing capacity and ductility of the wall increased, whereas the initial stiffness decreased. The change in axial compression ratio had a small effect on the energy dissipation capacity of the wall. In the early stage of loading, the influence of axial compression ratio on wall stiffness and strength degradation was unremarkable. In the later stage of loading, the stiffness and strength degradation of walls with high axial compression ratio were low. The displacement ductility coefficients of the wall under vertical pressure were more than 3.0 indicating that this wall type has good deformation ability. The limit values of elastic displacement angle under weak earthquake and elastic-plastic displacement angle under strong earthquake of the EBIMFCW were1/800 and 1/80, respectively.

Balloon Valvuloplasty for Congenital Pulmonary Valve Stenosis (풍선 카테터에 의한 폐동맥 판막 성형술)

  • Percutaneous pulmonary valvuloplasty was performed in 19 patients of congenital pulmonary valve stenosis. Pulmonary annulus diameter was estimated by cross sectional echocardiography and right ventricular cineangiography. The size of balloon dilatation catheter was chosen by the same size of the estimated pulmonary annulus in the first 3 patients and 20 \ulcorner30% greater than the annulus in the last 17 patients. After valvuloplasty a satisfactory results was obtained in most patients. Before dilatation, the right ventricular systolic pressure was 91.7 mmHg[range 58-150 mmHg] and it fell to 49.2mmHg[25-85 mmHg] after dilatation. The transvalvular gradient was 67.7 mmHg[33 \ulcorner120 mmHg] before dilatation and it fell to 23.7mmHg [5 \ulcorner62] after dilatation. Repeat cardiac catheterization has been scheduled in all patients 3 months after the initial valvuloplasty but follow up recatheterization was performed in only two patients; in one of them residual gradient of 50 mmHg was reduced to 30 mmHg by repeat valvuloplasty. The other patient showed no evidence of restenosis with transvalvular gradient of 20 mmHg The balloon used for valvuloplasty was single balloon for the first 10 cases and for the later 10 cases it was replaced by Trefoil balloon which was easier for inflation and deflation. There were no significant complications during and after the procedure. From our results, we conclude that balloon valvuloplasty for congenital pulmonary valve stenosis is the treatment of choice in most patients.

  • There have been several case reports of neurological manifestations in pediatrics as severe acute respiratory syndrome coronavirus-2 infection in children is being increased. We report a case of a 4-year-old boy who presented febrile seizure during follow-up in a negative pressure isolation room after confirmed coronavirus disease 2019, which has not yet been reported in Korea. He has no symptoms other than fever. The seizure was controlled after one dose of intravenous lorazepam, and there was no respiratory support during the hospitalization. He was discharged 12 days later without neurological sequelae.

The Influences of the Role-playing Analogy in Chemistry Concept Learning on Mapping Understanding and Mapping Errors (화학 개념학습에서 역할놀이 비유가 대응 관계 이해도 및 대응 오류에 미치는 영향)

  • In this study, we investigated the influences of the role-playing analogy in chemistry concept learning on mapping understanding and mapping errors by analogical reasoning ability level. Seventh-graders (N=151) at a middle school were assigned to the comparison group and the experimental group. The students of the experimental group were taught with the 'running in the circle' role-playing analogy. After the students were taught about 'the relation between volume and pressure of gas', the test of mapping understanding in the next class and the retention test four weeks later were administered. The students with typical mapping errors were also interviewed to investigate their mapping processes. The results revealed that the role-playing analogy in chemistry concept learning improved mapping understanding and its retention regardless of analogical reasoning ability level. It was also found that the students in the experimental group had fewer mapping errors than those in the comparison group. However, there were similar patterns of mapping errors in both groups, and there were no significant differences in the frequencies of each type of mapping errors by analogical reasoning ability level. Educational implication of these findings are discussed.

Metamorphism of gneiss complex in the Paju-Gimpo area, northwestern Gyeonggi massif, Korea (경기육괴 북서부의 파주-김포지역에 분포하는 편마암복합체의 변성작용)

  • Proterozoic gneisss complex of the Paju-Gimpo area, Northwestern Gyeonggi Massif, consists of mainly gneiss and schist with locally intercalated quartzite and metamorphic calcareous rocks. Mineral assemblages of the gneiss and schist are classified into two type: sillimanite free (garnet zone) and sillimanite bearing (sillimanite zone) assemblages. In the Goyang area, Kyanite occurs as metastable relict grain in two gneiss samples, in which sillimanite, garnet, biotite, K-feldspar and plagioclase occur. Cordierite bearing mineral assemblages of gneiss are biotite+garnet+sillimanite+cordierite+plagioclase+quartz ($\pm$K-feldspar, muscovite), and represent the upper amphibolite or granulite facies metamorphism. The metamorphic complex has experienced two different regional metamorphism. The prograde metamorphism is a medium-pressure type characteries by kyanite. The peak metamorphic P-T condition of the prograde metamorphism calculated from the kyanite bearing rock is 7.0~9.4 kb and $718~778^{\circ}C$. The retrograde metamorphism, after the prograde metamorphism, is the low-pressure type characteries by occurrence of cordierite. The peak metamorphic P-T condition of later calculated from the cordierite bearing rock is 3.6~5.5 kb and $750~889^{\circ}C$. Together with the occurrence of relict kyanite, garnet+biotite+plagioclase assemblage as relict in the cordierite, and the result of estimated P-T metamorphic conditions indicate a clockwise P-T path.
