Myxoma in the Laryngeal Ventricle and the True Vocal Cord:A Case Report (후두실과 진성대에 발생한 점액종 1예)

  • Myxoma is an uncertain mesenchymal cell origin, characterized by irregular round, stellate or spindle cells surrounded by the matrix containing abundant mucoid material and scant vascularity. Their occurrence in descending order of frequency is in the heart, subcutaneous tissue, bone and genitourinary tract. In the head and neck region, the most predilection sites are mandible and maxilla(more than 80%). Laryngeal myxoma is extremely rare:only 5 cases have been reported in the English literature. We report a rare case of laryngeal myxoma. A 60-year-old man with hoarseness visited the out-patient department. The mass was located between the vocal fold and the vocal ligament, filling with the left laryngeal ventricle. We planned the laryngo-microsurgery and successfully excised using $CO_2$ laser. The histopathologic finding revealed the myxoma. After 18 months of surgery, there is no evidence of recurrence and mucosal scarring in the vocal fold. This report is the first case of laryngeal myxoma involving the laryngeal ventricle and the true vocal cord together.

A case of prolapse of the laryngeal ventricle (후두실 탈출증 1례)

  • According to Bryce and Cranston, prolapse of the laryngeal ventricle is a protrusion of the ventricular mucosa into the laryngeal cavity from inflammatory hyperplasia or edema. This 53 year old female patient was admitted to our hospital March 11, 1977 with the chief complaints of dyspnea and hoarseness. On admission, evaluation of the larynx revealed smooth surfaced round pink-reddish mass protruding bilaterally beneath the false cord. The airway was almost completely obstructed by the tumor mass, but neither ulceration nor infiltration was seen, also no lymphnode was palpable in the neck. Tracheotomy was performed for the relief of dyspnea and biopsy was done endoscopically. Histologic study showed chronic inflammation. On April 2, 1977 laryngofissure was performed under general anesthesia. Laryngeal examination revealed smooth surfaced, plate like, pinkreddish mass protruding bilaterally from both laryngeal ventricles into the laryngeal cavity and extending the whole length of the ventricle antero-posteriorly and touching each other. Biopsy was taken of the mass and the result was non-specific inflammatory hyperplasia of the larynx. Postoperatively, the air way is completely clear of obstruction but mild hoarseness still remains. No recurrent mass is visible in the larynx. In view of the symptoms, operative findings and histological findings, this case is diagnosed as "Prolapse of the laryngeal ventricle". The prolapse of the laryngeal ventricle is a very rare lesion and is reported with review of the literature.

A Case of Laryngocele Combined with Laryngeal Carcinoma (후두낭에 병발한 후두암 1치험례)

  • A laryngocele is relatively uncommon disease, and it has abnormal dilatation of the laryngeal saccule or appendix of laryngeal ventricle. Etiology and pathogenesis of laryngocele has still dabated, but laryngocele has high relationship with glass blower, wind instrument player, chronic cough, laryngeal cancer, etc. We have experienced an unusual case of laryngocele combined with laryngeal carcinoma. We discuss the relationship of laryngocele and laryngeal carcinoma, and the current literature is reviewed.

Intractable Muscle Tension Dysphonia Treated by Injection Laryngoplasty and Lidocaine Injection (성대 주입술과 리도카인 주입술을 통해 치료한 난치성 근긴장성 발성장애)

  • Muscle tension dysphonia (MTD) is a voice disorder characterized by excessive tension of the laryngeal muscles during phonation. Voice therapy is the gold standard of treatment for MTD. However, patients with MTD do not always respond to voice therapy. Multidisciplinary approaches have been attempted to treat intractable MTD such as lidocaine instillation, lidocaine injection to recurrent laryngeal nerve, botox injection and excision of false ventricle using CO2 laser. Recently, injection laryngoplasty is suggested that assists in more efficient phonation and voice therapy to MTD patients. A patient with intractable MTD underwent lidocaine injection and injection laryngoplasty showed improved voice quality and remained stable until postoperative 3 months without any complications.

OBSERVATIONS ON EMBRYONAL DEVELOPMENT OF HUMAN LARYNX (사람 배아의 후두발달에 관한 형태학적 관찰)

  • 후두는 호흡기가 시작되는 부위로 후두의 태생학적 연구는 타장기에서와 같이 정상 후두의 해부학적 이해, 후두 기형의 병인의 이해 및 치료에 기초 자료가 된다. 그러나 사람 배아의 후두 발생에 관한 연구는 재료확보가 용이하지 않고 해부학적 난이성으로 매우 드물다. 이에 저자는 사람 배아의 후두 발달을 연구하고자 배령이 확인된 20례의 배아(배령 4 주에서 8 주 까지)의 연속절편에서 광학현미경하에 후두 발달의 형태학적 관찰을 시행하여 다음과 같은 결과를 얻었다. 1. 배령 4 주에 median pharyngeal groove, laryngotracheal sulcus 와 tracheoesophageal septum 이 관찰되었다. 2. 배령 5 주에 hypopharyngeal eminence, epithelial lamina of larynx와 arytenoid swelling이 관찰되었다. 3. 배령 6 주에 hyoid condensation과 tracheoesophagealfistula가 관찰되었다. 4. 배령 7 주에 epiglottis 가 확인되었고 hyoid bone, thyroid lamina 와 cricoid cartilage 의 condensation 및 muscular condensation 이 관찰되었다. 5. 배령 8 주에 laryngeal cartilage의 chondrification과 ventricle이 관찰되었으며, vestibulotracheal canal과 pharyngotracheal canal 의 교통은 관찰되지 않았다.

2 Cases of Malignant Changed Laryngeal Papilloma (악성변화를 일으킨 후두유두종 3례)

  • Papilloma is the common benign tumor of the larynx and the incidence of its malignant change was variable. The authors recently experienced 2 cases of squamous cell carcinoma of the larynx which were considered to be transformed from laryngeal papilloma. Case 1. A 58 year old male patient visited O.P.D. of Department of Otolaryngology of Busan National University Hospital, because of hoarseness for 3 years on May 13th, 1980. At that time, local finding of indirect laryngoscopy revealed whitish hypertrophic papillomatous mass on both vocal cords and anterior commissure, and dirty gray white pseudomembrane on left aryepiglottic fold, and the result of biopsy was squamous cell papilloma. So the laryngeal papilloma was removed under suspension laryngoscopy and then he had no specific treatment in spite of being recommended 5-FU topical spray. On March 5th, 1981, he visited O.P.D. again because of progressive exacerbation of hoarseness with mild dyspnea and histopathological finding was revealed squamous cell carcinoma of the larynx. Seven days later from that day, he visited emergency room due to severe dyspnea, and emergency tracheostomy was performed on sitting position. On April 7th 1981, total laryngectomy was performed successfully and postoperative irradiation therapy was recommended. Case 2. A 47 year old male patient visited our O.P.D. because of hoarseness for 5 years on Sep. 27, 1978. At that time, local finding of indirect laryngoscopy revealed papillomatous mass on left vocal cord and left ventricle and result of biopsy was squamous cell papilloma. So he had been treated with 11 times removal of papilloma, topical spray of 5-Fu and estrogen for 3 years, but the papilloma had been recurred. On Sep. 9th, 1981, he visited O.P.D. because of severe dyspnea and emergency tracheostomy and biopsy was performed. The result of biopsy was squamous cell carcinoma of larynx and total laryngectomy was performed successfully.
