Clinical Case Report on the Gait disturbance Patient with Osteoporosis and Lower Limb Fracture Surgery Treated by Chuna Therapy (골다공증을 동반한 하지부골절 수술 후 보행불리 환자 재활에 단순추나요법을 적용한 치험 1례)

  • Objectives : The purpose of this study was to show the effectiveness of Chuna Manual Therapy applied to a osteoporosis patient with gait disturbance due to lower limb fracture surgery. Method : We focused on Chuna therapy accompanied with acupuncture, herbal medicine, moxibustion, cupping treatment and rehabilitation exercise. This case was assessed using the Visual Analogue Scale (VAS), Pain Free Walking Distance (PFWD), Berg Balance Scale (BBS), and Modified Barthel Index (MBI). Result : This patient showed improvement on the VAS from 6 to 3, PFWD from 0 m to 2000 m using a walker and 50 m by self-walking, BBS from 2 to 28, and MBI from 56 to 89. Moreover the walking state improved from gait disturbance to self-walking. Conclusion : This study suggests that Chuna therapy could be used to improve gait disturbance in patients with osteoporosis and lower limb fracture surgery.

  • Velocity-dependent increase in tonic stretch reflexes is one of the prominent characteristics of spasticity. It is very important to evaluate spasticity objectively and quantitatively before and after treatment for physicians. An accurate quantitative biomechanical evaluation for the spasticity which is caused by the disorder of central nervous system is made in this study. A sudden leg dropper which is designed to generate objective testing environment at every trial gives very effective environment for the test. Kinematic data are archived by the 3-dimensional motion analysis system($Elite^{(R)}$, B.T.S., Italy). Kinematic data are angle and angular velocity of lower limb joints, and length and lengthening velocity of lower limb muscle. A program is also developed to analyze the kinematic data of lower limb, contraction and relaxation length of muscles, and dynamic EMG data at the same tim. To evaluate spasticity quantitatively, total 31 parameters extracted from goniogram, EMG and muscle model are analyzed. Statistical analysis are made for bilateral correlations for all parameters. The described instrumentation and parameters to make quantitative and objective evaluation of spasticity shows good results.

Effects of virtual reality cognitive rehabilitation program on cognitive function, physical function and depression in the elders with dementia

  • The aim of this study is to examine the effects of virtual reality cognitive rehabilitation program on cognitive function, physical function and depression of long-term care insurance nongrading elderly dementia using Daytime protection service. For achieve this purpose, 30 dementia elderly were randomly assigned in to the experimental group(n=15)and control group(n=15). All subjects performed a general therapeutic exercise and 20- 30minutes takes virtual reality cognitive rehabilitation program were conducted with experimental group 2 times a week for 8 weeks. The intervention effects were measured by using cognitive function is mini-mental states examination-Korean version(MMSE-K), visual perception is MVPT(Motor-Free Visual Perception Test), Berg's balance scales(BBS), depression test GDS-K and lower limb strength. The results of study represented that the training group showed significant improvement in MMSEK( p<.05), visual perception(p<.05), balance(p<.05), lower limb strength(p<.05) and depression (p<.05). In conclusion, the virtual reality cognitive rehabilitation training using improves visual perception included cognitive function, physical function included balance, and lower limb strength and depression. These results suggest that virtual reality training using virtual reality cognitive rehabilitation program is feasible and suitable for mild dementia.

A case report of urinary incontinence improved by Gyejigabuja-tang. (계지가부자탕(桂枝加附子湯)을 이용하여 호전된 요실금 치험 1례)

  • Objective : The purpose of this case report is to offer an unprecedented target for Gyejigabuja-tang by analyzing a case in which the symptoms were improved by the herbal decoction therapy, and to accumulate a fundamental knowledge in herbal medicine therapy for urinary incontinence. Method : We studied a patient's chart with the patient's consent and displayed patient's timeline and adherence in a figure according to the CARE guideline. The progress of urination symptoms were assessed with the frequency of urination and how long the patient can stand urine. The accompanied lower limb pain was estimated with Numerical Rating Scale(NRS) Results : The patient's adherence was good during the medication period and symptoms related with urination and accompanied lower limb pain were improved with no specific side effects. NRS of the lower limb pain decreased from 7 to 2, the frequency of urination also improved from over 10 times to 6~8 times per day, and the patient can stand urine from less than 1 second to 2~3 minutes. Conclusions : Gyejigabuja-tang was effective for an urinary incontinence patient which corresponds to the provision No.20 in Shanghanlun.

The Effects of Gaze Direction on the Stability and Coordination of the Lower Limb Joint during Drop-Landing (드롭랜딩 시 시선 방향의 차이가 하지관절의 안정성과 협응에 미치는 영향)

  • Objective: The purpose of this study was to investigate how three gaze directions (bottom, normal, up) affects the coordination and stability of the lower limb during drop landing. Method: 20 female adults (age: 21.1±1.1 yrs, height: 165.7±6.2 cm, weight: 59.4±5.9 kg) participated in this study. Participants performed single-leg drop landing task on a 30 cm height and 20 cm horizontal distance away from the force plate. Kinetic and kinematic data were obtained using 8 motion capture cameras and 1 force plates and leg stiffness, loading rate, DPSI were calculated. All statistical analyses were computed by using SPSS 25.0 program. One-way repeated ANOVA was used to compared the differences between the variables in the direction of gaze. To locate the differences, Bonferroni post hoc was applied if significance was observed. Results: The hip flexion angle and ankle plantar flexion angle were significantly smaller when the gaze direction was up. In the kinetic variables, when the gaze direction was up, the loading rate and DPSI were significantly higher than those of other gaze directions. Conclusion: Our results indicated that decreased hip and ankle flexion angles, increased loading rate and DPSI when the gaze direction was up. This suggests that the difference in visual information can increase the risk of injury to the lower limb during landing.

Effect of an 8-week Closed Kinetic Chain Styled Pilates Exercise on Lower Limb Alignment (8주간 닫힌 사슬형 필라테스 운동이 하지 정렬에 미치는 영향)

  • Objective: The aim of this study was to investigate the effect of an 8-week closed kinetic chain typed Reformer and Chair Pilates exercise on static and dynamic lower limb alignment for healthy female adults. Method: Ten healthy young female adults without musculoskeletal injury history in last 6 months (Age: 29.3 ± 3.5 yrs., Height: 165 ± 3.4 cm, Body mass: 58.2 ± 5.4 kg) participated in this study. All participants asked to join the 8-week closed kinetic chain typed Reformer and Chair Pilates exercise, and the program was conducted for 60 minutes twice a week. Participants were asked to be measure a static Q-angle and performed free squat one week before and after the program. A 3-D motion analysis with 8 infrared cameras and 5 channels of EMG was executed in this study. The effectiveness of the training was evaluated by paired t-test, and the significance level was set at .05. Results: A significantly decreased in internal rotation angles was found at hip joint during free squat after the training. Also, significantly decreased in lateral rotation angles were found at knee and ankle joint during free squat after training. Finally, significantly decreased in muscle activations were found at adductor longus and peroneus longus during free squat after training. Conclusion: From results of our study, it is concluded that an 8-week closed kinetic chain typed Pilates exercise positively effect on lower limb alignment during dynamic movement.

Comparison of Lower Limb Muscle Activity According to Different Gait Pattern and Speed on the Treadmill (트레드밀 전·후방 보행과 속도에 따른 다리근육의 근활성도 비교)

  • Purpose: This study is to compare muscle activation of leg muscles with forward and backward gait and treadmill speed. Method: The experimenter is a healthy ten male and female. They practice walking on the treadmill for 2 minutes and then walk 2km/h and 4km/h in front and back for 3 minutes. Muscle activities were recorded from the lower limb muscles (rectus femoris [RF], biceps femoris [BF], gastrucnemius [GCM]). Results: According to the study, lower extremity muscles have higher backward gait than forward gait. Conclusion: Muscle activity at the speed indicated that the muscular activity of the lower limbs was 4.0km/h, which is higher than 2.0km/h.

A study on the slacks pattern making according to the Movement-Fitness (Part I) (동작 적합성에 따른 Slacks Pattern 설계에 관한 연구 (Part I))

  • The purpose of this study was to investigate the slacks pattern making on the movement-fitness of the knee region. The experimental items were divided into the lower limb movement (M1~M5) and slacks pattern designs (4 : basic pattern, in-pleats, side-pleats, seam-line increase). This study was done by clothing pressure test, sensory evaluation test and the difference shape-transformation of wearing-slacks. The summarized finding resulted from experiments and investigation are suggested as follows ; First, the clothing pressure test showed that the order of clothing pressure in the movement was the rabbit leap movement (M3), the sit on knees (M4), the nobleman sitting (M5), the sit on chair (M2) from the highest to the lowest. In the comparison of clothing pressure tested by blocks of lower-limb, center front knee region shows the highest of 300g/cm2. In it by constructional methods, the clothing pressure of in-pleats, side-pleats methods shows lower than basic pattern, hem-line increase methods. Also in comparison of the two methods, in-pleats method shows lower than side-pleats method. Second, by the sensory evaluation test based on the lower-limb movements and constructional methods of slacks, it was found that the functional factor score of the rabbit leap movement was also the waist, hip, knee region was the lowest in all blocks. And in the comparison of constructional methods, in-pleats method was the highest in all methods. Third, in consideration of the difference in the shape-transformation of wearing-slacks on the movement-fitness, pleats pattern design methods showed that the ratio of shape-transformation was lower than basic on seam-line increase methods. But the difference of the side-pleats and in-pleats methods was litle. According to these investigation, the slacks pattern making caused by movement-fitness on the knee region, in-pleats design method was the functional slacks pattern.

Comparison Study on 30 Cases of HIVD Patients with Restricted on SLRT by Sa-Am Acupuncture Banggwangjeonggyeok(膀胱正格) and General Acupuncture (하지직거상검사(下肢直擧上檢査)상 제한을 보이는 요추추간판태출증(腰椎椎間板脫出症) 환자 30례에 대한 방광정격(膀胱正格)과 체침(體鍼)의 비교연구)

  • Objectives : This study was designed to evaluate the effects of treatment of HIVD patients with lower back pain, lower limb radiating pain and restricted on SLRT by the Sa-Am acupuncture. Methods : From March 2010 to July 2010, thirty HIVD patients who admitted to Cheonan oriental medical hospital in Daejeon university were divided into two groups. Group I was treated by Sa-Am acupuncture Banggwangjeonggyeok and general acupuncture, Group II by general acupuncture. We evaluated the treatment effect of each group with the Visual Analog Scale(VAS), Oswestry Disability Index(ODI), SLR test, dividing two period(from admission day to third day after admission and from third day to sixth day after admission). Results : 1. Group I is more effective than Group II in early pain and reducing rate of pain. 2. Group I is more effective than Group II in ODI score reducing rate from admission day to third day after admission, but there was no significant difference between the two groups from third day to sixth day after admission. 3. Group I is more effective than Group II in early SLR test improvement rate. Conclusions : Sa-Am acupuncture Banggwangjeonggyeok on HIVD patients with lower back pain, lower limb radiating pain and restricted on SLR test was effective in reducing pain and improving SLR test.

The Effects of Upper and Lower Limb Coordinated Exercise in One Leg Support or non Support on Gait Ability in Chronic Stroke Patients (한 발 지지 유무에 따른 상하지 협응운동이 만성 뇌졸중 환자의 보행능력에 미치는 영향)

  • The purpose of this study was to identify the effects of upper and lower limb coordinated exercise for gait ability in stroke patients. Upper and lower limb coordinated exercise method was conducted in two different groups; one is an one leg support group and the other is a non support group. In this study, 14 patients were participated. One leg support group was applied to 7 patients, and non support group was applied to 7 patients. Both group carried out 3 times a week for 30 minutes during 4 weeks. Data were analyzed statistically via Repeated two-way ANOVA, Mann-Whitney U test, and Friedman test. The results of the measurement analysis were summarized as follows: 1. There were significant differences in 10MWT among 2 groups after intervention(p<.05). 2. There were significant differences in F8WT, FSST among 2 groups after intervention(p<.05). According to Bonferroni test, one leg support group had significant increased from pre-intervention to post-4 week. However, there were no significant differences in nonsupport group. In conclusion, improvement of gait ability in chronic stroke patients was effect to upper and lower limb coordinated exercise.
