본 연구는 전통적인 LOSA방법론의 한계를 보완하여 진보된 형태(Advanced LOSA)의 필요성에서 시작된다. 진보된 형태의 LOSA는 기존의 AUDIT 적용의 단점을 해결하고 현재의 항공 안전 요구에 보다 효과적으로 대응하는 것을 말한다. Advanced LOSA 개발 연구를 하는 동기는 최근 발전된 항공운항 체계와 항공데이터의 분석이 과거보다 용이하여 활용가치가 증대되었기 때문이다. 과거의 LOSA는 주로 조종실 공간에서 조종행위를 수행하는 조종사들에게 초점을 맞추고 있다. 그러나 Advanced LOSA는 항공사의 전 분야로 확장하여 통합적으로 관리할 수 있다. 둘째, Advanced LOSA는 데이터 수집 및 분석 기술을 통합하여 미래 예측이 가능하다. 이를 통해 항공사는 예측된 위해 요인을 사전에 제거하여 항공안전을 유지하는 확고한 SMS 체계를 확립할 수 있다. 셋째, Advanced LOSA는 데이터 분석을 기반으로 세부 요인을 선별할 수 있으며 구체적인 해결 방법을 모색할 수 있다.

LOSA(line operation safety audit)는 정상적인 운항 상태에서 운항 승무원이 발생할 수 있는 사고를 수집하기 위한 선제적 데이터 수집 방식으로 비행 중 발생하는 TEM(threat error management)과 CRM(crew resource management) 등을 수집하는 안전관리프로그램이다. 기존의 LOSA 기록 방식은 워크시트(worksheet) 에 관찰자가 직접 작성하여 자료를 보관하고 관리를 수행하였으나, 자료 보관의 어려움과 비밀 보장이 어려운 문제점이 있다. 기존의 LOSA 수집방식 대신 웹(web) 형태로 한국형 LOSA(K-LOSA)를 구현과 TEM 분류 코드를 변경하므로 효과적인 자료 관리 및 비밀 보장성을 확보하였다. 본 논문에서는 LOSA에 대하여 살펴보고 K-LOSA 프로그램의 구성을 소개한다.

In this paper, it has implemented the design for the K-Line Operations Safety Audit(K-LOSA) designed to provide a proactive snapshot of system safety and flight crew performance before an incident and accident. It introduces the a LOSA and ten operating characteristics that have come to define LOSA methodology. According to the LOSA methodology, we explain the conceptual and detail design and implement the prototype for the data collected with LOSA collection tool.

Line Operations Safety Audit (LOSA) is a well known preventive aviation safety program for Threat and Error management (TEM). High quality LOSA data suitable for safety management is obtained when a flight crew flies at the same level of attention as ordinary flight. Factors contributing to LOSA data quality may include flight crew's understanding on LOSA purpose, observer's career, and characteristics of the organization responsible for LOSA operations. This study explored purposes of TEM and LOSA, as well as their relationship. Previous studies mentioned quality of LOSA data can be influenced by heuristic judgment, hawthorne effect, and priming effect. This study recognized the importance of LOSA data quality to be effectively used for preventive safety management. It was confirmed that the level of understanding on LOSA concept, experience of the observer, and the characteristics of the department in charge of LOSA operation could affect the quality of LOSA data.

LOSA is spreading rapidly over the world to improve safety of the airlines using threat and error management system. LOSA has shown its success on the major improvement on many areas including the riskiest phase called the blue box, which is the busiest during approach and landing. Asiana Airlines has completed data collection in 2004 and round table for the data cleaning, to implement safety change process. The propose of this paper is to introduce the LOSA, ICAO DOC 9803, and the experience of Asiana.

LOSA is a flight safety program that analyses human errors in normal operations. Trained pilot observers monitor the normal flights at the observer seat. LOSA is a proactive non jeopardy data collection tool using threat and error management(TEM) as a framework. With the analysis of crew behaviors through LOSA with The LOSA collaborative(TLC), the airlines can identify the behaviors of the crew during normal operations. The major objective of LOSA is to measure how the crew manage threats, errors and undesired aircraft deviations in the cockpit on day to day operations. The airlines are able to set up effective TEM training with practical six generation Crew recourse management(CRM) with data of error from LOSA instead of theoretical CRM courses. The Airlines can use TEM as an integral part of a Safety Management System(SMS) and uses monitoring and cross-checking skills in the flight operations to manage threats and errors effectively when we know the errors we make in the cockpit on daily operation. The result of LOSA indicates that the error detection rate should be enhanced since around the half of the errors went undetected. The areas which should be focused for enhancing the error detection are monitor, cross-check, the management of workload, automation and taxiway/ runway to manage errors effectively.

ICAO(International Civil Aviation Organization) Doc 9803의 국제규정에 의거하여 SMS(Safety Management System-안전관리시스템)의 대표적인 비행안전 모니터링 프로그램으로 인정된 신개념의 운항감사제도인 LOSA(Line Operations Safety Audit-항공운항 안전감사)를 항공사에서 실시하여 정상 운항시의 운항승무원을 관찰하여 실제의 안전취약 및 위협요소, Error를 포착하여 텍사스대학 인적요인 연구소에서 작성한 최종보고서의 분석을 통해 제도와 방안을 개선한다. 본 논문에서는 LOSA실행을 통한 승무원들의 위협 및 error의 유형과 발생율, 관리율 및 관리여부를 분석하여 설명하고 항공사내 개선인 SCP(Safety Change Process)를 소개하는데 있다.

LOSA는 Line Operations Safety Audit(항공사 운항안전 감사)의 약자이며 기존의 적발 위주의 기존 Line Audit제도와 달리 조종사의 자발적 참여와 철저한 비밀을 유지하며, 처벌 금지 약속을 통하여 참여자가 평소 습관대로 비행할 수 있게 한다. 훈련된 감사관이 이를 소정의 절차서에 의거 조종석에서 관찰하여 실제의 안전취약 및 위협요소, Error를 포착해서 수집하고 텍사스대학 인적요인 연구소에서 분석하여 최종보고서를 작성하여 제도를 개선하는 안전프로그램이다. 제도와 방안을 개선하는 신개념의 선진 운항감사제도로서 안전관리시스템의 대표적인 비행안전 프로그램으로 통상 3${\sim}$4년을 주기로 실시한다. ICAO, IATA, FAA 및 IFALPA 실행 권고사항으로 현재 약30여개의 항공사가 실시하였다. LOSA는 2009년1월부터 ICAO부속서 6에 의거하여 항공사에서 실행해야 되는 SMS(안전관리 시스템)의 가장 효율적인 Hazard 식별 및 위험 관리도구 중의 하나이다. 본 논문에서는 안전관리시스템의 효과적 도구인 LOSA를 설명하고 항공사내 실행방법을 소개하는데 있다.
