The association of dietary patterns with insulin resistance in Korean adults: based on the 2015 Korea National Health and Nutrition Examination Survey (한국 성인의 식사 패턴과 인슐린 저항성 간의 상관성: 2015년도 국민건강영양조사를 이용하여)

  • Purpose: This study was conducted to identify the association between insulin resistance and the major dietary patterns of Korean adults. Methods: This study used data from the 2015 Korea National Health and Nutrition Examination Survey. The subjects were 2,276 adults aged 19 to 64 years old. Based on the food frequency questionnaire data, 112 food items were reclassified into 30 food groups. The principal component analysis method was applied to identify major dietary patterns. We used homeostatic model assessment of insulin resistance (HOMA-IR) and quantitative insulin sensitivity check index (QUICKI) value as indicators of insulin resistance. The association between major dietary patterns and insulin resistance was investigated using logistic regression analysis. Results: Three major dietary patterns were identified and assigned descriptive names based on the food items with high loadings: 'healthy Korean meal pattern', 'western meal pattern', and 'white rice, alcohol, meat pattern'. As the 'white rice, alcohol, meat pattern' score increased, significant increasing trends for fasting glucose concentration and HOMA-IR and a significant decreasing trend for QUICKI were observed after adjusting for age and sex. The odds ratio of insulin resistance according to the 'healthy Korean meal pattern' and the 'western meal pattern' were not statistically significant. the 'white rice, alcohol, meat pattern' showed a significant positive association with the risk of insulin resistance after adjusting for covariates. Conclusion: These results suggest that the 'white rice, alcohol, meat pattern' is positively associated with the risk of insulin resistance. The white rice, alcohol, meat pattern was related to the high consumption of alcohol together with rice or meat. This pattern was also associated with the high intake of sodium and low intakes of vitamin C, calcium, potassium, and dietary fiber. To confirm the association, further longitudinal studies are required.

Classification of Student's School Violence During Middle School: Applying Multilevel Latent Profile Models to Test Individual and School Effects (다층 잠재프로파일 분석을 적용한 중학생의 학교폭력 집단 분류와 개인 및 학교요인 검증)

  • The purposes of this study are to identify the latent classes of school violence depending on bullying and victimized experience by type and the influences of individual-level and school-level variables on determining these latent profiles. For these research goals, the present study utilized data from the Seoul Education Longitudinal Study(SELS) 5th wave, containing data from 2,195 middle school students who experienced school violences more than once. Multilevel latent profile models were applied to examine school violence among students. Our results indicated that there were four latent classes; high-level bullying and victimized group (1.7%), mainly bullying group(2.1%), mainly victimized group(3.7%), verbal bullying and victimized group(92.5%). Gender, resilience, self-control, peer relationship, parental relationship were significant determinants of the latent profiles at student level. Teacher-student relationships, school violence prevention, gender ratio of school were significant determinants of the latent profiles at school level. The present study contributed to extending theoretical discussions by classifying students into groups based on frequency and different forms of bullying and victimization. Moreover, this study examined determinants of student and school level simultaneously by dealing with multilevel data.

A Study about the Effects of Outplacement Support Program for Discharged Soldiers on Job-seeking Efficacy and Psychological Stability -With Focus on the Mediating Effect of Learning Commitment and Program Satisfaction- (전직지원교육이 구직효능감과 심리적 안정감에 미치는 영향에 관한 연구 -학습몰입과 교육만족도의 매개효과를 중심으로-)

  • This study aims to identify effects of outplacement support program for discharged soldier to settle and adapt successfully in Ministry of Patriots and Veterans Affairs. The subject of this study are 323 discharged soldiers who completed the outplacement support program. A survey questionnaire is conducted to measure variables of this study. All data analysis is accomplished using the AMOS 7.0 version. The results of this study are as follows. First, learning motivation has direct effect on learning flow and program satisfaction positively. Second, organizational support has direct effect on program satisfaction, psychological stability positively and on learning flow negatively. Third, outplacement support program has direct effect on learning flow, program satisfaction, job-seeking efficacy and psychological stability positively. Fourth, learning flow has direct effect on program satisfaction positively but don't have direct effect on job-seeking efficacy and psychological stability significantly. Fifth, learning motivation, outplacement support program has direct effect on job-seeking efficacy and psychological stability with partial mediation effect of program satisfaction while without mediation effect of learning flow. Based on the results of the study, the following recommendations have been made. First, it is necessary to design the outplacement support program with contents in relation to discharged soldiers' successful settlement because discharged soldiers who completed the outplacement support program showed higher job-seeking efficacy and psychological stability. Second, it is needed to conduct a longitudinal study considering other variables such as employment, establishment success etc.

  • The purpose of this study is to explore various factors during the career development period affecting the formation of the perspective on work of young workers after they graduated from high school, and to analyze the influence of each factor. The panel data of the Korean Education Longitudinal Study 2005 was used for the study. Specifically, the data on the 303 employees in the 7th year(2011) of their employment was used. First, perspective of young workers on work was affected by social self (self-awareness-related variables), educational background of their father(family background-related variables), and intrinsic and extrinsic learning motivation(learning motivation-related variables) formed during the career development period. However, extrinsic learning motivation had a passive and negative effect on motivation and giving value to work. Second, the variables related to self-awareness in the career development period and the variables related to the family background were found to have a causal relationship with the formation of young workers' perspective on work. This study is significant in that the learning motives formed during the career development period are found to be very meaningful and important variables for the formation of young workers' perspective on work.

A Computed Tomography Analysis of the Success of Spinal Fusion Using Ultra-Low Dose (0.7 mg per Facet) of Recombinant Human Bone Morphogenetic Protein 2 in Multilevel Adult Degenerative Spinal Deformity Surgery

  • Study Design: Retrospective cohort study. Purpose: To report on spinal fusion assessment using computed tomography (CT) after adult spinal deformity (ASD) surgery using ultra-low dose recombinant human bone morphogenetic protein 2 (RhBMP-2). Overview of Literature: The reported dose of RhBMP-2 needed for successful spinal posterolateral fusion in ASD ranges from 10 to 20 mg per spinal level. This study reports the use of ultra-low dose of RhBMP-2 (0.07 mg per facet) to achieve spinal fusion in multilevel ASD surgery. Methods: Consecutive patients who underwent ASD surgery using ultra-low dose RhBMP-2 were recruited. Routine postoperative CT analysis for spinal fusion was performed by two spine surgeons. Inter-observer agreement was calculated for facet fusion (FF) and interbody fusion (IBF) at 6 and 12 months after the procedure. Results: Six consecutive ASD patients with a mean age of 62 years (28-72 years) were examined. Each patient received a total dose of 12 mg with an average dose of $0.69{\pm}0.2mg$ (0.42-1 mg) per single FF and $1.38{\pm}0.44mg$ (0.85-2 mg) for IBF. Total 131 FF and 15 IBF were examined in the study, with 88 FFs and nine IBFs being analyzed specifically at 6 months after the surgery. FF and IBF reported by surgeons A and B at 6 months were 97.7% vs. 91.9% FF, respectively (${\kappa}=0.95$) and 100% vs. 100% IBF, respectively (${\kappa}=1$). Two patients underwent longitudinal follow-up CT at 12 months, and the FF rates reported by surgeons A and B were 100% vs. 95.8%, respectively (${\kappa}=0.96$). Five out of nine facet (56%) non-unions were identified at the cross-links. The remaining four facet pseudarthrosis were noted at 1-2 spinal levels caudal to the cross-links. At the final clinical follow-up, there was no rod breakage, deformity progression, neurological deficit, or symptom recurrence. The Oswestry Disability Index improved by an average of $32.8{\pm}6.3$, while the mental component summary of the 36-item Short-Form Health Survey improved by an average of $4.7{\pm}2.1$, and physical component summary improved by an average of $10.5{\pm}2.1$. Conclusions: To our knowledge, this is the first study to report a CT that defined 92%-98% FF and 100% IBF using the lowest reported dose of RhBMP-2 in multilevel ASD surgery. The use of ultra-low dose RhBMP-2 reduces the RhBMP-2 related complications and healthcare costs.

Assessment of Field Application of Contaminated Sediment Removal Efficiency Using PVDF Combined Hybrid Tunnel Drainage (PVDF(Polyvinylidene Fluoride) 필름형 트랜스듀서 하이브리드 터널배수재에 대한 오염퇴적물 제거효율의 현장 적용성 평가)

  • Typically, contaminated sediments cause clogging of the drain pipe, which increases the residual water pressure in the drain pipe; this study constructed a system for improving drainage efficiency of tunnels by reducing physical and chemical obstructions through ultrasonic energy generated by a PVDF film. The developed hybrid drainage system utilized a PVDF material film fused with an existing drainage tunnel and maintenance system resulting in the ability to initialize the reverse piezoelectric effect, which was evaluated through an on site application. In order to investigate the maintenance performance of the tunnel drainage system, contaminated sediments were simulated in a drainage pipe to test the effect of ultrasonic conditions on drainage efficiency in the laboratory. As a result of applying the developed portable equipment, the ultrasonic energy was generated for about 20 minutes resulting in a reduction of 74.62% of the contaminated sediments and improving drainage efficiency. From the tunnel, acoustic pressure measurements were taken to calculate the response rate while taking into account the laboratory results. In addition, PVDF film was attached to the transverse and longitudinal side of the drainage pipes where contaminated sediments occur most often in the field tunnel. these calculations show contaminant removal was 90% effective.

Association of Social Exclusion on Depression in Middle and Older Elderly: Focusing on Gender Differences (사회적 배제와 중·고령 노인 우울의 연관성: 성별 차이를 중심으로)

  • The purpose of this study is to redefine 7 social exclusion factors and to derive association between social exclusion factors and depression of middle and older elderly, focused on gender differences. The elderly aged over 50 years old were extracted from the data of the Korean Longitudinal Study of Ageing (KLoSA) in 2016. The data was analyzed by descriptive analysis, chi-squared test, and binary logistic regression analysis, using SAS 9.4. As a result, the elderly who were excluded from labor, residence, health, and relationship were found to be associated to depression. In addition, male with resident exclusion were associated with depression compared to non-excluded. Therefore, it is necessary to improve the local community programs in which elderly people can participate in labor and cultural activity. It is necessary to improve labor and culture exclusion by increasing the participation opportunities of various cultural programs. In addition, it is needed to establish specific guidelines for eliminating the social exclusion of overall elderly.

Derivation of a 3D Arching Formula for Tunnel Excavation in Anisotropic Ground Conditions and Examination of Its Effects (비등방 지반에서 터널굴착을 위한 3차원 아칭식의 유도 및 그 영향 조사)

  • Terzaghi proposed a 2D formula for arching based on the assumption of a vertical sliding surface induced in the upper part due to the downward movement of a trapdoor. The formula was later expanded to consider 3D tunnel excavation conditions under inclined sliding surfaces. This study further extends the expanded formula to consider the effects of different ground properties and inclined sliding conditions in the transverse and longitudinal directions considering anisotropic ground conditions, as well as 3D tunnel excavation conditions. The 3D formula proposed in this study was examined of the induced vertical stress under various conditions (ground property, inclined sliding surface, excavation condition, surcharge pressure, earth pressure coefficient) and compared with the 2D Terzaghi formula. The examination indicated that the induced vertical stress increased as the excavation width and length increased, the inclination angle increased, the cohesion and friction angle decreased, the earth pressure coefficient decreased, and the surcharge pressure increased. Under the conditions examined, the stress was more affected at low excavation lengths and by the ground properties in the transverse direction. In addition, The comparison with the 2D Terzaghi formula showed that the induced vertical stress was lower and the difference was highly affected by the ground properties, inclined sliding conditions, and 3D tunnel excavation conditions. The proposed 3D arching formula could help to provide better understanding of complex arching phenomena in tunnel construction.

A study on the development and applicability of fire risk assessment method for small road tunnels passing only small cars (소형차 전용 도로터널의 화재 위험도 평가기법개발 및 적용성에 관한 연구)

  • A quantitative risk assessment method for quantitatively evaluating the fire risk in designing a road tunnel disaster prevention facilities has been introduced to evaluate the appropriateness of a disaster prevention facility in a large tunnel through which all vehicle types pass. However, since the quantitative risk assessment method of the developed can be applied only to the large sectional area tunnels (large tunnels), it is necessary to develop a quantitative risk assessment method for road tunnels passing only small cars which has recently been constructed or planned. In this study, fire accidents scenarios and quantitative risk assesment method for small road tunnels through small cars only which is based on the methods for existing road tunnels (large tunnels). And the risk according to the distance between cross passage is evaluated. As a result, in order to satisfy the societal risk assessment criteria, the distance of the appropriate distance between cross passages was estimated to be 200 m, and the effect of the ventilation system of the large port exhaust ventilation system was quantitatively analyzed by comparing the longitudinal ventilation system.

The Effect of Burden of Caring Unmarried Adult Children on Depression of Employed Women: Mediating Effects of Couple Communication and Job Satisfaction (취업 여성의 미혼 성인자녀 돌봄 부담이 우울에 미치는 영향 : 부부의사소통과 일 만족도의 매개효과)

  • The purpose of this study is to explore the effect of burden of caring unmarried adult children on depression of employed women and to verify the mediating effects of couple communication and job satisfaction. For the purpose, the data from the fourth year Korean Longitudinal Survey of Women and Families on 586 employed women with baby boomer husbands and unmarried adult children were analyzed by using structure equation modeling. The found results are as follows. First, burden of caring unmarried adult children of employed women had a positive effect on their depression, while it had a negative effect on couple communication and job satisfaction. Second, couple communication of employed women influenced negatively on their depression and had a partial mediating effect on the relationship between burden of caring unmarried adult children and depression. Third, the effect of burden of caring unmarried adult children on depression of employed women was partly mediated by their job satisfaction. Further, the practical programs to enhance couple communication and job satisfaction, as well to reduce depression, were presented.
