Spectral & Aerodynamic Analysis of Cries in Infants with Cleft Lip and Palate. (구순구개열 환아의 crying에 대한 음향학적 및 공기역학적 분석)

  • 언어 발달의 조기 단계를 이해하기 위한 일환으로 crying은 언어전 발달의 기초 단계로서 여러 학문적 분야에서 많은 연구가 있어왔다. 그러나 구순구개열(CLP))환아의 경우는cry-producing/control mechnism에 variation이 많은 이유로 이 분야의 연구는 거의 없는 실정이다. 이에 본 연구에서는 다음과 같은 의문점을 가지고 CLP환아의 cry feature에 대한분석을 하였다. 첫째, 정상아와 CLP환아의 cry에 전형적인 차이가 있는가? 둘째, CLP환아의 술전, 술후 cry feature에 변화가 있는가? 셋째, cry분석이 CLP환아의 이후 speech disorder에 대한 언어전 평가로서의 가치가 있는가? 넷째, 특정 parameter가 언어전 평가에 적절한 도구로 작용할 수 있는가? 생후 15개월 이내의 CLP 환아 3명과 유사한 나이대의 정상아 8명의 cry에 대한 공기역학 및 음향음성학적 분석을 통해 CLP 환아와 정상아, CLP환아의 술전, 술후 cry특성을 비교 분석하였다. 결과는 다음과 같다. 1 공기역학적 분석 1) airflow는 CLP 환아의 경우 정상아보다 약간 높았고 술 후 약간 증가하였다. 2)폐활량을 나타내는volume에서는 정상아보다 술전 CLP환자의 경우 보상적으로 더 큰 수치를 보였고 술후 약간 증가하였다. 3)강도를 나타내는 parameter(SPL)에서는 정상아 보다 술전 CLP환자의 계측치가 약간 작았으나 술 후 증가하는 양상을 보였다. 2. 음향음성학적 분석 1)기저 주파수 분석시 정상아에 비해 술 전 CLP환자의 경우 계측치가 약간 낮았으나 술 후 증가하여 정상군의 계측치에 근접하였다. 2)강도를 나타내는energy 측정시 정상아에 비해 술 전 CLP계측치가 보상성으로 약간 큰수치를 나타내었고 술 후 약간 더 증가하였다. 3) Shimmer에서는CUI환자의 술후계측치가술전에 비해 현저히 감소하여 정상군의 수치에 근접하였다.

Transforming Growth Factor-β3 Gene SfaN1 Polymorphism in Korean Nonsyndromic Cleft Lip and Palate Patients

  • The nonsyndromic cleft lip and palate (NSCL/P) is a congenital deformity of multifactorial origin with a relatively high incidence in the oriental population. Various etiologic candidate genes have been reported with conflicting results, according to race and analysis methods. Recently, the ablation of the TGF-${\beta}3$ gene function induced cleft palates in experimental animals. Also, polymorphisms in the TGF-${\beta}3$ gene have been studied in different races; however, they have not been studied in Koreans. A novel A $\rightarrow$ G single nucleotide polymorphism (defined by the endonuclease SfaN1) was identified in intron 5 of TGF-${\beta}3$ (IVS5+104 A > G). It resulted in different genotypes, AA, AG, and GG. The objective of this study was to investigate the relationship between the SfaN1 polymorphism in TGF-${\beta}3$ and the risk of NSCL/P in the Korean population. The population of this study consisted of 28 NSCL/P patients and 41 healthy controls. The distribution of the SfaN1 genotypes was different between the cases and controls. The frequency of the G allele was significantly associated with the increased risk of NSCL/P [odds ratio (OR) = 15.92, 95% confidence interval (CI) = 6.3-41.0]. The risk for the disease increased as the G allele numbers increased (GA genotype: OR = 2.11, 95% CI = 0.38-11.68; GG genotype: OR = 110.2, 95% CI = 10.67 - 2783.29) in NSCL/P. A stratified study in patients revealed that the SfaN1 site IVS5+104A > G substitution was strongly associated with an increased risk of NSCL/P in males (p < 0.001), but not in females. In conclusion, the polymorphism of the SfaN1 site in TGF-${\beta}3$ was significantly different between the NSCL/P patients and the control. This may be a good screening marker for NSCL/P patients among Koreans.

Effects of the long-term use of maxillary protraction facemasks with skeletal anchorage on pharyngeal airway dimensions in growing patients with cleft lip and palate

  • Objective: To investigate the effects of the long-term use of a maxillary protraction facemask with miniplate (FM-MP) on pharyngeal airway dimensions in growing patients with cleft lip and palate (CLP). Methods: The study included 24 boys with CLP (mean age, 12.2 years; mean duration of FM-MP therapy, 4.9 years), divided into two groups according to the amount of A point advancement to the vertical reference plane (VRP): Group 1, > 4 mm; Group 2, < 2 mm; n = 12/group. After evaluating the skeletodental and airway variables using lateral cephalograms acquired before and after FM-MP therapy, statistical analyses were performed. Results: Group 1 showed greater forward and downward displacements of the posterior maxilla (posterior nasal spine [PNS]-horizontal reference plane [HRP]; PNS-VRP), greater increase in ANB, more forward tongue position (tongue tip-Pt vertical line to Frankfort horizontal plane), and greater increase in the oropharynx (superior posterior airway space [SPAS]; middle airway space [MAS]) and upper nasopharynx (PNS-adenoid2) than did Group 2. While maxillary advancement (A-VRP and PNS-VRP) correlated with increases in SPAS, MAS, and PNS-adenoid2, downward displacement of the PNS (PNS-HRP) correlated with increases in SPAS, MAS, PNS-adenoid1, and PNS-adenoid2, and with a decrease in vertical airway length (VAL). Mandibular forward displacement and decrease in mandibular plane correlated with increases in MAS. Conclusions: FM-MP therapy had positive effects on the oropharyngeal and nasopharyngeal airway spaces without increases in VAL in Group 1 rather than in Group 2. However, further validation using an untreated control group is necessary.

Retinoic Acid Induces Abnormal Palate During Embryogenesis in Rat

  • In order to understand the effects of all-trans-RA on palate development, RA was injected into the abdominal cavity of pregnant mice and then the embryos were taken in the following days and analyzed morphologically as well as molecular biologically. When RA was administered at the stage of E11 or E15, the overall craniofacial development was retarded. The length from jaw to eye was shortened, compared to that of normal group. When the E11 embryos were exposed to RA, cleft lip was also found along with the cleft palate. In vitro palate culture experiment also revealed that RA caused cleft palate. When RT-PCR was performed, early stage administration of RA at E11 inhibited the upregulation of Hoxa7 expression at E15 through E17. Whereas in control group, high level of Hoxa7 expression was detected in the palate of E15 to E17. In the case of Bax, the expression was decreased from E16, while remaining constant in control group. When TUNEL analysis was performed following the RA treatment at E15, TUNEL positive cells were detected in the mesenchymal cells as well as epithelial cells of palatal shelves of E16 and in E17 embryos. Whereas in normal control, TUNEL positive cells were observed mostly at the epithelium around the nasal cavity and oral cavity where rugae is made. These results altogether indicate that exposure to RA during palate development causes facial deformity including cleft palate and cleft lip by modulating the expression of homeotic genes such as Hoxa7 as well as an apoptosis-related gene, Bax, and thus malregulating the apoptosis.

Effects of a Prefabricated Functional Orthodontic Appliance on Children with Class II Division 1 Malocclusion

  • Background: This study aimed to evaluate the effect of a prefabricated functional appliance (Myobrace®) on skeletal, dental, and soft tissue components in children with Class II, division 1 malocclusion. Methods: Thirteen patients with Class II, division 1 malocclusion (9 girls and 4 boys; mean age, 8.2±0.9 years at the start and 9.3±1.0 years at the end of the treatment) were treated with Myobrace® for a mean period of 12.9±4.0 months. Patients were instructed to use the appliance daily for 1 hour and overnight while sleeping. A control group of 10 patients with untreated Class II, division 1 malocclusion (3 girls and 7 boys; mean age, 9.0±1.6 years at the start and 10.4±2.1 years at the end of the observation) was included to eliminate possible growth effects. The mean observation period for this group was 17.7±11.2 months. Lateral cephalograms were taken at the start and end of the treatment, and findings from 41 measurements were analyzed using the V-CephTM program. The mean and standard deviation of cephalometric measurements were analyzed using paired and independent sample t-tests. Results: The treatment group showed significant changes in SNB, ANB, maxillary protrusion, ramus height, proclination of upper anterior teeth, interincisal angle, overjet, and upper lip protrusion compared with the control group. However, only decrease in ANB, maxillary protrusion, overjet, upper lip protrusion, and increase in interincisal angle were significantly higher in the treatment group than in the control group. Conclusion: The prefabricated functional appliance induced skeletal, dentoalveolar, and soft tissue changes, resulting in a significant reduction in anteroposterior discrepancy.

A STUDY ON THE DISTRIBUTION OF SEVERAL GROWTH FACTORS IN THE ARTIFICIALLY CHEATED CLEFT LIP WOUND HEALING OF RABBIT FETUSES (토끼 태자에 형성시킨 구순열상의 치유과정에서 수종 성장인자 분포에 관한 연구)

  • Recently several growth factors such as $TGF-{\beta}1$, $TGF-{\beta}1$, PDGF, bFGF are known to play an important role in scar formation following adult tissue injury. But there is little known about the role of growth factors in fetal tissue healing without scar formation. Therefore the purpose of this study is to investigate the distribution of growth factors which we involved with scar formation in the artificially created cleft lip wound healing of fetuses. The author had undergone hysterotomy and created cleft lip-like defects on fetuses of New Zealand White Rabbit in mid-third trimester (24 days). Fetuses were divided into 3 groups (the repaired group, the unrepaired group and the sham-operated control group). At 1, 2, 3, 5, 7 days after procedure, the repaired, the unrepaired and the control groups were obtained by Caeserean section. After documenting the viability of fetuses, fetuses were photographed to compare size and facial morphology and sectioned for histological examination by H & E stain and spatial and temporal deposition of $TGF-{\beta}1$, $TGF-{\beta}2$, PDGF, bFGF by immunohistochemical method. The findings are summarized as follows 1. There were lack of inflammation and scar formation and neovascularity in the repaired and the unrepaired group during experimental periods. 2. The reepithelialization of the unrepaired group was slower than that of repaired group. 3. There were no differences of distribution of bFGF in the control, the repaired and the unrepaired group. 4. PDGF was increased at post-op. first and second day and decreased after post-op. third day. Eventually, there were no differences in the control, the repaired and the unrepaired group. 5. $TGF-{\beta}1$ and $TGF-{\beta}2$ were slightly increased at post-op. first and second day and decreased after post-op. third day. Eventually there were no differences in the control, the repaired and the unrepaired group. And $TGF-{\beta}2$ is more densely stained than $TGF-{\beta}1$.

  • This paper deals with developmeant and application of coating thickness control system in hot dip galvanizing process. According to the line conditions, such as line speed, strip size and target coating weight, a predictive preset model sets the initial oprating conditions. Referring the zine coating informations from the gauge, mean coating value controller adjusts the chamber pressure and horizontal distance between strip and air knife, while coating deviation controller adjusts the lip gap profile of the air knife. All adaptive gains are interactively calculated by numeric models based on the theoretical analysis. The operating result with this system effectively reduces the coating deviation in transverse direction as well as in longitudinal direction.

A SPEECH-PHONETIC STUDY ON THE PRONUNCIATION OF THE OPENBITE PATIENTS (개교환자의 발성에 관한 언어 음성학적 연구)

  • This study aimed at examining speech defects of openbite patients, which were analized in terms of formant frequency for vowels and word pronunciation length for consonants. In addition, the upper and lower lip (perioral m.) activity was tested by the EMG. The tongue force was measured by the strain gauge, and the speech discrimination test was carried out. One experimental group and one control group were used for this study and they were respectively composed of six female openbite patients and six normal-occlusion females. Eight monophthongs, two fricatives and two affricatives were chosen for speech analysis. Speeches of the above-mentioned groups were recorded and then analized by the ILS/PC-1 software. Four hundred most frequently used monosyllables were also chosen for discrimination score. Openbite patients showed the following characteristics: 1. Abnormality in case of /a/, $/\varepsilon/$, /e/, /i/ $F_2$ and /e/, /a/ $F_1$. 2. Significantly elongated length in their pronunciation of /h/ and $/C^h/$ and somewhat elongated length also in their pronunciation of /s/ and /c/. 3. Significant upper lip activity according to the EMG test during pronunciation of the bilabial consonants. 4. Relatively weak tongue force according to the strain gauge measurement. 5. According to the speech discrimination test, high rate of misarticulation in case of (a) initial /p/ /s'/ and /ts'/, (b) /a/,$/\varepsilon/$,/e/,/je/,/o/, $/\phi/$,/jo/,/u/,/we/, and /i/ (c) final (equation omitted).

Improving User Satisfaction in Adaptive Multicast Video

  • Adaptability is the most promising feature to be applied in future robust multimedia applications. In this paper, we propose the Direct Algorithm to improve the degree of satisfaction at heterogeneous receivers in multi-layered multicast video environments. The algorithm relies on a mechanism that dynamically controls the rates of the video layers and is based on feedback control packets sent by the receivers. The algorithm also addresses scalability issues by implementing a merging procedure at intermediate nodes in order to avoid packet implosion at the source in the case of large multicast groups. The proposed scheme is optimized to achieve high global video quality and reduced bandwidth requirements. We also propose the Direct Algorithm with a virtual number of layers. The virtual layering scheme induces intermediate nodes to keep extra states of the multicast session, which reduces the video degradation for all the receivers. The results show that the proposed scheme leads to improved global video quality at heterogeneous receivers with no cost of extra bandwidth.

  • An experimental study has been conducted to investigate the suction performance of a designed inlet. Total pressure distortion in the inlet S-duct was measured and the effect of lip thickness and throat area was analyzed. The volume flow rate of air into the turbojet engine was controlled to observe the effect of engine RPM to the total pressure distortion. The pressure data from 36 air tubes were obtained using scanivalve, pressure transducer, and visicoder system.
